L'objectif de cet article est d'estimer le cout moyen de la prise en charge medicale et sociale pour un enfant porteur de la forme severe de ce syndrome (SAF), de sa naissance a son sixieme anniversaire.

La consommation d’argile ou geophagie est decrite dans le monde entier comme un comportement part... more La consommation d’argile ou geophagie est decrite dans le monde entier comme un comportement particulier des femmes enceintes dans certaines conditions, voire propre a certaines sous-populations. Les explications donnees a ce comportement ont varie dans le temps. Tout d’abord decrit comme une envie de la femme enceinte, attiree par toutes sortes de produits non ingeres habituellement, il a ensuite ete designe comme pica, puis groupe avec les autres troubles des comportements alimentaires. Dans tous les cas, la definition en est d’une envie irrepressible de consommer certaines substances, comportant eventuellement le besoin d’augmenter les quantites. Cette definition et la globalisation des addictions comportementales et a des substances amenent a l’integrer dans ce champ. Nous visons ici a interroger la nature de l’ingestion de l’argile au travers d’une revue historique puis au moyen de nos donnees de terrain sur les representations et/ou interpretations des soignants et des femmes concernes par ces pratiques, par l’exemple de la Guyane francaise.

Data on such consumption in Frensh Guyana are scattered and scarce, which prevents prevention policies from being adapted to the local context.

In France, since 2007, there is a compulsory warning recommending abstinence during pregnancy on ... more In France, since 2007, there is a compulsory warning recommending abstinence during pregnancy on every container of alcohol. Awareness of this warning, which consists of a small pictogram, is unknown. The aim of this study was to assess awareness of the warning and risk perceptions about prenatal drinking in pregnant and postpartum women. A cross-sectional survey was carried out by telephone five years after the introduction of the warning label. A total of 3603 pregnant or postpartum French women participated. A quota sampling method was used to ensure the sample reflected the population. Multivariate analyses examined the characteristics associated with knowledge of risks and with awareness of the warning label. The warning label had been noticed by 66.1% of women and 77.3% of drinkers. Of those who had noticed the warning, 98.6% thought that it suggested abstinence. Les donnees montrent que le modele moral predomine, qui attribue la responsabilite de l'addiction, comme la resolution du probleme, a l'individu. La responsabilite de la dependance est attribuee essentiellement a l'individu, surtout si le professionnel est une femme, un travailleur social, un medecin (plutot âge), residant dans des zones non urbaines

Changes in drug use in the general French population over the last decade have also been observed... more Changes in drug use in the general French population over the last decade have also been observed among pregnant women. There have also been significant changes in the treatment of substance users. Data relating to maternal drug use and the treatment of drug-using mothers were collected based on a retrospective cohort of 170 newborns with mothers using at least two different substances. The decrease in the proportion of heroin-users, the rise of opiate maintenance treatments and cannabis, and the development of new forms of treatment are linked to shorter maternity stays for newborns and shorter treatments for neonatal abstinence syndrome. The percentage of preterm births and of children placed in foster care is also on the decrease. Changes in drug use among mothers and new forms of treatment given to mother-infant dyads may account for the decrease in treatment for NAS, the shorter length of hospital stays, and the decreasing number of children placed in foster care.

This survey aimed to explore the issue of the associations of substances during pregnancy and to determine the consumer profiles. One hundred and seventy newborns whose mothers were psychoactive substances users were identified over the period 1999 to 2008. The data relating to maternal consumption, their reproductive history, and their living environment were collated. At the end of their pregnancy, the mothers reported using on average 3.14 substances. Three profiles were determined: 65 women were heroin users or had consumed it in their lifetime and were currently on substitution treatment, and had a very unfavourable social living environment; 30 women were mainly consumers of alcohol, with or without benzodiazepines or other psychotropic drugs, and had a history of abortions; 75 women were mainly tobacco and cannabis smokers, with or without substitution treatment, had good social living conditions and had wanted the pregnancy.
