Methods: In this study, we have retrospectively evaluated the outcome of pregnancies of which cesarean section was performed because of fetal stress, between 2003 and 2008 in Zekai Tahir Burak Women Health and Education Hospital. High-risk pregnancies were excluded in the study. Maternal ages, gestational weeks, number of pregnancies, fetal birth weight, gender, and Apgar scores of all cases were analyzed. All data were evaluated by the Logistic Regression Analyses. The minimum limit for significance was accepted as 0.05. Results: A total of 1747 pregnancies were evaluated. One thousand and twenty-six (58.7%) of them had male fetuses and 721 (41.3%) of them had female fetuses. To find the correlation between vascular and inflammatory reaction, the expressions of vascular endothelial growth factor (VEGF), inducible nitric oxide synthase (iNOS), CD34, and CD68 were investigated. Materials and methods: 30 cases of OLP and 20 normal oral tissues samples were subjected to immunohistochemistry for VEGF, iNOS, CD34 and CD68. CD34-positive (+) blood vessels (microvessel density, MVD), degree of lymphocytic infiltration, and CD68+ macrophages were correlated with the expression of VEGF and iNOS. OLP cases were histopathologically subdivided into active (non erosive and erosive/ulcerative) and non-active stages. Results: Expression of VEGF was observed in all epithelial layers of OLP while iNOS was in basal and parabasal layers. They were significantly higher (P < 0.001) than those in controls. The mean MVD (CD34+) was significantly increased in all cases of OLP (P < 0.05), especially in the active (erosive/ulcerative) stage (P < 0.01) compared with controls. The mean number of CD68+ cells was significantly increased (P < 0.01) in OLP, especially higher in the active (erosive/ulcerative) OLP (P < 0.001). The expressions of VEGF (P < 0.05) and iNOS (P < 0.01) were significantly correlated in OLP. There was significant positive correlations between the lymphocytic infiltration and the expressions of VEGF and iNOS (P < 0.05), MVD (P < 0.01), and CD68 counts (P < 0.001) in OLP. Conclusion: The present results indicated that the upregulation of VEGF and iNOS plays an important role in pathogenesis of OLP through activated angiogenesis and chronic inflammatory cell infiltrates. Novel strategies directed to inhibition of these markers could be developed for OLP treatment.

The aim of this study was to evaluate the success rate of chemically modified and conventional sa... more The aim of this study was to evaluate the success rate of chemically modified and conventional sandblasted acid-etched surface (SLA) titanium implants in irradiated oral squamous cell carcinoma patients. Material and methods: Twenty patients with a mean age of 61.1 years were treated with dental implants after ablative surgery and radio-chemotherapy of oral cancer. All patients were non-smokers. The placement of SLA and modSLA implants was performed bilaterally according to a split-mouth design. All 102 implants (50 SLA, 52 modSLA) placed showed an unloaded healing time of 6 weeks in the mandible and 10 weeks in the maxilla. Mean crestal bone changes using standardized orthopantomographies and clinical parameters like pocket depths, mPII and mBI were evaluated. Results: Of 102 implants, 55 implants (27 SLA implants, 28 modSLA) were located in the maxilla and 47 implants (23 SLA, 24 modSLA) in the mandible. The average observation period was 14.4 months. The amount of bone loss at the implant shoulder of SLA implants was 0.4 mm mesial and 0.4 mm distal. The modSLA implants displayed a bone loss of mesial 0.3 mm and distal 0.3 mm. Two SLA implants were lost resulting in a success rate of 96%. The success rate of modSLA implants was 100%. Conclusion: Regarding the data found in this investigation, we can conclude that implants with chemically modified and conventional SLA titanium surface show high success rates in irradiated patients. SLA implants with or without a chemically modified surface regardless of the location can be restored with a high predictability of success at least in the short time range observed.

