Health inequalities are rooted in inequality in vital resources for health, including nancial res... more Health inequalities are rooted in inequality in vital resources for health, including nancial resources, a supportive informal network, a stable living situation, work or daytime activities, or education and literacy. About 25% of Dutch citizens experience deprivation in such resources. Social policy consists of crucial instruments for improving resources in those groups, but can also have adverse effects and lead to additional burden. This project aims to contribute to the reduction of health inequalities through 1) better understanding how social policy interventions can contribute to reducing health inequality through the redistribution of burdens and resources, and 2) developing anticipatory governance strategies to implement those insights, contributing to a change in social policy systems. Two systems approaches are combined for establishing a systems change. Firstly, a realist approach enables insights into what in social policy interventions may impact health outcomes, for whom, and under what circumstances. Secondly, an institutional approach enables scaling up these insights, by acknowledging the crucial role of institutional actors for accomplishing a systems change. Together with stakeholders, we perform a realist review of the literature and identify existing promising social policy interventions. Next, we execute a realist evaluation of selected social policy interventions in seven municipalities, ranging from small, mid-size to large, and in both urban and rural settings. Simultaneously, through action research with (national) institutional actors, we facilitate development of anticipatory governance strategies. By combining a realist approach and an institutional approach, this study protocol not only sheds light on cross-domain impacts on health through realist review and evaluations, it also provides insight into how systems can be changed to bene t health, through action research. Thereby, it contributes to advancement of the concept "systems for health", relevant to both science and (policy)practice. The main ethical challenges of this project relate to the vulnerable positions of the populations under study. Extra attention is paid to prevent additional emotional burdening in the research process. We co-develop knowledge products and activities, with the targeted professionals, students, citizens and governments, to stimulate uptake of the knowledge produced in the project.

Als de stress hevig en chronisch is, kan dit flink doorwerken op ons gedrag. We gaan meer bij de dag leven, hebben meer moeite om verstandige beslissingen te nemen en om onze emoties en verlangens te reguleren. Het besef dat chronische stress negatief doorwerkt op het ‘doenvermogen’ van cliënten inspireert steeds meer organisaties om daar in hun hulp- en dienstverlening expliciet rekening mee te houden en erop aan te sluiten. Een van de manieren om daar invulling aan te geven is door te voorzien in psycho-educatie. Dit is een samenvoeging van de woorden psychotherapie en educatie en is erop gericht om cliënten meer inzicht te geven in de wijze waarop de door hen ervaren stress doorwerkt in hun (doelgerichte) gedrag, en om copingstrategieën te ontwikkelen die hen helpen om (lange) termijn doelen sneller en vaker te bereiken. Met opdracht van het programma Schouders Eronder heeft het lectoraat Schulden en Incasso van de Hogeschool Utrecht uitgewerkt hoe psycho-educatie bij de aanpak van financiële problemen vorm kan worden gegeven. In directe samenwerking met professionals en in indirecte samenwerking met cliënten is er een stappenplan en praatplaat ontwikkeld. De betrokken professionals werken bij drie organisaties die voorzien in ondersteuning bij financiële problemen:de regionale sociale diensten Avres en WIL Lekstroom en de gemeente Meppel. Na de ontwikkeling van de aanpak heeft het lectoraat Schulden en Incasso samen met lectoraat De Gezonde Stad van Hogeschool Windesheim onderzocht wat de toepassing van psycho-educatie volgens cliënten en professionals kan opleveren

