Extraction of glycolipid component from hypertrophic and cancerous prostatic tissue were performe... more Extraction of glycolipid component from hypertrophic and cancerous prostatic tissue were performed using chloroform-methanol solution and isopropanol-hexan-solution. The extract was separated into the "upper phase" and "lower phase" by Folchs fractionation technic. Each fraction was conjugated with acid treated salmonella minesota and than it was injected subcutaneously, intra-peritoneally and finally intravenously to BALB/c mice. Spleen cells from immunized mice were fused with P3X63, and antibody APG1 was obtained. APG1 shows a preferential reactivity towards prostatic tissue by the immunohistochemical analysis. Furthermore, the immunohistochemical analysis using enzyme and acid treated tissue and TLC immunostaining indicate that this monoclonal antibody specifically recognizes the sialylglycochain expressed in the prostatic tissue.

We examined the usefulness of prostate specific antigen density (PSAD) for selection of biopsy ca... more We examined the usefulness of prostate specific antigen density (PSAD) for selection of biopsy candidate with prostate specific antigen levels between 4.1 and 10.0 ng./ml. in prostate cancer screening retrospectively. The screening was conducted on male candidates in Natori city, aged 55 years or older, for 6 years from 1994 through 1999. We could analyze serum PSA levels and PSA density in 118 men with PSA levels between 4.1 and 10.0 ng./ml. All of 118 men underwent ultrasound guided systematic prostate biopsy regardless of findings of digital rectal examination and transrectal ultrasound. Prostate volume was estimated by transrectal ultrasound measurements using the prolate ellipse formula (pi/6 x length x width x height). PSAD was calculated by dividing serum PSA level by prostate volume. Serum PSA levels were determined by Tandem-R assay. In 118 men, twenty-five men had prostate cancer. Two cases of ectopic prostatic tissue or adenomatous polyps with prostatic type epithelium in the... more Two cases of ectopic prostatic tissue or adenomatous polyps with prostatic type epithelium in the prostatic urethra were reported. They were 30-and 33-year-old males suffering from gross-hematuria and from urethral pain on urination with terminal gross-hematuria. Endoscopic examination revealed papillary lesions from the dilated orifice of the prostatic utricle. Biopsied pathology demonstrated a tissue similar to that of the prostatic gland. PSA (prostatic specific antigen) stain using immunohistochemical methods was positive. Histogenesis and the importance of this lesion particularly as a cause of hematuria in the young male adult were discussed.

Patients who underwent radical nephroureterectomy for upper urinary tract urothelial carcinoma between 2000 and 2011 at 12 institutions participating in the Tohoku Urological Evidence-Based Medicine Study Group were included in the present study. Those who underwent prior or simultaneous radical cystectomy were excluded. The site of intravesical recurrence was investigated, and the survival curves after radical nephroureterectomy were analyzed retrospectively using the Kaplan-Meier method. Multivariate analyses of factors predicting survival were carried out. A total of 534 patients were eligible for the present study. Objective: No previous studies have reported the longitudinal health-related quality of life (HRQ... more Objective: No previous studies have reported the longitudinal health-related quality of life (HRQOL) for intensity modulated radiation therapy (IMRT). We compared HRQOL after IMRT with that after conventional and after conformal radiation therapy (XRT). Methods: A total of 110 patients underwent XRT (34 patients underwent conventional radiation therapy and 76 underwent conformal radiation therapy) and 30 underwent IMRT for clinically localized prostate cancer between 2000 and 2002. We measured the general and disease-specific HRQOL using the Medical Outcomes Study 36-Item Health Survey and University of California, Los Angeles, Prostate Cancer Index, respectively. Results: There were no significant differences in the preoperative characteristics and HRQOL scores of the two groups. Repeated measure analyses of variance revealed significantly different patterns of alteration in several general HRQOL domains between XRT and the IMRT groups. In the urinary domain, there was no difference in the alteration patterns between the two groups. The XRT group suffered worse bowel function at 3 and 6 months than the IMRT group (P < 0.05). In the XRT group, sexual function decreased at 3 months and remained substantially lower than the baseline level. However, the IMRT group showed no significant difference from the baseline level at any of the observation periods. At 18 months the XRT group showed worse sexual function than the IMRT group. Conclusion: The two approaches showed different longitudinal profiles regarding general and disease-specific HRQOL during the first 2 years after treatment. The IMRT approach produced little impairment in bowel and sexual function.

Purpose We evaluated the changes of health-related quality of life (HRQOL) during the Wrst 2 year... more Purpose We evaluated the changes of health-related quality of life (HRQOL) during the Wrst 2 years after radical prostatectomy (RP) or external beam radiation therapy (EBRT) in elderly men (70 years old or older) treated for prostate cancer. Patients and methods Between 2002 and 2005, a total of 319 elderly men who underwent RP or EBRT participated in our study. Patients completed the general and disease-speciWc HRQOL with the Short Form-36 (SF-36) and University of California, Los Angeles Prostate Cancer Index, respectively. The participants were asked to complete the questionnaire before and 3, 6, 12, 18 and 24 months after treatment. Results The RP group was younger and had more favorable tumor characteristics than the EBRT group. When compared with the EBRT group, the RP group had higher scores for several SF-36 subscales and sexual function, whereas other domains were comparable. In the EBRT group, there were no signiWcant diVerences in the general HRQOL scores between the baseline and any of the obser-vation periods. The RP group had a signiWcantly worse urinary function and bother scores postoperatively than the EBRT group. The score for sexual function declined over the 24 months, but more so in the RP group. Similarly, sexual bother scores were signiWcantly lower at each postoperative time point for the RP group. Conclusions Both aggressive treatments can oVer satisfactory functional outcomes from the perspective of HRQOL for selected elderly subjects with localized prostate cancer.

