Objective: In this study, we investigated late complications of circumcision and presence of urog... more Objective: In this study, we investigated late complications of circumcision and presence of urogenital anomalies which exist at the time of the circumcision, which were not noticed and/or not operated, among primary school children in Kastamonu region. Materials and methods: Genital physical examination was performed on 992 circumcised, fifth class, 7-9-yearold primary school children with no health problem between 2005 and 2010, and the results were noted. Results: Excess foreskin was the only late complication of circumcision. The other most common urogenital anomalies in order of frequency were as follows; retractile testis, cryptorchidism, grade 3 infantile varicocele, inguinal hernia, hydrocele, penile rotation, and hypospadias. The rate of operating room circumcision was 14.3%. While 2 (1.2%) of the circumcision complications were determined by specialist physicians, the remaining 126 (15%) were found by healthcare workers practicing circumcision. The number of missed anomalies was "zero" in circumcisions performed by specialist physicians. Conclusion: The complication rate of circumcision is higher in circumcisions conducted by uneducated people as well as in mass circumcisions. In such cases, urogenital anomalies that require a combined surgical approach with circumcision are missed, resulting in practice of unnecessary secondary surgical interventions.

Some information suggests that this disease may be associated with infertility. It is characterized by pain-free scrotal swelling. In this study, we reported a scrotal lipomatosis case presenting due to infertility and pain-free scrotal swelling. It was operated on with the initial diagnosis of varicocele, but once fatty tissue was observed in the scrotum, the case was diagnosed as scrotal lipomatosis. Here, we present this rare case with a literature review.

IntroductionInternalized stigmatization means a internalized reaction in the person with mentally... more IntroductionInternalized stigmatization means a internalized reaction in the person with mentally illness as a answer against the people reject and discrimination.ObjectiveThe peoples with mentally illness believe that most people reject and devalue people with mental illnesses, they may suffer a number of negative outcomes, such as demoralization, lowered self-esteem.AimThe aim of our study was to identify the relationship between the factors effecting the internalized stigmatization and self-esteem among patients with mentally illness.MethodsIn this study, we measured the prevalence of internalized stigma among 115 outpatients referring to the Psychiatry Service of Antalya Education and Research Hospital between 2009–2010 using Rosenberg Self-Esteem(SE) Scale, the Internalized Stigma of Mental Illness(ISMI) Scale, and Personel Datasheet.ResultsOf the patients, 53.9% had middle level of SE and the 22.00–70.00 ranging of ISMI, 30.4% of the patients were found to be 21–30 years-old. We found the increase of ISMI when SE was decreased in both of gender(p = 0.01, r = -0.67) and it was also found to be decreased of ISMI when the education level was increased in the patients(p = 0.004,r = -0.27). Recurrent hospitalization of the patients increased their ISMI points(p = 0.014, r = 0.23). There was no statistical relation between the living place of the patients and their ISMI points(p = 0.523).ConclusionsOur results suggest that some factors effected internalized stigma and showed the relationship between the internalized stigmatization and self-esteem among patients with mentally illness. To decrease and prevent internalized stigmatization, a specific education should be provided to patients, families and society by health professionals.

ARTIcLE InfO ______________________________________________________________ _____________________... more ARTIcLE InfO ______________________________________________________________ ______________________ Introduction: Aim of this study is to investigate bacterial growth on non-infected devices and compare antibiotic-coated and non-coated implants. Materials and methods: The charts of 71 patients who underwent revision surgeries for penile prosthesis between 1995 and 2013 were reviewed. Of those, 31 devices were antibiotic-coated prostheses, while 40 of the implants were non-coated. Swab cultures were routinely obtained from corporal, pump or reservoir site during the operation. If a bacterial biofilm was determined on the prosthesis, it was also cultured. Results: A total of 5 different organisms were cultured from 18 patients. Of them, 4 devices were antibiotic-coated and the other 14 were non-coated devices. Staphylococcus epidermidis was the most common organism, while Staphylococcus hominis, beta hemolitic streptococcus, Escherichia coli and Proteus mirabilis were also cultured. All patients who had positive cultures were treated with appropriate antibiotics for four weeks postoperatively. Median follow-up time was 41 months, ranging between 8 and 82 months. One prosthesis (non-coated) became clinically infected in the follow-up period with a totally different organism. Culture positivity rates of antibiotic-coated and non-coated devices were 13% and 35% respectively and the result was significant (p=0.00254). Conclusions: Positive bacterial cultures are present on non-infected penile prostheses at revision surgeries in some of the patients. Antibiotic coated prostheses have much less positive cultures than non-coated devices.

