Despite the lateral epicondylitis or tennis elbow is a common cause of pain in orthopaedic and sports medicine, the results of the different modalities of conservative treatment are still contradictory. The pourpose of this study was to evaluate the efficacy of radial shock wave therapy (RSWT) in the treatment of tennis elbow. Methods. In a prospective randomized controlled singleblind study, of 75 eligible patients, 62 with tennis elbow were randomly assigned to study group and control group. There were 31 patients in the study group and 31 patients in the control group. Both groups had received a treatment a week for 4 weeks; the study group had received 2 000 impulses of RSWT and the control group 20 impulses of RSWT. All patients were evaluated 3 times: before treatment, at the end of treatment and to 6 months follow-up. The evaluation consisted of assessments of pain, pain-free grip strength test, and functional impairment.

This injury generally occurs in individuals who engage in sports and recreational activities that require sudden acceleration and deceleration, and in athletes who perform repetitive movements such as jumping, climbing, and kicking. These activities are known to have the potential to overload the extensor apparatus of the knee. Lian et al, 13 in a study that included 612 athletes from different sports, reported a prevalence of patellar tendinopathy of 14.2%. This injury is twice as prevalent in men as it is in women 11 and keeps 33% of athletes away from their sports activities for more than 6 months, forcing 50% of these athletes into early retirement. 2 Furthermore, it affects approximately 45% of volleyball athletes, as this is a sport with actions that involve several risk factors for patellar tendinopathy. Due to the high prevalence of patellar tendinopathy in athletes, early diagnosis and treatment are essential to avoid extended periods away from sports activities. Clinical instruments to determine the severity and the level of disability resulting from patellar tendinopathy are highly useful because they can assess the evolution of the condition and serve as T T STUDY DESIGN: Clinical measurement. T T OBJECTIVES: To translate, adapt, and test the measurement properties of the Brazilian Portuguese version of the Victorian Institute of Sport Assessment-Patella (VISA-P) questionnaire.

ROC analysis indicated that baseline FSH level predicted the appearance of ejaculated spermatozoa after treatment [AUC = 0.811; 95 % Confidence Interval (CI) 0.6–0.9; p = 0.0029]. A cutoff level of FSH <10.06 mIU/mL identified 82.0 % of cases with ejaculated spermatozoa with a specificity of 81.8 % and a sensitivity of 83.3 %. Conclusion Selected patients with NOA may show ejaculated spermatozoa after a non-invasive repair of a left side varicocele, therefore avoiding testicular sperm extraction. Baseline serum FSH was a valuable predictor for ejaculated spermatozoa after treatment.

The severity of clinical activity of Crohn's disease is high during the first year after diagnosis and decreases thereafter. Approximately 50% of patients require steroids and immunosuppressants and 75% need surgery during their lifetime. The clinical course of patients with Crohn's disease first diagnosed at surgery has never been investigated.

The aim of this investigation was to determine whether unilateral paravertebral block (PVB) could provide better control of postoperative pain syndrome compared with unilateral subarachnoid block (SAB).A randomized controlled study was conducted using fifty patients with unilateral inguinal hernias. The patients were randomized to receive either PVB (S group) or SAB (C group). PVB was performed by injecting a total of 20 ml of 0.5% levobupivacaine from T9 to T12 under ultrasound guidance, whereas SAB was performed by injecting 13 mg of 0.5% levobupivacaine at the L3-L4 level. Data regarding anesthesia, hemodynamic changes, side effects, time spent in the post-anesthesia care unit (PACU), the Karnofsky Performance Status (KPS), acute pain and neuropathic disturbances were recorded.PVB provided good anesthesia of the inguinal region without patient or surgeon discomfort, with better hemodynamic stability and safety and with a reduced time to discharge from the PACU compared with SAB. During the post-surgical and post-hospital discharge follow-ups, rest and incident pain and neuropathic positive phenomena were better controlled in the S group than in the C group. The consumption of painkillers was higher in the C group than in the S group throughout the follow-up period.PVB can be considered a viable alternative to common anesthetic procedures performed for inguinal hernia repair surgery. PVB provided good management of acute postoperative pain and limited neuropathic postoperative disturbances.

Twenty-seven patients were submitted to radiation therapy alone and 34 received radiotherapy post limited surgery in early stages and post radical vulvectomy in advanced stages. The dose range varied from 59 to 63 Gy in post-operative patients and 65 Gy to 71 Gy in curative patients. Five-year Overall Survival (OS) and Disease-Free Survival (DFS) for patients treated with irradiation alone and for those treated with post-operative radiotherapy were 50.8% and 69.7%, respectively, without significant statistical difference. For OS multivariate analysis showed statistical difference for stage and age variables, and for stage variable in the case of DFS. In early stage vulvar cancer patients OS and DFS are good, with high control rate and low incidence of adverse effect. In loco-regionally-advanced patients, especially in those with st...
