ARAŞ TIRMA ÖZET Ösefagusun geriye dönüşsüz bir şekilde hasar görerek pasajı engellediği durumlard... more ARAŞ TIRMA ÖZET Ösefagusun geriye dönüşsüz bir şekilde hasar görerek pasajı engellediği durumlarda, ösefagus replasmanı amacıyla mide, jejunum, ileum ve kolon segmentleri kullanılabilmektedir. Bu tekniklerden kolon interpozisyonu en sık tercih edilen yöntemdir. Greftlerin kullanıldığı tüm diğer cerrahi yöntemler gibi ösefagus replasmanı amaçlı kullanılan kolon greftlerinde de reperfüzyon hasarı belirli oranda gelişmektedir. Eritropoetin hormonunun iskemi/reperfüzyonda dokuyu koruyucu bir etki gösterebileceği bilinmektedir. Bu çalışmada ratlarda, sol kolon grefti oluşturulduktan sonra greftin uç kısımlarında meydana gelen mukozal hücrelerdeki iskemik değişiklikler ile intraperitoneal eritropoetin uygulamasının bu değişiklikler üzerine olan etkileri araştırılmıştır. Toplam 56 denek rastgele Sham (n=14), Sham+eritropoetin (n=14), kolon grefti (n=14), kolon grefti+eritropoetin (n=14) olmak üzere dört ana gruba, her grup da kendi içinde 2 alt gruba ayrıldı. Birinci alt gruba 1 gün sonra, 2. alt gruba ise 7 gün sonra reoperasyon sonrası histopatolojik inceleme yapıldı. Kolon grefti ve kolon grefti+eritropoetin grupları arasında postoperatif 1. gün ve 7. gün proksimal uçlarının değerlendirilmesinde mukozal hasarın kolon grefti grubunda anlamlı olarak daha yüksek olduğu görüldü. Bu çalışma, preoperatif intraperitoneal eritropoetin uygulamasının, kolon grefti hazırlanmış ratlarda mukozal hasarlanmayı belirgin olarak azalttığını göstermektedir. Of these techniques colon interposition is the most preferred method. Reperfusion damage develops at a certain rate in colon grafts used for esophageal replacement as with all other surgical methods where grafts are used. It is known that erythropoietin hormone may have a tissue-protective effect in ischemia/ reperfusion. Ischemic changes developing in mucosal cells in the graft ends after creating a left colon graft in rats and the effect of intraperitoneal erythropoietin administration on these changes were investigated in the present study. A total of 56 subjects were randomly divided into four main groups of Sham (n=14), Sham+erythropoietin (n=14), colon graft (n=14) and colon graft+erythropoietin (n=14), and each group was divided into 2 subgroups. Histopathological examination was performed to the first subgroup one day and to the second subgroup 7 days after reoperation. There was significantly much more mucosal damage in the colon graft group in comparison to the postoperative day 1 and 7 proximal ends between the colon graft and colon graft+erythropoietin groups. Results of the present study showed that intraperitoneal erythropoietin administration significantly decreased mucosal damage in rats in which a colon graft was prepared.

It is commonly found in children and young adolescents as a slowly-growing nodule, mostly in the head and neck regions. It is relatively less common in the upper extremities. Pilomatrixomas are usually asymptomatic, solitary, hard, and mobile dermal and subcutaneous nodules and are mostly misdiagnosed despite being commonly seen in the practise. Most are smaller than 3 cm and usually around 1 cm in size while those larger than 5 cm are called pilomatrixoma and are less common. We present a seven-year-old girl with a giant pilomatrixoma on the upper arm in this study.

We reviewed the clinical signs and treatment of the congenital neck mass in children evaluated in our department. Material and Method: Hospital records of 63 patents admitted to our department between 1998 and 2008 with congenital cysts and fistulas of the neck were reviewed retrospectively, in terms of patient demographics, signs and symptoms, treatment modalities and therapy results. Results: Of the children, 38 were boys and 25 were girls. The mean age of children was 5.3 years (range; 0 day-14 years). Cases were classified as having 43 (% 68) midline and 20 (%32) lateral neck masses. The most frequent mass was thyroglossal duct cyst (n=19, %30), followed by fistula of the second branchial arch anomalies (n=14, %22). It is seen at a rate of 0.6-3.0% in prepubertal girls and the incidence peaks between the 12th and 23rd months. It is usually asymptomatic but can rarely cause problems such as dysuria, recurrent vulvar and urinary system infections, or difficulty urinating with wetting of underwear following urination. No treatment is needed for asymptomatic cases and those with small partial adherence while symptomatic or recurrent cases need medical or surgical treatment. The initial treatment is locally with ointments containing estrogen or low-dose cortisone. Manual separation is adequate in some cases not responding to this treatment while others require topical estrogen or low-dose cortisone ointment application for 2-3 weeks prophylactically following manual or surgical separation. Anorectal malformations are rare occurrences characterized by the absence or abnormal localizatio... more Anorectal malformations are rare occurrences characterized by the absence or abnormal localization of the anus. Clinical manifestations can vary from mild forms that require only minor surgery to more complicated cases that must be managed with multi-staged surgery. In this report, our aim is to present the clinical characteristics, management, and treatment outcome of an adult patient with an anorectal malformation with a vestibular fistula that was successfully repaired by posterior sagittal anorectoplasty (PSARP) and to discuss the case in the light of the relevant literature. We also would like to underline the fact that primary anorectal malformations can, although rarely, be observed in adult patients. A 26-year-old female patient presented with an imperforate anus and constipation with feces passing through a fistula located at the distal part of her vagina. She was continent for solid feces, but had leakage of flatus and fecal soiling. She also had concurrent left renal agenesis. A posterior sagittal anorectoplasty was successfully performed without a colostomy. The patient was discharged uneventfully on the postoperative 8th day. The patient was fully continent during a control visit after seven years of follow-up. Posterior sagittal anorectoplasty is a technique that can be successfully used in adult patients with anorectal malformations.