A urine sample was also collected from the infant, the cotinine concentration measured, and the measurement correlated with data provided by the mother. Eight percent of the infant urine cotinine values fell at or above 10 micrograms/L in the 305 households where no smoking was reported. Corresponding rates were 44 percent in the 96 households where a member other than the mother smoked, 91 percent in the 43 households where only the mother smoked, and 96 percent in the 74 households where both the mother and another household member smoked. In households where the mother smoked, infant urine cotinine levels were lower in the summer, and higher when the infant was breast-fed. We studied the predictivity of endometrial parameters in endometrial receptivity in over-aged, normogonadotrophic unexplained infertile women. Materials and methods: In this study, 150 women with an age range of 38-42 were included. The patients were divided into 4 groups: group 1 was applied recombinant FSH and intrauterine insemination, group 2 was applied clomiphene citrate and intrauterine insemination, group 3 was applied IVF, and group 4 (control group) was composed of fertile control patients. All patients in each study group had standard initiation dosages for ovulation induction. Endometrial thickness, volume, and colour Doppler results were evaluated according to their mean values. Results: The groups were homogeneous according to their age, body-mass-index, basal hormone levels, and maximum follicle diameter on the day of ovulation trigger injection. Endometrial thickness and pattern were significantly different between groups. The difference of endometrial volume was not statistically significant between groups. There were also no differences of uterine artery PI and RI between groups. The difference of spiral artery PI values between groups was statistically significant. When all groups are evaluated, effects of endometrial parameters on pregnancy were not different. Conclusion: Endometrial thickness, pattern, and volume, and RI and PI values for uterine and spiral arteries were found to have no predictive value on endometrial receptivity in over-aged, unexplained infertile women.

Our aim was to study the preoperative diagnostic value of CA 125 in the differential diagnosis of leiomyoma and uterine sarcoma. The subjects of the study included a total of 2,382 patients aged between 20-71 years operated for uterine myoma between the years 2005 and 2008 at our hospital, and in the same period 26 patients diagnosed with uterine sarcoma who were assessed retrospectively. Assessment of the predictivity of CA 125 values in the preoperative diagnosis of uterine sarcoma showed it was not significant according to the 95% CI related to the area below the curve. The assessment of CA 125 values in the uterine sarcoma group showed that those with carcinosarcoma had higher CA 125 mean values than other sarcoma groups. The objectives of this study are to compare the risk of psychological depression after medical and surgical abortions in first two trimesters and to evaluate the risk factors for post-abortion depression. Method. A retrospective study was conducted throughout 367 women who underwent surgical abortion and 458 women who underwent medical abortion between January 2006 and January 2007 in Dr. Zekai Tahir Burak Women's Health Hospital. Women were assessed by clinical psychologists one week after the intervention. The clinical characteristics and psychological assessment of these women were statistically correlated by means of non-parametric tests. Results. Of the study population, 27.1% was diagnosed with post-abortion depression. The frequency of post-abortion depression was 34.3% in surgical abortion patients and 22.8% in medical abortion patients. The women who underwent surgical abortion were found to have significantly elevated risk of post-abortion depression. The women with a high risk of post-abortion depression were significantly younger and had a more frequent history of psychiatric and depressive disorders. Conclusion. An important quotient of women experiences post-abortion mood depression which is significantly more frequent after surgical abortion. Women with past psychiatric and anxiety disorders should be carefully monitored for depression when they would undergo an abortion.

