Was to examine prospectively the longitudinal course of pregnancy in cases of ARED-Flow detected ... more Was to examine prospectively the longitudinal course of pregnancy in cases of ARED-Flow detected between 24/0 and 34/0 gw. and to compare outcome with a control group of gestational age-matched control group. Methods: During the 6-years study period 1995-2000, 60 fetuses of singleton pregnancies with ARED-Flow in the umbilical artery were included in the study. Fetuses with malformations and aneuploidy were excluded. Surveillance was performed by repeated Dopplermeasurements of arterial and venous vessels, CTG and maternal parameters. Delivery (c.s.) was dated when either reversed Flow in the DV, decelerations in CTG or maternal pre-eclampsia occurred. Results: Mortality 16 (27%) intrauterine death (birthweight in 9 cases < 500 g), 44 (73%) liveborns, 7 (12%) neonatal death. Indication for delivery: 72% deterioration of fetal parameters, 28% pre-eclampsia. Following our protocol, 50% of fetuses with ARED Flow will be delivered after 6 days. Control group: 44 preterm neonates born in the same period in same centre. Significant differences between ARED fetuses and control: lower pH values (p = 0.001), lower birth weight (p = 0.0001), more broncho-pulmonal dysplasia (p = 0.002), more intestinal complications (p = 0.01) in the ARED group. Other tested parameters (intraventricular hemorrhage, neurological development et al.) were not significantly different. Conclusions: Diagnosis of ARED-flow indicates a group of severely hypotrophic, acidemic fetuses with a high mortality rate. Although the incidence of intraventricular haemorrhage (20%) and neurological defects (40%) was high, difference to preterm neonates was not significant. It can be speculated, that prolongation of pregnancy using venous Doppler improves the outcome.

The modified Misgav-Ladach method (MML) is a minimally invasive cesarean section procedure compar... more The modified Misgav-Ladach method (MML) is a minimally invasive cesarean section procedure compared with the classic Pfannenstiel-Kerr (PK) method. The aim of the study was to compare the MML method and the PK method in terms of intraoperative and short-term postoperative outcomes. This prospective, randomized controlled trial involved 252 pregnant women scheduled for primary emergency or elective cesarean section between October, 2014 and July, 2015. The primary outcome measures were the duration of surgery, extraction time, Apgar score, blood loss, wound complications, and number of sutures used. Secondary outcome measures were the wound infection, time of bowel restitution, visual analogue scale (VAS) scores at 6 h and 24 h after the operation, limitations in movement, and analgesic requirements. The objective of this study was to determine the paternal depression rates and affecting factors ... more The objective of this study was to determine the paternal depression rates and affecting factors in prenatal and postpartum periods. The study, which was descriptive and cross-sectional, was conducted with 92 volunteer couples that suited the criteria for the sample. These participants were selected from couples that applied to the gynecology clinic of a private hospital between September 2014 and April 2015 and who were in their 37th-40th weeks of pregnancy. Data collection was planned in two interviews; the first one was done between the 37th and 40th weeks of the pregnancy, and the second was conducted between the 4th and 6th weeks after the birth. Interviews were completed with 84 couples. In this study, it was determined that there was no relationship between average paternal and maternal scores on the Edinburgh Postnatal Depression Scale (EPDS) in prenatal and postpartum periods. It was found that the average score of the Family-Work Conflict Scale increased and couples&#39; marriage harmony decreased as the EPDS average score for fathers increased. In addition, it was found that the EPDS average score for fathers that wanted the pregnancy was less compared to fathers that did not want the pregnancy, and the difference between the two groups was statistically meaningful.

In this study, we aimed to examine the pathogenic mechanisms of endometrial polyp development using immunohistochemistry. We evaluated the expression of galectin-3 and cyclooxgenase-2 (COX-2) during the menstrual cycle in premenopausal women with endometrial polyps or normal endometrium. Methods: Thirty-one patients with endometrial polyps and 50 healthy control patients were included in this study. The levels of expression of COX-2 and galectin-3 were studied by immunohistochemistry. Results: The percentage of COX-2-positive cells and the intensity of COX-2 staining in the endometrium did not vary during the menstrual cycle either in the control group or in patients with endometrial polyps. However, expression of galectin-3 was significantly lower in endometrial polyps and during the proliferative phase of the endometrium compared with the secretory phase. Conclusions: Our data suggests that the pathogenesis of endometrial polyps does not involve expression of COX-2 or galectin-3.

