In this study, we evaluated and described the findings of the Doppler ultrasound of the peripheral arteries performed at Aminu Kano Teaching Hospital (AKTH), Kano, Nigeria. All the findings of peripheral arterial Doppler ultrasound examinations performed at AKTH during a period of 18 months (from February 2012 to July 2013) were reviewed. All examinations were done using 7.5 MHz linear transducer connected to Mindary Digital Ultrasound Imaging System (Model DC-6; Shenzhen Mindray Biomed Electronics, Shenzhen, China). A 3.5 MHz convex transducer of the same machine was used in obese patients and those with severe subcutaneous oedema. The findings of 50 males and 28 females were reviewed. Background: Strokes in children with sickle cell anemia (SCA) are associated with significant mor... more Background: Strokes in children with sickle cell anemia (SCA) are associated with significant morbidity and premature death. Primary stroke prevention in children with SCA involves screening for abnormal transcranial Doppler (TCD) velocity coupled with regular blood transfusion therapy for children with abnormal velocities, for at least one year. However, in Africa, where the majority of children with SCA live, regular blood transfusions are not feasible due to inadequate supply of safe blood, cost, and the reluctance of caregivers to accept transfusion therapy for their children. Objectives: We describe the Primary Prevention of Stroke in Children with Sickle Cell Disease in Nigeria Trial [Stroke Prevention in Nigeria (SPRING) trial, NCT02560935], a three-center partially-blinded randomized controlled Phase III clinical trial to 1) determine the efficacy of moderate fixed-dose (20 mg/kg/day) versus low fixed-dose (10 mg/kg/day) hydroxyurea therapy for primary stroke prevention; 2) determine the efficacy of moderate fixed-dose hydroxyurea for decreasing the incidence of SCA-related hospitalization (pain, acute chest syndrome, infection, other) compared to low fixed-dose hydroxyurea. Design/Methods: We will test the primary hypothesis that there will be a 66% relative risk reduction of strokes in children with SCA and abnormal TCD measurements, randomly allocated, for a minimum of three years to receive moderate fixed-dose versus low fixed-dose hydroxyurea (total n=220). Results: The results of this trial will advance the care of children with SCA in Nigeria and sub-Saharan Africa, while improving research capacity for future studies to prevent strokes in children with SCA.