Although these cysts are mostly asymptomatic, they may cause various clinical pictures according to the dimension, localization, and content of the cyst and whether it is complicated. Children may therefore present with symptoms of abdominal pain, nausea, vomiting, constipation, diarrhea, weight loss, abdominal mass, distention, and may also suffer from various findings related to the cyst such as torsion, infection, hemorrhage and rupture. We present a mesenteric cyst case originating from the descending and sigmoid colon mesentery that presented with symptoms of fever, abdominal pain and diarrhea considered to be due to the infection of the cyst in the descending colon.

We present a case of venobiliary fistula due to umbilical venous catheter (UVC). UVC was inserted... more We present a case of venobiliary fistula due to umbilical venous catheter (UVC). UVC was inserted the day before surgery in a newborn who was scheduled for type IIIB jejunal atresia surgery. The UVC was superimposed on the liver. It was noted that the gastric drainage became chylous and increased to 790 and then 1977 mL daily. I.v. contrast tomography with 650 mL contrast showed that the opaque substance was dispersed around the catheter and a venobiliary fistula formed, with the administered fluid accumulating in the duodenum. Rapid improvement was seen in the clinical picture after the UVC was removed. Venobiliary fistula may develop in patients with UVC that is not placed appropriately, and can direct the fluid administered from the UVC to the gastrointestinal system through the choledochal duct. The importance of contrast computed tomography in the diagnosis of venobiliary fistula in the newborn is also emphasized.

Özellikle peripubertal çağdaki erkek çocuklarda, skrotal ağrı ve kitle yakınmasıyla ortaya çıkabileceği gibi, fizik muayene esnasında tesadüfen de tanı konabilir. Skrotal anomalisi olan çocuklarda yaşa bağlı olarak %5-14 oranında görülür. Etiyolojisi tam olarak bilinmese de, anneleri gebeliğinde dietilstilbestrol almış gençlerde daha sık (%20) görülür. Tanısı fizik muayene ve ultrasonografi ile konmaktadır. Olguların çoğunda kistler kendiliğinden gerilediğinden çok büyük ve ağrıya neden olmadıkları takdirde izlenmelerinde sakınca yoktur. Ancak, ağrıya neden olan kistlerin eksize edilmesi gerekebilir. Bu çalışmada sol skrotumda kitle ve ağrı yakınmasıyle başvuran ve 6 aylık izlem sonrasında ağrı yakınması geçmediği için eksizyon uygulanan 13 yaşındaki bir epididim kisti olgusu sunulmuştur.

Surgical treatment of the complications associated with Meckel's diverticulum is widely accepted,... more Surgical treatment of the complications associated with Meckel's diverticulum is widely accepted, but surgical management of an asymptomatic diverticulum found incidentally during laparotomy for other reasons is controversial. Appendectomy is the most common reason for laparotomy in many pediatric surgery clinics. The most common causes of an intestinal obstruction in the early period after appendectomy are an intra-abdominal abscess, adhesive small bowel obstruction and invagination. Obstruction due to Meckel's diverticulum is reported rarely. In this study, we present 2 cases, five and eleven years old, with an asymptomatic Meckel's diverticulum where appendectomy was performed due to acute and perforated appendicitis and obstruction developed due to Meckel's diverticulitis in the postoperative early stage. Meckel's diverticulitis must be considered in intestinal obstruction cases developing in the early stages after appendectomy although quite rare.

There was significantly much more mucosal damage in the colon graft group in comparison to the postoperative day 1 and 7 proximal ends between the colon graft and colon graft+erythropoietin groups. Results of the present study showed that intraperitoneal erythropoietin administration significantly decreased mucosal damage in rats in which a colon graft was prepared.