Objectives: The aim of our study was to evaluate the association of vitamin D deficiency (VDD) du... more Objectives: The aim of our study was to evaluate the association of vitamin D deficiency (VDD) during pregnancy with thymus size in full-term fetuses. Material and methods: In this prospective study, we evaluated mid-pregnancy serum 25-hydroxyvitamin D 3 (25(OH)D 3) concentrations. The fetal thymus size was measured by ultrasound in the third trimester. Neonatal 25(OH)D 3 levels were evaluated by umbilical cord blood sampling. Correlation of maternal and neonatal vitamin D levels and association between thymus size and both, maternal and neonatal vitamin D concentrations were investigated. Results: Serum 25(OH) D 3 concentrations were within the normal range in 48 (29.8%) mothers and 10 (13.1%) newborns. A strong correlation between mid-pregnancy maternal and neonatal 25(OH)D 3 concentration (r = 0.8, p < 0.001) was found. A significant linear correlation was observed between both, maternal and neonatal 25(OH)D 3 concentrations and thymus perimeter length (r = 0.45, p = 0.04 and r = 0.43, p < 0.01, respectively). Both, maternal and fetal VDDs were associated with decreased thymus perimeter (p = 0.04, p = 0.03). Conclusions: Vitamin D deficiency during pregnancy may be associated with smaller fetal thymus. Our data suggest that VDD in pregnancy may lead to systemic inflammatory response in the fetus.

To find out 'what is the leading cause of postpartum headache in patients undergoing caesarean se... more To find out 'what is the leading cause of postpartum headache in patients undergoing caesarean section with spinal anesthesia under elective conditions?' Methods: Our study was conducted with retrospective, controlled assessment of 304 patients who underwent caesarean section with spinal anesthesia under elective conditions at our institution between 1 June 2012 and 1 November 2012. The patients were assessed in terms of postpartum headache. They were divided into 2 groups: the group with headache versus the group without headache (the latter was the control group). Both groups were compared with respect to age, body mass index (BMI), number of previous pregnancies, indications for caesarean section, the spinal needle used during spinal anesthesia, preoperative and postoperative amount of fluid administration, and mobilization time. Results: None of the factors that are effective in development of headache, i.e. age, multiparity, the indication for caesarean section, BMI, and needle type, was statistically significant in logistic regression analysis. Only the needle type was significantly related to headache in Chi-Square test. All headache episodes were mild and improved with conservative therapy. We did not find any difference between groups with respect to age, BMI, number of previous pregnancies, indications for caesarean section, preoperative and postoperative amount of fluid administration, and mobilization time. Conclusion: In patients undergoing cesarean section with spinal anesthesia under elective conditions the main cause of headache is the type of the spinal needle used.

The aim of this study was to determine the relationship of the salivary levels of dehydroepiandro... more The aim of this study was to determine the relationship of the salivary levels of dehydroepiandrosterone sulfate (DHEA-S) and cortisol with factors related to depression and anxiety in patients with hyperemesis gravidarum (HG). Forty patients with a diagnosis of HG were selected for the study and matched with 40 control patients according to body mass index, parity, and age. Symptoms of depression and anxiety were investigated using the Beck Depression Inventory and Beck Anxiety Inventory for Adults, respectively. Saliva samples were collected in the morning and at night and subjected to enzyme linked immunosorbent assay for the determination of DHEA-S and cortisol levels. We observed a positive correlation between increased levels of depression and anxiety and increased salivary levels of cortisol and DHEA-S in patients with HG. Salivary cortisol and DHEA-S levels, as well as mood disorders, should be monitored in patients with HG, although further large, prospective studies are needed to confirm our results.

The aim of this study was to determine the relationship of the salivary levels of dehydroepiandro... more The aim of this study was to determine the relationship of the salivary levels of dehydroepiandrosterone sulfate (DHEA-S) and cortisol with factors related to depression and anxiety in patients with hyperemesis gravidarum (HG). Forty patients with a diagnosis of HG were selected for the study and matched with 40 control patients according to body mass index, parity, and age. Symptoms of depression and anxiety were investigated using the Beck Depression Inventory and Beck Anxiety Inventory for Adults, respectively. Saliva samples were collected in the morning and at night and subjected to enzyme linked immunosorbent assay for the determination of DHEA-S and cortisol levels. We observed a positive correlation between increased levels of depression and anxiety and increased salivary levels of cortisol and DHEA-S in patients with HG. Salivary cortisol and DHEA-S levels, as well as mood disorders, should be monitored in patients with HG, although further large, prospective studies are needed to confirm our results.
