Background Glanzmann thrombasthenia (GT) is a rare bleeding disorder with a high prevalence in co... more Background Glanzmann thrombasthenia (GT) is a rare bleeding disorder with a high prevalence in communities where consanguineous marriages are mainstream. Endometriosis is a chronic inflammatory disease, and its risk increases in women with menstrual periods of longer than six days. The phenotypic expression of endometriosis is determined by the frequency and rate of the menstrual flow, as well as genetic and environmental factors. Result and case presentation 14-year-old monozygotic twin sisters with GT who developed ovarian endometriosis were referred to Hazrat Rasoul Hospital due to severe dysmenorrhea. In ultrasonic examination, endometrioma cysts were reported in both patients. They both went under endometrioma cystectomy, and the bleeding was managed using antifibrinolytic drugs, followed by recombinant activated coagulation factor VII. Both were discharged after 3 days. In the ultrasound examination performed one year after the surgery, ovaries were normal in the first twin, while the second twin had a 28 × 30 hemorrhagic cyst in the left ovary. Discussion and conclusion Menstrual bleeding and genetic factors are two theories that could be related to GT and endometriosis association, and GT could be considered a risk factor for endometriosis.

Recently, it has been suggested that microbial infections play a role in the pathogenesis of endometriosis. One of the most commonly suggested infections associated with the pathogenesis of endometriosis is human papillomavirus (HPV) infection. The present study is aimed at evaluating the prevalence, types, and risk factors for HPV infection in women with endometriosis and at investigating the association of upper and lower genital tract involvement with HPV and the severity of endometriosis. Methods. This cross-sectional study was conducted on 81 patients with endometriosis, referred to Rasool Akram Medical Complex in Tehran, Iran, for laparoscopic surgery. The patients' demographic, clinical, and anthropometric data were extracted from their medical records, as well as interviews. The stage of disease was scored based on the revised American Society for Reproductive Medicine (rASRM) classification. The HPV-positive and HPV-negative cases were compared using the chi-square test for categorical variables and Student t-test for continuous variables. Results. Twenty (24.69%) out of 81 women with endometriosis were infected with HPV (nine cases of pelvic HPV, nine cases of vaginal HPV, and two cases of both pelvic and vaginal HPV). The HPV-infected women had a significantly lower infertility rate (15% vs. 45.9%; P = 0:014). The VAS scores for dysmenorrhea and dyspareunia were relatively the same in the two groups (P > 0:05). HPV 6 and HPV 11 were the most common types of HPV, reported in 35% and 30% of endometriosis cases, respectively. Conclusion. The prevalence of HPV was 24.69%, and low-risk genotypes were dominant. No significant association was found between HPV and the severity of endometriosis.

Background: The effect of soybean and omega-3 fatty acid supplementation on menopausal symptoms i... more Background: The effect of soybean and omega-3 fatty acid supplementation on menopausal symptoms in postmenopausal women was investigated. Materials and methods: In a randomized, double-blind, placebo-controlled trial postmenopausal women were randomly given either soybean (Soygan 500 mg capsule; n = 60), or omega-3 fatty acids (Omega-rex 1000 mg soft gel; n = 60), or placebo (n = 60) daily for 12 weeks. The primary outcome was a change on the menopause rating scale score (MRS), while the secondary outcome was any adverse symptoms and effect was effect of soybean or omega-3 fatty acid supplements on the blood lipids and thyroid hormone. Results: Based on the results of post-hoc in terms of overall MRS score there was a significant difference between the control and Soygan (p < 0.001) and Omega-rex groups (p = 0.03); however, there was no significant difference between the two intervention groups (p = 0.86). Soygan and Omega-rex had no effects on the blood lipids and thyroid hormone. Conclusion: Soygan and Omega-rex reduced the MRS score and improved menopausal symptoms in postmenopausal women.

