Findings: Following discussions at the Head and Neck MultiDisciplinary Team (MDT) meeting, this patient was TNM classified as T4N0M0 with Stage IV disease based on location. He subsequently underwent Right Segmental Mandibulectomy, Selective Neck Dissection and Free Fibula Flap reconstruction. Conclusion: This case highlights the discrepancies with the TNM staging and Tumour classification systems. Based on mere size of the tumour and depth of invasion of the cystic lining, this patient can be classified as T1N0M0 with Stage I disease. Follicular lymphoid hyperplasia is a very rare though benign reactive process of an unknown patho... more Follicular lymphoid hyperplasia is a very rare though benign reactive process of an unknown pathogenesis that may resemble a follicular lymphoma, clinically and histologically. Oral reactive follicular hyperplasia (RFH) has been described on the hard or soft palate and at the base of the tongue. We describe here the first case of RFH presenting as an aggressive tumor on the right posterior side of the maxilla in a 24-year-old male patient. The lesion had a clinical evolution of 18 months and was noticed after the surgical extraction of the right third molar, although we cannot assume a cause-effect relation with that surgical event whatsoever. His medical history was unremarkable. Following an incisional biopsy, histological examination revealed lymphoid follicles comprised by germinal centers surrounded by well-defined mantle zones. The germinal centers were positive for Bcl-6, CD10, CD20, CD21, CD23, CD79a, and Ki-67, while negative for Bcl-2, CD2, CD3, CD5, and CD138. The mantle and interfollicular zones were positive for Bcl-2, CD2, CD3, CD5, CD20, and CD138. Both areas were diffusively positive for kappa and lambda, showing polyclonality. The patient underwent a vigorous curettage of the lesion with no reoccurrences at 36 months of follow-up. This case report demonstrates that morphologic and immunohistochemical analyses are crucial to differentiate RFH from follicular lymphoma, leading to proper management.

The apurinic/apyrimidinic endonuclease 1 is a protein involved in DNA base excision repair and its expression still needs to be investigated in salivary gland tumors. The objective of this study is to analyze the immunoexpression of apurinic/apyrimidinic endonuclease 1 in pleomorphic adenomas and carcinomas ex pleomorphic adenomas of the salivary glands. A total of 33 pleomorphic adenomas and 16 carcinomas ex pleomorphic adenomas of the salivary glands underwent immunohistochemical study by the polymeric biotin-free technique. Immunopositive cells were analyzed quantitatively. For statistical analysis, Mann-Whitney test was performed and a significance level was set at p B 0.05. All analyzed tumors (n = 49) were positive for apurinic/apyrimidinic endonuclease 1. However, there was a higher median expression in carcinomas ex pleomorphic adenomas (p \ 0.001). There was no difference between this protein immunoexpression and tumors of major or minor salivary gland. Overexpression was found mainly in cases of carcinomas ex pleomorphic adenomas with lymph node metastasis (p = 0.002) and invasive growth (p = 0.003), when compared to cases without metastasis and without capsular invasion (intracapsular pattern). Our findings revealed that apurinic/apyrimidinic endonuclease 1 is downregulated in pleomorphic adenomas and overexpressed in carcinomas ex pleomorphic adenomas, suggesting that this protein is possibly deregulated in pleomorphic adenoma malignant transformation. Furthermore, the increased expression of this protein is associated with a more aggressive behavior in carcinomas ex pleomorphic adenomas, which suggests that this protein may represent a prognostic biomarker in the studied salivary gland tumors.