Endometriotic implants were induced surgically in female Wistar albino rats, which were randomly ... more Endometriotic implants were induced surgically in female Wistar albino rats, which were randomly divided into three groups. The rats in group I (n = 10) and group II (n = 9) were given 2.5 mg/kg/day intraperitoneal and oral atorvastatin, respectively, for 28 days. Group III (n = 9) was given no medication (control). The mean volume and weight of explants in group I were significantly lower (both P < 0.05) compared with group III. Histopathological score of the implants was significantly lower in groups I and II, when compared with group III (P < 0.01 and P < 0.05, respectively). There were significant reductions in explant concentrations of vascular endothelial growth factor and matrix metalloproteinase 9 in group I (P < 0.01 and P < 0.001, respectively) and group II (both P < 0.01) compared with group III while staining due to tissue inhibitor of metalloproteinase 2 was significantly higher in group I (P < 0.01) and group II (P < 0.01) compared with group III. Moreover, explant concentration of superoxide dismutase was significantly increased in groups I and II compared with group III (both P < 0.05). In conclusion, atorvastatin causes significant regression of endometriotic implants in rats. Moreover, intraperitoneal atorvastatin seems to be more effective than oral atorvastatin. RBMOnline

Results: Patients who had received Angeliq showed significant reduction in systolic blood pressure after 3 months (8.6mmHg) and 6 months (17.2mmHg) of treatment (p=0.04 and p=0.00 respectively). Even though no significant reduction in diastolic blood pressure occurred after 3 months in the Angeliq group, significant reduction was seen after 6 months (9.5mmHg, p=0.01). There was no significant difference in the blood pressure measurements in the Femoston-Conti group. However, there was significant reduction of body weight (2.3kg, p=0.01) and BMI (1.0. p=0.01) after 6 months of treatment. Conclusions: Angeliq has significant effect in reducing blood pressure in postmenopausal Malaysian women.

In order to determine the underlying physiological mechanisms and risk factors, the obesity-fecundity association is investigated in relation to parity, menstrual cycle regularity, smoking habits, and age. Methods: This was a retrospective cohort study of 22,840 women who gave birth between January 2006 and January 2007 in the Dr Zekai Tahir Burak Women's Health Research and Education Hospital. Age, parity, prepregnancy body mass index (BMI) values, time to pregnancy data related to smoking, and reproductive, medical, and gynecological history were obtained from the medical records. Results: Fecundity was reduced for overweight and obese women compared with optimal weight women, and this reduction was more evident for obese primiparous women. Fecundity remained reduced for overweight and obese women with normal menstrual cycles. Obese and overweight women were found to smoke significantly more than the optimal weight group. Conclusions: Obesity was found to be associated with reduced fecundity for all weight-adjusted groups of women and persisted for women with regular cycles. Weight loss should be encouraged initially during the treatment of infertile overweight and obese women.

One of these cytokines, IL-18, has a well-known association with obesity. Our aim in this study was to test the effect of serum and intrafollicular IL-18 and IL-18 binding protein (BP) levels on in vitro fertilization outcomes in obese, overweight, and non-obese patients. Serum and pooled follicular fluid levels of IL-18, IL-18BP, and the IL-18/IL-18BP ratios were evaluated in 21 non-obese (body mass index [BMI] &amp;amp;lt;25), 37 overweight (25&amp;amp;lt;or=BMI&amp;amp;lt;30), and 24 obese (BMI&amp;amp;gt;or=30) patients undergoing in vitro fertilization. IL-18 and IL-18BP levels in serum and follicular fluid did not change with obesity. There was no correlation among serum intrafollicular IL-18, IL-18BP levels, and the number of oocytes retrieved and transferred, embryo grades or pregnancy ratios. The levels of serum and intrafollicular IL-18, IL-18BP, and the IL-18/IL-18BP ratios do not seem to be related to in vitro fertilization outcomes in obese, overweight, and non-obese patients.

