Design/methodology/approach -The two meta-heuristic algorithms, tabu search and simulated annealing are hybridized using the properties of an optimal schedule identified in the existing literature to the problem. A lower bound is also suggested utilizing these properties. Finding -In a numerical experimentation with large size problems, the best-known heuristic algorithm to the problem is compared with the tabu search and simulated annealing algorithms. The study shows that the meta-heuristic algorithms outperform the heuristic algorithm. In addition, the developed meta-heuristics tend to be more robust against the problem-related parameters than the existing algorithm. Research limitations/implications -A future work may consider the possibility of machine failure along with the preventive maintenance. This relaxes the assumption that the machine cannot fail but it is rather maintained preventively. The multi-criteria scheduling can also be considered as an avenue of future work. The problem can also be considered with stochastic parameters such that the processing times of the jobs and the maintenance related parameters are random and follow a known probability distribution function. Practical implications -The usefulness of meta-heuristic algorithms is demonstrated for solving a large scale NP-hard combinatorial optimization problem. The paper also shows that the utilization of the directed search methods such as hybridization could substantially improve the performance of a meta-heuristic. Originality/value -This research highlights the impact of utilizing the directed search methods to cause hybridization in meta-heuristic and the resulting improvement in their performance for large-scale optimization.

Contrary to this belief, classical diphtheria continues to exist in many parts of India despite extensive immunizations campaigns raising the suspicion of resurgence. The absence of essential drugs in treatment of this condition has further worsened the situation. The expenditure involved in procuring these drugs is yet another constraint. We here by present one such case of adult diphtheria and discuss the problems associated with its diagnosis and treatment.

It is not yet established that both conditions are determinants of psychomotor development. The study investigated whether or not low birth weight leads to delayed psychomotor development of a child, and whether it can be mitigated by adequate postnatal growth. A cross-sectional study was conducted in 2002 in 15 rural and 11 urban communities of Sindh province, Pakistan. Assessment of 1234 children less than 3 years of age included Bayley's Scale of Infant Development II, socioeconomic questionnaire and anthropometry; WHO standards were used to calculate z-scores of height-for-age, weight-for-height and weight-for-age. The underlying study hypotheses were tested through multiple regression modelling. Out of 1219 children, 283 (23.2%) had delayed psychomotor development and 639 (52.4%) were undernourished according to the composite index of anthropometric failure. Strong negative associations with the psychomotor development index were detected between stunting and being underweight, with a larger magnitude of effect for stunting (p<0.001). The strong relationship persisted even when the analysis was restricted to non-malnourished children. The psychomotor index increased by 2.07 points with every unit increase in height-for-age z-score. The relationship between low birth weight and psychomotor development appears to be mediated largely by postnatal growth and nutritional status. This association suggests that among undernourished children there is significant likelihood of a group that is developmentally delayed. It is important to emphasize developmental needs in programmes that target underprivileged children.

Patients with hypothyroidism taking thyroxine can take their tablets on an empty stomach at bedtime instead of half an hour before Sehr. Patients with hyperthyroidism, on methimazole/carbimazole can continue their dose in once or twice daily regimes, while those on propylthiouracil need to be switched. Hyperthyroid patients with severe symptoms should start treatment immediately and can avoid fast for few days after a consultation with their religious scholar.
Subclinical hypothyroidism: Controversies to consensus

SCH is diagnosed after excluding all other causes of elevated TSH levels. Symptoms of SCH may vary from being asymptomatic to having mild nonspecific symptoms. The risk of progression to overt hypothyroidism is related to number of factors including initial serum TSH concentration, presence of auto antibodies, family history and presence goiter. Various screening recommendations for thyroid function assessment are in practice. There are still controversies surrounding SCH and associated risk of various cardiovascular diseases (CVDs), pregnancy outcomes, neuropsychiatric issues, metabolic syndrome, and dyslipidemia. Consensus will require more large randomized clinical studies involving various age groups and medical condition, especially in developing countries. All these efforts will definitely improve our understanding of disease and ultimately patient outcomes.
Life-saving systemic thrombolysis in a patient with massive pulmonary embolism and a recent hemorrhagic cerebrovascular accident

