It is essential to prevent troubles in healthcare processes for healthcare safety. Trouble prevention is based on trouble prediction using accumulated knowledge on processes, troubles, and countermeasures. However, information on troubles has not been accumulated in hospitals in the appropriate structure, and it has not been utilized effectively to prevent troubles. In the previous study, however a detailed knowledge acquisition process for trouble prediction was proposed, the knowledgebase for countermeasures was not involved. In this paper, we aim to propose the structure of the knowledgebase for countermeasures, in the knowledge acquisition process for trouble prediction in healthcare process. We first design the structure of countermeasures and propose the knowledge representation form on countermeasures. Then, we evaluate the validity of the proposal, by applying it into an actual hospital.

Medical accidents are typically classified into two types: accidents caused by healthcare staff w... more Medical accidents are typically classified into two types: accidents caused by healthcare staff while providing services, and accidents caused by patients. Among the latter type, patient falls account for a significant percentage, and have a significant impact on patients. The goal of our research here is to establish a methodology to prevent patient falls by identifying situations that are dangerous for patients and formulating concrete countermeasures based on the results of an assessment to prevent such situations from arising. Kato et al. (2013b) evaluated risk factors, those are included in the existing assessment sheet, through Cox regression analysis by using data from approximately 1,000 cases at Iizuka Hospital, located in Fukuoka Prefecture. However, the number of this dataset was insufficient for a detailed analysis. It was also needed to arrange the data format and consult models for recurrent event analysis. Furthermore, the statistical model and covariates need to be considered in detail. In this paper, we tried to statistically model patient falls based on multivariate analysis by using a logistic model, the Cox model, and models for recurrent events. We first discussed the statistical model as well as the covariates to be included in it. We then developed a multiple scoring system based on the results of multivariate analysis by using data from Iizuka Hospital from 2009 to 2011. Finally, we evaluated each scoring system by calculating the correlation between scores and probability of events.

Rehabilitation is a medical intervention implemented mainly by therapists under the prescription ... more Rehabilitation is a medical intervention implemented mainly by therapists under the prescription orders of medical doctors in hospitals. It is difficult to standardize rehabilitation intervention processes because they largely depend on individual therapists rather than medicines or instruments, and there are few quantitative clinical indicators. In such situations, there are differences in intervention processes and outcomes between therapists and hospitals. In this study, we aim to develop a method for standardization of rehabilitation intervention processes based on actual clinical data. In our method, we firstly prepare a standard intervention process that could include the differences between therapists and hospitals. Second, we prepare an investigation sheet, which is designed to record information on intervention, outcome, and patient background. Third, we accumulate actual clinical data on the standard intervention process. Fourth, we analyze the accumulated clinical data, based on the evaluation structure designed for the target rehabilitation intervention. Finally, we try to standardize the intervention processes based on the results of analysis among therapists and hospitals. We applied our method to 87 total cases of dysphagia rehabilitation for cerebral stroke patients at three hospitals as the first cycle of standardization, and 130 cases as the second cycle. We thus designed a standard intervention process to record the clinical processes actually implemented in three hospitals. In addition, we obtained findings both on the effects of patient background and on effective intervention which supported the validity of our proposal.

A deficit in medical insurance finance has been observed with an increase in national medical exp... more A deficit in medical insurance finance has been observed with an increase in national medical expenses due to rapid aging, and the premium income does not increase. Therefore, efficient functioning of all medical institutions, reinforcement of a coordinated medical care system, and effective practical use of this system are necessary. In this study, a coordinated medical treatment system is considered and a regional alliance path is designed as a tool that facilitates in improving the coordinated medical treatment system step-by-step. It clarifies the future of a patient's condition by providing appropriate management and helps to achieve effective coordination. Further, the regional alliance path helps to prescribe appropriate medical examination and treatment processes that each medical institution should follow by considering the processes followed by local clinics, and further, enables suitable distribution of community medical resources.

Under Japanese Long-term Care Insurance System, long-term care is provided based on long-term care programs, which were designed by care providers on the basis of long-term care service plans, which were designed by care managers. However, defined methodology for designing long-term care service plans and care programs has not been established yet. In this paper, we propose models for designing long-term care service plans and care programs for older people, both by incorporating the technical issues from previous studies and by redesigning the total methodology according to these studies.