Bu çalışmanın amacı Türkiye'deki erişkin erkek popülasyonunun seksüel performansı arttırmak için ... more Bu çalışmanın amacı Türkiye'deki erişkin erkek popülasyonunun seksüel performansı arttırmak için kullanılan ilaçlara ve bitkisel ürünlere ilgisini ve sonuçlarını değerlendirmek. Gereç ve Yöntem: Bu çalışma 2012 yılında yapılan kesitsel, gözlem çalışmasıdır. Katılımcılar Eurostat ve nomenclature of territorial units for statistics(NUTS) seviye-2 kriterlerine ve posta kodu listesine göre Türkiye genelini yansıtacak 19 ilden seçildi. 18 yaş ve üzeri erkekler çalışmaya dahil edildi. Son bir yıl içerisinde en az bir kez erektil disfonksiyon(ED) ürününü kullanan 410 erkekle yüzyüze görüşüldü. Bulgular: Katılımcıların %98'inde ED saptanmadı. ED bildirmeyenlerin % 63'ünün seksüel performans arttırmak amacıyla ilaç kullandığı saptandı. Sildenafil ve bitkisel ürünleri kullananlar arasında orta ve yüksek derecede tatmin oranı sırasıyla %85 ve %63 olarak bulundu. Bu tip ilaçları kulllanan erkeklerin partnerlerinin farkındalığı %25 idi. Bu ilaçları partnerlerinin kullandığının farkında olan kadınların tatmin oranı %63 olarak bulundu. Tartışma: Türk erkekleri arasında seksüel performansı arttırmak amacıyla PDE-5 inhibitörleri ve bitkisel ürünlerin kullanım prevelansı oldukça yüksek olup, bu erkeklerin genelde partnerlerini ilaç kullanımı hakkında bilgilendirmediği görüldü. Bilgilendirilmiş partnerlerin tatmin oranının yüksek olmasından dolayı bu ilaçların pozitif psikososyal etkilerine ve tedavi planına katkı sağlayacağına inanıyoruz.

To assess the impact of a diagnosis of bladder cancer on smoking behavior and to analyze the role... more To assess the impact of a diagnosis of bladder cancer on smoking behavior and to analyze the role of the physician in smoking cessation. Nonmuscle-invasive bladder cancer patients completed a phone survey of their smoking habits. They were asked about smoking behavior before and after the diagnosis of cancer and whether they had been informed by their physician about the relation between smoking and bladder cancer. Two hundred-twelve patients responded to the survey. The mean age was 60.03±6.36 years. Ninety-three of the 135 current smokers (68.9%) were advised to quit smoking, whereas nine of 20 nonsmokers (45%) were not commented about the future risks of smoking. The results showed that 13% of the current smokers and 35% of the current nonsmokers were not warned about the relation of bladder cancer with smoking. Statistical significance was not seen in terms of sex, disease stage, and tumor grade at diagnosis. The relation between smoking and bladder cancer has been well established. However, some patients are not informed to quit smoking by their physicians.

The procedure was completed successfully in 26 of 28 (92.9%) patients. Among these 25 of 26 (96%) patients reported mild pain. Visual analogue scale (VAS) was used for the evaluation of pain. All the patients were followed up for at least 6 months. Visual optical internal urethrotomy is a minimal invasive and successfull procedure for short (< 2 cm) strictures. Visual optical internal urethrotomy under local urethral anesthesia with lidocaine is a safe, comfortable and cost-effective procedure.