OBJECTIVE To retrospectively audit and analyse all the Transfontanelle Neurosonographic scans per... more OBJECTIVE To retrospectively audit and analyse all the Transfontanelle Neurosonographic scans performed at the Aminu Kano Teaching Hospital (AKTH), between January to September, 2004. METHODS Information on the gray scale appearance of the ventricles, cerebral and cerebellar parenchyma, extra axial fluid spaces, status of the sulci and gyri and the echogenic internal outline of the bony skull vault was extracted from the records of all the Transfontanelle neurosonographic scans performed at the specialist Ultrasound clinic of the AKTH, between January to September, 2004. RESULTS Eighteen infants were scanned, and 8 (44.4%) of them were neonates. Meningitis (9 patients) and increasing head size (7 patients) were the commonest clinical indication for referral. Communicating hydrocephalus was the only known complication seen in 4(44.4%) of the post meningitic patients. BACKGROUND Crossed fused renal ectopia is a relatively rare condition which may remain undiagnose... more BACKGROUND Crossed fused renal ectopia is a relatively rare condition which may remain undiagnosed for a long time. Renal function is usually preserved. It usually involves the left kidney. It is usually associated with mal-rotation, and may be a cause of urinary tract infection, hydronephrosis and renal calculi. MATERIAL AND METHOD We report a case of a 15-year old boy who was referred to our clinic with complaints of abdominal pain, and an ultrasound report of "absent right kidney". RESULTS Urine examination indicated a partially treated infection, but the tests of kidney functions were normal. Intravenous urography confirmed crossed fused renal ectopia, and a flush aortogram showed two anomalous right renal arteries arising from the left side of aorta. CONCLUSION An "absent" kidney in its normal location should prompt further investigations. In sub-Saharan Africa monthly blood transfusions for primary stroke prevention carries risks; hydroxyurea (HU) may be an alternative. We conducted the first, US NIH funded, SCD feasibility trial in sub-Saharan Africa (5R21NS080639-02) to: 1) assess the acceptability and willingness of families to participate in a HU trial; 2) develop a safety protocol for using HU in a trial setting in sub-Saharan Africa; and 3) prepare for a definitive phase III Trial. In the Sickle Cell Disease Stroke Prevention in Nigeria (SPIN) trial, our primary hypothesis in the internal feasibility trial is that 80% adherence for daily HU administration is feasible. In Kano, Nigeria, we conducted a feasibility trial to determine the acceptability of hydroxyurea therapy for primary stroke prevention in children with abnormal transcranial Doppler (TCD) measurements. Children with SCA and abnormal non-imaging TCD measurements (≥ 200 cm/s) received moderate fixed-dose hydroxyurea therapy (∼20 mg/kg/day). A comparison group of children with TCD measurements < 200 cm/s was followed prospectively. Approximately 90% (337 of 375) of families agreed to be screened, while 92% (25 of 27) of those with abnormal TCD measurements, on two separate occasions, enrolled in the trial. No participant elected to withdraw from the trial. The average mean corpuscular volume increased from 85 fL at baseline to 101.3 fL at 24 months, demonstrating adherence to hydroxyurea. It kills an estimated 2-3 million people a year with 95-98% of this mortality in developing countries. Nigeria ranks 4 th among 22 high burden countries in the world. It is associated with marked weight loss which can be evident at the lateral chest wall soft tissue thickness on the chest radiographs. Objectives: To compare the lateral chest wall soft tissue thickness on the chest radiographs of normal adult subjects and pulmonary TB patients in Zaria, also to determine sex difference in the lateral chest wall thickness in aforementioned subjects. Methodology: This prospective study was conducted over a six month period at the Radiology Department of Ahmadu Bello University Teaching Hospital, Zaria. Two hundred adults who were bacteriologically proven to have pulmonary tuberculosis were consecutively recruited into the study as cases. The same number of age and sex matched healthy individuals that had chest radiograph grossly normal were recruited as controls. Their lateral chest wall soft tissue thickness (subcutaneous fat and muscle layers) on the chest radiographs was measured. Results: A total of 400 patients, aged 18 to 70 years. Average age was 39.12 ± 11.89years.They were 135 (62.5%) females and 65 (37.5%) males The mean and standard deviation obtained for pre-treated PTB patients and control group were 12.66 ± 2.63 mm and 19.37 ± 2.65 mm respectively, and the difference was statistically significant (P < 0.0001). However, no significant increase is noted in lateral soft tissue thickness among different sexes. Conclusion: The lateral soft tissue thickness (muscle and fat) was generally lower in PTB patients compared to the healthy individuals.

Objective: This study was aimed to determine the pattern of fetal arterial blood flow of selected vessels in patients with PIH. Materials and Methods: A total of 34 pregnant women with PIH at a gestational age of 24-37 weeks were prospectively examined with Doppler ultrasound of the fetal middle cerebral artery (MCA), umbilical artery and placental blood flow (uterine artery). Results: The mean peak systolic velocity (PSV) of the fetal MCA was 8.23±3.96, resistance index (RI) was 0.763±0.07 and systolic diastolic (S/D) ratio was 4.558±1.36. The mean PSV of umbilical artery was 72.28±26.585, RI was 0.62±0.19 and S/D was 2.63±0.75. The mean placental blood flow (uterine artery) PSV was 141.34±70.58, RI was 0.59 and S/D was 2.42±1.07. Uterine artery PSV was normal in only six patients. Uterine artery was also not sonographically demonstrated in two patients. Conclusion: Doppler velocimetry of arterial blood vessels in pregnancy complicated with PIH reveals abnormal pattern; its application in PIH would be useful for further management.

Two multiparous women in early thirties presented for routine antenatal care. Routine ultrasonography revealed midtrimester abdominal pregnancies. Broad ligament pregnancy was diagnosed in both cases at laparotomy and both had excision of pregnancy and salpingo-ophorectomy with satisfactory recovery. Ultrasonography is a useful tool in the early diagnosis of abdominal pregnancy in our environment. Routine ultrasonography in pregnancy is advocated in our setting.

