The involvement of the oral mucosa or of the vermilion border of the lips is rare. CASE REPORTS: In our 5 cases, a small painless and more or less protruding nodule had been present for years on the mucosa of the hard palate (50 year-old man, 43 year-old woman), the soft palate (63 year-old woman), the vermilion border of the lower lip (48 year-old woman), and the gingiva (26 year-old man). The non specific clinical aspect did not permit a correct diagnosis. DISCUSSION: Histologically, the solitary circumscribed neuroma is formed by a proliferation of Schwann cells arranged in interlacing bundles, featuring one or several well-limited nodules, sometimes surrounded by a thin capsule. Low-grade myofibroblastic sarcoma (LGMS) is a mesenchymal tumor of myofibroblasts that occurs mor... more Low-grade myofibroblastic sarcoma (LGMS) is a mesenchymal tumor of myofibroblasts that occurs more frequently in adults. A series of three cases is presented to illustrate that LGMS may also occur within the oral cavity in children and adolescents. The first case (Case 1) occurred intra-osseously in the mandible, while the remaining two presented as gingival swellings and were purely restricted to soft tissue (Cases 2 and 3). The intra-osseous lesion arose in a 7-year-old girl, whereas the gingival lesions were observed in a 12-year-old girl (Case 2) and a 13-year-old boy (Case 3). Histopathologically, all cases were composed of spindle shaped cells arranged into long fascicles showing mild to moderate degree of nuclear atypia. Ki-67 (MIB-1) proliferation activity was relatively low, amounting to 3–5% in all cases. Cutaneous manifestations include café au lait spots, intertriginous freckling, and neurofibromas which appear during childhood and adolescence and are part of the clinical criteria to diagnose NF1. However, it is only recently that oral manifestations have been highlighted in many studies as frequently associated to NF1. This article aims to review oral and cutaneous manifestations related to NF1 and to report a case of a 51-year-old male with skin and oral neurofibromas related to NF1. Our patient presented with lesions on the gingiva, a rare localization that takes a hypertrophic form mimicking other various pathological conditions. Oral manifestations are common, and may remain exclusive to the oral mucosa without involvement of the skin or other mucosae. A differential diagnosis includes oral lichenoid drug reactions. Allopurinol, which is the first line hypo-uricemic treatment, is often quoted as being a possible offending drug, though oral reactions have rarely been reported. Case presentation: We describe a 59-year-old male gout patient, successfully treated with allopurinol, who developed acute onset of oral lichenoid lesions, involving bilaterally the buccal mucosa, the tongue and the labial mucosa. Histopathology was consistent with a lichen planus or a drug-induced lichenoid reaction. Improvement of the patient’s condition after withdrawal of allopurinol confirmed the lichenoid nature of the lesion. Remission was complete after a few weeks. BACKGROUND Heat shock proteins (HSPs) are released in response to stress situations, such as heat... more BACKGROUND Heat shock proteins (HSPs) are released in response to stress situations, such as heat, inflammation, infection. They are also involved in the tumor cell proliferation and prevention of apoptosis. Heat shock protein 105 (Hsp105/110) is a high-molecular-weight protein which has been reported in many cancer types but few studies have been carried out on oral squamous cell carcinoma (OSCC). In the current study we have focused on HSP105 expression on OSCC and evaluated their correlation with tumor clinicopathological parameters and patients' survival. METHODS A retrospective study included 70 patients with OSCC of which 50 patients (71.4%) were male and 20 (28.6%) were female. The patient´s information, including age, location, TNM stage, histological grade, regional metastasis, recurrence and survival, were collected. Immunohistochemical staining for HSP105 was performed. The healthy oral mucosa (n=10) was used as a control. The staining intensity and percentage of stained cells were semi-quantitatively evaluated and HSP105 expression was correlated with tumor clinicopathological features and patient survival. RESULTS Statistical analysis for HSP105 showed that there was no significant correlation with tumor clinicopathological features. However, HSP105 overexpression was associated with a decrease in the duration of patients' survival (p= 0.042). CONCLUSION This result suggests that the increased expression of the HSP105 in the OSCC could be a prognostic factor for malignancy.

