Linitis plastica is a rare variety of gastric cancer and is usually asymptomatic in early stages.... more Linitis plastica is a rare variety of gastric cancer and is usually asymptomatic in early stages. Often the presentation is late and is in the form of abdominal discomfort or gastrointestinal bleeding (with history of weight loss) or ascites. Here I report a case of linitis plastica where the patient presentation was that of intestinal obstruction. A patient with abdominal pain, vomiting and constipation, underwent surgery for suspected bowel obstruction. A subtotal colectomy with end ileostomy + mucus fistula formation was performed. The bowel specimen removed at surgery when sent for biopsy surprisingly revealed an adenocarcinoma with signet ring cell morphology. An oesophagogastroduodenoscopy was performed and biopsies taken from the stomach revealed that there was a primary tumour in the stomach - histologically linitis plastica.

Metabolism of a cancer cell is significantly different from that of a normal cell. Therefore the ... more Metabolism of a cancer cell is significantly different from that of a normal cell. Therefore the metabolites of a breast cancer cell are also different from the metabolites of a normal breast epithelial cell and identification of the altered metabolites in body fluids may help us identify the presense of cancer in the body. Metabolites produced by breast cancer cells can serve as potential biomarkers of breast cancer. In this study the breast cancer cell lines were cultured and the metabolites released into the culture media were obtained into three extracts. Liquid chromatography/mass spectrometry were then used to separate and analyze these metabolites. Data sets produced were aligned by mass lynx software and subsequently subjected to multivariate statistical analysis. Metabolites present in higher amounts in breast cancer cell lines were identified by comparison with the known masses in databases like METLIN. AmBisome [Injection] is a non-pyrogenic lyophilized product for intravenous infusion and each via... more AmBisome [Injection] is a non-pyrogenic lyophilized product for intravenous infusion and each vial contains amphotericin B [50 mg], intercalated into a liposomal membrane. The safety of Ambisome as prophylactic antifungal has been assessed in bone marrow transplant recepients, liver transplant recepients, renal transplant recepients and combined organ trans plant recepients. AmBisome was discontinued due to side-effects in 3% of the cases. In the current study involving small bowel transplant [SBT] patients it can be noted that all SBT patients tolerated the amBisome test dose well. However when intravenous infusion of amBisome was administered especially in neutropenic patients, it was noted that 3 patients (11%) developed a severe drug reaction and the drug had to be discontinued. The small bowel transplant patients in whom ambisome was discontinued there was a drug reaction which can be considered as a cumulative dose related reaction. It can be concluded that Ambisome is generally a safe drug for antifungal prophylaxis in small bowel transplant patients. The use of ambisome in neutropenic patients may amplify the cumulative dose reaction and may contribute to the picture that we have seen.

Background: All studies conducted to compare minimal access oesophagectomy (MAO) with open oesoph... more Background: All studies conducted to compare minimal access oesophagectomy (MAO) with open oesophagectomy (OO) till date, have the limitation that they are primarily based on non randomized studies. Only evidence based on randomised controlled trials (RC Ts) can establish MAO as the preferred surgery for resectable oesophageal cancer (OCA) and hence the current review.
Objectives: This review aims to establish a firm body of evidence in support of MAO. The objectives involve using PICO strategy and searching for relevant RCTs, extracting and analyzing data from them in order to derive conclusions that help establish
evidence in favour of MAO.
Data Sources: of the current review are RCTs that asess outcomes of MAO.
Review Methodology: Quantitave study has been designed through a systematic review and meta-analyses of RCTs.
Results: Blood loss during surgery, post pulmonary infection and duration of hospital stay favour MAO versus OO for resectable OCA management.
Conclusion:The ideal MAO strategy may involve thoracoscopic oesophagectomy in prone position with low tidal volume ventilation, perioperative administration of amino-acids and neutrophil elastase inhibitor plus immediate postoperative chest physio therapy and enteral feeding

Bile leakage in association with cholecystectomy is a well known complication. Common causes incl... more Bile leakage in association with cholecystectomy is a well known complication. Common causes include damage to the biliary ducts and slipping of clips/ties. Here I describe a rare case of bile leakage which resulted due to aberrant biliary anatomy. A thirty four year old lady undergoing laparoscopic cholecystectomy developed bile leakage. It was not clear where the bile was leaking from. Therefore an intraoperative cholangiogram was performed. We found an accessory hepatic duct in the liver of this patient was draining separately from the rest of the biliary system. The laparoscopic operation had to be converted to open cholecystectomy in order to control the bile leakage. It can be concluded that the anatomy of the biliary tree is highly variable and the importance of intra operative cholangiograms during cholecystectomy should be emphasized.

Appendicectomy is one of the most common surgical procedures performed. Published literature show... more Appendicectomy is one of the most common surgical procedures performed. Published literature shows that laparoscopic appendicectomy is a safe procedure. A 35 year old female came to the hospital complaining of abdominal pain for the past few hours. History and examination revealed a classical case of acute appendicitis. The patient underwent laparoscopic appendicectomy the same night. Immediate post operative period was uneventful and she was discharged. But one day after her discharge she returned with vomiting, abdominal pain, swelling and tightness in the abdominal wall. Investigations revealed necrotising fasciitis and pericarditis. This case report shows that Laparoscopic appendicectomy can lead to unusual complications such as necrotising fascitis and pericarditis which cause significant postoperative morbidity. Therefore it can be concluded that it would be useful to use prophylactic antibiotics routinely preoperatively in cases of seemingly uncomplicated appendicitis to prevent infections like necrotising fascitis. It would also be beneficial to do preoperative ECG in all patients of suspected appendicitis so as to get a baseline ECG in persons with no known comorbidities.

The patient had developed abdominal pain+vomiting along with swelling and tightness in the abdominal wall. Investigations revealed necrotising fasciitis and pericarditis. Laparoscopic appendectomy is considered to be a safe procedure in general. Infact nowadays almost all appendicectomies are laparoscopic appendicectomies because of the advantages associated with laparoscopic surgery. But it can be seen from this case that laparoscopic surgery can also cause morbidity due to complications such as intestinal obstruction, gas collection in abdominal muscles (necrotising fascitis) and a pericarditis.
