Material and methods: In the period 1995-2000, 20 patients (pts) with lung tuberculosis and hematological malignancies were analyzed, out of whom 7 (35%) were females, with mean age of 56 years, and 13 (65%) were males, with mean age of 55 years. Results and discussion: Among the affected, 8 (40%) patients had Non-Hodgkin's lymphoma, 5 (25%) suffered from chronic lymphocytic leukemia, 4 (2%) had Hodgkin's disease, 1 (5%) manifested a myelodysplastic syndrome, 1 (5%) multiple myeloma and 1 (5%) had chronic myeloid leukemia. The average time from completing a specific cancer therapy and development of tuberculous infection was 20 months, except in 3 cases who had a hematological malignancy and lung tuberculosis at the time of initial diagnosis. The diagnosis was made on the basis of positive findings of sputum smear in 15 (75%) pts, by bronchological examination and sputum analysis in 3 (15%) pts, lymph node biopsy and sputum analysis in 1 (5%) case, pleural biopsy in 1 (5%) patient, and finally confirmed by culture in 19 (95%) pts. Radiological examination revealed fibrocaseous tuberculosis in 14 (70%) pts, out of whom 12 with bilateral and 2 with unilateral changes, 3 (15%) manifested miliary tuberculosis, and 3 (15%) had pleural effusion and tuberculosis. Conclusion: Successful outcome was achieved in 18 (90%) pts, while lethal outcome was reported in 2 (10%) cases with miliary pulmonary tuberculosis.

Miliary TB is a treatable, potentially lethal form of TB resulting from massive lympho-hematogenous dissemination of Mycobacterium tuberculosis. Impaired cell-mediated immunity underlies the disease's development. We present a case of specific synovitis in a 21-year-old Caucasian HIV-seronegative woman. She presented with high fever and swelling of the right knee. Chest radiograph revealed bilateral nodular opacities in upper pulmonary lobes and signs of pleural effusions. Sputum samples were negative for Acid Fast Bacilli (AFB) and Löwenstein-Jensen (L-J) culture negative. Diagnosis was confirmed histologically by pleural biopsy and positive L-J cultures of knee puncture, also documented by MRI. Treatment outcome was successful with anti-tuberculous drugs following standardized treatment regimen.

The aim of this study was to determine impact of these haematological parameters in patients of different ages with NSCLC. MAteRIAl And MethOdS: Among 2,050 patients who underwent lung resection for NSCLC in the 5-year period 2002-2007, 200 were reviewed, of whom 93 were aged above 70 years. ReSUltS: The frequency of preoperative leukocytosis, anaemia and thrombocytosis was 21% (42/200), 32.5% (65/200), 16.5% (33/200), respectively. The 5-year survival of the patients with and without leukocytosis, anaemia and thrombocytosis was 26.5% vs 27.4%, 28.9% vs 27.2% and 31.7% vs 26.6%, respectively. No significant difference was observed in the 5-year survival according to either the presence or absence of preoperative leukocytosis, anaemia and thrombocytosis, or the age group, <70 years and ≥70 years. Significant difference was found in the haemoglobin (Hb) level between the different age groups, 3 (p=0.0025), 6 (p<0.001) and 12 (p=0.033) months postoperatively. Leukocytosis, anaemia and thrombocytosis were frequently found in earlier stages of NSCLC and in connection with extended types of surgical resection. cOnclUSIOnS: Preoperative anaemia, leukocytosis and thrombocytosis are not related with patient survival after lung resection for NSCLC, although the measurement is inexpensive and routinely used. An abnormal blood cell count in patients with cancer is not always a tumour-related phenomenon. Pneumon 2011, 24(1):35-39.

The increased morbidity and mortality rates are most frequently observed in patients with COVID-19-associated pulmonary aspergillosis (CAPA) and COVID-19-associated candidiasis (CAC). CAPA is the most frequently encountered infection with an incidence rate of 0.7–7.7%, while CAC is a less common and less studied fungal infection in COVID-19 patients.Materials and methodsThe present article is a prospective observational single-center study that was conducted between 1 September 2021 and 24 December 2021, involving 6,335 patients who were admitted to COVID Hospital “Batajnica,” University Clinical Center of Serbia, Belgrade.ResultsOf the 6,335 patients hospitalized during the four-month period of the study, 120 patients (1.86%) who had a proven diagnosis of IFD were included in the study. Most patients require anticoagulant therapy which predisposes them to the development of hemorrhagic syndrome. In critically ill COVID-19 patients secondary infections due to opportunistic pathogens are associated with a high mortality rate. Case report: Herein, we present a COVID-19 patient with severe hemorrhage at unusual sites complicated with invasive candidiasis and an extensively drug-resistant (XDR) strain of Klebsiella enterobacter. Conclusions: Clinicians should be aware of the possibility for invasive fungal infections in severely ill patients with SARS-CoV-2 infection due to pre-existing conditions, risk factors, and COVID-19 associated pathological mechanisms. Management of invasive candidiasis is challenging because of the high prevalence of comorbidities, risk of toxicities, and drug interactions.