The objective of the study is to present the clinico-pathological features of cystic and classic ... more The objective of the study is to present the clinico-pathological features of cystic and classic adenomatoid odontogenic tumors (AOTs) in order to identify the differences between the two variants of AOT. Materials and method: The study sample comprised of 41 AOTs, which were categorized into cystic and classic AOTs. Cystic AOTs are diagnosed as such when macroscopic and microscopic evidence of a cyst is present together with histopathological criteria of AOT (WHO–2017). Results: The study sample comprised of eleven cystic and thirty classic AOTs. Eight cystic AOTs were regarded as arising from dentigerous cysts as these lesions were attached to the cemento-enamel junction of the impacted teeth. Though not statistically significant, in contrast to classic AOTs which showed female predilection, cystic AOTs were more prevalent in males. Oral non-Hodgkin lymphomas are uncommon, and those arising on the gingiva are rare. A case of non... more Oral non-Hodgkin lymphomas are uncommon, and those arising on the gingiva are rare. A case of non-Hodgkin lymphoma of the gingiva that was erroneously diagnosed and treated as a dental abscess is presented. Clinical examination showed a firm and ulcerated mass extending from the mandibular right first premolar region to the retromolar trigone. Resorption of the alveolar ridge was radiographically observed. Histologic analysis revealed a large B-cell non-Hodgkin lymphoma. No other evidence of the disease was found except in the mandible. The patient was treated with chemotherapy that resulted in complete regression. As primary non-Hodgkin lymphoma of the gingiva can be misdiagnosed as a trivial inflammatory lesion, a histologic examination should be mandatory in those cases in which symptoms do not subside after specific treatment.

The nasopalatine duct cyst (NPDC) is a developmental cyst of the anterior palate's midline, usual... more The nasopalatine duct cyst (NPDC) is a developmental cyst of the anterior palate's midline, usually presenting as an asymptomatic swelling located just behind the maxillary central incisors. It is the most common non-odontogenic cyst of the jaws but is seen rarely in children. The purpose of this paper was to report an unusual case of nasopalatine duct cyst in a 7-year-old boy who presented with a slow-growing, slight swelling of the anterior palate together with malpositioned permanent maxillary central incisors. Although rare in children, NPCD should be included in the differential diagnosis of anterior palate swelling, particularly if associated with malpositioned maxillary central incisors.

Las lesiones orales son importantes, porque en algunas ocasiones preceden a las manifestaciones intestinales y sistemicas. En este estudio retrospectivo, se revisan los datos clinicos e histopatologicos de las fichas de 5 pacientes con enfermedad de Crohn diagnosticados en el Departamento de Medicina Oral y Patologia Maxilofacial de la Facultad de Odontologia de la Universidad Aristotle de Tesalonica, Grecia y de la Division de Estomatologia y Cirugia Oral de la Facultad de Medicina de la Universidad de Ginebra de Ginebra, Suiza. Los signos clinicos que se encontraron en los 5 pacientes fueron erosiones, ulceras profunndas, mucosa bucal en empedrado, lenguetas mucosas, e inflamacion labial. El examen histopatologico revelo una inflamacion granulomatosa con granulomas no caseificantes y fisuras profundas de la mucosa oral. En los 5 pacientes, las lesiones orales fueron los primeros signos de la enfermedad. El diagnostico de la enfermedad de Crohn se confirmo mediante colonoscopia y biopsia de las lesiones colonicas. Las lesiones orales pueden reepresentar lesiones importantes y/o signos iniciales de la enfermedad de Crohn. El reconocimiento de estas lesiones puede permitir un diagnostico precoz.

In the head and neck region, synovial sarcomas (SS) are rare tumours. We describe the diagnostic ... more In the head and neck region, synovial sarcomas (SS) are rare tumours. We describe the diagnostic approach to SS based on two cases which developed in a 26-year-old male in the face and in a 53-year-old female on the alveolar mucosa of the upper jaw. The demographic profile of the patients was compatible with the literature. Histopathologically, both tumours presented as unencapsulated spindle cell tumours arranged into short fascicles. Although the chromosomal translocation of t(X;18)(p11.2;q11.2), transducin-like enhancer of split 1 (TLE-1) and SMARCB1 antibodies derived from gene expression studies are considered as the most sensitive makers to diagnose SS, these facilities were not available. Therefore, our cases were diagnosed as monophasic fibrous SS, utilizing a panel of immunohistochemical markers, including cytokeratins, EMA, Bcl-2, and CD99 as positive indicators and CD34, SMA, MYO-D, and S-100 as negative indicators. PAS staining was used to identify glycogen and to exclude spindle cell carcinomas and leiomyosarcoma, while Alcian blue was used to identify myxoid ground substance and to exclude nodular fasciitis. In conclusion, SS, although rare, should be included in the differential diagnosis of spindle cell tumours of the face and oral mucosa.

