Visceral artery aneurysms (VAAs) and visceral artery pseudoaneurysms (VAPAs) are rare conditions ... more Visceral artery aneurysms (VAAs) and visceral artery pseudoaneurysms (VAPAs) are rare conditions and are potentially lethal when they rupture. They are usually found as incidental findings on computed tomography (CT) scans of asymptomatic patients. Although conventional open surgery is currently considered the gold standard treatment, the endovascular approach has gained prominence as a minimally invasive procedure with lower surgical risk. In this approach, use of coil embolization in saccular VAAs and VAPAs and implantation of flow-modulating stents constitute alternative treatments for fusiform aneurysms. We present the case of a 51-year-old female patient complaining of acute abdominal pain, tachycardia, and hypotension, with evidence of abdominal bleeding on CT angiography, who was diagnosed with a ruptured ileocolic artery (ICA) pseudoaneurysm. She underwent early endovascular treatment for ICA embolization, which was successful and achieved clinical improvement.

It can also be found in patients with superficial venous thrombosis (SVT) that extends to a deep vein. There are still no defined criteria for treatments described in the literature, which range from anticoagulation and fibrinolytic treatments with vena cava filter implants, through open or endovascular thrombectomies, to more invasive procedures such as surgical interruption with ligation of the venous system. Objectives To describe a series of cases of prophylactic uterine catheterization performed to prevent significant postpartum bleeding or during caesarean delivery in pregnant women with a previous diagnosis of accretion. Methods A retrospective analysis was conducted of medical records of cases of uterine artery catheterization performed during elective or emergency caesarean sections of patients at high risk of postpartum bleeding. Porous three-dimensional collagen/chitosan scaffolds combined with poly (ethylene glycol) (PEG) a... more Porous three-dimensional collagen/chitosan scaffolds combined with poly (ethylene glycol) (PEG) and hydroxyapatite were obtained through a freeze-drying method. Physical cross-linking was examined by dehydrothermal treatment. The prepared materials were characterized by different analyses, eg, scanning electron microscopy (SEM), measurements of porosity and swelling, mechanical properties, and resistance to enzymatic degradation. The porosity of scaffolds and their swelling ratio decreased with the addition of hydroxyapatite. Moreover, after exposure to collagenase, the collagen/chitosan matrices containing PEG showed much faster degradation rate than matrices with the addition of hydroxyapatite. The results indicated that the addition of hydroxyapatite led to improvement of stiffness. The highest degree of porosity and swelling were demonstrated by collagen/chitosan/PEG matrices without hydroxyapatite.

and to estimate the effect of the Nursing Activities Score on the response time of the users to t... more and to estimate the effect of the Nursing Activities Score on the response time of the users to the alarms of the multiparameter monitors. Methods: Cross-sectional, observational study with open cohort performed in a 10-bed intensive care unit, where the timing of alarms triggered by the multiparameter monitors that presented clinical alteration was timed. Patients who were receiving cardiopulmonary resuscitation maneuvers were excluded. The ethical premises were respected. The data were analyzed in statistical software R version x 64 3.1.1. Descriptive statistics, logistic regression analysis, and survival analysis were used. Results: 254 alarms were triggered, where 11% were attended and 89% presented alarm fatigue, totaling 168 hours of observation. The average number of alarms triggered was 4.5 alarms / day with average user time of 64 seconds. The Nursing Activities Score average was 59.2. An association was evidenced of the alarms attended with the complexity of the patient (P b .05), with an increase of almost 5% in the chance of attendance at each point increase in the value of Nursing Activities Score. It was also estimated that at each point of the Nursing Activities Score, the alarm has a 4% chance of being attended to every second (P b .05). Conclusion: The Nursing Activities Score is directly related to the response time to alarms triggered by multiparameter monitors.

