As COVID-19 continues to spread around the world, people in all countries are being encouraged to... more As COVID-19 continues to spread around the world, people in all countries are being encouraged to take preventive measuress to prevent transmission, including in many countries by staying at home and physical distancing. But a pandemic does not erase other diseases and their impact. While many services, including some health services, are being scaled back, the risk of further outbreaks of infectious diseases grows. The healthcare disruptions caused by COVID-19 could have a devastating impact on child mortality.
Birth asphyxia is a common cause of neonatal mortality. Identification of severity of asphyxia is... more Birth asphyxia is a common cause of neonatal mortality. Identification of severity of asphyxia is vital for prompt management. Biomarkers can be used for timely diagnosis of asphyxia and also helps in prognosis. Serum, CSF, urine, cord blood & magnetic resonance biomarkers for asphyxia in neonates have been studied. Cytokines like IL-6 in CSF, NSE in CSF, Protein S-100b in serum, urine & cord blood, LDH in serum and saliva, CK in serum and urine, cord BDNF, Urinary UA/Cr ratio, GFAP, Glutamate in CSF, PGE2, AST, ALT, Activin A have been studied with varied diagnostic accuracy and feasibility. Research directed towards newer biomarkers like NPBI, Hypoxanthine, total hydroperoxides, AOPP, UCHL-1 & pNFH-1 for early identification of severe asphyxia have shown promising results. Cardiac biomarkers like Troponin T & I, BNP, CK-MB may aid in longterm outcome. NRBC count still remains as the oldest and best-described biomarker of asphyxia. Use of Proton & Nuclear Magnetic Resonance Spectroscopy on day 1 apart from conventional MRI have opened a new era of MR Biomarkers in neonatal asphyxia. Early identification of severity of asphyxia with judicious use of biomarkers can make a huge difference in the management and outcome of birth asphyxia.

ECMO is a technique of providing cardiac & respiratory support to sustain life. Nowadays it is be... more ECMO is a technique of providing cardiac & respiratory support to sustain life. Nowadays it is being increasingly used in newborns. Respiratory ECMO is used for severe respiratory distress syndrome, pneumonia & meconium aspiration syndrome. Cardiac ECMO is used after open heart surgery, myocarditis or myopathy. There are two types of ECMO, Veno-Venous (V-V) bypass & Arterio-Venous (A-V) bypass. ECMOcan be used to bridge patients with heart failure as they await heart transplantation & as an adjunct to cardiopulmonary resuscitation. ECMO should be used in reversible diseases only after analysing various factors. Qualifying criteria for ECMO are applied only when the infant has reached maximal ventilatory support. Only babies with gestational Age > 34 weeks, Birth Weight > 2,000 grams, without major coagulopathy, with mechanical ventilation < 14 days & reversible lung disease are eligible for ECMO. Absolute contraindications include Grade 3 or 4 IVH, irreversible brain injury, lethal malformations, non-treatable congenital heart disease & significant coagulopathy. The ECMO circuit consists of a cannula to drain deoxygenated blood from the patient, a pump, an artificial lung to provide oxygenation & ventilation, heat exchanger, a second cannula to return oxygenated blood back to the patient Management during ECMO includes Oxygenation, Inotropic support, Rest Ventilation & Sedation. Complications include Bleeding, Infection, myocardial stun, neurodevelopmental problems, Pneumothorax, pulmonary haemorrhage, bronchial asthma, Sensorineural disabilities,acute tubular necrosis, Feeding difficulty, metabolic derangements, Psychosocial morbidity & Neuromotor deficits .Early initiation of ECMO, monitoring & prompt management of expected complications will improve survivalwithout severe disability.