The association between endometriosis and outcome of pregnancy is one of the interesting topics. ... more The association between endometriosis and outcome of pregnancy is one of the interesting topics. Patients' pain is alleviated with pregnancy, but it seems that endometriosis causes adverse outcomes in pregnancy. The main accused is systemic chronic in ammation caused by higher level of cytokines, growth factors and angiogenic factors. In this retrospective cohort study, the case group included 125 women with endometriosis who referred to Hazrat-e-Rasool hospital in 2015-2019, and the control group included 125 women without endometriosis who went to the hospital for delivery in 2015-2019. Data on pregnancy, delivery and newborns of the endometriosis and control group was extracted from the hospital information system and the questionnaires were completed. The average age of patients was 32.74 ± 4.10 and 31.7 ± 5.53 years in endometriosis and control group respectively. In terms of pregnancy complications, placenta previa, placenta accreta, placenta abruption pre-eclampsia, GDM and postpartum hemorrhage remarkably increase in the endometriosis group compared to the control group (P < 0.05). In bowel endometriosis SGA was signi cantly higher than patients without bowel endometriosi. (P < 0.05). Conservative or surgical treatment of rectal endometriosis had no signi cant effect on pregnancy outcome (P > 0.05). As well as the presence of adenomyosis made pregnancy outcomes signi cantly unfavorable compared to people without adenomyosis. (P < 0.05). NICU admission rate was notably higher in infants of endometriosis group compared to controls (40.7% vs. 24.8%, P-value 0.009). Infant mortality rate had no signi cant difference in both groups (P = 0.12). Our ndings showed that endometriosis and adenomyosis are related to bad obstetric consequences.

Background: Endometriosis is one of the common gynecological problems during the reproductive yea... more Background: Endometriosis is one of the common gynecological problems during the reproductive years, affecting the quality of life, fertility, and sexual function of women. It is known that sexual dysfunction and quality of life are interrelated. Therefore, this study aimed to evaluate the effect of resection of endometriosis lesions via laparoscopic surgery on the improvement of sexual dysfunction in women with endometriosis. Methods: This clinical trial was performed on 30 patients with endometriosis. The Female Sexual Function Index, Endometriosis Health Profile-30, and Visual Analog Scale were completed for the patients before laparoscopic surgery and three, six, and 12 months after surgery. The results were examined and compared before and after the intervention using the ANOVA test. Results: The present results showed that the mean pain score of the patients (dysmenorrhea, dyspareunia, and pelvic pain) was significant after laparoscopic surgery (P<0.005). The female sexual function improved after laparoscopic surgery compared to the preoperative phase, and changes in the domains of psychological stimulation, humidity, and sexual orgasm were significant (P<0.005). Moreover, the female quality of life scores increased in all dimensions compared to the preoperative phase, although these improvements were not statistically significant. Conclusion: The present results revealed that laparoscopic surgery is an effective treatment, leading to a considerable improvement in female sexual function.

Methods: Women with PPROM and gestational age less than 34 weeks were enrolled in this cohort study. PPROM was confirmed with sterile speculum examination or the AmniSure test. A blood sample was drawn from mothers for complete blood count (CBC) and differential count (Diff) before administering antibiotics and corticosteroids. Another blood sample was drawn for CBC-Diff tests at the time of pregnancy termination. The status of mothers and fetuses was carefully monitored during hospitalization through the following indicators: vital signs, daily abdominal examination, uterus discharge, daily non-stress test, and weekly ultrasonography. The evidence on genotype-specific risk in women infected with human papillomavirus (HPV) with nor... more The evidence on genotype-specific risk in women infected with human papillomavirus (HPV) with normal cytology and the importance of the distinction of high-risk (HR)-HPV genotypes in the management of low-grade lesions suggest that the distinction of HR-HPV genotypes has the potential to improve the follow-up of patients treated for high-grade cervical lesions. The aims of this study were to define the persistence of the different HR-HPV in the follow-up of surgical treated women, to detect the changes of genotypes from the pre-to the post-operative status, and to evaluate whether genotype-specific persistence can predict the development of residual or recurrent disease during the follow-up. HR-HPV detection and genotyping was carried out by the Linear Array HPV Genotyping Test 1 on cervical cytological samples from 72 women treated by surgery. The 6-month post-operative HPV status was correlated with the pre-operative HPV genotype and with the residual or recurrent disease within 24 months. Foreign body is a less common etiology due to the increasing rate of cesarean delivery. We are confronting some uncommon complications of cesarean section. Uterine incision closure is a substantial step in cesarean section. Since suture material hydrolysis is affected by patient different features, impaired suture material degradation in uterus scar may cause some complications like abnormal uterine bleeding. Case Presentation: We reported a 49-year-old woman who had abnormal uterine bleeding with prior history of multiple cesarean section. During hysteroscopy, abnormal mass was seen in anterior surface of uterine cavity compatible with the level of internal os. Polypectomy and removal of foreign body followed by endometrial curettage were performed. After hormonal assay and sonogra-phy evaluation, the patient underwent hysteroscopy and foreign body was observed in anterior surface of uterus. She was followed and visited 6 month after operation and she had no abnormal bleeding. Perhaps this foreign body led to abnormal uterine bleeding. Conclusions: Patient characteristics has important role in suture material degradation. Impaired suture hydrolysis in uterus incision may cause abnormal uterine bleeding. It is interesting to demonstrate whethersuture degradation in previous cesarean section has a role in formation of placenta accrete or not .This issue needs more investigation. Because there is no similar case in literature, we cannot compare our article with others.