In sub-Saharan Africa monthly blood transfusions for primary stroke prevention carries risks; hydroxyurea (HU) may be an alternative. We conducted the first, US NIH funded, SCD feasibility trial in sub-Saharan Africa (5R21NS080639-02) to: 1) assess the acceptability and willingness of families to participate in a HU trial; 2) develop a safety protocol for using HU in a trial setting in sub-Saharan Africa; and 3) prepare for a definitive phase III Trial. In the Sickle Cell Disease Stroke Prevention in Nigeria (SPIN) trial, our primary hypothesis in the internal feasibility trial is that 80% adherence for daily HU administration is feasible. In Kano, Nigeria, we conducted a feasibility trial to determine the acceptability of hydroxyurea therapy for primary stroke prevention in children with abnormal transcranial Doppler (TCD) measurements. Children with SCA and abnormal non-imaging TCD measurements (≥ 200 cm/s) received moderate fixed-dose hydroxyurea therapy (∼20 mg/kg/day). A comparison group of children with TCD measurements < 200 cm/s was followed prospectively. Approximately 90% (337 of 375) of families agreed to be screened, while 92% (25 of 27) of those with abnormal TCD measurements, on two separate occasions, enrolled in the trial. No participant elected to withdraw from the trial. The average mean corpuscular volume increased from 85 fL at baseline to 101.3 fL at 24 months, demonstrating adherence to hydroxyurea. It kills an estimated 2-3 million people a year with 95-98% of this mortality in developing countries. Nigeria ranks 4 th among 22 high burden countries in the world. It is associated with marked weight loss which can be evident at the lateral chest wall soft tissue thickness on the chest radiographs. Objectives: To compare the lateral chest wall soft tissue thickness on the chest radiographs of normal adult subjects and pulmonary TB patients in Zaria, also to determine sex difference in the lateral chest wall thickness in aforementioned subjects. Methodology: This prospective study was conducted over a six month period at the Radiology Department of Ahmadu Bello University Teaching Hospital, Zaria. Two hundred adults who were bacteriologically proven to have pulmonary tuberculosis were consecutively recruited into the study as cases. The same number of age and sex matched healthy individuals that had chest radiograph grossly normal were recruited as controls. Their lateral chest wall soft tissue thickness (subcutaneous fat and muscle layers) on the chest radiographs was measured. Results: A total of 400 patients, aged 18 to 70 years. Average age was 39.12 ± 11.89years.They were 135 (62.5%) females and 65 (37.5%) males The mean and standard deviation obtained for pre-treated PTB patients and control group were 12.66 ± 2.63 mm and 19.37 ± 2.65 mm respectively, and the difference was statistically significant (P < 0.0001). However, no significant increase is noted in lateral soft tissue thickness among different sexes. Conclusion: The lateral soft tissue thickness (muscle and fat) was generally lower in PTB patients compared to the healthy individuals.

Objective: This study was aimed to determine the pattern of fetal arterial blood flow of selected vessels in patients with PIH. Materials and Methods: A total of 34 pregnant women with PIH at a gestational age of 24-37 weeks were prospectively examined with Doppler ultrasound of the fetal middle cerebral artery (MCA), umbilical artery and placental blood flow (uterine artery). Results: The mean peak systolic velocity (PSV) of the fetal MCA was 8.23±3.96, resistance index (RI) was 0.763±0.07 and systolic diastolic (S/D) ratio was 4.558±1.36. The mean PSV of umbilical artery was 72.28±26.585, RI was 0.62±0.19 and S/D was 2.63±0.75. The mean placental blood flow (uterine artery) PSV was 141.34±70.58, RI was 0.59 and S/D was 2.42±1.07. Uterine artery PSV was normal in only six patients. Uterine artery was also not sonographically demonstrated in two patients. Conclusion: Doppler velocimetry of arterial blood vessels in pregnancy complicated with PIH reveals abnormal pattern; its application in PIH would be useful for further management.

Renal function is usually preserved. It usually involves the left kidney. It is usually associated with mal-rotation, and may be a cause of urinary tract infection, hydronephrosis and renal calculi. We report a case of a 15-year old boy who was referred to our clinic with complaints of abdominal pain, and an ultrasound report of "absent right kidney". Urine examination indicated a partially treated infection, but the tests of kidney functions were normal. Intravenous urography confirmed crossed fused renal ectopia, and a flush aortogram showed two anomalous right renal arteries arising from the left side of aorta. An "absent" kidney in its normal location should prompt further investigations. Two multiparous women in early thirties presented for routine antenatal care. Routine ultrasonography revealed midtrimester abdominal pregnancies. Broad ligament pregnancy was diagnosed in both cases at laparotomy and both had excision of pregnancy and salpingo-ophorectomy with satisfactory recovery. Ultrasonography is a useful tool in the early diagnosis of abdominal pregnancy in our environment. Routine ultrasonography in pregnancy is advocated in our setting.