Occupational stress is increasingly faced by doctors. Upto some extent, stress improves performan... more Occupational stress is increasingly faced by doctors. Upto some extent, stress improves performance but when excessive, it becomes harmful. Doctors have more stress related mental health problems than general population. Burnout is inability to cope with emotional stress at work. It consists of 3 domains; emotional exhaustion, depersonalization & lack of personal accomplishment. Various models like demand-control imbalance, effort-reward imbalance & support model explain evolution of work related stress. Common causes of stress are related to patient care, decision making, taking responsibility, increasing use of technology, continuing rises in health care costs, turbulence within work environment, long working hours & overworking. Stress causes physical symptoms, mental derangements, emotional & behavioural disturbances. Research suggests that high stress levels may impair immune system with increased risk of cardiovascular disease & cancer. Stress can be modified by appropriate stress management skills like avoiding cynicism, reading, joining a support group, attending a wellness conference, strengthening communication skills, addressing to spiritual needs, meditation & decreasing caffeine. Burnout prevention should be done both on both personal & organizational levels. Appropriate interventions like improving inter-professional relationships, changes in work environment, modifying off duty time & streamlining decision making should be done to mitigate the effect of stress on doctors.

Background: Anganwadi centers are the focal units for the implementation of the Integrated Child ... more Background: Anganwadi centers are the focal units for the implementation of the Integrated Child Development Services projects in each block. Anganwadi centres cater to the children up to the age of 6 years, pregnant and lactating mothers, adolescent girls and women in the reproductive age group. There are 32 anganwadi centres in the Mullerkad-Jyothinagar urban field practice area of Father Muller Medical College, Mangalore. Under the public private partnership model our institution provides immunization, health education, health checkup and referral services to these anganwadi centres. Objective: To assess the infrastructural facilities of the anganwadis and quality of services provided. Methodology: Study design: Cross Sectional; Study area: Field practice area of UTHC Mullerkad-Jyothinagar, Father Muller Medical College, Mangalore; Sample size: 24; Study duration: March 2013-June 2013; Data collection tool: Interview schedule cum observation checklist. Results: 9 of anganwadi workers were educated up to SSLC. Most of the anganwadi workers were residing within 15 minutes distance of the respective anganwadi centres. Registers were maintained properly and kept updated in all the anganwadis. 10 anganwadi were not having separate space of storage and 3 did not have even toilet facility available. Cooking area was separate in most of the anganwadis 14. Conclusion: Basic amenities like space for storage, kitchen and toilet and playing area for the children should be there in all the anganwadis. Pregnant and lactating mothers should be followed up to confirm whether they have been registered at any health centre and have received the requisite number of IFA and TT doses.

Background: Acinetobacter baumanii is an increasingly troublesome nosocomial pathogen, often with... more Background: Acinetobacter baumanii is an increasingly troublesome nosocomial pathogen, often with multidrug and pandrug resistant strains, mainly in intensive care units (ICUs), being responsible for different types of nosocomial infections. Objectives: The objective was to determine prevalence of carriers of Acinetobacter Spp. and predisposing risk factors among healthcare workers (HCWs) in ICUs and their role as source in nosocomial infection. Materials and Methods: Specimens from nose, throat, axilla and hands of all HCWs (55 (40 staff nurses, 15 attenders)) from ICUs and 30 HCWs from general wards were collected and processed by standard laboratory procedures. Results: Prevalance of A.baumanii carriers was 18.18%. Higher carrier rate was observed in male than female HCWs (12.72% Vs 5.5%). carrier rate was 22.2% and 18.75% in SICU and PICU respectively with none from general wards. Carrier rate was 7.3% at axilla and hands.Conclusion:Health care workers are responsible for colonization of hands by various hospital organism followed by spread to other contacts.

Objectives: The objective was to determine prevalence of carriers of Acinetobacter Spp. and predisposing risk factors among healthcare workers (HCWs) in ICUs and their role as source in nosocomial infection. Materials and Methods: Specimens from nose, throat, axilla and hands of all HCWs (55 (40 staff nurses, 15 attenders)) from ICUs and 30 HCWs from general wards were collected and processed by standard laboratory procedures. Results: Prevalance of A.baumanii carriers was 18.18%. Higher carrier rate was observed in male than female HCWs (12.72% Vs 5.5%). carrier rate was 22.2% and 18.75% in SICU and PICU respectively with none from general wards. Carrier rate was 7.3% at axilla and hands.Conclusion:Health care workers are responsible for colonization of hands by various hospital organism followed by spread to other contacts.

Objectives: The objective was to determine prevalence of carriers of Acinetobacter Spp. and predisposing risk factors among healthcare workers (HCWs) in ICUs and their role as source in nosocomial infection.
