A partir de la crisis de diciembre de 2001 en la Argentina y de los cuestionamientos del modelo n... more A partir de la crisis de diciembre de 2001 en la Argentina y de los cuestionamientos del modelo neoliberal aparece en el sentido de lo político el rescate de una axiología basada en lo colectivo. Esto se manifestó en la oposición a los valores que caracterizaron la década de los noventa y en el retorno de otras temáticas que revalorizaron la cuestión nacional, la solidaridad y la exaltación de la moral en la práctica política. Analizamos el retorno de lo colectivo en los discursos de campaña electoral de Elisa Carrió y en el discurso de asunción presidencial de Néstor Kirchner. Henoch-Schönlein purpura (HSP) is the most common small vessel leukocytoclastic vasculitis during childhood. It is characterized by palpable purpura and/or joint, abdominal or renal involvement. Objective. To describe the epidemiological, clinical, laboratory, and evolutionary characteristics of patients with HSP. Population and methods. Observational, descriptive, and retrospective study. Patients younger than 15 years who met the Ankara criteria for HSP diagnosis were included in the period between 1/1/2008 and 12/31/2017. Results. A total of 339 patients with HSP were included; the male/female distribution was similar; their average age at onset was 7.02 years (± 3.02). The typical form was observed in 78 % and the atypical form, in 22 %, with joint involvement prior to purpura onset. Joint manifestations: 82 %; abdominal involvement: 54 %; renal manifestations: 19 %. Among patients with renal involvement, 68 % experienced it in the first month after onset. The most common symptom was isolated hematuria, and 22 % of patients with renal involvement had moderate to severe nephritis. Recurrences were observed in 15 % of patients. Conclusion. Joint manifestations were more common than what has been described in other series, whereas renal involvement was less common. Although it usually occurs in the first months of disease onset, it is worth noting that these patients need follow-up with urinary and blood pressure controls as part of their pediatric assessment.

A systematic review with conventional and network meta-analyses (NMA) was conducted to examine th... more A systematic review with conventional and network meta-analyses (NMA) was conducted to examine the outcomes of loco-regional hyperthermia (HT) with radiotherapy (RT) and-/or chemotherapy (CT) in locally advanced cervix cancer, IIB-IVA (LACC). 217 abstracts were screened from five databases and reported as per PRISMA guidelines. Only randomized trials with HT and RT±CT were considered. The outcomes evaluated were complete response (CR), long-term loco-regional control (LRC), patients alive, acute and late grade III/IV toxicities. Eight articles were finally retained. Six randomized trials with HTRT (n = 215) vs. RT (n = 212) were subjected to meta-analysis. The risk difference for achieving CR and LRC were greater by 22% (p < 0.001) and 23% (p < 0.001) respectively with HTRT compared to RT. Aim Predicting the impact of climate change on marine ecosystems or how fish and other species ar... more Aim Predicting the impact of climate change on marine ecosystems or how fish and other species are adapting to rising sea temperatures is still subject to much uncertainty, despite considerable progress in recent years. In this study we assess whether our understanding of the impact of sea warming on marine fish can be enhanced with an interdisciplinary approach that collates data from fisheries, fishermen and scientific research. By doing this, we aim to shed light on the major changes in the abundance and diversity of warm and cold water fish in recent decades in relation to sea warming. Location This study was conducted in the northwestern Mediterranean, where the impacts of global warming are particularly critical because range shifts are physically constrained. Methods We collected and combined statistical data from fisheries, the traditional ecological knowledge of fishermen (TEK), reproductive data (histological gonad analyses and ichthyoplankton surveys) and extensive research into the relevant literature (including systematic catalogues and museum collections and their databases). Results We have found that changes in the abundance of fish have followed a particular spatio-temporal sequence, with three different phases of colonization in the case of warm-water species (occasional occurrence, common presence and establishment), and three phases of regression (abundance reduction, range contraction and disappearance from the catch) in the case of cold-water species. Main conclusions Overall, the results show that this multidisciplinary approach, combining qualitative and quantitative information from different sources, provides new insight into the observed changes in fish diversity and abundance in relation to climate change.