Two types of priapism have been described: low and high flow. High flow priapism oc curs secondary to penile or perineal trauma. The more common low-flow priapism occurs depending on various blood disorders, alcohol spinal cord lesions, and various medications. Medications are the most common cause in the etiology of lowflow priaspsism. Antipsychotic agents in drug-induced priapism are responsible for 15-26% of cases. Althoughthe periferal and central effect of psychotropic agents are known, dose, intake duration, and the etiology causing priapism is yet to be clarified. In this report, along with a review of the literature, we present two priapism cases which developed in different patients who took single dose chlorpromazine for 4 weeks.

Objective: To compare the efficacy and side effects of a cyclosporine microemulsion and tacrolimu... more Objective: To compare the efficacy and side effects of a cyclosporine microemulsion and tacrolimus in immunosuppressive therapy of renal transplantation. Material and methods: Between March 2003 and June 2005, the patients who had undergone kidney transplantation surgery and who were administered either basiliximab, a cyclosporine microemulsion, mycophenolate mofetil and prednisolone or basiliximab, tacrolimus, mycophenolate mofetil and prednisolone for baseline immunosuppressive therapy were recruited to our study. We evaluated the results of an 18-month follow-up period. The donors were called back weekly for a follow-up in the first month, fortnightly in the second month and then monthly for 18 months after discharge. A total of 41 patients were included in the study. The patients were evaluated as for demographic characteristics, acute rejection, cardiovascular and metabolic side effects, graft function, infections, hirsutism, gingival hyperplasia, cosmetic side effects, nephrotoxicity, drug changes and the survival rates. Results: There were no significant differences among the patients with regard to age, sex, donor type, dialysis periods, preoperative and postoperative systolic blood pressures, creatinine levels, hepatotoxicity, nephrotoxicity, occurrence of diabetes mellitus and the incidence of infection. The duration of hospitalization was prolonged in the cyclosporine A group. Acute rejection emerged in 5 patients (23.8%) in the tacrolimus group and in 4 patients (20%) in the cyclosporine A group. In the cyclosporin A group, the cholesterol and triglyceride levels were significantly higher than the tacrolimus group. The cosmetic side effects (gingival hyperplasia and hirsutism) as a reason for a change in medication were only observed in the cyclosporin A group, not in the tacrolimus group. A medication change was made in 8 patients in the cyclosporine A group and in 1 patient in the tacrolimus group. No death was observed in either group. Graft loss was observed in only 1 patient in the cyclosporine A group. Conclusion: Regarding the cosmetic side effects and hyperlipidemia, tacrolimus was found to be superior to cyclosporine A. Where hyperlipidemia is considered to be a risk factor for cardiovascular disease, tacrolimus use should be considered as a more acceptable treatment modality. However, the immunosuppressive regimen should be evaluated individually.

Background: Apolipoprotein E (ApoE) is a potential inhibitor of cell proliferation, immune regula... more Background: Apolipoprotein E (ApoE) is a potential inhibitor of cell proliferation, immune regulation and modulation of cell growth and differentiation; it also has a substantial role in antioxidant activity. ApoE has a potential role in prostate cancer progression. Materials and methods: ApoE genotyping was performed using real-time polymerase chain reaction (RT-PCR) for blood samples from a group of patients with prostate cancer (n=68) and a control group (n=78). Results: The frequency of the E3/E3 genotype was significantly higher in patients compared to controls (p=0.004). E3/E3 genotype carriers were 3.6-fold more likely to be patients than controls (odds ratio=3.67, 95% confidence interval=1.451-9.155; p=0.004). Additionally, the patients with E3/E3 genotype had significantly higher Gleason score (p=0.017), and more patients with this genotype had a Gleason score higher than 7 (p=0.007). Individuals carrying the E4 allele were significantly more common in the control group (p=0.006). The frequency of the E3/E4 genotype was found to be significantly higher in controls compared to patients (p=0.007), and patients were significantly less likely to have this genotype than controls (odds ratio=0.89, 95% confidence interval=0.833-0.967, p=0.007). Individuals carrying the E2/E3 genotype had a significantly lower Gleason score (p=0.049)-all of the patients with this genotype had a Gleason score lower than 7 (p=0.024). Conclusion: E3/E3 genotype may be a potential risk factor for prostate cancer and high Gleason scoring. The E4 allele maybe a risk-reducing factor for prostate cancer.

