As Latinas increasingly enter the profession, we are now having our first opportunity to supervise another Latina. This opportunity is often greeted with great excitement and anticipation; however, it also presents unique professional challenges. Ethnic identity is theoretically linked to the psychological health of U.S. Latinas/os and has pl... more Ethnic identity is theoretically linked to the psychological health of U.S. Latinas/os and has played a preeminent role in the limited literature focused on this group. However, research does not uniformly confirm a positive relationship between ethnic identity and psychological health among U.S. Latinas/os. Emerging work suggests that inconsistencies in the existing literature might be better understood if ethnic identity conceptualizations accounted for context and the developmental period under study. It also has been suggested that the narrow collection of methods and analytical tools used in ethnic identity research prevent a complete understanding of this construct. This article introduces the rationale for consideration of context, incorporation of a life span perspective, and expansion of methodological approaches in the study of U.S. Latina/o ethnic identity, describing the manner in which the four empirical studies in this major contribution illustrate these issues. The practice implications of current findings are summarized after offering suggestions for future research.

Adherence to antipsychotic medication is important in preventing relapse and re-hospitalization i... more Adherence to antipsychotic medication is important in preventing relapse and re-hospitalization in patients with severe and persistent psychosis. We explored multiple measures of adherence to oral atypical antipsychotics in a group of patients followed naturalistically for three months post discharge from a large state psychiatric inpatient facility. Thirty-eight schizophrenia patients received 3 predose blood draws immediately after discharge while medication intake was closely monitored to determine the coefficient of variation (%CV) in plasma level under controlled conditions. Two random pre-dose draws were then obtained 3 months post discharge. Percent CV at 3 months and Cp/dose difference between baseline and 3 months were used as adherence indicators. Pill counts and serf-report were also collected. Based upon all available data, less than 8% of subjects refused to take medication at all. Combining %CV and Cp/dose difference to classify patients we identified 77% of total subjects as intermittently or poorly adherent by 3 months. The mean Cp/dose difference was 11.8 -+ 8.4% for appropriately adherent subjects compared to 76.5 -+ 70.7% for intermittently/poorly adherent subjects (p < 0.00001). Pill count data identified 65% of subjects as intermittently/poorly adherent, and demonstrated that only 8% of subjects (n=3) had taken the exact number of pills prescribed. In contrast, 60% percent of all subjects rated themselves as taking all doses of medication as prescribed. Agreement among different measures of adherence was poor, ranging from. 17 to .28 (Kappa statistic). Problems in misidentification of cases may occur with all methods of adherence assessment currently available. Our preliminary data indicated that few patients who made it to three months were refusing medication. However, the majority of patients were intermittently/poorly adherent to medication following discharge from a state hospital. Overall, it appears patients who participated in our study were willing to take medication but were taking it incorrectly. While conventional depot atypical antipsychotic medications may improve adherence in this partially adherent group, the side effects of these medications make this option unrealistic for many patients.