Current practice guidelines include the immediate administration of thrombolytic therapy in the absence of contraindications. However, thrombolysis for pulmonary embolism is said to be absolutely contraindicated in the presence of recent hemorrhagic stroke and other conditions. The current contraindications to thrombolytic therapy have been extrapolated from data on acute coronary syndrome and are not specific for venous thromboembolic disease. Some investigators have proposed that the current contraindications be viewed as relative, rather than absolute, in cases of high-risk pulmonary embolism. We present the case of a 60-year-old woman in whom massive pulmonary embolism led to cardiac arrest with pulseless electrical activity. Eight weeks earlier, she had sustained a hemorrhagic cerebrovascular accident-a classic absolute contraindication to thrombolytic therapy. Despite this practice guideline, we administered tissue plasminogen activator systemically in order to save the patient's life. This therapy did not evoke intracranial bleeding, and the patient was eventually discharged from the hospital. Until guidelines specific to venous thromboembolic disease are developed, we think that the current contraindications to thrombolysis should be considered on an individual basis in patients who are at high risk of death from massive pulmonary embolism.

This amount may result in significant health, environmental, aesthetic, land-use resources, and economic concerns if not managed properly. Waste to Energy (WTE) is a viable option for disposal of MSW and energy generation. This paper presents a brief review of WTE technologies; reviews the current market status of the WTE; and forecasts the potential global annual investment in this sector. The research findings show that incineration is the dominant technology utilized to date. Advanced technologies such as Plasma Arc Gasification are gaining momentum in developed countries, whereas biomethanation technology is expanding at a high rate in developing countries in the last decade. Globally, WTE will play a significant role in minimizing MSW challenges and will contribute to the emerging renewable energy market in the near future.

The properties of an optimal schedule are identified and utilized to develop a constructive heuristic and a lower bound estimate. The properties are also utilized to hybridize Tabu search and Simulated Annealing algorithms. A numerical study with over 3200 randomly generated problems is reported to demonstrate the performance of the proposed solution methods. The study shows that the effectiveness of the proposed lower bound and constructive heuristic is sensitive to maintenance related parameters. We also show that hybridized Tabu search and Simulated Annealing algorithms are efficient approaches to solve the problem.

The proposed methods employ Direct and Parallel QR decomposition for tall and skinny matrix where many more rows than columns are involved. Data matrix of deterministic and stochastic system has been used. The proposed methods compared to a standard QR decomposition show a significant reduction in the computational time and the complexity of the system in terms of number of operations and memory storage. The system matrices are identified by making use of Kalman filter states and Canonical Variate Algorithm (CVA). The simulation results illustrate that proposed methods require less processing time and low complexity, and provide high accuracy in identifying the system parameters compared to the standard QR decomposition.

Growth regulators, naphthalene acetic acid, gibberellic acid, and 2, 4-dichloro phenoxy acetic acid at the rates of (15, 25, and 35 ppm) were sprayed at full bloom stage on a popular mango cultivar ‘Dusehri’ growing in the subtropical region of Faisalabad in Pakistan. All of the treatments significantly influenced the number of fruits retained at pea, marble, and at harvest stages of fruit growth as compared to the control. Among the treatments, gibberellic acid sprayed at 35 ppm proved to be very effective with 31% more fruit retention than the control. In regard to fruit quality, gibberellic acid (35 ppm) and 2, 4-dichloro phenoxy acetic acid (15 ppm) resulted in a significant enhancement of fruit size in terms of fruit weight (140.8 and 130.1 g), which was 17.6 and 8.7% that of the control (119.8 g), respectively. The treatment 2, 4-D (35 ppm) recorded significantly higher TSS (19.5 °Brix) and TSS to titratable acid ratio over the control. Ascorbic acid content was significantly higher with 35 ppm NAA (61.9 mg/100 ml), while total sugars (20.4%) were significantly higher in 2, 4-D (35 ppm) than the control (11.4%). In general, application of growth regulators resulted in improved fruit retention and fruit quality.