Four consensus guidelines have been published (European Organization for Research and Treatment of Cancer, Faculty of Radiation Oncology Genito-Urinary Group, Princess Margaret Hospital, Radiation Therapy Oncology Group) with the aim of standardizing the clinical target volume (CTV) delineation and improve outcomes. To date, no attempt has been made to compare these guidelines in terms of treatment volumes or organ at risk (OAR) irradiation. The extent to which the guideline-derived plans meet the dosimetric constraints of present trials or of the Quantitative Analysis of Normal Tissue Effects in the Clinic (QUANTEC) trial is also unknown. Our study also explored the dosimetric benefits of intensity-modulated radiotherapy (IMRT). METHODS AND MATERIALS A total of 20 patients treated with postoperative RT were included. The three-dimensional conformal radiotherapy (3D-CRT) plans were applied to cover the guideline-generated planning target volumes (66 Gy in 33 fractions). Dose-volume histograms (DVHs) were analyzed for CTV/planning target volume coverage and to evaluate OAR irradiation. The OAR DVHs were compared with the constraints proposed in the QUANTEC and Radiotherapy and Androgen Deprivation In Combination After Local Surgery (RADICALS) trials. 3D-CRT plans were compared with the tomotherapy plans for the Radiation Therapy Oncology Group planning target volume to evaluate the advantages of IMRT. RESULTS The CTV differed significantly between guidelines (p < 0.001). The European Organization for Research and Treatment of Cancer-CTVs were significantly smaller than the other CTVs (p < 0.001). Differences in prostate bed coverage superiorly accounted for the major volumetric differences between the guidelines. Using 3D-CRT, the DVHs rarely met the QUANTEC or RADICALS rectal constraints, independent of the guideline used. The RADICALS bladder constraints were met most often by the European Organization for Research and Treatment of Cancer consensus guideline (14 of 20). The tomotherapy IMRT plans resulted in significant OAR sparing compared with the 3D-CRT plans; however, the RADICALS and QUANTEC criteria were still missed in a large percentage of cases. CONCLUSION Treatment volumes using the current consensus guidelines differ significantly. For the four CTV guidelines, the rectal and bladder DVH constraints proposed in the QUANTEC and RADICALS trials are rarely met with 3D-CRT. IMRT results in significant OAR sparing; however, the RADICALS dose constraints are still missed for a large percentage of cases. The rectal and bladder constraints of RADICALS should be modified to avoid a reduction in the CTVs.

The goals of this study were to examine cases of proximal ureteral stones in which semirigid or f... more The goals of this study were to examine cases of proximal ureteral stones in which semirigid or flexible ureteroscopes alone were insufficient for endoscopic treatment, requiring the combination of both. A total of 137 patients were retrospectively evaluated. Holmium laser was used as the energy source for stone fragmentation. Each operation was begun with a 6/7.5 Fr semirigid ureteroscope (URS), and continued with a 7.5 Fr flexible URS in those procedures that failed to reach the stone or push-up. Double J stents were inserted into those patients in whom the flexible URS failed. Shock wave lithotripsy (SWL) or a repeat ureteroscopy (after 2-4 weeks) was planned in those patients who were considered to be treated unsuccessfully. The demographic features of the patients, stone sizes, treatment outcomes, need for additional treatment, complications, and the results of the postoperative 1-month early followup were evaluated. The mean age of the patients (77 males and 60 females) was 38 ± 6.7 years old, the mean stone size was 12.3 ± 3.7 mm, and the number of patients with persistent hydronephrosis was 86 (62.8 %). A stone-free diagnosis was achieved in a total of 124 patients (90.5 %), using a semirigid URS in 80 patients and a flexible URS in 44 patients. Treatment using a flexible URS was administered in 38 patients (27.7 %) due to push-up, and in 6 patients (4.3 %) because of the failure to advance the semirigid URS into the ureter. The treatment failed in 13 patients (9.4 %) despite the use of both methods. Treatment using low-caliber semirigid ureteroscopy and a holmium laser is possible, regardless of the stone size, in female patients without hydronephrosis. However, the need for combined treatment with flexible ureteroscopy is increased in male patients with hydronephrosis.