Abstract Objective The most common oral neurogenic lesions are traumatic neuroma, neurilemmoma, a... more Abstract Objective The most common oral neurogenic lesions are traumatic neuroma, neurilemmoma, and neurofibroma. They share clinical and histological features, but have a different prognosis and treatment. Our aim was to use a small panel of immunohistochemistry antibodies to distinguish among them. Methods Eight cases of traumatic neuroma, 9 cases of neurilemmoma, and 11 cases of neurofibroma were selected and stained with primary antibodies against S-100 protein, laminin, and epithelial membrane antigen (EMA) using the streptavidin-biotin-peroxidase method of immunohistochemistry. Results The S-100 protein was positive in the endoneurium of traumatic neuroma, Schwann cells, and Verocay bodies of the neurilemmomas and revealed a variable number of positive cells amongst spindle-shaped cells that compose neurofibromas. Laminin was positive in the endoneurium and in the perineurium of traumatic neuromas. In neurilemmomas, all neoplasic cells and the capsule were positive for laminin. For neurofibromas, laminin positivity followed the patterns observed for S-100 protein. EMA was positive in the perineurium of traumatic neuroma and in the capsule of neurilemmomas. Most cases of neurofibroma were completely negative for EMA, but three cases showed rare, scattered positive cells. Conclusion The use of this small immunohistochemical panel to evaluate the presence and localization of S-100 protein, laminin, and EMA can help to distinguish among these three lesions and provide a more specific diagnosis. Consequently, this will provide a better prognosis for patients.

Syphilis, a sexually transmitted infection, has increased in incidence over the years, especially... more Syphilis, a sexually transmitted infection, has increased in incidence over the years, especially among the adult population. The oral manifestation of primary syphilis is usually characterized by a clinical course variable in which it progresses to the next stages if not treated. In this report, we present two cases of oral manifestation of primary syphilitic lesion with peculiar clinical characteristics, and we emphasize the importance of a correct diagnosis. Biopsies were needed for differential diagnosis and serological exams were requested. The histopathological analysis suggested the diagnosis and the immunohistochemistry confirmed the presence of Treponema pallidum. Antibiotic therapy was initiated with complete remission of the lesion. Therefore, a thorough anamnesis and a thorough clinical examination are essential for establishing the correct diagnosis, to begin the treatment of syphilis as soon as possible, thus avoiding the disease’s progression.

Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare vascular proliferation of unknown pa... more Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare vascular proliferation of unknown pathogenesis that may be related to trauma. Although it affects mainly the head and neck, the zygomatic area is rarely involved. We report a case that affected the zygomatic region of a 46-year-old black man. The lesion had been present for about a year and the patient reported that it appeared after a facial injury during a soccer match. Clinical and tomographic investigations suggested a benign tumour, and the lesion was excised through an intraoral approach. Histopathological examination showed the unexpected diagnosis of ALHE. This tumour was interesting because of its rarity, and also because of its unusual site within the head and neck region. The diagnosis of ALHE is hardly ever considered in the differential diagnosis of zygomatic nodules.

Previous studies have shown their potential as biomarkers in oral squamous cell carcinoma (OSCC). Aiming to understand more clearly the function of MTs in OSCC we evaluated, for the first time, the gene expression profile of MTs in this neoplasm.Materials and resultsTissue samples from 35 cases of tongue and/or floor of mouth OSCC, paired with their corresponding non‐neoplastic oral mucosa (NNOM), were retrieved (2007–09). All tissues were analysed for the following genes using TaqMan® reverse transcription–quantitative polymerase chain reaction (RT–qPCR) assays: MT1A, MT1B, MT1E, MT1F, MT1G, MT1H, MT1X, MT2A, MT3 and MT4. The expression of MT1B and MT1H was seldom detected in both OSCC and NNOM. A significant loss of MT1A, MT1X, MT3 and MT4 expression and gain of MT1F expression was observed in OSCC, compared to NNOM. Frente à necessidade de utilização de animais vivos para as práticas de hemostasia, foram ministradas aulas usando animais da espécie Ga!!usga!!us (galinha doméstica), onde, treinou-se desde a anestesia (diazepam, xilazina e quetamina) à eutanásia. O reto do animal simulou o pedículo ovariano, ausente nesta espécie e, no membro, foi utilizado o torniquete, para realização de amputação. Outra didática utilizada foi à confecção de vídeos, ilustrando assuntos como: ambiente cirúrgico, profilaxia das infecções e algumas cirurgias, que foram mostrados durante as aulas teóricas da disciplina. O método empregado para treinamento da técnica hemos tática das três pinças, foi a utilização de bexigas vermelhas de látex número 00 acopladas a um equipo com soro fisiológico. A extremidade do equipo, contendo a bexiga, foi colocado sob tensão e passando para o interior de um cilindro de espuma que representava a cavidade. A realização das aulas práticas alternativas para o treinamento das principais técnicas cirúrgicas demonstrou ser de grande interesse e aprendizado para os alunos. Estes puderam participar mais efetivamente das aulas, sem o estresse e o medo que ocorrem quando se deparam logo na primeira prática com os animais vivos; treinar passo a passo, cada um dos assuntos que constam no plano de ensino da disciplina Técnica Cirúrgica Veterinária e aprender as diversas técnicas de diérese, excerese, hemostasia e síntese. Os resultados obtidos evidenciam que a corrente mundial de substituição de animais vivos, é viável, e não compromete o aprendizado.