Material and methods: Twenty patients with a mean age of 61.1 years were treated with dental implants after ablative surgery and radio-chemotherapy of oral cancer. All patients were non-smokers. The placement of SLA and modSLA implants was performed bilaterally according to a split-mouth design. All 102 implants (50 SLA, 52 modSLA) placed showed an unloaded healing time of 6 weeks in the mandible and 10 weeks in the maxilla. Mean crestal bone changes using standardized orthopantomographies and clinical parameters like pocket depths, mPII and mBI were evaluated. Results: Of 102 implants, 55 implants (27 SLA implants, 28 modSLA) were located in the maxilla and 47 implants (23 SLA, 24 modSLA) in the mandible. The average observation period was 14.4 months. The amount of bone loss at the implant shoulder of SLA implants was 0.4 mm mesial and 0.4 mm distal. The modSLA implants displayed a bone loss of mesial 0.3 mm and distal 0.3 mm. Two SLA implants were lost resulting in a success rate of 96%. The success rate of modSLA implants was 100%. Conclusion: Regarding the data found in this investigation, we can conclude that implants with chemically modified and conventional SLA titanium surface show high success rates in irradiated patients. SLA implants with or without a chemically modified surface regardless of the location can be restored with a high predictability of success at least in the short time range observed.

A urine sample was also collected from the infant, the cotinine concentration measured, and the measurement correlated with data provided by the mother. Eight percent of the infant urine cotinine values fell at or above 10 micrograms/L in the 305 households where no smoking was reported. Corresponding rates were 44 percent in the 96 households where a member other than the mother smoked, 91 percent in the 43 households where only the mother smoked, and 96 percent in the 74 households where both the mother and another household member smoked. In households where the mother smoked, infant urine cotinine levels were lower in the summer, and higher when the infant was breast-fed. We studied the predictivity of endometrial parameters in endometrial receptivity in over-aged, normogonadotrophic unexplained infertile women. Materials and methods: In this study, 150 women with an age range of 38-42 were included. The patients were divided into 4 groups: group 1 was applied recombinant FSH and intrauterine insemination, group 2 was applied clomiphene citrate and intrauterine insemination, group 3 was applied IVF, and group 4 (control group) was composed of fertile control patients. All patients in each study group had standard initiation dosages for ovulation induction. Endometrial thickness, volume, and colour Doppler results were evaluated according to their mean values. Results: The groups were homogeneous according to their age, body-mass-index, basal hormone levels, and maximum follicle diameter on the day of ovulation trigger injection. Endometrial thickness and pattern were significantly different between groups. The difference of endometrial volume was not statistically significant between groups. There were also no differences of uterine artery PI and RI between groups. The difference of spiral artery PI values between groups was statistically significant. When all groups are evaluated, effects of endometrial parameters on pregnancy were not different. Conclusion: Endometrial thickness, pattern, and volume, and RI and PI values for uterine and spiral arteries were found to have no predictive value on endometrial receptivity in over-aged, unexplained infertile women.

The subjects of the study included a total of 2,382 patients aged between 20-71 years operated for uterine myoma between the years 2005 and 2008 at our hospital, and in the same period 26 patients diagnosed with uterine sarcoma who were assessed retrospectively. Assessment of the predictivity of CA 125 values in the preoperative diagnosis of uterine sarcoma showed it was not significant according to the 95% CI related to the area below the curve. The objectives of this study are to compare the risk of psychological depression after medical and surgical abortions in first two trimesters and to evaluate the risk factors for post-abortion depression. Method. A retrospective study was conducted throughout 367 women who underwent surgical abortion and 458 women who underwent medical abortion between January 2006 and January 2007 in Dr. Zekai Tahir Burak Women's Health Hospital. Women were assessed by clinical psychologists one week after the intervention. The clinical characteristics and psychological assessment of these women were statistically correlated by means of non-parametric tests. Results. Of the study population, 27.1% was diagnosed with post-abortion depression. The frequency of post-abortion depression was 34.3% in surgical abortion patients and 22.8% in medical abortion patients. The women who underwent surgical abortion were found to have significantly elevated risk of post-abortion depression. The women with a high risk of post-abortion depression were significantly younger and had a more frequent history of psychiatric and depressive disorders. Conclusion. An important quotient of women experiences post-abortion mood depression which is significantly more frequent after surgical abortion. Women with past psychiatric and anxiety disorders should be carefully monitored for depression when they would undergo an abortion.