Objective: In this study, we investigated late complications of circumcision and presence of urog... more Objective: In this study, we investigated late complications of circumcision and presence of urogenital anomalies which exist at the time of the circumcision, which were not noticed and/or not operated, among primary school children in Kastamonu region. Materials and methods: Genital physical examination was performed on 992 circumcised, fifth class, 7-9-yearold primary school children with no health problem between 2005 and 2010, and the results were noted. Results: Excess foreskin was the only late complication of circumcision. The other most common urogenital anomalies in order of frequency were as follows; retractile testis, cryptorchidism, grade 3 infantile varicocele, inguinal hernia, hydrocele, penile rotation, and hypospadias. The rate of operating room circumcision was 14.3%. While 2 (1.2%) of the circumcision complications were determined by specialist physicians, the remaining 126 (15%) were found by healthcare workers practicing circumcision. The number of missed anomalies was "zero" in circumcisions performed by specialist physicians. Conclusion: The complication rate of circumcision is higher in circumcisions conducted by uneducated people as well as in mass circumcisions. In such cases, urogenital anomalies that require a combined surgical approach with circumcision are missed, resulting in practice of unnecessary secondary surgical interventions.

It was operated on with the initial diagnosis of varicocele, but once fatty tissue was observed in the scrotum, the case was diagnosed as scrotal lipomatosis. Here, we present this rare case with a literature review.

IntroductionInternalized stigmatization means a internalized reaction in the person with mentally... more IntroductionInternalized stigmatization means a internalized reaction in the person with mentally illness as a answer against the people reject and discrimination.ObjectiveThe peoples with mentally illness believe that most people reject and devalue people with mental illnesses, they may suffer a number of negative outcomes, such as demoralization, lowered self-esteem.AimThe aim of our study was to identify the relationship between the factors effecting the internalized stigmatization and self-esteem among patients with mentally illness.MethodsIn this study, we measured the prevalence of internalized stigma among 115 outpatients referring to the Psychiatry Service of Antalya Education and Research Hospital between 2009–2010 using Rosenberg Self-Esteem(SE) Scale, the Internalized Stigma of Mental Illness(ISMI) Scale, and Personel Datasheet.ResultsOf the patients, 53.9% had middle level of SE and the 22.00–70.00 ranging of ISMI, 30.4% of the patients were found to be 21–30 years-old. We found the increase of ISMI when SE was decreased in both of gender(p = 0.01, r = -0.67) and it was also found to be decreased of ISMI when the education level was increased in the patients(p = 0.004,r = -0.27). Recurrent hospitalization of the patients increased their ISMI points(p = 0.014, r = 0.23). There was no statistical relation between the living place of the patients and their ISMI points(p = 0.523).ConclusionsOur results suggest that some factors effected internalized stigma and showed the relationship between the internalized stigmatization and self-esteem among patients with mentally illness. To decrease and prevent internalized stigmatization, a specific education should be provided to patients, families and society by health professionals.