Intense positivity of smooth muscle markers highlighted the surrounding vessel wall. Based on these features, a definitive diagnosis of intravascular fasciitis was rendered. Conclusions: Intravascular fasciitis has been infrequently reported in the oral cavity. The lesional cellularity and brisk mitotic activity, as seen in the family of pseudosarcomatous myofibroblastic proliferations, could lead to a misdiagnosis of sarcoma. Recognition of the typical histologic features of intravascular fasciitis is important for the practicing oral pathologist.

Background: The ability of coronavirus SARS-CoV-2 to spread is one of the determinants of the COV... more Background: The ability of coronavirus SARS-CoV-2 to spread is one of the determinants of the COVID-19 pandemic status. Until June 2020, global COVID-19 cases surpassed 10 million. Asymptomatic patients, with no respiratory impairment, are believed to be responsible for more than 80% of the transmission. Other viruses have been consistently detected in periodontal tissues. Objective: The aim of this study was to investigate the presence of SARS-CoV-2 in periodontal tissue. Methods: We conducted video-endoscope minimally invasive post-mortem biopsy in seven fatal cases of COVID-19, using a regular endoscope video system associated with a smartphone to locate periodontal tissue. We analyzed the samples using RT-PCR, to identify the SARS-CoV-2 RNA and histopathological analysis. Results: The seven studied autopsies with positive laboratory tests for COVID-19 included 57.14% of female patients at the average age of 47.4 (range 8 to 74). In five cases, periodontal tissue was positive for SARS-CoV-2 (RT-PCR). Histopathologic analyses showed morphologic alterations in the keratinocytes of the junctional epithelium, a vacuolization of the cytoplasm and nucleus and nuclear pleomorphism. Conclusion: We presented a biomolecular analysis obtained from minimally invasive autopsies. This is the first study to demonstrate the presence of SARS-CoV-2 in periodontal tissue in COVID-19 positive patients.

Heterotopic gastrointestinal mucosa (HGM) is a rare entity observed in the head and neck region a... more Heterotopic gastrointestinal mucosa (HGM) is a rare entity observed in the head and neck region and occurs more frequently in male infants and children. The floor of the mouth and anterior aspects of the tongue are the most commonly affected sites. Histologically, HGM resembles gastric, intestinal or colonic mucosa with areas of squamous epithelium, which can be cystic or solid. In the current report, 2 cases of HGM affecting the oral mucosa in patients over 35 years of age are presented, and one of these cases presented 2 nodules in different locations. Both cases represented solid lesions instead of the more common cystic presentation showing gastric glandular-type tissue that was lined with pseudostratified columnar epithelium containing crypts and fundic glands. Immunohistochemical analysis revealed positive expression of cytokeratins 7, 8 and 18 and smooth muscle actin (SMA). In both cases, the final diagnosis was HGM, and conservative surgical excision was performed.

We describe a previously unreported case of a combined cyst composed of glandular odontogenic cyst and ghost cell keratinization that was present for 33 years. We also reviewed the published 38 cases of glandular odontogenic cyst found in the world literature and add this additional case.

