In order to assess the frequency of peripheral organizing pneumonia (OP) in patients with resecte... more In order to assess the frequency of peripheral organizing pneumonia (OP) in patients with resected lung tumours and to describe its differential features, a cross-sectional study with prospective data collection was realized in a community teaching hospital. Demographic and clinical data were collected from clinical records. The lung specimens removed with a curative purpose in 89 consecutive patients with lung tumours were studied and the clinical and pathological characteristics of patients with and without OP were compared. In 33 of 89 patients (37%) included, OP in the vicinity of neoplasm was found. Areas of other types of fibrosis were evident in 21 patients (24%). Male gender, smoker, epidermoid histological type and the presence of lipid pneumonia were found with a significant higher frequency in patients with OP. Although without significant differences, the presence of symptoms and the bronchial stenosis were found more frequently in patients with OP. In conclusion, OP pattern adjacent to lung cancer, frequently associated to lipid pneumonia, is a common pathological finding. Male gender, a history of tobacco use and epidermoid histological type appear as risk factors for developing this pathologic pattern. Given the lack of distinctive clinico-pathological features, cancer adjacent OP could be confounded with other etiologic forms of this fibrotic process.

We report the case of a woman with membranoproliferative glomerulonephritis type I who presented ... more We report the case of a woman with membranoproliferative glomerulonephritis type I who presented with acute respiratory failure secondary to alveolar haemorrhage. The persistence of the respiratory failure, once the alveolar haemorrhage had ceased, was apparently due to diffuse alveolar damage in organization. The recognition of this proliferative reaction through the performance of transbronchial biopsy allowed the initiation of a correct treatment with which a favourable response was obtained.

The purpose of this study was to review our experience with patients who had a definitive diagnos... more The purpose of this study was to review our experience with patients who had a definitive diagnosis of follicular bronchiolitis (FB), and to describe in detail the clinical and pathological findings, looking for common clinical aspects that may help to identify this entity. Ours is a community 750 bed teaching hospital that acts as a tertiary referral center for several subspecialties, including thoracic surgery. Six patients with a morphological diagnosis of FB, defined by the presence of coalescent germinal centers adjacent to airways, were included. Lung biopsy was obtained by thoracotomy in all patients (2 women and 4 men, mean age 53 years). In one patient FB was associated with advanced AIDS, and in another with prolonged exposure to polyethylene-flock. In 4 patients no condition previously associated with FB was found. Five patients had a history of repeated respiratory infections, 3 patients complained of dyspnea and none had peripheral blood eosinophilia. After a mean follow-up of 25 months, 2 patients responded well to steroid therapy; 3 patients suffered symptomatic exacerbations that required an increase in the steroid dose and 1 patient was not treated with steroids. The most important contribution of this series is the description of a subset of patients with FB who were not associated with other processes. These patients present relatively homogeneous clinical and pathological pictures that do not differ greatly from secondary forms.

To validate a cardiorespiratory polygraphy system (BITMED NGP 140) by comparing it to conventiona... more To validate a cardiorespiratory polygraphy system (BITMED NGP 140) by comparing it to conventional polysomnography in the diagnosis of sleep apnea-hypopnea syndrome. Polysomnography and cardio-respiratory polygraphy were performed simultaneously on 103 consecutive patients referred because of suspected sleep apnea-hypopnea syndrome. The Bland and Altman method and intraclass correlation coefficients were used to assess agreement between the 2 methods of measurement. Receiver operating characteristic (ROC) curves were used to calculate the yield of cardiorespiratory polygraphy compared to that of conventional polysomnography. Ninety-two valid studies were obtained for 72 men and 20 women (mean [SD] age: 52.4 [12] years). By the Bland and Altman method, the difference between the respiratory event index obtained with the BITMED NGP140 and the apnea-hypopnea index (AHI) obtained by conventional polysomnography was 7.6 (13.2) in the manual analysis (95% confidence interval [CI], 4.9-10....

Although it is generally accepted that a malignant transient pleural transudate may appear during... more Although it is generally accepted that a malignant transient pleural transudate may appear during the early stages of lymphatic obstruction, cases demonstrating such probability are rare in literature. A 67-year-old woman was admitted to hospital because a lymphangitic carcinomatosis and a transudative infrapulmonary pleural effusion with a cytology positive for adenocarcinoma. One month later the effusion keeps being positive for adenocarcinoma but exudative in character. Lymphatic obstruction appears as the cause of the initial transudative characteristics of the pleural effusion.

The presence of urine in the pleural space (urinothorax) is a rarely recognized cause of pleural ... more The presence of urine in the pleural space (urinothorax) is a rarely recognized cause of pleural effusion. To date, only 58 cases have been reported. In this article the features of urinothorax are analyzed, and clinical and biochemical characteristics are reviewed in order to propose a classification, founded on pathogenic criteria, that will be useful in achieving the diagnosis. Recently reported cases of urinothorax provide a more detailed description of the biochemical characteristics that allow a better understanding of this entity. Urinothorax can be divided into two categories: (1) obstructive urinothorax, due to bilateral obstructive uropathy; and (2) traumatic urinothorax, due to unilateral traumatic injury of the urinary system, mostly iatrogenic. In patients with urinothorax, the pleural effusion usually has the biochemical characteristics of a transudate, with a pH lower than 7.30 and a pleural fluid/serum creatinine ratio higher than 1. These characteristics are not always present, however, and individually are shared by a significant number of pleural effusions of different etiology.
