In politics, a scandal may be said to occur when important public figures are widely suspected of... more In politics, a scandal may be said to occur when important public figures are widely suspected of contravening prevailing standards of acceptable public or private behaviour and often of attempting to conceal their actions through further illegal or deceitful acts as well (Lowi, 1989, pp. vii–viii). Scandals of this sort have been a very prominent feature of recent French politics. In the 1985 Greenpeace affair, for example, the Minister of Defence, the president’s personal military attache and the director of the Secret Service apparently hatched and implemented a bizarre plan to protect the country’s nuclear testing activities in the South Pacific by having military frogmen blow up a boat belonging to the Green-peace organisation while it was moored in Auckland, New Zealand. The French Government earnestly denied any responsibility for the attack, but persistent efforts by French journalists led to the gradual revelation of the involvement not only of French undercover agents but also of the Minister of Defence, Charles Hernu, and to suspicions that the president himself had personally approved the project. Although the president and the prime minister managed to dodge the bullet, Hernu was forced to resign (Derogy and Pontaut, 1986; Bornstein, 1989).

Important arguments and conclusions are offered but often remain tangential and weakly related to the data. Despite this shortcoming, Korpi provides an important theoretical perspective, a richness of historical data on electoral activity and public opinion in Sweden, and useful comparative material on elections and industrial conflict in advanced industrial societies.

At present, patients in Newfoundland and Labrador who require a PET scan must travel out of the province to either Alberta or Quebec at a substantial cost to the provincial health system as well as to the patient and his/her family.

The province of Newfoundland and Labrador has the highest rate in the country of newly diagnosed ... more The province of Newfoundland and Labrador has the highest rate in the country of newly diagnosed patients over the age of 65 years with end-stage renal (kidney) disease (ESRD). 1 Here, as elsewhere in Canada, the profile of patients undergoing dialysis has changed, with a growing number of older, and more medically frail, patients being offered dialysis. In January, 2008, there were 380 patients on dialysis in this province, 65% of whom were being treated by hemodialysis in main hospital-based dialysis units in St. John's and Corner Brook, and in Grand Falls-Windsor, a satellite of St. John's that operates much like a main unit.

In this commentary, we articulate strategies for positioning PhD-trained scientists at the forefront of this transformation, including refreshing a competency framework that outlines the skill set required for maximum impact, exploring opportunities to expand embedded research career pathways and considering new ways to support the evolution of learning health systems. We conclude highlighting the need to modernize how real-world research impact is recognized.

Despite an increase in prevalence of complex chronic conditions and dementia, longterm care servi... more Despite an increase in prevalence of complex chronic conditions and dementia, longterm care services are being continuously pushed out of institutional settings and into the home and community. The majority of people living with dementia in Canada and the United States (U.S.) live at home with support provided by family, friends or other unpaid caregivers. Ten dementia care policy programs and service delivery models across five different North American jurisdictions in Canada and the U.S. are compared deductively using a comparative policy framework originally developed by Richard Rose. One aim of this research was to understand how different jurisdictions have worked to reduce the fragmentation of dementia care. Another aim is to assess, relying on the theory of smart policy layering, the extent to which these policy efforts 'patch' health system structures or add to system redundancies. We find that these programs were introduced in a manner that did not fully consider how to patch current programs and services and thus risk creating further system redundancies. The implementation of these policy programs may have led to policy layers, and potentially to tension among different policies and unintended consequences. One approach to reducing these negative impacts is to implement evaluative efforts that assess 'goodness of fit'. As Robyn Tamblyn and her colleagues (2016) note in this issue, the Institute of Health Services a... more As Robyn Tamblyn and her colleagues (2016) note in this issue, the Institute of Health Services and Policy Research (IHSPR) of CIHR has identified one of its key strategic goals for the 2015–2019 period as “the creation of learning health systems and the next generation of researchers with the skills to partner in health system learning and transformation.” As part of its effort to realize that goal, the Institute led the creation of a multi-sectoral “alliance” of organizations concerned with health systems and policy research (HSPR) and that Alliance spawned a “Working Group on Training Modernization” whose role was to examine the ways in which our current models of doctoral training in HSPR need to be enhanced and updated to meet the Institute’s strategic goal. As co-chair (alongside Dr. Adalsteinn Brown of the University of Toronto), I am submitting this commentary to inform the readers of this journal about the activities, outputs and plans of that Working Group. The Issue In 2009, the Government of Newfoundland & Labrador announced the creation of two youth ... more The Issue In 2009, the Government of Newfoundland & Labrador announced the creation of two youth residential treatment (YRT) centres for youth with complex needs. YRT is a multifaceted form of care that requires programming in the areas of treatment, milieu (i.e., plans to manage group interactions among clients and staff), and the residence itself. A broad range of multi-disciplinary research-based evidence can inform the design and implementation of these YRT centres. For this evidence to be useful to decision makers, it needs to be interpreted in the context of Newfoundland & Labrador. Providing health decision makers with the best available evidence that is attuned to the capacities and characteristics of the province is the goal of the Contextualized Health Research Synthesis Program (CHRSP). In February 2022, a task force appointed by the Premier of Newfoundland and Labrador submitted a ... more In February 2022, a task force appointed by the Premier of Newfoundland and Labrador submitted a report that he had commissioned 14 months earlier. The task force’s objective was to devise a 10-year strategy for modernizing and reforming the province's healthcare system and bringing health outcomes in the province up to Canada's high standards. The report will be followed this month (June 2022) by two more documents, an 'implementation blueprint' and an 'evidence report.' This commentary examines the report, summarizing both its devastating findings on the current health system and its comprehensive and ambitious proposals for reform.
