A clinical trial of 21 cases was conducted to evaluate the effects of a pulsed magnetic field on ... more A clinical trial of 21 cases was conducted to evaluate the effects of a pulsed magnetic field on pseudoarthroses. Nineteen out of 21 cases had successful results; the remaining 2 failed to respond. Two-coiled and four-coiled systems were used in this study. It was concluded that the four coiled system is more effective in spite of its practical disadvantages and larger size.

In a clinical and radiological study of seven neonates with bladder extrophy (BX), separation of ... more In a clinical and radiological study of seven neonates with bladder extrophy (BX), separation of pubic bones from the midline, lateral and caudal tilting of the iliac alae and external rotation of the acetabula, with consequent early predisposition to bilateral congenital hip dislocation (CHD), who were treated by a combined bilateral innominate osteotomy and reconstruction of the urinary bladder, there were only two failures due to CHD and three due to BX, which were dealt with at a later session. Although the bilateral transverse iliac osteotomy procedure is very rarely, if ever, employed in the neonatal period, it may well be combined with the reconstructive surgical procedures that involve pelvic skeletal structures while dealing with soft tissue procedures. Bilateral transverse iliac osteotomy was preferred to the conventional two-stage posterior iliac vertical osteotomies because of its easiness and the simple approach provided by the supine positioning of the patient.

Their management include conservative or surgical especially in case of neurological symptoms. However, indications for surgical treatment were less well defined, because of different evaluation criteria of risk of future displacement and canal compromise. We present a patient with burst fracture of fourth lumbar vertebra who was treated operatively although she had no neurological symptoms. And we demonstrate that why anterior aproach was performed to this patient.

Torokolomber omurga kırığı tanısı ile cerrahi uygulanan hastaların klinik ve radyolojik sonuçları... more Torokolomber omurga kırığı tanısı ile cerrahi uygulanan hastaların klinik ve radyolojik sonuçlarının karşılaştırılması amaçlanmıştır. Gereç ve Yöntemler: Çalışma, 1995 ile 2009 tarihleri arasında torokolomber omurga kırığı tanısı ile cerrahi uygulanan 38 (23 kadın, 15 erkek, ortalama yaş 46.5) hastadan oluşmaktadır. Klinik veriler Visual Analog Skala (VAS), Oswestry ve Frankel skalaları ile; radyolojik veriler ise hastaların cerrahi öncesi, sonrası ve son kontrollerindeki çekilen X-ray görüntülerinde yapılan sagital indeks (Sİ) değerleri, lokal kifoz açıları (LKA) ve anterior korpus yükseklik kaybı (AKYK) yüzdeleri ölçülerek değerlendirilmiştir. Bulgular: Hastaların ortalama takip süresi 96,5 ay (6-183 ay) dır. VAS ortalaması 13,5 mm, Oswestry ortalaması 15'tir. Ameliyat öncesi Sİ açıları arttıkça son kontrol açıları da yükselmekte ve ameliyat sonrası açılar ne kadar yüksekse son kontrol açıları da aynı oranda yüksek olmaktadır (P<0,05). LKA arttıkça ameliyat sonrası LKA da yükselmekte, ameliyat sonrası LKA arttıkça son kontrol açısı da yüksek olmaktadır (P<0,05). Cauda equina syndrome is the result of any lesion that compresses or paralyzes cauda equina roots... more Cauda equina syndrome is the result of any lesion that compresses or paralyzes cauda equina roots which are both motor and sensory. It is an uncommon syndrome, which features low back pain, sciatica, variable lower extremity motor and sensory loss with possible bladder and bowel dysfunction. It is an emergency situation as it may cause significant morbidity such as permanent paralysis, impaired bladder and/or bowel control or loss of sexual sensation. We present the case of a patient who was admitted to the emergency department with a traumatic posterior L5-S1 dislocation, low back pain and bladder dysfunction 8 days following an initial trauma. Open L5-S1 reduction and posterior stabilization was performed and the dural sac was decompressed. Most of the patient's neurological deficits resolved over several years, following the initial surgery.
Objective: Comparison between the clinical and radiological findings in patients that underwent s... more Objective: Comparison between the clinical and radiological findings in patients that underwent surgery due to thoracolumbar fractures was aimed in this study. Methods: Between 1995 and 2009, 38 patients (15 male, 23 female, mean age 46.5 years) that underwent operation after diagnosis of thoracolumbar fractures were admitted to the study. Clinic values were evaluated with Visual Analog Scale (VAS) and Frankle and scale of Oswestry scales. Radiological values were evaluated with preoperative, postoperative and last control X-ray images. Value of sagittal index (SI), value of local kyphosis angle (LKA) and the percentage of anterior corpus height loss (ACHL) were measured using X-ray images.Results: The mean follow-up period was 96.5months, the mean VAS value was 13,5 mm, and the mean Oswestry value was 15. Preoperative SI angles increased, as well as final control measurements. On the other hand the final control measurements became higher as postoperative angles increased (P<0,05). We also observed increased LKA, postoperative LKA and measured angle of the final controls (P<0,05). Preoperative ACHL percentage, postoperative ACHL percentage, and the ACHL percentage at the final control were all significantly higher (P<0,05). However, there was no significant correlation between the clinical and the radiological parameters.Conclusion: As a result, clinical data were not associated with radiological data.

The aim of this biomechanical experimental study was to evaluate the resistance of each posterior... more The aim of this biomechanical experimental study was to evaluate the resistance of each posterior ligamentous complex structure of the thoracic and lumbar spine to compression forces and to measure the shifting load to the intervertebral disc when each PLC structure was interrupted. Method: The study was conducted on 4 groups for thoracic and lumbar region as intact, supraspinous ligament interrupted, interspinous ligament/ligamentum flavum combination interrupted and facet joint capsule interrupted. Pre and post anterior vertebral body height, the highest compression force and pressure changes in the intervertebral disc during 40 N loading were measured. Results: A significantly different degree of resistance to compression force was determined in each posterior ligamentous complex structure in the thoracic and lumbar spine samples. The combination of interspinous ligament and ligamentum flavum was found to be the most effective structure to resist compression forces (p ¼ 0.001 in both groups). The effect of the supraspinous ligament in thoracic and lumbar segments was found to be similar to that of the interspinous ligament and ligamentum flavum combination (p ¼ 0.008 and p ¼ 0.006, respectively). The least effective structure was observed to be the facet joint capsule. Compression forces were significantly increased in the intervertebral disc as a result of the disruption of supraspinous ligament (p ¼ 0.0032 and p ¼ 0.0029, respectively in thoracic and lumbar segments) and combination of interspinous ligament/ligamentum flavum (p ¼ 0.0019 and p ¼ 0.0021, respectively in thoracic and lumbar segments). Conclusion: The interspinous ligament/ligamentum flavum combination and supraspinous ligament are the largest contributor to resisting applied compression moments in the sheep thoracic and lumbar spine. As a result of the loss of resistance to compression forces, there will be a shift of a great proportion of this force onto the intervertebral disc.
