Purpose: To evaluate the diagnostic value of ultrasonography (US) in detecting intraabdominal inj... more Purpose: To evaluate the diagnostic value of ultrasonography (US) in detecting intraabdominal injuries in patients with blunt abdominal trauma. Materials and methods: Blunt trauma patients admitted to the emergency department from January 2002 to August 2003 were retrospectively evaluated. A total of 454 patients with blunt abdominal trauma who underwent US examination were included. Ultrasonography results were compared with findings of CT, diagnostic peritoneal lavage, laparotomy and clinical course. Sensitivity, specificity, positive and negative predictive values of US in detecting free fluid, intraabdominal parenchymal organ injury or both were calculated. Results: Computed tomography, diagnostic peritoneal lavage and laparotomy results showed that intraabdominal organ injury was present in 37 of 454 patients. Ultrasonography examinations were positive in 51 patients. True-positive findings were seen in 32 of these patients. In these 32 patients, US examination showed free fluid in 19, fluid and abdominal organ injury in 11 and only abdominal organ injury in 2. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of US in detecting intraabdominal injury were 86.5%, 95.4%, 62.7%, 98.7% and 94.7%, respectively. Conclusion: Ultrasonography has high diagnostic performance in the screening of patients with blunt abdominal trauma. Abdominal US is a useful and valuable diagnostic tool after clinical evaluation in patients with blunt abdominal trauma. Because of its high negative predictive value, we recommend that clinical follow up is adequate for patients whose US results are negative for intraabdomial organ injury.

Anahtar kelimeler: BNP, kronik kalp yetersizli¤i, nesiritid ✔ ✔ The Role of B-Type Natiuratic Peptide in Heart Failure B type natriuretic peptide (BNP) is a cardiac neurohormone secreted mainly from ventricles in response to volume expansion and pressure overload and it has diuretic, natriuertic and vasodilatator effects. Increased BNP level is important in determining heart failure in a patient presenting with respiratory failure. Additionally, it has been reported that BNP is useful in the diagnosis and classification of cardiac failure, determining the prognosis of cardiac failure, and assessing the efficiency of treatment in cardiac failure. Recently, it has been reported that nesiritid, a recombinant BNP is useful to use in addition to other therapy methods in severe cardiac failure. In this paper, we discuss the role in the diagnosis and prognosis of cardiac failure, and assessing the efficiency of treatment in cardiac failure.

Results: A total of 18,034 patients applied to the Emergency Department of Ondokuz Mayıs University Hospital during the study period and 284 of whom were followed-up as in patients in the emergency observation unit. The study was carried out with 565 randomly selected patients meeting the inclusion criteria. Of these patients, 187 (33.10%) were treated in the emergency observation unit and 378 (66, 90%) were treated in the emergency outpatient clinic. The rate of the patients who stated that they were satisfied with the service they received was 80.4% (304) in those receiving treatment in the emergency outpatient clinic while 88.2% (1165) in those receiving treatment in the emergency observation unit. Conclusion: Behaviors of nurses and physicians as well as their care and interventions were determined to be the most influential factors on patient satisfaction. In addition, it was revealed that there was a negative correlation between satisfaction and patient education level, where as a positive correlation between satisfaction and patient age and inpatient follow-up.

Ancak akut pankreatit mantar zehirlenmesinin nadir bir komplikasyonudur. Altmış altı yaşında kadın hasta yabani mantarı yemesinden 24 saat sonra bulantı, kusma ve karın ağrısı yakınmaları ile acil servisimize başvurdu. Hastanın fizik muayenesinde epigastrik bölgede hassasiyet vardı. Başvuru anında serum hepatik transaminazlar, amilaz ve lipaz düzeylerinde yükseklik saptandı. Çekilen bilgisayarlı batın tomografisinde peripankreatik mayi görüldü. Hasta mantar zehirlenmesine ve akut pankreatit tanılarıyla acil yoğun bakım ünitesine yatırıldı. Takip süresince klinik durumu iyi olan hastanın hepatik transaminazları ve pankreatik enzim düzeyleri progresif olarak azaldı. Komplikasyon gelişmeyen hasta şifa ile taburcu edildi. Mantar zehirlenmesinin ve akut pankreatitin erken semptomları benzerdir. Mantar zehirlenmeli hastalarda diğer organ yetersizliklerinin yanı sıra akut pankreatit olasılığı düşünülmelidir. Akut pankreatit ve mantar zehirlenmesinin erken tanısı ve uygun tedavisi iyi prognozun sağlanması ve komplikasyonların önlenmesine katkı sağlayabilir.