This study review the prevalence of selenium deficiency after bariatric surgery, incidence, and s... more This study review the prevalence of selenium deficiency after bariatric surgery, incidence, and symptoms. A systematic literature search and meta-analysis was performed in PubMed and Scopus for articles published by November 1, 2021, including the keywords “Roux-en Y gastric bypass”, “RYGB”, “Omega bypass”, “Mini bypass”, “One anastomosis gastric bypass”, “Bariatric surgery”, “Weight loss surgery”, “Metabolic surgery”, “Gastric bypass”, “Loop gastric bypass”, “Selenium”, “Selenium deficiency”, or a combination of aimed tothem in the title or abstract. In this review, nine studies examining a total of 1174 patients were included in this meta-analysis. The mean age of the patients was 41.14 ± 7.69 years. The mean interval between bariatric surgery and selenium deficiency was 40.36 ± 43.29 months. Mean BMI before surgery and at the time of selenium deficiency was 43.68 ± 4.91 kg/m2 and 28.41 ± 9.09 kg/m2, respectively. Additionally, the results showed a prevalence of 16% and 2% of selenium deficiency at 1- and 2-year follow-up after bariatric surgery, respectively. Symptoms included weakness, myopathy, and cardiomyopathy, loss of muscle mass, erythematous desquamating eruption, lethargy, dyspnea, and bilateral lower extremity pitting edema. Forty percent of studies reported “Selenium orally (100 μg once daily)” as treatment option. A multidisciplinary team of healthcare professionals, including dietitians, should be involved in the bariatric patient’s care. As a result, clinicians should encourage patients to take supplements for the rest of their lives, and patients should be monitored after surgery if necessary.

Multiple treatment options have been offered for venous repair. In this regard, the present study used the peritoneum as a conduit for the superior mesenteric vein (SMV) reconstruction during colon cancer surgery.Case presentation: The case was a 55-year-old woman with colon adenocarcinoma. The SMV damage was 6 cm in length. The defect was replaced with peritoneal conduit originated from parietal peritoneum of the abdominal wall. Coalition and flow of the SMV were confirmed by contrast-enhanced computed tomography. The patient had a good condition during a 6 months follow-up. Conclusions: Due to its availability and lower overall costs, peritoneal conduit might be a promising choice for SMV defects, in emergent conditions.

Thus, in this study, we evaluated whether there is an association between 25-hydroxyvitamin D [25(OH)D] and susceptibility to endometriosis in Iranian women. METHODS Women at reproductive age, including 56 healthy women and 54 patients with endometriosis, were enrolled in the study. Serum levels of 25(OH)D, calcium, parathyroid hormone (PTH), and peritoneal fluid (PF) levels of 25(OH)D were assessed. RESULTS The serum and PF levels of 25(OH)D in the patients with endometriosis were significantly lower than the control group (P = 0.001 and P = 0.03, respectively). Subjects with serum levels of 25(OH)D lower than 20 ng/mL had a 2.7 times higher risk of endometriosis than people with 25(OH)D serum levels higher than 20 ng/mL (non-deficient) (OR = 2.7, 95 % confidence interval: 1.24-5.80, P = 0.01). Foreign body is a less common etiology due to the increasing rate of cesarean delivery. We are confronting some uncommon complications of cesarean section. Uterine incision closure is a substantial step in cesarean section. Since suture material hydrolysis is affected by patient different features, impaired suture material degradation in uterus scar may cause some complications like abnormal uterine bleeding. Case Presentation: We reported a 49-year-old woman who had abnormal uterine bleeding with prior history of multiple cesarean section. During hysteroscopy, abnormal mass was seen in anterior surface of uterine cavity compatible with the level of internal os. Polypectomy and removal of foreign body followed by endometrial curettage were performed. After hormonal assay and sonogra-phy evaluation, the patient underwent hysteroscopy and foreign body was observed in anterior surface of uterus. She was followed and visited 6 month after operation and she had no abnormal bleeding. Perhaps this foreign body led to abnormal uterine bleeding. Conclusions: Patient characteristics has important role in suture material degradation. Impaired suture hydrolysis in uterus incision may cause abnormal uterine bleeding. It is interesting to demonstrate whethersuture degradation in previous cesarean section has a role in formation of placenta accrete or not .This issue needs more investigation. Because there is no similar case in literature, we cannot compare our article with others.

