It is a slow growing neoplasia of unknown etiogenesis, asymptomatic and of variegated histology. They are more common in males but our case was a female patient. The presentation may mimic sarcoma at times. Case Report An unusual case of a very large neck mass in a 62 years old female patient is reported. The growth involved the dorso-cervical region over a period of approximately 8 years but remained asymptomatic. The mass was excised and post excision histopathology was reported as nuchal fibroma. Discussion The case reported is large compared to the usual size of nuchal fibroma. Association with diabetes mellitus and Gardner’s syndrome has been reported in literature. MRI is the imaging of choice to establish the differential diagnosis.

Melanocytes originate from the neural crest cells and then migrate to various tissues in the body during the course of development. Sinonasal melanoma are rare tumours and the aetiopathogenesis, treatment and staging of the tumours are still an area of debate. There remains paucity of studies dealing exclusively with sinonasal melanoma. We attempt to make a comprehensive study of the disease by reviewing the existing literature on sinonasal melanoma. METHODOLOGY Review of existing literature on the melanoma of nose and paranasal sinus. RESULTS AND DISCUSSION The melanoma of the sinonasal tract are rare and account for 1% of all melanoma. The aetiology of the tumour is still debated and the behaviour of the tumour remains conspicuously different both genetically and clinically from its cutaneous counterpart. Surgery remains the mainstay of treatment, though newer modalities are being explored. The prognosis of these tumours remain grim with a 5-year survival rate of about 25%. CONCLUSION Early diagnosis, histopathological confirmation and aggressive control of the primary tumour is the standard modality of treatment. The molecular biology and genetics of the tumour are an interesting field of research and targeted therapy may hold the key to improve the outcome in the future.

Foreign body impaction in the upper digestive tract is not an infrequent emergency in every clini... more Foreign body impaction in the upper digestive tract is not an infrequent emergency in every clinical setup. In the present retrospective study, we have tried to analyze the incidence of impacted foreign body in the upper digestive tract at our center. The most frequent presentation is history of foreign body ingestion or aspiration. They are chiefly produced by accidental swallowing of the foreign body in the extremes of age and deliberate attempts in case of psychiatric and criminal background subjects. Varied manifestations of dysphagia, drooling of saliva and retro-sternal pain is presented. Additional symptoms of vomiting and subjective sensation of breathlessness are present occasionally. Removal of foreign body at times is a challenge for the gastrointestinal endoscopist.. If not intervened expeditiously, can lead to horrific outcome. In our review study, rigid endoscopy resolved the problem in majority of cases by removal of the foreign body or dislodgement of the impacted fleshy meat ball in to the stomach. We report a retrospective study of incident and types of impacted foreign body in the upper digestive tract. The study was carried out at a Tertiary Care Center at Gangtok, India, spread over 5 years. The review of the case details was tabulated and the incidence types and outcome was calculated to review the clinical condition and its management.

Introduction Foreign bodies are common in ENT practice universally. At times they may present as emergency requiring urgent intervention and many a times they go unnoticed as these are not suspected. Nasopharyngeal foreign bodies are rare in any age group. Case Report A 10 months old female child was brought with the history of persistent drooling of saliva and refusal to feed after accidental ingestion of a bunch of stapler pin. Plain radiograph showed a bunch of stapler pin in the nasopharynx with enlarged adenoid tissue and prevertebral soft tissue shadow. Conclusion Bunch of stapler pin is an unusual foreign body in the nasopharynx. Care should be taken while removing these foreign bodies.