We retrospectively reviewed the medical records of 1268 men with prostate cancer treated using radical prostatectomy without neoadjuvant therapy. The association between various risk factors and biochemical recurrence was then statistically evaluated. The Kaplan-Meier method, log-rank tests and Cox proportional hazards models were used for statistical analysis. In the intermediate-risk group, 96 patients (14.5%) experienced biochemical recurrence during a median follow up of 41 months. In the intermediate-risk group, preoperative prostate-specific antigen level, prostate volume and prostate-specific antigen density were significant preoperative risk factors for biochemical recurrence, whereas other factors including age, primary Gleason 4, clinical stage >T2 and percentage of positive biopsies were not. Through March 1995 to April 2009, a total of 184 patients were instilled 3-day consecutive intravesical instillation of pirarubicine hydrochloride (THP) (30 mg/40 ml in disinfected distilled water) started within a few hours after TURBT. 184 patients were diagnosed as untreated fresh non-muscle-invasive urothelial bladder cancer with no concomitant carcinoma in situ (CIS), no concurrent upper urinary tract urothelial cancer and no past history of upper urinary tract urothelial cancer. Solitary fibrous tumor (SFT) of the retroperitoneal space is rare. We report a case of retroperit... more Solitary fibrous tumor (SFT) of the retroperitoneal space is rare. We report a case of retroperitoneal tumor, diagnosed as SFT. A 69-year-old woman presented with right lower abdominal swelling, and was referred to our hospital with suspicion of right renal tumor. Abdominal ultrasound and computerized tomography (CT) showed a mass (about 15 x 14 x 10 cm) in the right abdomen. The tumor was thought to be right renal rumor, and right radical nephrectomy was performed. In the excised specimen the tumor was not connected to gastrointestinal tract, peritoneum, or right kidney. The histological and immunohistochemical examination of the specimen revealed SFT. The tumor has malignant potential with partially increased mitotic activity and cellularity in the histological examination. The patient is healthy and without evidence of recurrence or metastasis 26 months from surgery.

In the urinary domain, there was no difference in the alteration patterns between the two groups. The XRT group suffered worse bowel function at 3 and 6 months than the IMRT group (P < 0.05). In the XRT group, sexual function decreased at 3 months and remained substantially lower than the baseline level. However, the IMRT group showed no significant difference from the baseline level at any of the observation periods. At 18 months the XRT group showed worse sexual function than the IMRT group. Conclusion: The two approaches showed different longitudinal profiles regarding general and disease-specific HRQOL during the first 2 years after treatment. The IMRT approach produced little impairment in bowel and sexual function.

Purpose We evaluated the changes of health-related quality of life (HRQOL) during the Wrst 2 year... more Purpose We evaluated the changes of health-related quality of life (HRQOL) during the Wrst 2 years after radical prostatectomy (RP) or external beam radiation therapy (EBRT) in elderly men (70 years old or older) treated for prostate cancer. Patients and methods Between 2002 and 2005, a total of 319 elderly men who underwent RP or EBRT participated in our study. Patients completed the general and disease-speciWc HRQOL with the Short Form-36 (SF-36) and University of California, Los Angeles Prostate Cancer Index, respectively. The participants were asked to complete the questionnaire before and 3, 6, 12, 18 and 24 months after treatment. Results The RP group was younger and had more favorable tumor characteristics than the EBRT group. When compared with the EBRT group, the RP group had higher scores for several SF-36 subscales and sexual function, whereas other domains were comparable. In the EBRT group, there were no signiWcant diVerences in the general HRQOL scores between the baseline and any of the obser-vation periods. The RP group had a signiWcantly worse urinary function and bother scores postoperatively than the EBRT group. The score for sexual function declined over the 24 months, but more so in the RP group. Similarly, sexual bother scores were signiWcantly lower at each postoperative time point for the RP group. Conclusions Both aggressive treatments can oVer satisfactory functional outcomes from the perspective of HRQOL for selected elderly subjects with localized prostate cancer.

We retrospectively reviewed the medical records of 1268 men with prostate cancer treated using radical prostatectomy without neoadjuvant therapy. The association between various risk factors and biochemical recurrence was then statistically evaluated. The Kaplan-Meier method, log-rank tests and Cox proportional hazards models were used for statistical analysis. In the intermediate-risk group, 96 patients (14.5%) experienced biochemical recurrence during a median follow up of 41 months. We report a case of retroperitoneal tumor, diagnosed as SFT. A 69-year-old woman presented with right lower abdominal swelling, and was referred to our hospital with suspicion of right renal tumor. Abdominal ultrasound and computerized tomography (CT) showed a mass (about 15 x 14 x 10 cm) in the right abdomen. The tumor was thought to be right renal rumor, and right radical nephrectomy was performed. In the excised specimen the tumor was not connected to gastrointestinal tract, peritoneum, or right kidney. The histological and immunohistochemical examination of the specimen revealed SFT. The tumor has malignant potential with partially increased mitotic activity and cellularity in the histological examination. The patient is healthy and without evidence of recurrence or metastasis 26 months from surgery.
