However, angiosarcoma is a malignant tumor of vascular origin and presents as an infiltrating mass. Primary angiosarcoma of the mediastinum is very rare. Few cases have been reported in the atrium, pulmonary vein, and lung and present with chest pain. Angiosarcoma arising from a benign tumor like a schwannoma is very rare; only a handful of cases have been reported in the literature. This case is presented for its rarity and awareness that even a schwannoma presenting as well-circumscribed mediastinal mass can harbor a life-threatening malignancy. Case Report: A 51-year-old male presented with chest pain, and on investigation, there was an extrapulmonary and posterior mediastinal mass in the left paravertebral region with a benign appearance and his serum tumor markers were normal. Aim: The aim of this study was to study intratumoral heterogeneity in the expression of estrogen ... more Aim: The aim of this study was to study intratumoral heterogeneity in the expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) between two different cores/blocks of core needle biopsy (CNB), two different blocks of the same lesion in resection specimen (RS), and in between the CNB and RS. Materials and Methods: This is a prospective observational study of all the cases of breast carcinoma received in the department of histopathology for 2 years. All the patients with breast carcinoma diagnosed by CNB and RS were analyzed by three pathologists independently by immunohistochemistry. Results: Of the 75 cases, 74 were females and 1 male with a peak incidence of 60–70 years. Invasive breast carcinoma of no special type (97%) was the most common, followed by invasive lobular carcinoma (3%). A 51-year-old gentleman developed a rapidly progressing inguinal mass three months post corona vi... more A 51-year-old gentleman developed a rapidly progressing inguinal mass three months post corona virus disease (covid-19) infection. On examination, there was generalized lymphadenopathy confirmed by CT scans and a lymphoma was suspected. On light microscopy there were features of Rosai Dorfman disease with associated necrotizing granulomas amidst the large cells in the sinusoids, which have not been described before in Rosai Dorfman disease. Gene x-pert for tuberculosis, bacterial, fungal and acid fast bacilli cultures were negative. We present this case report for documenting this presentation post COVID-19 infection with review of literature.

Vesicles and bullae are fluid filled cavities present within or beneath the epidermis. They are autoimmune blistering disorders in which autoantibodies are directed against target antigens present in the epidermis and dermoepidermal junction. Aim: To study and analyse the clinical, histopathological and immunofluorescence findings in bullous lesions of the skin and to determine the contribution of immunofluorescence in diagnosing these conditions when there is a histological overlap. Materials and Methods: A total of 50 cases were selected in a two years span. Punch biopsy specimens of the skin taken from early lesions, sent for histopathological examination and Direct Immunofluorescence (DIF), were processed routinely. This is the first case report of a mucinous cystadenoma of the thymus. So this case is reported f... more This is the first case report of a mucinous cystadenoma of the thymus. So this case is reported for awareness that mucinous cystadenomas can occur in the mediastinum with a review of literature and the need for complete surgical excision. A twelve year old girl with a cystic mediastinal mass, presented with dry cough for three weeks. The mass was completely excised as it was huge and causing pressure symptoms. On light microscopy, there was thymic tissue with a mucinous cystadenoma. This is the first report of a mucinous cystadenoma in the thymus. Mucinous cystadenomas can grow into large sizes and become symptomatic due to pressure or rupture prompting investigation. Complete surgical excision is advised as mucinous cystadenocarcinoma of the thymus has to be ruled out by adequate sampling and careful analysis of the morphology.

Drug-related injury has been noted in virtually all organ systems, and recognition of the pattern... more Drug-related injury has been noted in virtually all organ systems, and recognition of the patterns of injury associated with medication enables modification of treatment and reduces the morbidity associated with the side effects of drugs. With the large number of new drugs being developed, documentation of the morphology of the changes seen as an adverse effect becomes important to characterize the pattern of injury. The pathologist is often the first to identify these abnormalities and correlate them with a particular drug. Kayexalate or sodium polystyrene sulfonate (SPS), a linear polymer derived from polystyrene containing sulfonic acid and sulfonate functional groups is used to treat hyperkalemia. It is usually administered with an osmotic laxative sorbitol orally or as retention enema. This combination has been implicated in causing damage to different parts of the gastrointestinal (GI) tract especially the colon and causes an established pattern of injury, recognizable by the presence of characteristic crystals, is presented to create a greater awareness of the Kayexalate colitis. This entity should be included in the differential diagnosis of lower GI mucosal injury in a setting of uremia and hyperkalemia.

