Introduction Acute dyspnea is a common presenting complaint to the emergency department (ED) [1,2... more Introduction Acute dyspnea is a common presenting complaint to the emergency department (ED) [1,2]. Often, it is caused by decompensated congestive left-heart failure (CHF); which requires rapid diagnosis for prompt and appropriate treatment. However, this is often difficult in the context of emergency, especially in elderly [3] or obese patients [4] or those with underlying chronic lung disease [5]. The symptoms may be nonspecific, and physical findings are not sensitive enough to make the diagnosis which can have detrimental effects for the patient with the corresponding risks for underand overtreatment [6-8]. N terminal probrain natriuretic peptide (NtproBNP) has been described as an important biomarker able to assess diagnosis and severity of heart failure (HF) as well as predict outcome and potentially guide therapy even in the emergency setting [9]. It has been recommended in international guidelines for the diagnosis and management of HF [10].

Acute hypokalemic paralysis is one of its clinical presentation forms. It is a rare but treatable cause of acute weakness. We report a case that presented with hypokalemic paralysis in emergency room. Case Report: We report a rare case of a 36-year-old female presented with sudden onset flaccid left hemiparesis with no significant past clinical history. Initially the patient was considered as an ischemic stroke, but she was later found to have hypokalemia due to distal renal tubular acidosis and further diagnosed as case of Sjögren's syndrome. Conclusion: Acute flaccid neuromuscular weakness is a common presenting symptom in emergency room. Hypokalemia is an important differential diagnosis. The cause of hypokalemia should be sought. The diagnosis of distal renal tubular acidosis must be considered in patients presenting with hypokalemic paralysis and renal stones, and may be the first clue ...

En Tunisie, parmi les envenimations par animaux terrestres, les morsures de vipéridés sont, après... more En Tunisie, parmi les envenimations par animaux terrestres, les morsures de vipéridés sont, après les piqûres de scorpions, les plus fréquentes. La présence d'un syndrome hémorragique est un des principaux critères de gravité. Nous rapportons le cas fatal d'une envenimation vipérine chez un patient de 37 ans sans antécédents qui a consulté nos urgences dans les suites d'une morsure de vipère. L'examen initial note un oedème local de la main gauche avec des ecchymoses et la trace de morsure. Initialement le patient est conscient. La biologie initiale est strictement normale. Quatre heures après son admission, le patient présente brutalement une altération de l'état de conscience. Le scanner cérébral pratiqué en urgence conclut à la présence d'hémorragie méningée, d'hématomes intra cérébraux et de lésions ischémiques diffuses. La biologie de contrôle révèle une coagulation intravasculaire disséminée (CIVD). L'évolution est rapidement défavorable avec décès du patient dans un tableau de coma dépassé.

In Tunisia, there are about 478 species of plants commonly used in folk medicine. Medicinal plant... more In Tunisia, there are about 478 species of plants commonly used in folk medicine. Medicinal plants and herbal remedies used are responsible for 2% of intoxications listed by Tunisian National Poison Center. Most cases are related to confusion between edible plants and toxic plants lookalikes or to an excessive consumption of therapeutic plants. We report the case of a 58-year-old man admitted to the Emergency Department of the Regional Hospital of Zaghouan (Tunisia), with renal failure and rhabdomyolysis. The patient reported having daily consumption of a homemade tea based onMediterranean Buckthornroots, during the last 6 months to treat type 2 diabetes. The aim of this work was to establish an association between the consumption of the herbal remedy and the occurrence of both renal failure and rhabdomyolysis. No similar cases have been reported in recent literature.

Patients with isolated clavicle fractures are frequent in the emergency department. However, unus... more Patients with isolated clavicle fractures are frequent in the emergency department. However, unusual clavicle fractures complications, such as pneumothorax, are rare. Previous reports indicated that all pneumothorax cases were treated via performing thoracostomy. Conservatively, the treatment of the clavicle fracture, like in our case, was successful. Despite the fact that isolated clavicle fractures rarely cause complications and generally heal with immobilization, serious complications may occur requiring urgent treatment. It has been proven that physical examinations, with particular attention to the neurovascular and chest examinations, and radiographs of the clavicle are necessary to prevent overlooking these potentially dangerous complications.