Purpose We aimed to compare the histomorphological features of oral lymphoepithelial cysts (OLC) ... more Purpose We aimed to compare the histomorphological features of oral lymphoepithelial cysts (OLC) in different locations of the oral cavity and to verify the association between oral lymphoepithelial cysts and subgemmal neurogenous plaque (SNP) on biopsies from the lateral border of the tongue. Methods All cases diagnosed as OLC from the Oral Pathology Service at the School of Dentistry/University of São Paulo were retrieved. For all the cysts located in the tongue, their association to SNP was analyzed. Immunohistochemical staining against S100 was performed to confirm the diagnosis of SNP. Results Thirty-one cases were selected for morphological analysis and SNP was identified in 40% of cases, all of them positive for S100. In these cases, macroscopic analysis demonstrated two independent fragments. Microscopic analysis showed that cysts located in the floor of the mouth and ventral surface of the tongue showed less intense exocytosis, less formation of lymphoid follicles, and predominantly a moderate inflammatory infiltrate. Conclusions OLC have a similar histological pattern regardless the location within the oral cavity and. In some cases, the biopsy specimen may be accompanied by a SNP as an adjacent independent structure.

Komercijalno čisti titanij (CP Ti) se koristi u dentalnoj medicini zbog biokompatibilnosti, dobri... more Komercijalno čisti titanij (CP Ti) se koristi u dentalnoj medicini zbog biokompatibilnosti, dobrih mehaničkih svojstava i otpornosti na koroziju. Konvencionalni proizvodni procesi izrade takvog titanija mogu utjecati na kvalitetu površine uzoraka i rezultirati slabim vezanjem CP Ti s dentalnom keramikom. Zato se uvode nove tehnologije proizvodnje titanija, primjerice metalurgija praha i oblikovanje na erozimatu s žicom (WEDM). Svrha ovog istraživanja jest odrediti utjecaj WEDM-a na površinu uzoraka P/M CP Ti proizvedenih za ispitivanje vezne čvrstoće prema normi ISO 9693. Materijali i metode: Osam uzoraka P/M CP Ti dimenzija prema normi ISO 9693 proizvedeno je korištenjem WEDM-a i podijeljeno u dvije grupe-u neobrađene i brušene. Površine obje grupe uzoraka analizirane su metodama SEM, EDS i XDR. Rezultati: Analize neobrađenih uzoraka metodama SEM i EDS pokazuju tanki sloj različitog sastava i frakture. Brušeni uzorci imaju homogenu strukturu bez fraktura. Analiza metodom XDR pokazuje visoku koncentraciju titanijevih oksida na površini neobrađenih uzoraka, a nakon brušenja dobivena je samo čista α-faza. Zaključak: WEDM je metoda prikladna za proizvodnju uzoraka prema normi ISO 9693, ako se uzorci naknadno bruse. The oral manifestation of primary syphilis is usually characterized by a clinical course variable in which it progresses to the next stages if not treated. In this report, we present two cases of oral manifestation of primary syphilitic lesion with peculiar clinical characteristics, and we emphasize the importance of a correct diagnosis. Biopsies were needed for differential diagnosis and serological exams were requested. The histopathological analysis suggested the diagnosis and the immunohistochemistry confirmed the presence of Treponema pallidum. Antibiotic therapy was initiated with complete remission of the lesion. Therefore, a thorough anamnesis and a thorough clinical examination are essential for establishing the correct diagnosis, to begin the treatment of syphilis as soon as possible, thus avoiding the disease’s progression.

Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare vascular proliferation of unknown pa... more Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare vascular proliferation of unknown pathogenesis that may be related to trauma. Although it affects mainly the head and neck, the zygomatic area is rarely involved. We report a case that affected the zygomatic region of a 46-year-old black man. The lesion had been present for about a year and the patient reported that it appeared after a facial injury during a soccer match. Clinical and tomographic investigations suggested a benign tumour, and the lesion was excised through an intraoral approach. Histopathological examination showed the unexpected diagnosis of ALHE. This tumour was interesting because of its rarity, and also because of its unusual site within the head and neck region. The diagnosis of ALHE is hardly ever considered in the differential diagnosis of zygomatic nodules.