In lateral regions where cells were in close apposition, the majority of assembling viral particles in the process of budding from the producing cell had their apex already engaged in clathrin-coated pits of the neighbouring cell surface. These observations suggest that the viral envelope-plasma membrane interaction triggers the focal formation of clathrin-coated pits; they also show how VSV infection could spread throughout a tissue with only minimal exposure to a host's extracellular environment. Oral malignant melanoma (OMM) is rare, representing less than 0.5% of all oral malignancies. The ... more Oral malignant melanoma (OMM) is rare, representing less than 0.5% of all oral malignancies. The most affected sites are the palate and the maxillary gingiva. Histological examination is important to establish the diagnosis of any suspicious pigmented lesion in the oral cavity, mainly if a precise clinical diagnosis is not possible. We present one case of OMM that was initially diagnosed as a racial pigmentation elsewhere 2 years earlier. Clinical examination showed multiple macules and nodules located on the hard and soft palate, gingiva and superior alveolar mucosa. These lesions were painless and presented a color variation going from dark blue to black. Histological analysis showed sheets and nests of atypical melanocytes displaying a range of shapes such as plasmacytoid, epithelioid, and round cells, located in the superficial corium extending to the deep tissues. A few tumor cells contained variable amounts of melanin. There was no invasion of blood vessels or nerve fibers. Immunohistochemical analysis revealed that the neoplastic cells were positive for HMB-45, melan-A, S-100 and negative for AE1/AE3, confirming the diagnosis of melanoma. The Ki-67 labeling index was around 25%. The patient refused any treatment and died 11 months later.

Objective. To evaluate the association between sinus bone graft and the development of fungus bal... more Objective. To evaluate the association between sinus bone graft and the development of fungus ball (FB) of the maxillary sinus. Study Design. The charts of all patients seen for surgical treatment of maxillary sinus FB following sinus bone grafting between 2006 and 2014 were reviewed. The charts of 49 participants were selected from our internal registry for comparison as controls. The association between FB and age, gender, smoking habits, associated co-morbidities, and bone grafting material was evaluated. FB of the maxillary sinus was estimated by using an odds ratio with a Yates' correction. P values were computed using Fisher's exact test, and the statistical significance was set at a P value < .05. Results. All 13 cases (100%) with FB of the maxillary sinus had received anorganic bovine bone as the bone substitute (P ¼ .0001). There were significantly more women in the case group than in the control group (84.6% in the cases vs 40.8% in the controls; P ¼ .01). Conclusions. This study demonstrated a significant association between a specific deproteinized bovine bone substitute use as sinus bone graft material and subsequent development of FB of the maxillary sinus.

PAS staining was used to identify glycogen and to exclude spindle cell carcinomas and leiomyosarcoma, while Alcian blue was used to identify myxoid ground substance and to exclude nodular fasciitis. In conclusion, SS, although rare, should be included in the differential diagnosis of spindle cell tumours of the face and oral mucosa.

In lateral regions where cells were in close apposition, the majority of assembling viral particles in the process of budding from the producing cell had their apex already engaged in clathrin-coated pits of the neighbouring cell surface. These observations suggest that the viral envelope-plasma membrane interaction triggers the focal formation of clathrin-coated pits; they also show how VSV infection could spread throughout a tissue with only minimal exposure to a host's extracellular environment. Oral malignant melanoma (OMM) is rare, representing less than 0.5% of all oral malignancies. The ... more Oral malignant melanoma (OMM) is rare, representing less than 0.5% of all oral malignancies. The most affected sites are the palate and the maxillary gingiva. Histological examination is important to establish the diagnosis of any suspicious pigmented lesion in the oral cavity, mainly if a precise clinical diagnosis is not possible. We present one case of OMM that was initially diagnosed as a racial pigmentation elsewhere 2 years earlier. Clinical examination showed multiple macules and nodules located on the hard and soft palate, gingiva and superior alveolar mucosa. These lesions were painless and presented a color variation going from dark blue to black. Histological analysis showed sheets and nests of atypical melanocytes displaying a range of shapes such as plasmacytoid, epithelioid, and round cells, located in the superficial corium extending to the deep tissues. A few tumor cells contained variable amounts of melanin. There was no invasion of blood vessels or nerve fibers. Immunohistochemical analysis revealed that the neoplastic cells were positive for HMB-45, melan-A, S-100 and negative for AE1/AE3, confirming the diagnosis of melanoma. The Ki-67 labeling index was around 25%. The patient refused any treatment and died 11 months later.

To evaluate the association between sinus bone graft and the development of fungus ball (FB) of the maxillary sinus. Study Design. The charts of all patients seen for surgical treatment of maxillary sinus FB following sinus bone grafting between 2006 and 2014 were reviewed. The charts of 49 participants were selected from our internal registry for comparison as controls. The association between FB and age, gender, smoking habits, associated co-morbidities, and bone grafting material was evaluated. FB of the maxillary sinus was estimated by using an odds ratio with a Yates' correction. P values were computed using Fisher's exact test, and the statistical significance was set at a P value < .05. Results. All 13 cases (100%) with FB of the maxillary sinus had received anorganic bovine bone as the bone substitute (P ¼ .0001). There were significantly more women in the case group than in the control group (84.6% in the cases vs 40.8% in the controls; P ¼ .01). Conclusions. This study demonstrated a significant association between a specific deproteinized bovine bone substitute use as sinus bone graft material and subsequent development of FB of the maxillary sinus.