Resumo A trombose venosa superficial (TVS), também denominada de tromboflebite superficial, carac... more Resumo A trombose venosa superficial (TVS), também denominada de tromboflebite superficial, caracteriza-se pela formação de trombos dentro de veias superficiais, com suboclusão ou oclusão da luz e reação inflamatória no seu trajeto, sendo mais comum nos membros inferiores. Afeta de 3 a 11% da população geral. O diagnóstico costuma ser clinicamente fácil, mas sua extensão e possíveis complicações tromboembólicas precisam de exames subsidiários para confirmação. A TVS pode estar associada a trombose venosa profunda em 6 a 40% dos casos, a embolia pulmonar assintomática (EP) em 20 a 33%, e a EP sintomática em 2 a 13%. Apesar de sua morbidade e das suas complicações, ainda não existem diretrizes brasileiras para a TVS. Nesta diretriz, foram analisadas as principais dúvidas quanto à definição, terminologia e etiologia da TVS, com recomendações para seu diagnóstico e tratamento.

The aim of our study was to assess the use of chest X-rays (CXRs) as a screening method of intrathoracic neoplasms in patients who with idiopathic DVT. In observational, cross-sectional study, the medical records of 99 patients with idiopathic DVT, who were submitted to CXR (during the hospital stay or within 30 days after discharge), were investigated. Of these patients, 15.1% had images suggestive of thoracic neoplasias (single or multiple pulmonary nodules and mediastinal widening). Neoplasms were diagnosed in 4 patients in the follow-up with CXR. The CXR is an option of early screening for thoracic neoplasms and other nonmalignant diseases in patients with idiopathic DVT.

ABSTRACT The use of scaffolds composed of natural biodegradable matrices represents an attractive... more ABSTRACT The use of scaffolds composed of natural biodegradable matrices represents an attractive strategy to circumvent the lack of cell engraftment, a major limitation of stem cell therapy in cardiovascular diseases. Bovine derived non-porous collagen scaffolds with different degrees of cross-linking (C0, C2, C5 and C10) were produced and tested for their mechanical behavior, in vitro biocompatibility with adipose derived stem cells (ADSC) and tissue adhesion and inflammatory reaction. Uniaxial tensile tests revealed an anisotropic behavior of collagen scaffolds (2x0.5cm size) and statistically significant differences in the mechanical behavior between cross-linked and non-cross-linked scaffolds (n=5). In vitro, ADSCs adhered homogenously and showed a similar degree of proliferation on all four types of scaffolds (Cellsx10(3)/cm(2) at day 7: C0:94.7±37.1; C2:91.7±25.6; C5:88.2±6.8; C10:72.8±10.7; p=n.s.; n=3). In order to test the in vivo biocompatibility, a chronic myocardial infarction model was performed in rats and 1.2x1.2cm size collagen scaffolds implanted onto the heart 1month post-infarction. Six animals per group were sacrificed 2, 7 and 30 days after transplant. Complete and long-lasting adhesion to the heart was only observed with the non-cross-linked scaffolds with almost total degradation 1 month post-transplantation. After 7 and 30 days post-implantation, the degree of inflammation was significantly lower in the hearts treated with non-cross-linked scaffolds (Day 7: C0:10.2±2.1%; C2:16.3±2.9%; C5:15.9±4.8%; C10:17.4±4.1%; p<0.05 vs. C0; Day 30: C0:1.3±1.3%; C2:9.4±3.0%; C5:7.0±2.1%; C10:9.8±2.5%; p<0.01 vs. C0). In view of the results, the non-cross-linked scaffold (C0) was chosen as an ADSC-carrier sheet and tested in vivo. One week post-implantation, 25.3±7.0% of the cells transplanted were detected in those animals receiving the cell-carrier sheet whereas no cells were found in animals receiving cells alone (n=3 animals/group). We conclude that the biocompatibility and mechanical properties of the non-cross-linked collagen scaffolds makes them a useful cell carrier that greatly favors tissue cell engraftment and may be exploited for cell transplantation in models of cardiac disease.

The average number of alarms triggered was 4.5 alarms / day with average user time of 64 seconds. The Nursing Activities Score average was 59.2. An association was evidenced of the alarms attended with the complexity of the patient (P b .05), with an increase of almost 5% in the chance of attendance at each point increase in the value of Nursing Activities Score. It was also estimated that at each point of the Nursing Activities Score, the alarm has a 4% chance of being attended to every second (P b .05). Conclusion: The Nursing Activities Score is directly related to the response time to alarms triggered by multiparameter monitors.