Materials and methods: The charts of 71 patients who underwent revision surgeries for penile prosthesis between 1995 and 2013 were reviewed. Of those, 31 devices were antibiotic-coated prostheses, while 40 of the implants were non-coated. Swab cultures were routinely obtained from corporal, pump or reservoir site during the operation. If a bacterial biofilm was determined on the prosthesis, it was also cultured. Results: A total of 5 different organisms were cultured from 18 patients. Of them, 4 devices were antibiotic-coated and the other 14 were non-coated devices. Staphylococcus epidermidis was the most common organism, while Staphylococcus hominis, beta hemolitic streptococcus, Escherichia coli and Proteus mirabilis were also cultured. All patients who had positive cultures were treated with appropriate antibiotics for four weeks postoperatively. Median follow-up time was 41 months, ranging between 8 and 82 months. One prosthesis (non-coated) became clinically infected in the follow-up period with a totally different organism. Culture positivity rates of antibiotic-coated and non-coated devices were 13% and 35% respectively and the result was significant (p=0.00254). Conclusions: Positive bacterial cultures are present on non-infected penile prostheses at revision surgeries in some of the patients. Antibiotic coated prostheses have much less positive cultures than non-coated devices.

Gereç ve Yöntem: Bu çalışma 2012 yılında yapılan kesitsel, gözlem çalışmasıdır. Katılımcılar Eurostat ve nomenclature of territorial units for statistics(NUTS) seviye-2 kriterlerine ve posta kodu listesine göre Türkiye genelini yansıtacak 19 ilden seçildi. 18 yaş ve üzeri erkekler çalışmaya dahil edildi. Son bir yıl içerisinde en az bir kez erektil disfonksiyon(ED) ürününü kullanan 410 erkekle yüzyüze görüşüldü. Bulgular: Katılımcıların %98'inde ED saptanmadı. ED bildirmeyenlerin % 63'ünün seksüel performans arttırmak amacıyla ilaç kullandığı saptandı. Sildenafil ve bitkisel ürünleri kullananlar arasında orta ve yüksek derecede tatmin oranı sırasıyla %85 ve %63 olarak bulundu. Bu tip ilaçları kulllanan erkeklerin partnerlerinin farkındalığı %25 idi. Bu ilaçları partnerlerinin kullandığının farkında olan kadınların tatmin oranı %63 olarak bulundu. Tartışma: Türk erkekleri arasında seksüel performansı arttırmak amacıyla PDE-5 inhibitörleri ve bitkisel ürünlerin kullanım prevelansı oldukça yüksek olup, bu erkeklerin genelde partnerlerini ilaç kullanımı hakkında bilgilendirmediği görüldü. Bilgilendirilmiş partnerlerin tatmin oranının yüksek olmasından dolayı bu ilaçların pozitif psikososyal etkilerine ve tedavi planına katkı sağlayacağına inanıyoruz.

Nonmuscle-invasive bladder cancer patients completed a phone survey of their smoking habits. They were asked about smoking behavior before and after the diagnosis of cancer and whether they had been informed by their physician about the relation between smoking and bladder cancer. Two hundred-twelve patients responded to the survey. The mean age was 60.03±6.36 years. Ninety-three of the 135 current smokers (68.9%) were advised to quit smoking, whereas nine of 20 nonsmokers (45%) were not commented about the future risks of smoking. The results showed that 13% of the current smokers and 35% of the current nonsmokers were not warned about the relation of bladder cancer with smoking. Statistical significance was not seen in terms of sex, disease stage, and tumor grade at diagnosis. The relation between smoking and bladder cancer has been well established. However, some patients are not informed to quit smoking by their physicians.

The procedure was completed successfully in 26 of 28 (92.9%) patients. Among these 25 of 26 (96%) patients reported mild pain. Visual analogue scale (VAS) was used for the evaluation of pain. All the patients were followed up for at least 6 months. Visual optical internal urethrotomy is a minimal invasive and successfull procedure for short (< 2 cm) strictures. Visual optical internal urethrotomy under local urethral anesthesia with lidocaine is a safe, comfortable and cost-effective procedure.

Two types of priapism have been described: low and high flow. High flow priapism oc curs secondary to penile or perineal trauma. The more common low-flow priapism occurs depending on various blood disorders, alcohol spinal cord lesions, and various medications. Medications are the most common cause in the etiology of lowflow priaspsism. Antipsychotic agents in drug-induced priapism are responsible for 15-26% of cases. Althoughthe periferal and central effect of psychotropic agents are known, dose, intake duration, and the etiology causing priapism is yet to be clarified. In this report, along with a review of the literature, we present two priapism cases which developed in different patients who took single dose chlorpromazine for 4 weeks.

