Sometimes surrounding tissues are not healthy enough or quantity is not favorable to provide adequate pedicle flaps. Microvascular free flap can provide healthy tissue of adequate amount from distant area for those difficult situations.15 microvascular free flaps were performed from October 2011 to February 2013. Radial forearm free flap was done in 8 and Latissimusdorsi (LD) flap in 7 cases. 10 flaps done in foot, ankle & lower leg region (radial forearm-5, LD-5) and 5 flaps were done in face and scalp region (radial forearm-4, LD-1).12 flaps healed uneventfully with good coverage of the defect. Average ischemia time was 135 min (range 100-240 min) and average anastomosis time was 75 min (average 60-100 min). 2 flaps failed. FNAC from lesions of both sites showed it as a case of actinomycosis. He was successfully treated by antibiotics & local care of the affected area. No surgical intervention was required. Here we like to present our experience of diagnosing & treating this rare disease. Amputation of the penis is a rare condition reported from various parts of the world. They report... more Amputation of the penis is a rare condition reported from various parts of the world. They reported as isolated cases or small series of patients in scientific research. The common etiology is self amputation by sharp weapon or an avulsion or crush injury in an industrial accident or following violence. Complete reconstruction of anatomical structures should be done in one stage which provides the best chance for full rehabilitation of the patient. We report here a single case of sub-total amputation of the penis at it's proximal third in a schizophrenic patient. The penis was successfully replanted using a microsurgical technique. After surgery, near-normal appearance and function including a good urine flow and absence of urethral stricture, capabilities of erection and near normal sensitivity were observed. Introduction: The therapeutic use of oxygen under pressure is known as hyperbaric oxygen therapy ... more Introduction: The therapeutic use of oxygen under pressure is known as hyperbaric oxygen therapy (HBOT) and has been used to assist wound healing for almost 40 years. The purpose of the present study was to see the outcome of hyperbaric oxygen therapy following its use in non healing wounds. Methods: This prospective observational study was conducted in the Department of Plastic Surgery, Dhaka Medical College Hospital, Dhaka, from March 2016 to February 2017 for a period of 12 (twelve) months. The study was carried out on patients with non healing wounds. Patients were initially assessed with detailed history, clinical examination and investigations, and then treated with hyperbaric oxygen therapy for 90 minutes per session in 6 days a week for 25 cycles. This site of lipoma is rare. A case of a young male patient who consulted for removal of a penile lump is presented. The small, regular, wellcircumscribed pedunculated tumor localized on the ventral aspect of the penis was excised. The histopathological analysis showed a benign lipoma. A review of the literature showed lipoma of the penis to be very uncommon. This study was planned to evaluate the efficacy of this technique. This prospective interventional study was conducted in the department of Plastic Surgery, Dhaka Medical College Hospital, over a period of 16 months. A total of 28 procedures of subcutaneous pedicled rhomboid flap were performed on 12 patients. The contractures were released successfully by the rhomboid flaps. The mean percentage gain in length of the scar band was 77.28%. Most of the cases had complication free recovery. The clinical results indicated that the subcutaneous pedicled rhomboid flap is an effective and versatile technique for releasing scar contractures in different parts of the body. Burns are a global public health problem, accounting for an estimated
180 000 deaths every year, according to WHO. The vast majority
occur in low and middle-income countries. Non-fatal burns often
require lengthy and gruesome hospitalisations, where multiple treatments
are administered, early rehabilitation and life-long follow up.
The impact of burns on burns victims and the society can never be
overstated. Many factors, beginning from the cause of burns, affects the final burns outcome. This study aims to evaluate the epidemiology
of burns in Dhaka.
Materials and Methods
A survey form was employed to standardise the interview process.
Survey form was prepared in English. Doctors were approached randomly
at the local Burns unit at Dhaka, while they were at work. All
interviews were conducted, one-to-one, with the assistance of a
translator and with the doctors’ verbal consent.
Results
15 doctors participated in the study. 60% of them were males
while the rest were females. 80% (12) chose English to be the
language medium of choice whereas 13.3% (2) was other languages
and 6.7% (1) was Bengali. The two most common burn
injuries were thermal (15) and electrical burns (15). 100% agreed
that thermal burns were more prevalent than electrical burns.
Other types of burns reported were chemical (9), friction (7), radiation
(2), lightning (1) and abuse (1). 100% of them agreed that
there were not enough safety precautions in place to prevent
electrical burns. 80% of them thought that education was lacking
in the community. Other popular causes reported were inadequate
general public awareness (73%, 11), overcrowding in poor
housing conditions (67%, 10), poor education levels (53%, 8) and
poor city/home layout/hazard placements (53%, 8). 11 (73.3%)
thought that doctors, instead of nurses should lead Burns Resuscitation
and 9 (60%) thought that doctors, instead of nurses
should lead Burns Dressings. According to this group of local
doctors, the complication of Burn Injuries which patients complain
the most of was disfigurement.
Conclusions
The high prevalence of burns, undeniably is attributed to anthropogenic
reasons. Thus, burns are preventable, excluding that caused by
natural disasters for example in this study, lightning. The consensus
among these Dhaka doctors is that much effort is still needed to
help the burns community. While the Dhaka authorities formulate
their policies and plan their city, the authors recognise that a new
tertiary level hospital is on the way in Dhaka.

is essential to know how they occur and which population groups,
environments, types, and mode of burn can be targeted for prevention
work.
METHODS: Retrospective study.
RESULTS: The aim of this study was to determine the epidemiological
characteristics of burns leading to hospitalization in a
resource poor country like Bangladesh. In 2015, 6213 burn victims
were hospitalized; 43,100 victims were treated in the emergency
and outpatient of the only burn and plastic surgery department.
Males constituted 56% of victims where mean age was 22 years.
The most severe burns occurred between the ages of 18 and 32
years, and were mainly flame related. However, scalds representing
45% of burns cases and 47% were flame burns. Both in flame and
non-flame burns, women suffered more severe burns and mortality
than men and the majority of the severe cases in children were under
the age of 5 years. The most important injury patterns, more
common among women, were getting burned while using a cooking
stove. The main container was the rice cooking pot in 37%,
followed by water kettles in 32%, and pots in 22% cases. The difference
between flame and non-flame burns in the distribution of
burns in extremities was not statistically significant, but head and
neck burns were 3.7 times more likely to be caused by flame.
CONCLUSIONS: Safety education for caregivers and close supervision
of young children is important to reduce pediatric burns.
Occupational safety education could prevent burns caused by scald
and flame.