Finding of the concomitant venous anomaly of inferior vena cava revealed the diagnosis of bronchopulmonary sequestration. Case Report: A 22-year-old Caucasian woman presented with a history of chronic cough, initially treated as bronchial asthma for a year. Chest X-ray showed fibrocaseous cavernous tuberculosis on the right lung. Acid Fast Bacilli (AFB) were found in sputum samples. Patient was treated for 6 months with usual antituberculous regiment. Control chest X-ray showed subatelectasis of the upper right lobe. Six months later the first thorax computed tomography (CT) showed complete atelectasis of the right lung. Patient was admitted to the hospital again after 6 years due to the persistent fever and cough. Endoscopic finding and histopathological analysis confirmed EBTB. Thoracic CT scan revealed duplication of inferior vena cava which ...

Neurofibromatosis type 1 (NF1), also known as von Recklinghausen disease, is one of the most common single-gene disorders (mutation on chromosome 17q) and usually associated with cutaneous, musculoskeletal and neurological disorders in humans. NF1 is generally complicated with one or more neurobehavioral disorders or tumors located in the peripheral nervous system such as neurofibromas, peripheral nerve sheath tumor, pheochromocytoma, etc. In the available medical literature, the thoracic manifestations of NF1 have been rarely described in these patients. There are few reports about intrathoracic neurogenic tumors, kyphoscoliosis, pneumonitis and pulmonary fibrosis in patients with NF1. Case report. A 65-year-old female was admitted to the Intensive Care Unit at the Lung Clinic of Belgrade University Clinical Center of Serbia. The patient?s general condition was poor with shortness of breath and present cyanosis. At the same time, the skin changes similar to NF1 were n...

Bronchobiliary fistula (BBF) is a pathological communication between the bronchial system and the biliary tree that presents with bilioptysis. Many conditions can cause its development. There is still no optimal therapy for BBF. Conservative treatment is rarely indicated, as was published before in a few cases. Case report. We presented a 71-year-old Caucasian Serbian woman with BBF secondary to previous laparotomy due to multiple echinococcus liver cysts. The diagnosis was established by the presence of bilirubin and bile acids in sputum and magnetic resonance cholangiopancreatography (MRCP). A repeat MRCP performed after conservative procedure, did not reveal fistulous communication. Conclusion. We suggest that in small and less severe fistulas between the biliary and the bronchial tract, conservative treatment may be used successfully, and invasive treatment methods are not needed in all patients.

The most frequent histological types of lung cancer in tissue specimens obtained by bronchoscopy or percutaneous needle biopsy were analysed in terms of age and gender. The studied population included 6289 patients (16.6% females and 83.1% males). Statistical significance was established by x(2) test at the level p<0.05. Squamous cell carcinoma (SCC) prevailed in the total number of patients in all investigated years (58.0%), and separately in male (60.4%) and female (45.7%) patients. This histological type was predominant in all age groups in both genders (41.6% in males and 38.1% in females). SCC has the highest incidence in Serbia. Continuous campaign against smoking and helping its cessation, improving working and socioeconomic conditions is a strategy for decreasing all histological types of lung cancer patients.

Pneumonia caused by this fungus has been reported as a post mortem diagnosis in a patient with haematologic malignancy. A 46-year-old unemployed Caucasian man, smoker, was admitted on emergency in a serious condition, with bilateral pulmonary infiltrates (chest x-ray) and incapsulated mediastinal pleural effusions (computed tomography scans). Blastoschizomyces capitatus (Trichosporon capitatum) has been isolated from two sputum samples and bronchial secretion aspirate obtained by bronchoscopy. Pneumonia caused by Blastoschizomyces capitatus can develop in subjects without underlying malignancy but with history of heavy alcohol intake, tobacco smoking and malnutrition and may have favorable disease outcome.