We reviewed the clinical signs and treatment of the congenital neck mass in children evaluated in our department. Material and Method: Hospital records of 63 patents admitted to our department between 1998 and 2008 with congenital cysts and fistulas of the neck were reviewed retrospectively, in terms of patient demographics, signs and symptoms, treatment modalities and therapy results. Results: Of the children, 38 were boys and 25 were girls. The mean age of children was 5.3 years (range; 0 day-14 years). Cases were classified as having 43 (% 68) midline and 20 (%32) lateral neck masses. Clinical manifestations can vary from mild forms that require only minor surgery to more complicated cases that must be managed with multi-staged surgery. In this report, our aim is to present the clinical characteristics, management, and treatment outcome of an adult patient with an anorectal malformation with a vestibular fistula that was successfully repaired by posterior sagittal anorectoplasty (PSARP) and to discuss the case in the light of the relevant literature. We also would like to underline the fact that primary anorectal malformations can, although rarely, be observed in adult patients. A 26-year-old female patient presented with an imperforate anus and constipation with feces passing through a fistula located at the distal part of her vagina. She was continent for solid feces, but had leakage of flatus and fecal soiling. She also had concurrent left renal agenesis. A posterior sagittal anorectoplasty was successfully performed without a colostomy. The patient was discharged uneventfully on the postoperative 8th day. The patient was fully continent during a control visit after seven years of follow-up. Posterior sagittal anorectoplasty is a technique that can be successfully used in adult patients with anorectal malformations.

Although these cysts are mostly asymptomatic, they may cause various clinical pictures according to the dimension, localization, and content of the cyst and whether it is complicated. Children may therefore present with symptoms of abdominal pain, nausea, vomiting, constipation, diarrhea, weight loss, abdominal mass, distention, and may also suffer from various findings related to the cyst such as torsion, infection, hemorrhage and rupture. We present a mesenteric cyst case originating from the descending and sigmoid colon mesentery that presented with symptoms of fever, abdominal pain and diarrhea considered to be due to the infection of the cyst in the descending colon.

UVC was inserted the day before surgery in a newborn who was scheduled for type IIIB jejunal atresia surgery. The UVC was superimposed on the liver. It was noted that the gastric drainage became chylous and increased to 790 and then 1977 mL daily. I.v. contrast tomography with 650 mL contrast showed that the opaque substance was dispersed around the catheter and a venobiliary fistula formed, with the administered fluid accumulating in the duodenum. Rapid improvement was seen in the clinical picture after the UVC was removed. Venobiliary fistula may develop in patients with UVC that is not placed appropriately, and can direct the fluid administered from the UVC to the gastrointestinal system through the choledochal duct. The importance of contrast computed tomography in the diagnosis of venobiliary fistula in the newborn is also emphasized.

Özellikle peripubertal çağdaki erkek çocuklarda, skrotal ağrı ve kitle yakınmasıyla ortaya çıkabileceği gibi, fizik muayene esnasında tesadüfen de tanı konabilir. Skrotal anomalisi olan çocuklarda yaşa bağlı olarak %5-14 oranında görülür. Etiyolojisi tam olarak bilinmese de, anneleri gebeliğinde dietilstilbestrol almış gençlerde daha sık (%20) görülür. Tanısı fizik muayene ve ultrasonografi ile konmaktadır. Olguların çoğunda kistler kendiliğinden gerilediğinden çok büyük ve ağrıya neden olmadıkları takdirde izlenmelerinde sakınca yoktur. Ancak, ağrıya neden olan kistlerin eksize edilmesi gerekebilir. Bu çalışmada sol skrotumda kitle ve ağrı yakınmasıyle başvuran ve 6 aylık izlem sonrasında ağrı yakınması geçmediği için eksizyon uygulanan 13 yaşındaki bir epididim kisti olgusu sunulmuştur.

Of these techniques colon interposition is the most preferred method. Reperfusion damage develops at a certain rate in colon grafts used for esophageal replacement as with all other surgical methods where grafts are used. It is known that erythropoietin hormone may have a tissue-protective effect in ischemia/ reperfusion. Ischemic changes developing in mucosal cells in the graft ends after creating a left colon graft in rats and the effect of intraperitoneal erythropoietin administration on these changes were investigated in the present study. A total of 56 subjects were randomly divided into four main groups of Sham (n=14), Sham+erythropoietin (n=14), colon graft (n=14) and colon graft+erythropoietin (n=14), and each group was divided into 2 subgroups. Histopathological examination was performed to the first subgroup one day and to the second subgroup 7 days after reoperation. Surgical treatment of the complications associated with Meckel's diverticulum is widely accepted,... more Surgical treatment of the complications associated with Meckel's diverticulum is widely accepted, but surgical management of an asymptomatic diverticulum found incidentally during laparotomy for other reasons is controversial. Appendectomy is the most common reason for laparotomy in many pediatric surgery clinics. The most common causes of an intestinal obstruction in the early period after appendectomy are an intra-abdominal abscess, adhesive small bowel obstruction and invagination. Obstruction due to Meckel's diverticulum is reported rarely. In this study, we present 2 cases, five and eleven years old, with an asymptomatic Meckel's diverticulum where appendectomy was performed due to acute and perforated appendicitis and obstruction developed due to Meckel's diverticulitis in the postoperative early stage. Meckel's diverticulitis must be considered in intestinal obstruction cases developing in the early stages after appendectomy although quite rare.
