Several preclinical and observational studies have shown that anti-diabetic medications (ADMs) ca... more Several preclinical and observational studies have shown that anti-diabetic medications (ADMs) can modify the risk of hepatocellular cancer (HCC) in patients with diabetes mellitus (DM). We performed a systematic review and meta-analyses of studies evaluating the effect of metformin, thiazolidinediones (TZDs), sulfonylureas, and/or insulin on the risk of HCC. We conducted a systematic search of Medline, EMBASE, and Web of Science up to August 2012. Studies were included if they (1) evaluated and clearly defined exposure to metformin, TZDs, sulfonylureas, and/or insulin, (2) reported HCC outcomes in patients with DM, and (3) reported relative risks or odds ratio (OR) or provided data for their estimation. Summary OR estimates with 95% confidence intervals (CIs) were estimated using the random-effects model. Nickel hypersensitivity has been reported in up to 15% of the general population. However, there ... more Nickel hypersensitivity has been reported in up to 15% of the general population. However, there are few reported cases of patients developing nickel allergy after cardiac device implantation. This phenomenon is generally not well understood and many authorities believe that these devices are safe. Most reported cases of nickel allergy have presented with non-specific symptoms such as chest pain, palpitations, or pericarditis and the majority of patients have undergone atrial septal defect closure with devices containing nickel, such as the Amplatzer septal occluder device (St Jude Medical). Herein, we describe a case of hypersensitivity pericarditis complicated with cardiac tamponade that responded dramatically to anti-inflammatory therapy. We hypothesize that this hypersensitivity was secondary to nickel allergy.

YouTube is a popular, consumer-generated, video-sharing website, which can be a source of information on RA. We investigated the quality of information on RA on YouTube and analyzed audience interaction.Methods.YouTube was searched using the term “Rheumatoid Arthritis,” for videos uploaded on RA. Two physicians independently classified videos as useful, misleading, or patient views, and rated them on a 5-point global quality scale (GQS; 1 = poor quality, 5 = excellent quality). Useful videos were rated for reliability and content, on a 5-point scale (higher scores represent more reliable and comprehensive videos). Source of videos was also noted. and the Risk of Hepatocellular Carcinoma in Patients With Hepatitis B Virus Infection.” In this p... more and the Risk of Hepatocellular Carcinoma in Patients With Hepatitis B Virus Infection.” In this population-based cohort study that used the Taiwan National Health Insurance Research Database, the authors reported that statin use may reduce the risk of hepatocellular carcinoma (HCC) in patients with hepatitis B virus (HBV) infection. Tsan et al 1 followed 33,413 patients with HBV infection from 1997 to 2008, of which 8.3% used statins and noted 1,021 incident HCC cases in this HBV cohort. The incidence rate (per 100,000 person-years) of HCC in patients on statins was 210.9, compared with an incidence rate of 319.5 in nonusers (P .001). The crude and adjusted hazard ratios (95% CIs) for HCC in statin users compared with nonusers was 0.66 (0.51 to 0.86) and 0.47 (0.36 to 0.61), respectively. The authors appropriately adjusted for potential confounders (age, sex, cirrhosis, diabetes, and medications such as anti-HBV treatment, aspirin, and angiotensin converting enzyme inhibitors) and acknowledged unmeasured confounders (alcohol intake, smoking status, and body mass index) as a limitation of their study. However, in their analysis, Tsan et al 1 failed to account for the impact of antidiabetic medications on altering the risk of HCC. It is now well established that diabetes is an independent risk factor for HCC as reported in several observational studies from Asia, Europe, and North America and subsequent metaanalyses. 2-4 Interestingly, recent studies have reported that antidiabetic medications also alter the risk of multiple cancers, including HCC. Gastric variceal bleeding is associated with significant morbidity and mortality in patients with... more Gastric variceal bleeding is associated with significant morbidity and mortality in patients with portal hypertension. N-butyl cyanoacrylate (NBC), mixed with lipiodol, has been shown to be effective in controlling bleeding, but is associated with the risk of distal embolization. To study the efficacy and safety of undiluted NBC in the management of gastric varices (GV). Prospective cohort study. Single tertiary care center. 170 consecutive patients with GV. Standardized technique of undiluted NBC injection for management of GV. Achievement of initial hemostasis, rate of rebleeding, procedure-related complications, and mortality. GV were identified in 170 patients, 87 of whom were treated with 261 injections of undiluted NBC. Among 46 patients with active bleeding of GV, initial hemostasis was achieved in 84.8%. Rebleeding was seen in 23.4% patients over a mean follow-up of 16 months. No case of clinical distal embolization was seen. Large GV size, fundal location, and large esophageal variceal size were predictive of GV bleed. The mortality was 8.8% for all patients with GV; 10.3% for patients with GV treated with NBC, and 7.2% for those with GV not treated with NBC. Child-Pugh status was the only predictor of mortality. Only 1 intervention group. Undiluted NBC is safe and effective in the management of gastric variceal bleeding.