Previous studies have shown their potential as biomarkers in oral squamous cell carcinoma (OSCC). Aiming to understand more clearly the function of MTs in OSCC we evaluated, for the first time, the gene expression profile of MTs in this neoplasm.Materials and resultsTissue samples from 35 cases of tongue and/or floor of mouth OSCC, paired with their corresponding non‐neoplastic oral mucosa (NNOM), were retrieved (2007–09). All tissues were analysed for the following genes using TaqMan® reverse transcription–quantitative polymerase chain reaction (RT–qPCR) assays: MT1A, MT1B, MT1E, MT1F, MT1G, MT1H, MT1X, MT2A, MT3 and MT4. Frente à necessidade de utilização de animais vivos para as práticas de hemostasia, foram ministradas aulas usando animais da espécie Ga!!usga!!us (galinha doméstica), onde, treinou-se desde a anestesia (diazepam, xilazina e quetamina) à eutanásia. O reto do animal simulou o pedículo ovariano, ausente nesta espécie e, no membro, foi utilizado o torniquete, para realização de amputação. Outra didática utilizada foi à confecção de vídeos, ilustrando assuntos como: ambiente cirúrgico, profilaxia das infecções e algumas cirurgias, que foram mostrados durante as aulas teóricas da disciplina. O método empregado para treinamento da técnica hemos tática das três pinças, foi a utilização de bexigas vermelhas de látex número 00 acopladas a um equipo com soro fisiológico. A extremidade do equipo, contendo a bexiga, foi colocado sob tensão e passando para o interior de um cilindro de espuma que representava a cavidade. A realização das aulas práticas alternativas para o treinamento das principais técnicas cirúrgicas demonstrou ser de grande interesse e aprendizado para os alunos. Estes puderam participar mais efetivamente das aulas, sem o estresse e o medo que ocorrem quando se deparam logo na primeira prática com os animais vivos; treinar passo a passo, cada um dos assuntos que constam no plano de ensino da disciplina Técnica Cirúrgica Veterinária e aprender as diversas técnicas de diérese, excerese, hemostasia e síntese. Os resultados obtidos evidenciam que a corrente mundial de substituição de animais vivos, é viável, e não compromete o aprendizado.

Intense positivity of smooth muscle markers highlighted the surrounding vessel wall. Based on these features, a definitive diagnosis of intravascular fasciitis was rendered. Conclusions: Intravascular fasciitis has been infrequently reported in the oral cavity. The lesional cellularity and brisk mitotic activity, as seen in the family of pseudosarcomatous myofibroblastic proliferations, could lead to a misdiagnosis of sarcoma. Recognition of the typical histologic features of intravascular fasciitis is important for the practicing oral pathologist.

Until June 2020, global COVID-19 cases surpassed 10 million. Asymptomatic patients, with no respiratory impairment, are believed to be responsible for more than 80% of the transmission. Other viruses have been consistently detected in periodontal tissues. Objective: The aim of this study was to investigate the presence of SARS-CoV-2 in periodontal tissue. Methods: We conducted video-endoscope minimally invasive post-mortem biopsy in seven fatal cases of COVID-19, using a regular endoscope video system associated with a smartphone to locate periodontal tissue. We analyzed the samples using RT-PCR, to identify the SARS-CoV-2 RNA and histopathological analysis. Results: The seven studied autopsies with positive laboratory tests for COVID-19 included 57.14% of female patients at the average age of 47.4 (range 8 to 74). In five cases, periodontal tissue was positive for SARS-CoV-2 (RT-PCR). Histopathologic analyses showed morphologic alterations in the keratinocytes of the junctional epithelium, a vacuolization of the cytoplasm and nucleus and nuclear pleomorphism. Conclusion: We presented a biomolecular analysis obtained from minimally invasive autopsies. This is the first study to demonstrate the presence of SARS-CoV-2 in periodontal tissue in COVID-19 positive patients.