Resumo A trombose venosa superficial (TVS), também denominada de tromboflebite superficial, carac... more Resumo A trombose venosa superficial (TVS), também denominada de tromboflebite superficial, caracteriza-se pela formação de trombos dentro de veias superficiais, com suboclusão ou oclusão da luz e reação inflamatória no seu trajeto, sendo mais comum nos membros inferiores. Afeta de 3 a 11% da população geral. O diagnóstico costuma ser clinicamente fácil, mas sua extensão e possíveis complicações tromboembólicas precisam de exames subsidiários para confirmação. A TVS pode estar associada a trombose venosa profunda em 6 a 40% dos casos, a embolia pulmonar assintomática (EP) em 20 a 33%, e a EP sintomática em 2 a 13%. Apesar de sua morbidade e das suas complicações, ainda não existem diretrizes brasileiras para a TVS. Nesta diretriz, foram analisadas as principais dúvidas quanto à definição, terminologia e etiologia da TVS, com recomendações para seu diagnóstico e tratamento.

The aim of our study was to assess the use of chest X-rays (CXRs) as a screening method of intrathoracic neoplasms in patients who with idiopathic DVT. In observational, cross-sectional study, the medical records of 99 patients with idiopathic DVT, who were submitted to CXR (during the hospital stay or within 30 days after discharge), were investigated. Of these patients, 15.1% had images suggestive of thoracic neoplasias (single or multiple pulmonary nodules and mediastinal widening). Neoplasms were diagnosed in 4 patients in the follow-up with CXR. The CXR is an option of early screening for thoracic neoplasms and other nonmalignant diseases in patients with idiopathic DVT.

ABSTRACT The use of scaffolds composed of natural biodegradable matrices represents an attractive... more ABSTRACT The use of scaffolds composed of natural biodegradable matrices represents an attractive strategy to circumvent the lack of cell engraftment, a major limitation of stem cell therapy in cardiovascular diseases. Bovine derived non-porous collagen scaffolds with different degrees of cross-linking (C0, C2, C5 and C10) were produced and tested for their mechanical behavior, in vitro biocompatibility with adipose derived stem cells (ADSC) and tissue adhesion and inflammatory reaction. Uniaxial tensile tests revealed an anisotropic behavior of collagen scaffolds (2x0.5cm size) and statistically significant differences in the mechanical behavior between cross-linked and non-cross-linked scaffolds (n=5). In vitro, ADSCs adhered homogenously and showed a similar degree of proliferation on all four types of scaffolds (Cellsx10(3)/cm(2) at day 7: C0:94.7±37.1; C2:91.7±25.6; C5:88.2±6.8; C10:72.8±10.7; p=n.s.; n=3). In order to test the in vivo biocompatibility, a chronic myocardial infarction model was performed in rats and 1.2x1.2cm size collagen scaffolds implanted onto the heart 1month post-infarction. Six animals per group were sacrificed 2, 7 and 30 days after transplant. Complete and long-lasting adhesion to the heart was only observed with the non-cross-linked scaffolds with almost total degradation 1 month post-transplantation. After 7 and 30 days post-implantation, the degree of inflammation was significantly lower in the hearts treated with non-cross-linked scaffolds (Day 7: C0:10.2±2.1%; C2:16.3±2.9%; C5:15.9±4.8%; C10:17.4±4.1%; p<0.05 vs. C0; Day 30: C0:1.3±1.3%; C2:9.4±3.0%; C5:7.0±2.1%; C10:9.8±2.5%; p<0.01 vs. C0). In view of the results, the non-cross-linked scaffold (C0) was chosen as an ADSC-carrier sheet and tested in vivo. One week post-implantation, 25.3±7.0% of the cells transplanted were detected in those animals receiving the cell-carrier sheet whereas no cells were found in animals receiving cells alone (n=3 animals/group). We conclude that the biocompatibility and mechanical properties of the non-cross-linked collagen scaffolds makes them a useful cell carrier that greatly favors tissue cell engraftment and may be exploited for cell transplantation in models of cardiac disease.