Widespread use of ultrasonography in the prenatal period has increased the rate of early diagnosis of congenital pulmonary airway malformations, thus adult presentation of this condition is extremely unusual. Adenocarcinoma in situ is seen very rarely in adolescent and adult patients with long-standing congenital pulmonary airway malformations. We describe a 28-year-old man with type I congenital pulmonary airway malformation with pulmonary Kirsten rat sarcoma viral oncogene homolog mutation positive mucinous adenocarcinoma in situ.

Pulmonary dirofilariasis, caused by Dirofilaria immitis, rarely affects humans and is usually asy... more Pulmonary dirofilariasis, caused by Dirofilaria immitis, rarely affects humans and is usually asymptomatic, but may present as chest pain, cough, hemoptysis, wheezing, low-grade fever, and malaise. The dead and dying worms obstruct branches of the pulmonary artery, causing infarction and a granulomatous reaction. Coin lesions are apparent on radiography, raising concern of malignancy. Complete surgical excision is the treatment of choice and an anthelmintic can be administered if residual lesions are present or the patient is from an endemic area. We present two cases of pulmonary dirofilariasis presenting as coin lesions in the lung, which were clinically suggestive of malignancy.

Rosai-Dorfman disease or sinus histiocytosis is a nonmalignant disease chiefly presenting as a pa... more Rosai-Dorfman disease or sinus histiocytosis is a nonmalignant disease chiefly presenting as a painless cervical lymphadenopathy, although extranodal involvement is also frequent. Involvement of the tracheobronchial tree is extremely rare, and there are no clear guidelines on management. Rosai-Dorfman disease usually presents as a tracheal tumor, although it may also present with extraluminal compression. Only 12 cases have been reported in the English language literature. Histopathology is required to confirm the diagnosis. We describe what we think is India’s first reported case of histologically proven tracheal Rosai-Dorfman disease in a 40-year-old otherwise healthy lady.

Renal involvement in microsporidiosis is very rarely seen in these recipients. We describe two cases of pediatric renal microsporidiosis, diagnosed on renal biopsies, following bone marrow transplantation presenting as severe acute kidney injury. The first patient died, whereas the second survived due to early diagnosis based on high index of suspicion and prompt treatment with Albendazole. We believe these are the first such reported cases of renal microsporidiosis in pediatric bone marrow transplant recipients.

Early recognition and treatment are essential to prevent morbidity and mortality. Cytoreduction with Steroids, Hydroxyurea and Imatinib are the main stay of treatment. Molecular studies like Fip1-like-1 fused with platelet derived growth factor receptor alpha (FIP1L1-PDGFRα) etc., are recommended in view of therapeutic implication. In this paper we report a rare case of HES developing in a lady 6 months after surgical removal of lymphangioma of spleen, which in itself is rare.

Most common sites are gonads, sacrococcygeal, mediastinal, retroperitoneal, and pineal region. Here, we report a case of 2 -year-old previously healthy female toddlerwho presented with fever and right upper quadrant abdominal pain. Magnetic resonance imaging (MRI) of the abdomen revealed a right suprarenal nonenhancing cystic mass. Focal calcification was seen in the inferomedial aspect of the cyst. Complete surgical resection of the right adrenal gland was done. The patient had an uneventful postoperative course and was free of recurrence at 8 months follow-up. Primary retroperitoneal teratomas of the adrenal gland are very uncommon (4% of all primary teratomas) and can mimic a primary adrenal tumor, hence should be included in the differential diagnosis of adrenal masses.