Als dagloners, kan het vooruitzicht geëerd te zullen worden voor de rechterstoel van Christus met een bijzondere beloning, voor hen alleen maar een machtige aanmoediging zijn om trouw te blijven. Maar, meer nog dan aan hun eigen vreugde, moesten zij denken aan die van hun Heer, wanneer Hij in tegenwoordigheid van Myriaden heiligen, al de voldoening van Zijn hart zal uitdrukken, voor de trouw, die zij in hun leven op aarde getoond hebben. Dan zal de beproeving van hun geloof blijken te zijn tot lof en heerlijkheid en eer bij de openbaring van Jezus Christus (1 Petr. 1:7). De publicaties vragen aandacht voor de dynamiek van chronische stress die mensen als het ware gijzelt om hun situatie met succes te ontgroeien. De taaiheid van de problematiek en het niet behalen van de beleidsdoelen zijn dus niet eenvoudig toe te schrijven aan een tekort aan motivatie bij de doelgroep. De inzichten over de effecten van schaarste op het cognitieve functioneren vormen de basis van de aanpak Mobility Mentoring®. Deze aanpak uit de Verenigde Staten is ontwikkeld door de organisatie EMPath en heeft als doel om economische zelfredzaamheid te bevorderen. Om eerste beelden te verzamelen van het belang van een ontlokkende en vriendelijke inrichting, zi... more Om eerste beelden te verzamelen van het belang van een ontlokkende en vriendelijke inrichting, zijn er in het najaar van 2019 twee spreekkamers van de Jeugdgezondheidszorg opnieuw ingericht. Het team van Social Design Studio Joes + Manon heeft de inrichting ter hand genomen op de locaties De Gravin en F.C. Donderstraat. Het lectoraat schulden & Incasso van de Hogeschool Utrecht heeft vanuit haar ontwerpende rol in de aanpak van het stress-sensitief werken een vragenlijst voor een voor- en nameting ontwikkeld en de ingevulde vragenlijsten geanalyseerd. In het totaal zijn er 58 ouders bevraagd over de oude en 52 ouders over de nieuwe spreekkamer. Het resultaat van het onderzoek is dat de uitstraling van de spreekkamers op beide locaties significant positiever wordt beoordeeld na de herinrichting van de spreekkamers. Purpose Obesity and high physical workload are both associated with poor work ability, but the in... more Purpose Obesity and high physical workload are both associated with poor work ability, but the interaction between obesity and high physical workload on work ability is not yet fully understood. Obesity and high physical workload may share a common mechanical pathway, possibly leading to a synergistic negative effect on work ability. The purpose of this study was to investigate the effect of obesity on work ability in workers with high versus low physical work load. Methods A longitudinal study was conducted among 36,435 Dutch construction workers who participated in at least two periodic medical examinations during the years 2008-2015. Logistic regression analyses were used to investigate the effect of manual material handling and strenuous work postures in sports on the association between obesity and work ability. Work ability was measured using the self-reported Work Ability Index consisting of seven dimensions. Confounding effects were tested for age, educational level, smoking, vigorous physical activity, psychosocial work demands, and working hours. Additive interaction between obesity and physical workload on work ability was tested using the relative excess risk due to interaction (RERI). Results Construction workers with overweight (OR = 1.09; 95% CI 1.02-1.16) or obesity (OR = 1.27; 95% CI 1.17-1.38) had an increased risk of poor/moderate work ability. Exposure to manual material handling (OR = 1.58; 95% CI 1.49-1.68) or strenuous work postures (OR = 1.80; 95% CI 1.70-1.90) also increased the risk of poor/moderate work ability. The effect of the combination of obesity with high physical workload was greater than the sum of the individual effects (strenuous work postures: RERI = 0.39; 95% CI 0.10-0.67; manual material handling: RERI = 0.26; 95% CI 0.02-0.51). Conclusions Obesity and high physical workload were associated with poor work ability and had a synergistic, negative effect on work ability. Interventions that prevent obesity and high physical workload might have a beneficial effect on work ability.

However, it is unknown to what extent employees use implemented SE measures. OBJECTIVE: To determine the utilization of 1) SE measures offered by employers, 2) employee SE strategies, and 3) to identify barriers and facilitators of SE strategies. METHODS: Survey data were collected among 731 blue collar and 879 white collar workers to determine the utilization of employer SE measures. Focus groups were held with 16 blue collar and 17 white collar workers to identify employee SE strategies and their barriers and facilitators. RESULTS: Utilization of employer SE measures was highest for personal development measures. Strategies applied by blue collar workers included using equipment, suggesting improvements of their working conditions, and seeking promotion to a less physically demanding job. White collar workers named engaging in leisure time physical activity and seeking an adequate work-life balance. Implementation of these strategies was influenced by employee awareness and self-efficacy, the accessibility and costs and benefits of the strategy, management support and company culture. CONCLUSION: Usage of employer SE measures was generally low and recommendations are given for both blue and white collar workers to improve SE strategies.

