Introduction This is a very interesting case of retained homicidal foreign body in the nose in co... more Introduction This is a very interesting case of retained homicidal foreign body in the nose in contrast to most of the foreign bodies which are accidental. Case Report A 27 year old male presented to ENT emergency with alleged history of assault over face with sharp object following which patient developed nasal bleed. On examination vertical laceration of approximately 8 cm in length was present along left naso-orbital groove extending superiorly from medial canthus of left eye and inferiorly to nasal alar cartilage. On anterior rhinoscopy a metallic foreign body was seen in both nasal cavities, which appeared to be crossing from left to right side piercing the nasal septum. Foreign body was removed via open approach. Discussion Penetrating maxillofacial injury with foreign body impaction are less common. High index of suspicion is required in diagnosing these cases. Antrochoanal polyp is a benign solitary sinonasal polyp arising within the maxillary sinus and ex... more Antrochoanal polyp is a benign solitary sinonasal polyp arising within the maxillary sinus and extending into nasopharynx via sinus ostium. It is reported commonly in young adults mainly in 3rd to 5th decades. Antrochoanal polyp accounts 4% to 6% of sinonasal polyps in general population and approximately 33% in paediatric population. Here we are reporting a case of antrochoanal polyp in just six year child because of its relative rarity. We report a case of six year old female patient who presented with left sided nasal obstruction for 3 months. It was associated with nasal discharge. Anterior rhinoscopy showed presence of a single polypoidal mass filling left nasal cavity, posterior extent of the mass could not be assessed clinically due to small age of the patient. Non-contrast computed tomography of nose and paranasal sinuses was done to see the exact extent of the mass. Mammary analogue secretary carcinoma are salivary gland tumors similar to secretary carcinoma of ... more Mammary analogue secretary carcinoma are salivary gland tumors similar to secretary carcinoma of breast histologically. It usually affects adults with mean age group of 45 years. Clinically it is usually slow growing, low grade malignancy with favorable prognosis. We are reporting a case of 47 years old female presented with complaints of swelling behind left ear for last 7 years. It was slow growing, painless and persistent. On examination, 4 x 3 cm swelling was present just below left ear lobule. It was nontender, firm, irregular in shape with smooth surface with slight mobility and overlying skin pinchable. Fine needle aspiration cytology suggested benign tumor with cystic change. On contrast enhanced computed tomography scan, there was a lobulated hypodense lesion measuring 24×35×32 mm with internal septations and enhancement of wall in expected location of superficial as well as deep lobe of left parotid gland. Case Presentation We, hereby, present a case of 25-year-old male, a case of chronic suppurative o... more Case Presentation We, hereby, present a case of 25-year-old male, a case of chronic suppurative otitis media atticoantral disease who underwent canal wall down mastoidectomy with tympanoplasty. He presented with complaints of facial nerve paralysis, 7 days after the surgery. Patient had a House-Brackmann grade-3 paralysis. Treatment was initiated with oral corticosteroids and oral acyclovir. Patient was recovered completely after the therapy. Discussion DPOFNP is an uncommon complication following uneventful tympanomastoid surgery. It is due to mechanical reactivation of HSV-1 virus in geniculate ganglion following mechanical disturbance of chorda tympani or operating in close vicinity to the facial nerve. It can also occur following neuro-otological surgery like acoustic neuroma and vestibular neurectomy, following stapedectomy or cochlear implantation. Conclusion Patient presenting with facial nerve paralysis a few days after uneventful tympanomastoid surgery should be diagnosed as DOPFNP. The patient should be reassured and given a course of oral corticosteroids and acyclovir. Overall prognosis is good.

Collaural fistula or cervico-aural fistula is a rare anomaly accounting for less than 8% of first... more Collaural fistula or cervico-aural fistula is a rare anomaly accounting for less than 8% of first branchial cleft anomaly. Aberrant development of first branchial cleft may lead to formation of a cervical cyst or sinus in the region of ear. We reported a case of a 4 year old girl who presented with recurrent swelling in right infra-auricular region from 6 months of age. She had undergone incision and drainage of the swelling three times at various peripheral hospitals over past 3.5 years. On examination two sinuses were noticed surrounding lobule of right pinna of which one was present posterior to the lobule and second was present just above lobule at lateral most part of conchal cartilage. Diagnosis of recurrent infected collaural fistula requires detailed clinical examination for presence of multiple sinuses surrounding the lobule and external auditory canal. In case of non visibility of sinus in external auditory canal, microscopic examination should be done. CT sonogram and MRI...