This study review the prevalence of selenium deficiency after bariatric surgery, incidence, and s... more This study review the prevalence of selenium deficiency after bariatric surgery, incidence, and symptoms. A systematic literature search and meta-analysis was performed in PubMed and Scopus for articles published by November 1, 2021, including the keywords “Roux-en Y gastric bypass”, “RYGB”, “Omega bypass”, “Mini bypass”, “One anastomosis gastric bypass”, “Bariatric surgery”, “Weight loss surgery”, “Metabolic surgery”, “Gastric bypass”, “Loop gastric bypass”, “Selenium”, “Selenium deficiency”, or a combination of aimed tothem in the title or abstract. In this review, nine studies examining a total of 1174 patients were included in this meta-analysis. The mean age of the patients was 41.14 ± 7.69 years. The mean interval between bariatric surgery and selenium deficiency was 40.36 ± 43.29 months. Mean BMI before surgery and at the time of selenium deficiency was 43.68 ± 4.91 kg/m2 and 28.41 ± 9.09 kg/m2, respectively. Additionally, the results showed a prevalence of 16% and 2% of selenium deficiency at 1- and 2-year follow-up after bariatric surgery, respectively. Symptoms included weakness, myopathy, and cardiomyopathy, loss of muscle mass, erythematous desquamating eruption, lethargy, dyspnea, and bilateral lower extremity pitting edema. Forty percent of studies reported “Selenium orally (100 μg once daily)” as treatment option. A multidisciplinary team of healthcare professionals, including dietitians, should be involved in the bariatric patient’s care. As a result, clinicians should encourage patients to take supplements for the rest of their lives, and patients should be monitored after surgery if necessary.

Multiple treatment options have been offered for venous repair. In this regard, the present study used the peritoneum as a conduit for the superior mesenteric vein (SMV) reconstruction during colon cancer surgery.Case presentation: The case was a 55-year-old woman with colon adenocarcinoma. The SMV damage was 6 cm in length. The defect was replaced with peritoneal conduit originated from parietal peritoneum of the abdominal wall. Coalition and flow of the SMV were confirmed by contrast-enhanced computed tomography. The patient had a good condition during a 6 months follow-up. Conclusions: Due to its availability and lower overall costs, peritoneal conduit might be a promising choice for SMV defects, in emergent conditions.

MATERIALS AND METHODS In a randomized, double-blind, placebo-controlled trial postmenopausal women were randomly given either soybean (Soygan 500 mg capsule; n = 60), or omega-3 fatty acids (Omega-rex 1000 mg soft gel; n = 60), or placebo (n = 60) daily for 12 weeks. The primary outcome was a change on the menopause rating scale score (MRS), while the secondary outcome was any adverse symptoms and effect was effect of soybean or omega-3 fatty acid supplements on the blood lipids and thyroid hormone. RESULTS Based on the results of post-hoc in terms of overall MRS score there was a significant difference between the control and Soygan (p < 0.001) and Omega-rex groups (p = 0.03); however, there was no significant difference between the two intervention groups (p = 0.86). Thus, in this study, we evaluated whether there is an association between 25-hydroxyvitamin D [25(OH)D] and susceptibility to endometriosis in Iranian women. METHODS Women at reproductive age, including 56 healthy women and 54 patients with endometriosis, were enrolled in the study. Serum levels of 25(OH)D, calcium, parathyroid hormone (PTH), and peritoneal fluid (PF) levels of 25(OH)D were assessed. RESULTS The serum and PF levels of 25(OH)D in the patients with endometriosis were significantly lower than the control group (P = 0.001 and P = 0.03, respectively). Subjects with serum levels of 25(OH)D lower than 20 ng/mL had a 2.7 times higher risk of endometriosis than people with 25(OH)D serum levels higher than 20 ng/mL (non-deficient) (OR = 2.7, 95 % confidence interval: 1.24-5.80, P = 0.01).