Diabetics are at an increased risk of restenosis and adverse events after coronary stenting. Drug... more Diabetics are at an increased risk of restenosis and adverse events after coronary stenting. Drug-eluting stents may, therefore, be useful in these patients. Our objective was to evaluate the use of sirolimus-eluting stents in diabetics with complex coronary lesions. Between May 2002 and August 2003, we treated 231 patients with 260 complex coronary lesions using sirolimus-eluting stents. Of these patients, 56% did not have diabetes (ND), 22% had non-insulin-dependent diabetes (NIRD), and 20% had insulin-dependent diabetes (IRD). The primary clinical endpoint was target vessel failure at 1 year. The primary angiographic endpoints in the stent were late loss and binary restenosis at 6 months. At 6 months, late loss was greater in the IRD group (0.35 [0.71] mm) than in the ND group (0,096 [0.54] mm; P =.016) or the NIRD group (0.058 [0.52] mm; P=.017), and restenosis was more frequent (IRD, 16.3%; ND, 6.3%; and NIRD 7.8%; P=.05 for linear trend). At one year, target vessel failure occurred more frequently in the IRD group (IRD, 17.4%; NIRD, 7.7%; ND, 7.7%; P=.07 for linear trend) and the rate of survival free of target vessel failure was lower in the IRD group (82.1%) compared with the ND group (92.3%, P=.06) or the NIRD group (92.3%, P=NS). The only independent predictor of restenosis and target vessel failure was female sex. Despite IRD patients having greater late lumen loss and more frequent restenosis at six months and a trend towards a poorer clinical outcome at 1 year, no independent relationship was found between type of diabetes and clinical outcome.

Los pacientes diabéticos tienen un mayor riesgo de complicaciones tras el implante de stents convencionales; por ello, los stents farmacoactivos pueden ser útiles en estos pacientes. El objetivo es evaluar la utilidad de los stents recubiertos de rapamicina en diabéticos con lesiones coronarias complejas. Pacientes y método. Entre mayo de 2002 y agosto de 2003 tratamos a 231 pacientes con 260 lesiones complejas con stents recubiertos con rapamicina. Un 56% no tenía diabetes (ND), un 22% eran pacientes diabéticos no insulinodependientes (DMNID) y un 20% eran pacientes diabéticos insulinodependientes (DMID). The aim of present study was to compare the efficacy of SES with bare metal stents (BMS) for the treatment of such lesions. 96 patients with severe pLAD stenosis treated with SES were included. Clinical follow-up were performed during a 24 month period. A 98 patient sample with pLAD lesions treated with BMS was taken as control group. Death, angiographic restenosis, new target lesion revascularization (TLR) and target vessel failure (TVF) were registered. The combination of chronic oral anticoagulants (COA) with antiplatelet therapy has been related to an increased risk of hemorrhage. The aim of this study was to evaluate the level of the antiplatelet therapy observance in those patients and the incidence of adverse events after 1 year. Methods: Patients with prior COA treatment with coronary lesions suitable for stenting were included. Clinical assessment was performed on admission, with follow-up at 1, 6, and 12 months. Antiplatelet and COA treatment, adverse cardiac events, and hemorrhagic episodes were registered. Results: A total of 70 patients were included. Mean age was 70.5 ± 8.7 years. Introduction: Sirolimus-eluting stents (SES) have demonstrated low target vessel revascularizatio... more Introduction: Sirolimus-eluting stents (SES) have demonstrated low target vessel revascularizations and low incidence of angiographic restenosis in several clinical scenarios. The aim of the present study was to assess the efficacy and safety of SES for the treatment of proximal left anterior descending coronary artery (pLAD) lesions. Methods: Ninety-six patients with severe pLAD stenosis were enrolled. Angiographic and clinical follow-up were performed at 6 and 24 months, respectively. Death, myocardial infarction (MI), new target lesion revascularization (TLR), and target vessel failure (TVF) were registered. Clinical, angiographic, and procedural variables were analyzed to identify predictors of restenosis. Results: Mean clinical follow-up was 858 ± 158 days (26.5 ± 8.3 months). Angiographic procedural success was 100%. Angiographic follow-up showed 8.4% of binary restenosis without edge-restenosis phenomenon. Late loss was 0.15 ± 0.65 mm; 15.6% of patients had an adverse cardiac event, with 1% of death, 5.2% of MI, 6.3% of TLR, and 9.4% of TVF. At 2 years, the probabilities of cumulative TVF-and TLR-free survival were 90.6% and 93.7%, respectively. Interestingly, no adverse cardiac events were registered between the first and second years. Female gender (OR 10.7 CI 95% [1.7-66.7]) and in-stent restenosis (OR 8.2, CI 95% [1.2-56.4]) were found as independent predictors of binary restenosis. Advanced chronic renal failure showed a strong trend toward worse outcome in terms of binary restenosis (P = 0.063). Conclusions: SES for the treatment of pLAD stenosis proved safe and effective in a long-term follow-up with low incidence of adverse cardiac events and restenosis. Female gender and in-stent restenosis were predictors of binary restenosis.