The rats in group I (n = 10) and group II (n = 9) were given 2.5 mg/kg/day intraperitoneal and oral atorvastatin, respectively, for 28 days. Group III (n = 9) was given no medication (control). The mean volume and weight of explants in group I were significantly lower (both P < 0.05) compared with group III. Histopathological score of the implants was significantly lower in groups I and II, when compared with group III (P < 0.01 and P < 0.05, respectively). There were significant reductions in explant concentrations of vascular endothelial growth factor and matrix metalloproteinase 9 in group I (P < 0.01 and P < 0.001, respectively) and group II (both P < 0.01) compared with group III while staining due to tissue inhibitor of metalloproteinase 2 was significantly higher in group I (P < 0.01) and group II (P < 0.01) compared with group III. Moreover, explant concentration of superoxide dismutase was significantly increased in groups I and II compared with group III (both P < 0.05). In conclusion, atorvastatin causes significant regression of endometriotic implants in rats. Moreover, intraperitoneal atorvastatin seems to be more effective than oral atorvastatin. RBMOnline

Patients were also asked to fill up the Women's Health Questionaire. They were seen every 3 months to evaluate the response of therapy and side effects. Results: Patients who had received Angeliq showed significant reduction in systolic blood pressure after 3 months (8.6mmHg) and 6 months (17.2mmHg) of treatment (p=0.04 and p=0.00 respectively). Even though no significant reduction in diastolic blood pressure occurred after 3 months in the Angeliq group, significant reduction was seen after 6 months (9.5mmHg, p=0.01). There was no significant difference in the blood pressure measurements in the Femoston-Conti group. However, there was significant reduction of body weight (2.3kg, p=0.01) and BMI (1.0. p=0.01) after 6 months of treatment. Conclusions: Angeliq has significant effect in reducing blood pressure in postmenopausal Malaysian women.

In order to determine the underlying physiological mechanisms and risk factors, the obesity-fecundity association is investigated in relation to parity, menstrual cycle regularity, smoking habits, and age. Methods: This was a retrospective cohort study of 22,840 women who gave birth between January 2006 and January 2007 in the Dr Zekai Tahir Burak Women's Health Research and Education Hospital. Age, parity, prepregnancy body mass index (BMI) values, time to pregnancy data related to smoking, and reproductive, medical, and gynecological history were obtained from the medical records. Results: Fecundity was reduced for overweight and obese women compared with optimal weight women, and this reduction was more evident for obese primiparous women. Fecundity remained reduced for overweight and obese women with normal menstrual cycles. Obese and overweight women were found to smoke significantly more than the optimal weight group. Conclusions: Obesity was found to be associated with reduced fecundity for all weight-adjusted groups of women and persisted for women with regular cycles. Weight loss should be encouraged initially during the treatment of infertile overweight and obese women.

One of these cytokines, IL-18, has a well-known association with obesity. Our aim in this study was to test the effect of serum and intrafollicular IL-18 and IL-18 binding protein (BP) levels on in vitro fertilization outcomes in obese, overweight, and non-obese patients. Serum and pooled follicular fluid levels of IL-18, IL-18BP, and the IL-18/IL-18BP ratios were evaluated in 21 non-obese (body mass index [BMI] &amp;amp;lt;25), 37 overweight (25&amp;amp;lt;or=BMI&amp;amp;lt;30), and 24 obese (BMI&amp;amp;gt;or=30) patients undergoing in vitro fertilization. IL-18 and IL-18BP levels in serum and follicular fluid did not change with obesity. There was no correlation among serum intrafollicular IL-18, IL-18BP levels, and the number of oocytes retrieved and transferred, embryo grades or pregnancy ratios. The levels of serum and intrafollicular IL-18, IL-18BP, and the IL-18/IL-18BP ratios do not seem to be related to in vitro fertilization outcomes in obese, overweight, and non-obese patients.
