The telomere plays a crucial role in regulating cellular responsiveness to stress and growth stimuli as well as maintaining the integrity of the Deoxyribonucleic Acid (DNA), and aging leads to the progressive decline in the telomere length (TL) due to continuous cell division. The aim of this study was to determine the relationship between TL and advancing age and the impact of metabolic syndrome (MetS) on TL. Firstly, we determined the association of advancing age and TL, by measuring telomere length (T/S ratio) in healthy volunteers (n = 90). The TL was compared between normal population and patients with metabolic syndrome (n = 298). The age matched controlled and uncontrolled MetS patients (n = 149) were also compared for their TL T/S ratio. The TL showed negative correlation with advancing age, whereas the significant change was observed at the cutoffs of 40 and 70 years defining 40 with longer TL and 70 as shorter TL. The longest T/S ratio at 2.46 was measured at the age range of 1 year in healthy volunteers, while elderly population showed considerably shorter TL. The patients older than 60 years with poor or uncontrolled MetS had shorter TL, as compared to the controlled MetS. In conclusion our findings suggest that TL was negatively correlated with advancing age. Uncontrolled metabolic syndrome appeared to have worsening effects on TL. Telomere length appears to have potential to be used a parameter to determine age. However, further large scale studies are recommended to make firm guidelines. Globally, 150,000 individuals die daily and nearly 2/3rd of them die due to age related factors. Aging process however is not constant, but the process gets modified or expedited due to comorbidities such as, cardiovascular and metabolic disorders. Aging is a genetically controlled phenomenon, and telomeres are essential element to maintain the integrity of the genome and regulating cellular responses to stress and growth factors 3. In normal cells, under physiological conditions, the telomere length (TL) decreases with each cell division. Given that TL decreases with each cell division, it progressively shortens with advancing age, therefore can be considered a biological marker of aging. This age-related decline in TL has been associated with a number of age-related diseases, including hypertension, diabetes, cancer, Alzheimer's disease, and many others 4. Patiehts and Methods This cross sectional descriptive study of six months was carried at Liaquat University Hospital Hyderabad Sindh, Pakistan from February 2008 to July 2008. All patients above 12 years of age, of either gender, were known / already diagnosed and newly diagnosed cases of lichen planus (LP) were came through outdoor patient department (OPD), indoor patient and causality outdoor department (COD) were evaluated and enrolled in the study. All such patients were screened for Helicobacter pylori infection by Helicobacter pylori stool antigen (HpSA). For getting result the data was saved and analyzed in SPSS version 10.00. RESULT: One hundred and five patients (105) were identified as lichen planus and screened for Helicobacter pylori infection. The Helicobacter pylori stool antigen (HpSA) was positive in 81 (77%) subjects with mean age 42.89 ± ±13.32 (SD). Majority of the patient were presented in the department of dermatology; were females and belonged to the rural areas of Sindh province. Other features and presentation identified in such patients were epigastric pain (69%), retrosternal pain (17%) and haemetemesis in 07 patients.CONCLUSION: The patients with lichen planus are more prone to acquire the Helicobacter pylori infection.

Subjects and methods: All the patients diagnosed as case of uterine rupture was taken as study subject. Data was collected in a pre-designed porforma. Variable analyzed were frequency, demographic characters, maternal morbidity and mortality. Results: During the study, 34 patients out of 1400 deliveries presented with uterine rupture resulting in a frequency of 2.42%or 1: 41deliveries. 64.7%(22) patients were between 26 to 35 years age and 67.64%(23) were multipara.26.47%(9) presented with extension of tears, 17.6%(6) with shock,20.5%(7) with septicemia and 20.5%(7) with wound infection. Conclusions: Uterine rupture is yet a common obstetrical emergency in our area. The significant morbidity and mortality deserves our special attention by a collaborative approach. Regular ante natal checkups, careful selection of patients for vaginal delivery, vigilantly monitoring during labor with smooth switch over to operative delivery can reduce this drastic obstetrical complication.

It was a prospective study carried out from February 2008 to January 2011. Data of all patients undergoing elective laparoscopic cholecystectomy during the study period and fulfilling the selection criteria was collected and entered in standardized proforma and analyzed on SPSS 10. A total of 100 patients underwent laparoscopic cholecystectomy with an overwhelming majority of females (87 out of 100). The age range from 22 to 60 years, mean age being 38 years. The overall complication rate was 9%. The complications included bleeding (4/100, 4%) from cystic artery and gall bladder bed, port site infection (4/100, 4%) One patient developed collection in pouch of Morrison and one patient developed stitch granuloma. There was no bile duct injury and colonic injury. The common causes of these complications were accident injury to cystic artery and gross spillage of infected bile. Bleeding and Port site infections were the commonest complications. The commonest cause of bleeding was cystic artery injury whereas the commonest cause of port site infection was gross spillage of infected bile.