Whether late mortality with DES may be higher than with bare metal stents is controversial and factors influencing death after DES implantation are not well known. The present observational registry analyses predictors of death during follow up after coronary stenting with sirolimus-eluting stents (SES). Methods: We performed an analysis of the causes of death on 233 patients included in a prospective registry that evaluated angiographic and clinical outcomes after SES implantation in complex scenarios. We analysed the relation of death with: 1) clinical, angiographic and procedural variables, and 2) clinical events, after a median follow up of 3.7 [3.4-3.9] years. We performed Cox regression analysis to assess predictors of mortality. Results: Nineteen patients (8.1%) died. Survival free of death was 91.85%. Two deaths (0.8%) were due to ictus, 2 (0.8%) were due to cancer, 2 (0.8%) were of unknown causesnot cardiacand 13 (5.6%) were cardiac. After multivariate analysis independent predictors of death were depressed left ventricular function (HR 10.8 [3.31-35.14], p = 0.0001), age (HR 1.1 [1.04-1.19], p = 0.001), AMI during follow up (HR 7.03 [2.04-24.29], p = 0.002), proximal LAD lesions (HR 0.15 [0.04-0.59], p = 0.006), female gender (HR 3.54 [1.18-10.57], p = 0.023), renal insufficiency (HR 5.82 [1.25-27.11], p = 0.025) and IIb-IIIa inhibitors (HR 0.31 [0.09-1.03], p = 0.054). Conclusions: Mortality after SES implantation seems to be related to basal conditions of the patient, use of IIb-IIIa inhibitors, left ventricular dysfunction and progression of disease.

Angiographic follow-up was carried out in the 6th month in 201 patients. Clinical, angiographic, and procedural variables were analyzed to assess predictors of restenosis. Of the patients, 42.6% were diabetic and 20.2% were insulin dependent. A total of 263 lesions was treated. Mean lesion length was 16.5 +/- 11 mm and mean vessel diameter was 2.54 +/- 0.7 mm. A total of 327 stents was implanted to fully cover the entire lesion, in most cases after predilatation (89.1%). The mean segment length covered by the stent was 26 +/- 12 mm and the stent length/lesion length ratio was 1.7 +/- 0.9. Binary restenosis was found in 8.7%, but there was no edge restenosis. A total of 339 patients with HSP were included; the male/female distribution was similar; their average age at onset was 7.02 years (± 3.02). The typical form was observed in 78 % and the atypical form, in 22 %, with joint involvement prior to purpura onset. Joint manifestations: 82 %; abdominal involvement: 54 %; renal manifestations: 19 %. Among patients with renal involvement, 68 % experienced it in the first month after onset. The most common symptom was isolated hematuria, and 22 % of patients with renal involvement had moderate to severe nephritis. Recurrences were observed in 15 % of patients. Conclusion. Joint manifestations were more common than what has been described in other series, whereas renal involvement was less common. Although it usually occurs in the first months of disease onset, it is worth noting that these patients need follow-up with urinary and blood pressure controls as part of their pediatric assessment.

