Introduction: Dengue virus infection is one of the most important vector-borne human arboviral in... more Introduction: Dengue virus infection is one of the most important vector-borne human arboviral infection and has emerged as a notable public health problem in recent decades. AIMS: To study the trend of seroprevalence of dengue infection and to highlight the usefulness of rapid NS1 antigen test in early diagnosis of dengue. Materials and Method: A retrospective study was done in the Department of Microbiology of a tertiary care centre during the study period of October to December in the year 2017, 2018 and 2019. The serum samples were subjected to immuno-chromatographic Dengue NS1 antigen, IgM / IgG Antibodies rapid kit test and Dengue IgM Mac ELISA test. Results: In October to December 2019, 1138 samples of suspected dengue cases were sent to our lab for detection by rapid kit tests. Out of which 161 (14.1%) samples were NS1 positiv, 38 (3.3%) were only IgM positive and 4 (0.4%) were IgG positive. Inadvertent consumption of toxic varieties of fish mistaken to be edible, results in the developm... more Inadvertent consumption of toxic varieties of fish mistaken to be edible, results in the development of toxic signs and symptoms and fatality, and this phenomenon of toxicity by poisonous fish is referred to as ichthyotoxicosis. Pufferfish is a variety of poisonous fish, which contains a potent toxin called “tetrodotoxin,” which is a neurotoxin. This case report deals with a case of poisoning following inadvertent consumption of pufferfish by a child. The child developed vomiting, altered sensorium and respiratory depression and was declared dead on arrival at the casualty despite emergency management. Histopathological examination revealed congestion and haemorrhages in the viscera, and blood culture showed growth of Aeromonas hydrophila.

Fifty-three strains of Aeromonas (7.7 percent) were isolated along with other enteric pathogens. Among the media used Ampicillin Blood agar proved to be most useful yielding 88.67 percent of Aeromonas isolates. Biochemical speciation identified A. caviae to be predominant biotype (83.8 percent). The pathogenic potential of A. hydrophila as correlated to the presence of cellular exudate.

With hyperamylasemia, and a CT scan suggestive of pancreatic abscess as a result of acute pancreatitis, a CT -guided aspiration was done which confirmed the diagnosis of a pancreatic abscess. Background: Drug resistant tuberculosis has frequently been encountered in India. The prevalence ... more Background: Drug resistant tuberculosis has frequently been encountered in India. The prevalence of extrapulmonary multi drug resistant tuberculosis in this country is found to be 3% in new cases and 12-17% in treated cases, which needs to be evaluated. Settings and Design: A retrospective analysis of processed extrapulmonary tubercular samples in Intermediate Reference Laboratory of Department of Microbiology, Goa Medical College, was carried out over a period of one year, from November 2013 to October 2014. Methods: Lowenstein Jensen Media was used for culture processing of 202 samples from clinically suspected cases of extra-pulmonary tuberculosis patients. Drug susceptibility testing was done on the isolated strains using the economic variant of proportion method. Results: Out of 202 samples, 30 (14.85%) were observed to be culture positive for Mycobacterium tuberculosis of which six (21.43%) were found to be resistant to both Isoniazid and Rifampicin. Conclusion: With the advent of immunocompromised states, one has to be aware of multi drug resistant extra-pulmonary tuberculosis as a double edged sword. The early detection in such cases is a need of the hour to initiate prompt treatment.

Scrub typhus is one of the differential diagnoses of haemorrhagic fevers especially if associated... more Scrub typhus is one of the differential diagnoses of haemorrhagic fevers especially if associated with jaundice and/or renal failure. Goa State in the western region of India has been witnessing increased incidence of such fevers, therefore, the present study was undertaken to identify whether scrub typhus is the aetiological agent. Adult patients presenting with undiagnosed febrile illness between June 2009 to October 2010, were evaluated. Testing was done using a commercial ELISA kit for specific IgM antibodies against Orientia tsutsugamushi. Of the 44 patients included in the study, 15 (34%) were found to be positive for IGM antibodies against O. tsutsugamushi. The cases were seen mainly in the months between July to November. The common symptoms noted were fever, myalgias, gastrointestinal complaints, followed by breathlessness, rash and jaundice. The study was a clinico-mycological approach to find out the various clinical types of dermatophy... more The study was a clinico-mycological approach to find out the various clinical types of dermatophytoses and as well as the species of dermatophytes in Goa. Various socio-demographic and host factors were assessed to determine the predisposing factors for dermatophytoses. A detailed clinical history of the patients was recorded. Samples were collected aseptically in sterile black paper envelopes. The material was subjected to microscopy using potassium hydroxide mount. Culture was done by inoculating the material in Sabouraud's peptone glucose agar slants. Tinea corporis was found to be the most common clinical presentation (44.2 %). Among the dermatophytes, Trichophyton rubrum predominated (38.2 %), followed by Trichophyton mentagrophytes (27.2 %). Dermatophytoses was more common among the lower and upper lower strata of society. Majority of the patients were in the 21-40 years age group (62 %) with males predominating women in ratio 2.4:1.