Objective: To compare the efficacy and side effects of a cyclosporine microemulsion and tacrolimu... more Objective: To compare the efficacy and side effects of a cyclosporine microemulsion and tacrolimus in immunosuppressive therapy of renal transplantation. Material and methods: Between March 2003 and June 2005, the patients who had undergone kidney transplantation surgery and who were administered either basiliximab, a cyclosporine microemulsion, mycophenolate mofetil and prednisolone or basiliximab, tacrolimus, mycophenolate mofetil and prednisolone for baseline immunosuppressive therapy were recruited to our study. We evaluated the results of an 18-month follow-up period. The donors were called back weekly for a follow-up in the first month, fortnightly in the second month and then monthly for 18 months after discharge. A total of 41 patients were included in the study. The patients were evaluated as for demographic characteristics, acute rejection, cardiovascular and metabolic side effects, graft function, infections, hirsutism, gingival hyperplasia, cosmetic side effects, nephrotoxicity, drug changes and the survival rates. Results: There were no significant differences among the patients with regard to age, sex, donor type, dialysis periods, preoperative and postoperative systolic blood pressures, creatinine levels, hepatotoxicity, nephrotoxicity, occurrence of diabetes mellitus and the incidence of infection. The duration of hospitalization was prolonged in the cyclosporine A group. Acute rejection emerged in 5 patients (23.8%) in the tacrolimus group and in 4 patients (20%) in the cyclosporine A group. In the cyclosporin A group, the cholesterol and triglyceride levels were significantly higher than the tacrolimus group. The cosmetic side effects (gingival hyperplasia and hirsutism) as a reason for a change in medication were only observed in the cyclosporin A group, not in the tacrolimus group. A medication change was made in 8 patients in the cyclosporine A group and in 1 patient in the tacrolimus group. No death was observed in either group. Graft loss was observed in only 1 patient in the cyclosporine A group. Conclusion: Regarding the cosmetic side effects and hyperlipidemia, tacrolimus was found to be superior to cyclosporine A. Where hyperlipidemia is considered to be a risk factor for cardiovascular disease, tacrolimus use should be considered as a more acceptable treatment modality. However, the immunosuppressive regimen should be evaluated individually.

Background: Apolipoprotein E (ApoE) is a potential inhibitor of cell proliferation, immune regula... more Background: Apolipoprotein E (ApoE) is a potential inhibitor of cell proliferation, immune regulation and modulation of cell growth and differentiation; it also has a substantial role in antioxidant activity. ApoE has a potential role in prostate cancer progression. Materials and methods: ApoE genotyping was performed using real-time polymerase chain reaction (RT-PCR) for blood samples from a group of patients with prostate cancer (n=68) and a control group (n=78). Results: The frequency of the E3/E3 genotype was significantly higher in patients compared to controls (p=0.004). E3/E3 genotype carriers were 3.6-fold more likely to be patients than controls (odds ratio=3.67, 95% confidence interval=1.451-9.155; p=0.004). Additionally, the patients with E3/E3 genotype had significantly higher Gleason score (p=0.017), and more patients with this genotype had a Gleason score higher than 7 (p=0.007). Individuals carrying the E4 allele were significantly more common in the control group (p=0.006). The frequency of the E3/E4 genotype was found to be significantly higher in controls compared to patients (p=0.007), and patients were significantly less likely to have this genotype than controls (odds ratio=0.89, 95% confidence interval=0.833-0.967, p=0.007). Individuals carrying the E2/E3 genotype had a significantly lower Gleason score (p=0.049)-all of the patients with this genotype had a Gleason score lower than 7 (p=0.024). Conclusion: E3/E3 genotype may be a potential risk factor for prostate cancer and high Gleason scoring. The E4 allele maybe a risk-reducing factor for prostate cancer.