A systematic review with conventional and network meta-analyses (NMA) was conducted to examine th... more A systematic review with conventional and network meta-analyses (NMA) was conducted to examine the outcomes of loco-regional hyperthermia (HT) with radiotherapy (RT) and-/or chemotherapy (CT) in locally advanced cervix cancer, IIB-IVA (LACC). 217 abstracts were screened from five databases and reported as per PRISMA guidelines. Only randomized trials with HT and RT±CT were considered. The outcomes evaluated were complete response (CR), long-term loco-regional control (LRC), patients alive, acute and late grade III/IV toxicities. Eight articles were finally retained. Six randomized trials with HTRT (n = 215) vs. RT (n = 212) were subjected to meta-analysis. The risk difference for achieving CR and LRC were greater by 22% (p < 0.001) and 23% (p < 0.001) respectively with HTRT compared to RT. Aim Predicting the impact of climate change on marine ecosystems or how fish and other species ar... more Aim Predicting the impact of climate change on marine ecosystems or how fish and other species are adapting to rising sea temperatures is still subject to much uncertainty, despite considerable progress in recent years. In this study we assess whether our understanding of the impact of sea warming on marine fish can be enhanced with an interdisciplinary approach that collates data from fisheries, fishermen and scientific research. By doing this, we aim to shed light on the major changes in the abundance and diversity of warm and cold water fish in recent decades in relation to sea warming. Location This study was conducted in the northwestern Mediterranean, where the impacts of global warming are particularly critical because range shifts are physically constrained. Methods We collected and combined statistical data from fisheries, the traditional ecological knowledge of fishermen (TEK), reproductive data (histological gonad analyses and ichthyoplankton surveys) and extensive research into the relevant literature (including systematic catalogues and museum collections and their databases). Results We have found that changes in the abundance of fish have followed a particular spatio-temporal sequence, with three different phases of colonization in the case of warm-water species (occasional occurrence, common presence and establishment), and three phases of regression (abundance reduction, range contraction and disappearance from the catch) in the case of cold-water species. Main conclusions Overall, the results show that this multidisciplinary approach, combining qualitative and quantitative information from different sources, provides new insight into the observed changes in fish diversity and abundance in relation to climate change.

Drug-eluting stents may, therefore, be useful in these patients. Our objective was to evaluate the use of sirolimus-eluting stents in diabetics with complex coronary lesions. Between May 2002 and August 2003, we treated 231 patients with 260 complex coronary lesions using sirolimus-eluting stents. Of these patients, 56% did not have diabetes (ND), 22% had non-insulin-dependent diabetes (NIRD), and 20% had insulin-dependent diabetes (IRD). The primary clinical endpoint was target vessel failure at 1 year. The primary angiographic endpoints in the stent were late loss and binary restenosis at 6 months. At 6 months, late loss was greater in the IRD group (0.35 [0.71] mm) than in the ND group (0,096 [0.54] mm; P =.016) or the NIRD group (0.058 [0.52] mm; P=.017), and restenosis was more frequent (IRD, 16.3%; ND, 6.3%; and NIRD 7.8%; P=.05 for linear trend). At one year, target vessel failure occurred more frequently in the IRD group (IRD, 17.4%; NIRD, 7.7%; ND, 7.7%; P=.07 for linear trend) and the rate of survival free of target vessel failure was lower in the IRD group (82.1%) compared with the ND group (92.3%, P=.06) or the NIRD group (92.3%, P=NS). The only independent predictor of restenosis and target vessel failure was female sex. Despite IRD patients having greater late lumen loss and more frequent restenosis at six months and a trend towards a poorer clinical outcome at 1 year, no independent relationship was found between type of diabetes and clinical outcome.