However, the role of psychological factors during the early part of the continuum of care has so far been under-investigated. The objective of the Umeed study was to evaluate the impact of Common Mental Disorder (CMD), hazardous alcohol use and low cognitive functioning upon attendance for post-test counselling and linkage to care among people attending for HIV-testing in Goa, India. The study was a prospective cohort design. Participants were recruited at the time of attending for testing and were asked to complete a baseline interview covering sociodemographic characteristics and mental health exposures. HIV status, post-test counselling (PTC) and Antiretroviral Treatment (ART) Centre data were extracted from clinical records. Analysis of cross-sectional baseline data (plus HIV status) from a prospective cohort study. Participants were recruited at the time of coming for HIV testing. Consistent with associations found among general population samples, among our sample of 1934 participants, we found that indicators of psychosocial adversity were associated with CMD (common mental disorder - major depression, generalised anxiety and panic disorder) among people coming for testing for HIV. Background: The majority of research on HIV/AIDS and mental health has been carried out among cli... more Background: The majority of research on HIV/AIDS and mental health has been carried out among clinical populations: the time of onset of comorbid depression and the mechanisms for this are therefore unclear. Although there is evidence to suggest that asymptomatic people living with HIV/AIDS exhibit some cognitive deficits, the prevalence of poor cognitive functioning among people in low income settings at an early, pre-clinical stage has not yet been investigated. Methods: We used a cross-sectional survey design to test the hypotheses that symptoms of Common Mental Disorder (CMD) and low scores on cognitive tests would be associated with seropositivity among participants coming for testing for HIV/AIDS. Participants were recruited at the time of coming for testing for HIV/AIDS; voluntary informed consent was sought for participation in research interviews and data linkage with HIV test results. Baseline questionnaires including sociodemographic variables and measures of mental health (PHQ-9, GAD-7, panic disorder questions, AUDIT and delayed word list learning and recall and animal naming test of verbal fluency) were administered by trained interviews. HIV status data was extracted from clinical records. Results: CMD and scoring below the educational norm on the test of verbal fluency were associated with testing positive for HIV/AIDS in bivariate analysis (OR = 2.26, 1.31-3.93; OR = 1.77, 1.26-2.48, respectively). After controlling for the effects of confounders, the association between CMD and seropositivity was no longer statistically significant (AOR = 1.56, 0.86-2.85). After adjusting for the effects of confounders, the association between low scores on the test of verbal fluency and seropositivity was retained (AOR = 1.77, 1.27-2.48). Conclusions: Our findings provide tentative evidence to suggest that low cognitive test scores (and possibly depressive symptoms) may be associated with HIV status among people who have yet to receive their HIV test results. Impaired cognitive functioning and depression-like symptoms may be the result of the same underlying neurological damage. CMD and cognitive impairment may overlap to a greater extent than previously assumed. If replicated, this may have implications for the way in which we measure and treat CMD and cognitive functioning among people living with HIV/AIDS.

One week later, vivax malaria was diagnosed at a health center, but the patient developed respiratory distress and lost consciousness. She arrived at emergency department in shock, breathless, and comatose. She died within minutes. Two independent laboratories later confirmed Plasmodium vivax by microscopy (140,000/μL) and by nested and real-time polymerase chain reaction methods. Post-mortem examination showed congestion of alveolar capillaries by heavy monocytic infiltrates, along with diffuse damage to alveolar membranes consistent with acute respiratory distress syndrome. Parasites seen in lung tissue were roughly proportionate to both peripheral hyperparasitemia and those seen in other organs without lesions. In this patient, vivax malaria caused a rapidly fatal respiratory distress.

While scrub typhus was reported from Goa, other rickettsial infections have not been reported earlier. The present study was planned to identify pattern of rickettsial infections in Goa. All patients presenting with undiagnosed acute febrile illness were recruited over a two-year period. Other causes of febrile illness were ruled out by appropriate tests. Sera of the patients were subjected to Weil Felix testing. Patients were labelled as probable rickettsial infection if the titres to any one antigen was >1:80 as per DHR-ICMR guidelines. Clinical details were obtained retrospectively from case records. Sixty-one patients met inclusion criteria, of which six were excluded because of alternative diagnosis. An otherwise healthy 20-year-old woman in Goa, India, received antibiotics after a diagnosis of u... more An otherwise healthy 20-year-old woman in Goa, India, received antibiotics after a diagnosis of upper respiratory tract infection. One week later, vivax malaria was diagnosed at a health center, but the patient developed respiratory distress and lost consciousness. She arrived at emergency department in shock, breathless, and comatose. She died within minutes. Two independent laboratories later confirmed Plasmodium vivax by microscopy (140,000/μL) and by nested and real-time polymerase chain reaction methods. Post-mortem examination showed congestion of alveolar capillaries by heavy monocytic infiltrates, along with diffuse damage to alveolar membranes consistent with acute respiratory distress syndrome. Parasites seen in lung tissue were roughly proportionate to both peripheral hyperparasitemia and those seen in other organs without lesions. In this patient, vivax malaria caused a rapidly fatal respiratory distress.

While scrub typhus was reported from Goa, other rickettsial infections have not been reported earlier. The present study was planned to identify pattern of rickettsial infections in Goa. All patients presenting with undiagnosed acute febrile illness were recruited over a two-year period. Other causes of febrile illness were ruled out by appropriate tests. Sera of the patients were subjected to Weil Felix testing. Patients were labelled as probable rickettsial infection if the titres to any one antigen was >1:80 as per DHR-ICMR guidelines. Clinical details were obtained retrospectively from case records. Sixty-one patients met inclusion criteria, of which six were excluded because of alternative diagnosis.
