To secure business continuity is indispensable for hospitals to fulfill its social responsibility... more To secure business continuity is indispensable for hospitals to fulfill its social responsibility under disasters. Although to back up the data of the hospital information system (HIS) at multiple remote sites is a key strategy of business continuity plan (BCP), the requirements to treat privacy sensitive data jack up the cost for the backup. The secret sharing is a method to split an original secret message up so that each individual piece is meaningless, but putting sufficient number of pieces together to reveal the original message. The secret sharing method eases us to exchange HIS backups between multiple hospitals. This paper evaluated the feasibility of the commercial secret sharing solution for HIS backup through several simulations. The result shows that the commercial solution is feasible to realize reasonable HIS backup exchange platform when template of contract between participating hospitals is ready.

This study aimed to describe the temporal trends in the administration of life-sustaining treatments (LSTs) and intensive care unit (ICU) admissions just before death in older inpatients in Japan. We utilized the National Database of Health Insurance Claims and Specific Health Checkups of Japan (NDB). Inpatients who were aged ≥65 years and died in October of 2012, 2013, or 2014 were analyzed. The numbers of decedents in 2012, 2013, and 2014 were 3362, 3473, and 3516, respectively. The frequencies of receiving cardiopulmonary resuscitation (CPR) (11.0% to 8.3%), mechanical ventilation (MV) (13.1% to 9.8%), central venous catheter (CVC) insertion (10.6% to 7.8%), and ICU admission (9.1% to 7.8%), declined between 2012 and 2014. After adjusting for age, sex, and type of ward,...

Creating a dataset appropriate for specific analysis requires technical expertise and familiarity with data analysis. The aim of our research is to develop a data warehouse (DW) accessible to researchers of epidemiology without this expertise. Methods: We began by adding commonly used attributes in the epidemiological field to the National Database of Health Insurance Claims of Japan (NDB), to construct a Research Question Oriented DB. Secondly, we developed a versatile analysis unit schema by which the Research Question Oriented DW was reconstructed as perpatient units, covering demographics including sex, age group etc. We then proposed a pattern relational calculus by which research-specific attributes can be added without expert knowle...

Because they intermingle with players’ lives and naturally promote more casual gameplay, they could be a powerful strategy to stimulate physical activity among older adults. However, to use these games more effectively, it is necessary to understand how design elements of the game affect player behavior. OBJECTIVE The aim of this study was to evaluate how the presence of a specific design element, namely social interaction, would affect levels of physical activity. METHODS Participants were recruited offline and randomly assigned to control and intervention groups in a single-blind design. Over 4 weeks, two variations of the same pervasive game were compared: with social interaction (intervention group) and with no social interaction (control group). In both versions, players had to walk to physical locations and collect virtual cards, but the social...

Access to a centralized health record system is ideal but not always available to physicians. A technological gap exists in the context of PI sharing regarding patients’ representative and their in-situ involvement as an alternative PI source. The purpose of this study is to understand emergency physicians’ perceptions about patients’ representative and our design concept for improvement of PI sharing to physicians. Methods: Observations focused on physician’s interaction with representatives were used to understand the emergency setting which led to a mobile PI sharing application design. Semi-structured interviews were conducted with three experienced physicians to obtain their perceptions about the usefulness of our proposed design. Results: Physicians shared their experiences about interactions with patient representatives and perceptions about potential use our propo...

In this paper, a NISHIJIN e-textile sensor belt prototype designed for ambulatory measurement (pre-hospital 12-lead ECG measurement) was tested on twenty volunteers. The recorded data was evaluated through the result of commercial automatic ECG analyzer. The obtained results are almost equivalent with the ones obtained through conventional ECG electrodes. However, the results suggest slight modification of the prototype.

We prototyped a system which obtains context from positions of nurses and list of expected clinical procedures. A WoZ test showed that the proposed approach has potential to decrease incidents caused by information delivery error.