El objetivo es evaluar la utilidad de los stents recubiertos de rapamicina en diabéticos con lesiones coronarias complejas. Pacientes y método. Entre mayo de 2002 y agosto de 2003 tratamos a 231 pacientes con 260 lesiones complejas con stents recubiertos con rapamicina. The combination of chronic oral anticoagulants (COA) with antiplatelet therapy has been related to an increased risk of hemorrhage. The aim of this study was to evaluate the level of the antiplatelet therapy observance in those patients and the incidence of adverse events after 1 year. Methods: Patients with prior COA treatment with coronary lesions suitable for stenting were included. Clinical assessment was performed on admission, with follow-up at 1, 6, and 12 months. Antiplatelet and COA treatment, adverse cardiac events, and hemorrhagic episodes were registered. Results: A total of 70 patients were included. Mean age was 70.5 ± 8.7 years. The aim of the present study was to assess the efficacy and safety of SES for the treatment of proximal left anterior descending coronary artery (pLAD) lesions. Methods: Ninety-six patients with severe pLAD stenosis were enrolled. Angiographic and clinical follow-up were performed at 6 and 24 months, respectively. Death, myocardial infarction (MI), new target lesion revascularization (TLR), and target vessel failure (TVF) were registered. Clinical, angiographic, and procedural variables were analyzed to identify predictors of restenosis. Results: Mean clinical follow-up was 858 ± 158 days (26.5 ± 8.3 months). Angiographic procedural success was 100%. Angiographic follow-up showed 8.4% of binary restenosis without edge-restenosis phenomenon. Late loss was 0.15 ± 0.65 mm; 15.6% of patients had an adverse cardiac event, with 1% of death, 5.2% of MI, 6.3% of TLR, and 9.4% of TVF. At 2 years, the probabilities of cumulative TVF-and TLR-free survival were 90.6% and 93.7%, respectively. Interestingly, no adverse cardiac events were registered between the first and second years. Female gender (OR 10.7 CI 95% [1.7-66.7]) and in-stent restenosis (OR 8.2, CI 95% [1.2-56.4]) were found as independent predictors of binary restenosis. Advanced chronic renal failure showed a strong trend toward worse outcome in terms of binary restenosis (P = 0.063). Conclusions: SES for the treatment of pLAD stenosis proved safe and effective in a long-term follow-up with low incidence of adverse cardiac events and restenosis. Female gender and in-stent restenosis were predictors of binary restenosis.

Whether late mortality with DES may be higher than with bare metal stents is controversial and factors influencing death after DES implantation are not well known. The present observational registry analyses predictors of death during follow up after coronary stenting with sirolimus-eluting stents (SES). Methods: We performed an analysis of the causes of death on 233 patients included in a prospective registry that evaluated angiographic and clinical outcomes after SES implantation in complex scenarios. We analysed the relation of death with: 1) clinical, angiographic and procedural variables, and 2) clinical events, after a median follow up of 3.7 [3.4-3.9] years. We performed Cox regression analysis to assess predictors of mortality. Results: Nineteen patients (8.1%) died. Survival free of death was 91.85%. Two deaths (0.8%) were due to ictus, 2 (0.8%) were due to cancer, 2 (0.8%) were of unknown causesnot cardiacand 13 (5.6%) were cardiac. After multivariate analysis independent predictors of death were depressed left ventricular function (HR 10.8 [3.31-35.14], p = 0.0001), age (HR 1.1 [1.04-1.19], p = 0.001), AMI during follow up (HR 7.03 [2.04-24.29], p = 0.002), proximal LAD lesions (HR 0.15 [0.04-0.59], p = 0.006), female gender (HR 3.54 [1.18-10.57], p = 0.023), renal insufficiency (HR 5.82 [1.25-27.11], p = 0.025) and IIb-IIIa inhibitors (HR 0.31 [0.09-1.03], p = 0.054). Conclusions: Mortality after SES implantation seems to be related to basal conditions of the patient, use of IIb-IIIa inhibitors, left ventricular dysfunction and progression of disease.

Angiographic follow-up was carried out in the 6th month in 201 patients. Clinical, angiographic, and procedural variables were analyzed to assess predictors of restenosis. Of the patients, 42.6% were diabetic and 20.2% were insulin dependent. A total of 263 lesions was treated. Mean lesion length was 16.5 +/- 11 mm and mean vessel diameter was 2.54 +/- 0.7 mm. A total of 327 stents was implanted to fully cover the entire lesion, in most cases after predilatation (89.1%). The mean segment length covered by the stent was 26 +/- 12 mm and the stent length/lesion length ratio was 1.7 +/- 0.9. Binary restenosis was found in 8.7%, but there was no edge restenosis.