Objective: To determine the measures employers in the construction industry take to promote susta... more Objective: To determine the measures employers in the construction industry take to promote sustainable employability, the barriers and facilitators that influence implementation, and employer needs. Results: Employers expressed a need for alternative jobs for workers who can no longer perform physically demanding tasks, as well as means to stimulate proactive employee behavior. Measures frequently targeted the work environment (95%) and employee health (79%), less frequently personal development (63%) and organization (65%). Implementation was influenced by economic factors, rules and regulations, client demands, employee demands, company vision, company culture and time/manpower/expertise. Implementation of measures aimed at reducing physical load and the promotion of personal development are needed.

The aim of this study was to monitor the national scale up of the effective lifestyle interventio... more The aim of this study was to monitor the national scale up of the effective lifestyle intervention Health Under Construction in the Dutch construction industry. Data were collected on seven process indicators, ie, reach, dose delivered, dose received, fidelity, competence, satisfaction, and barriers. The intervention reached 2.4% of the target group. Thirty-eight percent of the participants received five to seven consultations and 41% discussed all six intervention components. None of the counselors attained motivational interviewing proficiency. Participants perceived their counselor as competent and were satisfied with the intervention. Counselors were moderately satisfied with the intervention and experienced various barriers. Even though important conditions for scale up were met, the implementation was characterized by a low reach, a high drop-out rate, and a low quality of the counseling technique.

Adolescent mothers tend to be from disadvantaged backgrounds and raising children often interferes with their education and economic prospects. It is also related to depression, insecure attachment styles, external locus of control, and low self-efficacy. Furthermore, adolescent pregnancies are not without medical risk, for instance of early birth and perinatal mortality. Next to these negative consequences for teenage mothers, there may be a number of negative consequences for their children. There is evidence that these are likely to suffer numerous health and psychosocial disadvantages. In the Netherlands, birthrates among adolescents are available from 1950 onward. The highest birthrates were recorded in the late 1960s and early 1970s. At that time, the birthrate among girls under 20 years old was 23 per 1,000. Most of these births occur among 18 and 19-year-olds. In 2007, this was reduced to less than a quarter of that number. This decrease has drawn the attention of researchers and policy makers around the world. The Netherlands are often seen as a forerunner among Western countries with regard to reproductive health. Particularly an open sexual climate, easy accessibility of contraceptives, comprehensive sexuality education, and a nonjudgmental attitude toward young people’s sexuality are believed to contribute to low rates of unintended pregnancies.

Om eerste beelden te verzamelen van het belang van een ontlokkende en vriendelijke inrichting, zi... more Om eerste beelden te verzamelen van het belang van een ontlokkende en vriendelijke inrichting, zijn er in het najaar van 2019 twee spreekkamers van de Jeugdgezondheidszorg opnieuw ingericht. Het team van Social Design Studio Joes + Manon heeft de inrichting ter hand genomen op de locaties De Gravin en F.C. Donderstraat. Het lectoraat schulden & Incasso van de Hogeschool Utrecht heeft vanuit haar ontwerpende rol in de aanpak van het stress-sensitief werken een vragenlijst voor een voor- en nameting ontwikkeld en de ingevulde vragenlijsten geanalyseerd. In het totaal zijn er 58 ouders bevraagd over de oude en 52 ouders over de nieuwe spreekkamer. Het resultaat van het onderzoek is dat de uitstraling van de spreekkamers op beide locaties significant positiever wordt beoordeeld na de herinrichting van de spreekkamers. De uitstraling is de optelsom van hoe vriendelijk, veilig en verwelkomend de spreekkamer op mensen overkomt. Daarbij voelen mensen zich ook significant vrijer om te praten...