Heterotopic gastrointestinal mucosa (HGM) is a rare entity observed in the head and neck region a... more Heterotopic gastrointestinal mucosa (HGM) is a rare entity observed in the head and neck region and occurs more frequently in male infants and children. The floor of the mouth and anterior aspects of the tongue are the most commonly affected sites. Histologically, HGM resembles gastric, intestinal or colonic mucosa with areas of squamous epithelium, which can be cystic or solid. In the current report, 2 cases of HGM affecting the oral mucosa in patients over 35 years of age are presented, and one of these cases presented 2 nodules in different locations. Both cases represented solid lesions instead of the more common cystic presentation showing gastric glandular-type tissue that was lined with pseudostratified columnar epithelium containing crypts and fundic glands. Immunohistochemical analysis revealed positive expression of cytokeratins 7, 8 and 18 and smooth muscle actin (SMA). In both cases, the final diagnosis was HGM, and conservative surgical excision was performed.

Purpose We aimed to compare the histomorphological features of oral lymphoepithelial cysts (OLC) ... more Purpose We aimed to compare the histomorphological features of oral lymphoepithelial cysts (OLC) in different locations of the oral cavity and to verify the association between oral lymphoepithelial cysts and subgemmal neurogenous plaque (SNP) on biopsies from the lateral border of the tongue. Methods All cases diagnosed as OLC from the Oral Pathology Service at the School of Dentistry/University of São Paulo were retrieved. For all the cysts located in the tongue, their association to SNP was analyzed. Immunohistochemical staining against S100 was performed to confirm the diagnosis of SNP. Results Thirty-one cases were selected for morphological analysis and SNP was identified in 40% of cases, all of them positive for S100. In these cases, macroscopic analysis demonstrated two independent fragments. Microscopic analysis showed that cysts located in the floor of the mouth and ventral surface of the tongue showed less intense exocytosis, less formation of lymphoid follicles, and predominantly a moderate inflammatory infiltrate. Conclusions OLC have a similar histological pattern regardless the location within the oral cavity and. In some cases, the biopsy specimen may be accompanied by a SNP as an adjacent independent structure.

Komercijalno čisti titanij (CP Ti) se koristi u dentalnoj medicini zbog biokompatibilnosti, dobri... more Komercijalno čisti titanij (CP Ti) se koristi u dentalnoj medicini zbog biokompatibilnosti, dobrih mehaničkih svojstava i otpornosti na koroziju. Konvencionalni proizvodni procesi izrade takvog titanija mogu utjecati na kvalitetu površine uzoraka i rezultirati slabim vezanjem CP Ti s dentalnom keramikom. Zato se uvode nove tehnologije proizvodnje titanija, primjerice metalurgija praha i oblikovanje na erozimatu s žicom (WEDM). Svrha ovog istraživanja jest odrediti utjecaj WEDM-a na površinu uzoraka P/M CP Ti proizvedenih za ispitivanje vezne čvrstoće prema normi ISO 9693. Materijali i metode: Osam uzoraka P/M CP Ti dimenzija prema normi ISO 9693 proizvedeno je korištenjem WEDM-a i podijeljeno u dvije grupe-u neobrađene i brušene. Površine obje grupe uzoraka analizirane su metodama SEM, EDS i XDR. Rezultati: Analize neobrađenih uzoraka metodama SEM i EDS pokazuju tanki sloj različitog sastava i frakture. Brušeni uzorci imaju homogenu strukturu bez fraktura. Analiza metodom XDR pokazuje visoku koncentraciju titanijevih oksida na površini neobrađenih uzoraka, a nakon brušenja dobivena je samo čista α-faza. Zaključak: WEDM je metoda prikladna za proizvodnju uzoraka prema normi ISO 9693, ako se uzorci naknadno bruse.