Objective: To determine the precipitating factors of hepatic encephalopathy (HE) in patients with... more Objective: To determine the precipitating factors of hepatic encephalopathy (HE) in patients with liver cirrhosis at Liaquat University Hospital Hyderabad/Jamshoro. Methods: This hospital based descriptive study was conducted from April 2007 to September 2007. All the patients who were more than 12 years of age and were diagnosed as hepatic encephalopathy were studied. During this period, 87 patients of hepatic encephalopathy were admitted. All patients were carefully examined, relevant investigations were performed and data was collected through pre-designed proforma. Results: Male patients were 65 (75%), above 40 years of age 58 (67%), belonging to interior/periphery of Sindh 54 (62%), in grade IV of hepatic encephalopathy 70 (80%) and Anti-HCV positive were 52 (60%). The most common precipitating factors detected were infection 58 (67%), constipation 43 (49%) and gastrointestinal bleeding 39 (45%). Out of 87 patients, 68 had increased total leucocytes count, 09 patients had hypokalaemia, 24 patients, hyponatraemia, 64 hypoalbuminaemia and 54 patients had a disturbed coagulation profile. Fifty nine patients recovered and were discharged while 20 patients expired. Majority of expired patients had Child-Pugh score 10-15 and were in grade IV of hepatic encephalopathy. Conclusion: The study concluded that there were different factors which play a key role in hepatic encephalopathy. In these factors, infection was the most common.

Method: the outcome of patients between 60 and 70 years of age and patients over 70 who underwent laparoscopic cholecystectomy for symptomatic non-malignant gallbladder disease was comparatively analysed. All patients over 60 years of age with symptomatic gallbladder disease and without cholecholithiasis, septic shock, diffuse peritonitis, gallbladder malignancy, portal hypertension or contraindication for general anaesthesia were selected for simple laparoscopic cholecystectomy (n = 158). This group represents over 80% of all elderly patients undergoing biliary surgery at our department over this period. Group A (n = 97) included patients from 60 to 69 years of age. Group B (n-61) comprised patients over 70 years. Results: there was no difference in sex distribution between groups. Operative time and conversion rates were similar in both groups. The overall morbidity rate was 14.5%, with no statistically significant increase in group B (11% for group Avs20% for group B). No perioperative mortality occurred. Recurrent biliary surgery was required in two patients from group B (3%). Postoperative endoscopic retrograde cholangiography and sphincterotomy was done in four patients from group A (4%). The mean postoperative stay was longer for older patients (group A, 3.1 (2.5) days; group B, 4.2 (4.3) days; i>=0.05). Conclusion: simple laparoscopic cholecystectomy is safe in the aged, even for patients over 70. This procedure is associated with a short hospital stay and low rates of re-admission and recurrent biliary surgery.

Methods: This hospital based descriptive study was conducted from April 2007 to September 2007. All the patients who were more than 12 years of age and were diagnosed as hepatic encephalopathy were studied. During this period, 87 patients of hepatic encephalopathy were admitted. All patients were carefully examined, relevant investigations were performed and data was collected through pre-designed proforma. Results: Male patients were 65 (75%), above 40 years of age 58 (67%), belonging to interior/periphery of Sindh 54 (62%), in grade IV of hepatic encephalopathy 70 (80%) and Anti-HCV positive were 52 (60%). The most common precipitating factors detected were infection 58 (67%), constipation 43 (49%) and gastrointestinal bleeding 39 (45%). Out of 87 patients, 68 had increased total leucocytes count, 09 patients had hypokalaemia, 24 patients, hyponatraemia, 64 hypoalbuminaemia and 54 patients had a disturbed coagulation profile. Fifty nine patients recovered and were discharged while 20 patients expired. Majority of expired patients had Child-Pugh score 10-15 and were in grade IV of hepatic encephalopathy. Conclusion: The study concluded that there were different factors which play a key role in hepatic encephalopathy. In these factors, infection was the most common.

Method: the outcome of patients between 60 and 70 years of age and patients over 70 who underwent laparoscopic cholecystectomy for symptomatic non-malignant gallbladder disease was comparatively analysed. All patients over 60 years of age with symptomatic gallbladder disease and without cholecholithiasis, septic shock, diffuse peritonitis, gallbladder malignancy, portal hypertension or contraindication for general anaesthesia were selected for simple laparoscopic cholecystectomy (n = 158). This group represents over 80% of all elderly patients undergoing biliary surgery at our department over this period. Group A (n = 97) included patients from 60 to 69 years of age. Group B (n-61) comprised patients over 70 years. Results: there was no difference in sex distribution between groups. Operative time and conversion rates were similar in both groups. The overall morbidity rate was 14.5%, with no statistically significant increase in group B (11% for group Avs20% for group B). No perioperative mortality occurred. Recurrent biliary surgery was required in two patients from group B (3%). Postoperative endoscopic retrograde cholangiography and sphincterotomy was done in four patients from group A (4%). The mean postoperative stay was longer for older patients (group A, 3.1 (2.5) days; group B, 4.2 (4.3) days; i>=0.05). Conclusion: simple laparoscopic cholecystectomy is safe in the aged, even for patients over 70. This procedure is associated with a short hospital stay and low rates of re-admission and recurrent biliary surgery.
