Lymphangiomas are benign hamartomatous tumors of lymphatic origin which results from sequestratio... more Lymphangiomas are benign hamartomatous tumors of lymphatic origin which results from sequestration of lymphhatic tissues that do not communicate with other lymphatic channels. They may occur with hemangiomas and have high predilections for head and neck region (50-70%). They may occur at birth (60-70%) or till two years of age (90%) and rarely in adults. The most common site of occurrence in the mouth is dorsum of tongue being followed by lips, buccal mucosa, soft palate and floor of mouth. Tongue lymphangioma may present as localized or diffused growth that may result in macroglossia, impaired speech and difficulty in mastication. There are different treatment options; surgical excision being the most effective mode of treatment. Here we have reported three different cases of oral lymphangiomas, two of which occurring in the tongue and one in the buccal mucosa.

Meningiomas represent one of the commonest benign neural tissue neoplasms, but it is an extremely... more Meningiomas represent one of the commonest benign neural tissue neoplasms, but it is an extremely rare finding in the oral cavity. It tends to arise from the arachnoid cap cells pertaining to dura matter of the Meninges, the choroid plexus, or the brain stem. Herein, we report the case of a 36-year-old male patient, who visited our Department with the chief complaint of facial swelling associated with asymmetry. Based on thorough clinical, radiological, histopathological and immunohistochemical analyses, a diagnosis of Primary Extracranial Meningioma (PEM) of the Maxilla was made. Later, the tumor was surgically removed, and on follow up, the patient is doing well. Our presented case is extremely unique-it represents a PEM involving the oral cavity.

Oral potentially malignant disorders, in many cases, are superseded by Oral carcinoma. These prem... more Oral potentially malignant disorders, in many cases, are superseded by Oral carcinoma. These premalignant lesions usually manifest as erythematous or whitish patches within the oral mucosa. Histo-pathological grading of these lesions still remains the gold standard in diagnostic pathology; being interpreted as Epithelial Dysplasia. It refers to a premalignant change in epithelium characterized by a combination of cellular and architectural alterations. Histopathological grading of epithelial dysplasia poses many challenges to Oral and diagnostic pathologists, including inter and intra observer variability and choice of optimum parameters based on which the grading should be done. Pathologists must resort to using uniform standards and defined criteria for interpretation of dysplasia. Herein, we have briefly discussed regarding various grading systems of Epithelial dysplasia.

The concept "field effect in cancer" originated in 1953, from the histopathological observations ... more The concept "field effect in cancer" originated in 1953, from the histopathological observations of Slaughter and colleagues regarding the occurrence of multiple primary oral squamous cell carcinomas and their local recurrences. There are two fundamental theories regarding the development of oral field cancerization (OFC) which includes the "classical" and the "clonal" mechanism. The classical view hypothesizes that the entire epithelial surface of the upper aerodigestive tract has an increased risk for the development of premalignant disorders because of multiple genetic abnormalities in the whole tissue region; owing to prolonged exposures of susceptible carcinogens as a result of tobacco or alcohol consumption. While the clonal theory believes that there always exists a field with genetically altered cells having a high risk of developing premalignant and malignant disorders. Here, the developing separate tumors from a genetically individually altered stem cell in the extended field, share the same clonal origin, and some degree of genetic similarity. Modern molecular and histological technologies exploring the tissue abnormalities in OFC have been performed. Herein, we have briefly discussed regarding the concepts of field cancerization along with the various diagnostic molecular techniques and the therapeutic aspects.

Oral Lichen Planus (OLP) is a relatively common, systemic, immune based disease. It usually, invo... more Oral Lichen Planus (OLP) is a relatively common, systemic, immune based disease. It usually, involves the oral mucosa, but other sites like the skin, genital mucosa, and scalp are also affected. In this auto-cytotoxic muco-cutaneous disease, the T lymphocytes trigger apoptosis of epithelial cells leading to chronic inflammation. OLP is mediated by both antigen specific and nonspecific pathways. Antigen presentation through basal keratinocytes and antigen-specific keratinocyte destruction by CD8+ cytotoxic T-cells are within the specific pathway spectrum. Whilst the nonspecific methods involve mast cell degeneration; leading to granular release and matrix metalloproteinase (MMP) activation. The process ultimately results in clustering of T cells in the upper layer of lamina propria, basal lamina disruption, T-cell inflow within the epithelium, along with apoptotic death of keratinocytes. The normal oral mucosa may be an immune privileged site and breach of the immune barrier could result in OLP and possibly other autoimmune oral mucosal diseases owing to deficient antigen-specific TGF-β1-mediated immunosuppression. However, many among these immuo pathologic mechanisms are still shroud in mystery.