Obesity and high physical workload may share a common mechanical pathway, possibly leading to a synergistic negative effect on work ability. The purpose of this study was to investigate the effect of obesity on work ability in workers with high versus low physical work load. Methods A longitudinal study was conducted among 36,435 Dutch construction workers who participated in at least two periodic medical examinations during the years 2008-2015. Logistic regression analyses were used to investigate the effect of manual material handling and strenuous work postures in sports on the association between obesity and work ability. Work ability was measured using the self-reported Work Ability Index consisting of seven dimensions. Confounding effects were tested for age, educational level, smoking, vigorous physical activity, psychosocial work demands, and working hours. Additive interaction between obesity and physical workload on work ability was tested using the relative excess risk due to interaction (RERI). Results Construction workers with overweight (OR = 1.09; 95% CI 1.02-1.16) or obesity (OR = 1.27; 95% CI 1.17-1.38) had an increased risk of poor/moderate work ability. Exposure to manual material handling (OR = 1.58; 95% CI 1.49-1.68) or strenuous work postures (OR = 1.80; 95% CI 1.70-1.90) also increased the risk of poor/moderate work ability. The effect of the combination of obesity with high physical workload was greater than the sum of the individual effects (strenuous work postures: RERI = 0.39; 95% CI 0.10-0.67; manual material handling: RERI = 0.26; 95% CI 0.02-0.51). Conclusions Obesity and high physical workload were associated with poor work ability and had a synergistic, negative effect on work ability. Interventions that prevent obesity and high physical workload might have a beneficial effect on work ability.

However, it is unknown to what extent employees use implemented SE measures. OBJECTIVE: To determine the utilization of 1) SE measures offered by employers, 2) employee SE strategies, and 3) to identify barriers and facilitators of SE strategies. METHODS: Survey data were collected among 731 blue collar and 879 white collar workers to determine the utilization of employer SE measures. Focus groups were held with 16 blue collar and 17 white collar workers to identify employee SE strategies and their barriers and facilitators. RESULTS: Utilization of employer SE measures was highest for personal development measures. Strategies applied by blue collar workers included using equipment, suggesting improvements of their working conditions, and seeking promotion to a less physically demanding job. White collar workers named engaging in leisure time physical activity and seeking an adequate work-life balance. Implementation of these strategies was influenced by employee awareness and self-efficacy, the accessibility and costs and benefits of the strategy, management support and company culture. CONCLUSION: Usage of employer SE measures was generally low and recommendations are given for both blue and white collar workers to improve SE strategies.

Objective: To determine the measures employers in the construction industry take to promote susta... more Objective: To determine the measures employers in the construction industry take to promote sustainable employability, the barriers and facilitators that influence implementation, and employer needs. Results: Employers expressed a need for alternative jobs for workers who can no longer perform physically demanding tasks, as well as means to stimulate proactive employee behavior. Measures frequently targeted the work environment (95%) and employee health (79%), less frequently personal development (63%) and organization (65%). Implementation was influenced by economic factors, rules and regulations, client demands, employee demands, company vision, company culture and time/manpower/expertise. Implementation of measures aimed at reducing physical load and the promotion of personal development are needed.

The aim of this study was to monitor the national scale up of the effective lifestyle interventio... more The aim of this study was to monitor the national scale up of the effective lifestyle intervention Health Under Construction in the Dutch construction industry. Data were collected on seven process indicators, ie, reach, dose delivered, dose received, fidelity, competence, satisfaction, and barriers. The intervention reached 2.4% of the target group. Thirty-eight percent of the participants received five to seven consultations and 41% discussed all six intervention components. None of the counselors attained motivational interviewing proficiency. Participants perceived their counselor as competent and were satisfied with the intervention. Counselors were moderately satisfied with the intervention and experienced various barriers. Even though important conditions for scale up were met, the implementation was characterized by a low reach, a high drop-out rate, and a low quality of the counseling technique.

Adolescent mothers tend to be from disadvantaged backgrounds and raising children often interferes with their education and economic prospects. It is also related to depression, insecure attachment styles, external locus of control, and low self-efficacy. Furthermore, adolescent pregnancies are not without medical risk, for instance of early birth and perinatal mortality. Next to these negative consequences for teenage mothers, there may be a number of negative consequences for their children. There is evidence that these are likely to suffer numerous health and psychosocial disadvantages. In the Netherlands, birthrates among adolescents are available from 1950 onward. The highest birthrates were recorded in the late 1960s and early 1970s. At that time, the birthrate among girls under 20 years old was 23 per 1,000. Most of these births occur among 18 and 19-year-olds. In 2007, this was reduced to less than a quarter of that number. This decrease has drawn the attention of researchers and policy makers around the world. The Netherlands are often seen as a forerunner among Western countries with regard to reproductive health. Particularly an open sexual climate, easy accessibility of contraceptives, comprehensive sexuality education, and a nonjudgmental attitude toward young people’s sexuality are believed to contribute to low rates of unintended pregnancies.
