Transthoracic echocardiography showed presence of а giant left atrial cardiac mass with atypical location for thrombus. Cardiac Magnetic Resonance (CMR) Imaging was performed for histological discrimination, and showed a large cardiac mass wall attached in the left atrium, homogeneous, with diameter of 3.4cm x 3.2cm. Late Gadolinium Enhancement sequences revealed black avascular tissue without signal, confirming the characterization of a thrombus. The patient refused hospital initiation of low molecular weight heparin treatment and started treatment with Rivaroxaban. Six weeks later he presented with NYHA class II, almost complete dissolving of the thrombus on CMR scan. Seven months from the initial CMR scan, echocardiography screen was done showing complete absence of the left atrial mass. With this case report we have demonstrated the significance of the CMR as o...

AIM: We report on the first fifty consecutive patients using this imaging modality. METHODS AND MATERIALS: Strict protocol for CMR procedure, imaging quality assessment, contraindications, and informed consent were established. Patients selected for CMR were enrolled in a prospective registry. Visualizing the heart chambers, heart muscle and heart valves, resulted in acquiring complex imaging of the heart structure and function. When applicable, patients received gadolinium contrast agent for Late Gadolinium Enhancement (LGE). Adenosine was used for stress induced myocardial perfusion study. In this study, we report on the initial CMR procedures in the first 15 months. RESULTS: The age of the patients ranges from 17 to 82 and the number of male and female patients was well balanced. No absolute contraindications were met in any patient. ...

Research paper thumbnail of Novel Experience with Combined Fix-Dose Therapy for Hypertension

In spite of the general improvement in treatment strategy and novel medications for hypertension,... more In spite of the general improvement in treatment strategy and novel medications for hypertension, still there are many patients with unsatisfactory results. In this study we retrospectively analyzed the local registry for 280 hypertension patients, referred during 18 months. Aims: To access the efficacy and safety of combined fix-dose therapy approach. Patients were grouped according to the number of antihypertensive medications and mono or fix-combined therapy and followed for 1 month in respect of obtaining target blood pressure control. Patients were informed about taking proper measurements and in respect of drug side effects. Clinical data on previous medical history findings were obtained. Statistical analysis was performed using SPSS 23.0 software package. Parameters were expressed as mean ± SD and percentages for nominal values. Comparative analysis was performed using X2 test.Statistical significance was set at p<0.05.Results: The mean age of study participants was over ...

In this hospital-based study we tried to assess predictors for development of CIN in patients undergoing cardiac catheterization. A total of 5604 patients undergoing coronary angiogram/PCI from 2007-2017 were enrolled in the study. Multivariate predictors of CIN were identified by logistic regression using stepwise selection with entry and exit criteria of p < 0.1. A two-sided 95% confidence interval (CI) was constructed around the point estimate of the odds (OR). A p value <0.05 was considered significant. CIN occurred in 6 (1‰) patients. The mean age of patients suffering from CIN was higher than in the whole population (66.5± 31.15 vs. 58.66± 28.57, p=0.03). Characteristics of patients who developed CIN were: older age, diabetes, higher creatinine and lo...