Four consensus guidelines have been published (European Organization for Research and Treatment of Cancer, Faculty of Radiation Oncology Genito-Urinary Group, Princess Margaret Hospital, Radiation Therapy Oncology Group) with the aim of standardizing the clinical target volume (CTV) delineation and improve outcomes. To date, no attempt has been made to compare these guidelines in terms of treatment volumes or organ at risk (OAR) irradiation. The extent to which the guideline-derived plans meet the dosimetric constraints of present trials or of the Quantitative Analysis of Normal Tissue Effects in the Clinic (QUANTEC) trial is also unknown. Our study also explored the dosimetric benefits of intensity-modulated radiotherapy (IMRT). METHODS AND MATERIALS A total of 20 patients treated with postoperative RT were included. The three-dimensional conformal radiotherapy (3D-CRT) plans were applied to cover the guideline-generated planning target volumes (66 Gy in 33 fractions). Dose-volume histograms (DVHs) were analyzed for CTV/planning target volume coverage and to evaluate OAR irradiation. The OAR DVHs were compared with the constraints proposed in the QUANTEC and Radiotherapy and Androgen Deprivation In Combination After Local Surgery (RADICALS) trials. 3D-CRT plans were compared with the tomotherapy plans for the Radiation Therapy Oncology Group planning target volume to evaluate the advantages of IMRT. RESULTS The CTV differed significantly between guidelines (p < 0.001). The European Organization for Research and Treatment of Cancer-CTVs were significantly smaller than the other CTVs (p < 0.001). Differences in prostate bed coverage superiorly accounted for the major volumetric differences between the guidelines. Using 3D-CRT, the DVHs rarely met the QUANTEC or RADICALS rectal constraints, independent of the guideline used. The RADICALS bladder constraints were met most often by the European Organization for Research and Treatment of Cancer consensus guideline (14 of 20). The tomotherapy IMRT plans resulted in significant OAR sparing compared with the 3D-CRT plans; however, the RADICALS and QUANTEC criteria were still missed in a large percentage of cases. CONCLUSION Treatment volumes using the current consensus guidelines differ significantly. For the four CTV guidelines, the rectal and bladder DVH constraints proposed in the QUANTEC and RADICALS trials are rarely met with 3D-CRT. IMRT results in significant OAR sparing; however, the RADICALS dose constraints are still missed for a large percentage of cases. The rectal and bladder constraints of RADICALS should be modified to avoid a reduction in the CTVs.

The goals of this study were to examine cases of proximal ureteral stones in which semirigid or f... more The goals of this study were to examine cases of proximal ureteral stones in which semirigid or flexible ureteroscopes alone were insufficient for endoscopic treatment, requiring the combination of both. A total of 137 patients were retrospectively evaluated. Holmium laser was used as the energy source for stone fragmentation. Each operation was begun with a 6/7.5 Fr semirigid ureteroscope (URS), and continued with a 7.5 Fr flexible URS in those procedures that failed to reach the stone or push-up. Double J stents were inserted into those patients in whom the flexible URS failed. Shock wave lithotripsy (SWL) or a repeat ureteroscopy (after 2-4 weeks) was planned in those patients who were considered to be treated unsuccessfully. The demographic features of the patients, stone sizes, treatment outcomes, need for additional treatment, complications, and the results of the postoperative 1-month early followup were evaluated. The mean age of the patients (77 males and 60 females) was 38 ± 6.7 years old, the mean stone size was 12.3 ± 3.7 mm, and the number of patients with persistent hydronephrosis was 86 (62.8 %). A stone-free diagnosis was achieved in a total of 124 patients (90.5 %), using a semirigid URS in 80 patients and a flexible URS in 44 patients. Treatment using a flexible URS was administered in 38 patients (27.7 %) due to push-up, and in 6 patients (4.3 %) because of the failure to advance the semirigid URS into the ureter. The treatment failed in 13 patients (9.4 %) despite the use of both methods. Treatment using low-caliber semirigid ureteroscopy and a holmium laser is possible, regardless of the stone size, in female patients without hydronephrosis. However, the need for combined treatment with flexible ureteroscopy is increased in male patients with hydronephrosis.