TCA zehirlenmesi olan hastaların değerlendirilmesinde “Antidepresan Doz Aşımı Risk Değerlendirmesi” (ADORA) kriterleri kullanılmıştır. Bununla birlikte, bu kriterlerin klinik uygulamadaki yararları hakkındaki veriler, ADORA kriterlerini doğrulamak için yetersizdir. Bu çalışmanın amacı, ADORA kriterlerinin klinik sonuç ile ilişkisini belirlemektir. Gereç ve Yöntem: Çalışmaya yıllık toplam 133 hasta dahil edildi. Hastalar ADORA kriterlerine göre yüksek risk ve düşük risk gruplarına ayrıldı. Bulgular:Ortanca yaş 25 (18-55 yaş aralığında) idi. Hastalar genellikle TCA ajanı olarak amitriptilin kullanmışlardır. This cross-sectional study was conducted among the patients with acute CO poisoning who referred to the emergency department in a 1-year period. Serum levels of S100B protein, NSE, and GFAP were determined on admission. A total of 55 CO-poisoned patients (mean age ± standard deviation, 45 ± 20.3 years; 60% women) were included in the study. The control group consisted of 25 healthy adults. The patients were divided into two groups according to whether they were conscious or unconscious. The serum levels of S100B, NSE, and GFAP were higher in patients than that in the control group. The S100B is a calcium-binding peptide and is used as a parameter of glial activation and/or death in many disorders of the central nervous system (CNS). It plays important roles in normal CNS development and recovery after injury. Although S100B is mainly found in astroglial and Schwann cells, it also has extracerebral sources. S100B is a useful neurobiochemical marker of brain damage such as in circulatory arrest, stroke and traumatic brain injury. S100B is also associated with neurodegenerative diseases like Alzheimer's disease or other chronic neurological diseases. Moreover, S100B may have a potential in predicting the efficiency of treatment and prognosis. In this review, an updated overview of the role of S100B in human neurological disorders is presented.

Pediyatrik hastaların anatomik ve fizyolojik farklılıkları ve bu hastalara yaklaşım konusunda standart sağlanamamış olması klinisyenlerin karşılaştıkları zorluklardır. Organize olmayan yaklaşımlar bu hasta grubunda mortalitenin artmasına neden olmaktadır. Pediyatrik travma konusunda özelleşmiş merkezlerin kurulması en iyi yöntemdir. Bu mümkün olamıyorsa acil servis çalışanlarının eğitim ve tecrübe seviyesi arttırılmalı ve diğer bölümlerle iş birliği içinde belli standartların saptanması gerekmektedir. Travmalı hastalar için belli algoritmaların belirlenmesi ve uygulanması hastalar için hayati önem taşıdığı kadar, acil serviste çalışanlar açısından iş yükünün azaltılmasında ve zaman kazanmada fayda sağlayacaktır. Biz bu çalışmamızda, güncel literatür ışığında pediyatrik travmaya yaklaşımın sistematik hale getirilmesine katkı sağlamayı amaçladık.

Objective: To assess the possible use of hyaluronic acid (HA) and interleukin-6 (IL-6) together a... more Objective: To assess the possible use of hyaluronic acid (HA) and interleukin-6 (IL-6) together as a biochemical marker of liver damage in mushroom poisoning (MP). Methods: We prospectively studied patients with MP who were admitted to the emergency service, between April 2005 and April 2007, Samsun, Turkey. Twenty-seven patients with MP were included in the study. Serum HA and IL-6 levels of the patients were determined using enzyme-linked immunosorbent assay daily for a total of 3 days. Ten healthy adults were included in the study to serve as controls. The patients were divided into survivors, and non-survivors. Results: There was no significant difference between the patients and controls with respect to serum HA levels on admission (p>0.05). However, IL-6 levels on admission were significantly higher in the patients than the control group (p<0.01). Serum HA and IL-6 levels on admission, and the following days were significantly higher in non-surviving patients (n=5) than in surviving patients (n=22) (p<0.05). There was a significant correlation between HA and IL-6 (r=0.42, p<0.05) on admission. The HA concentration was also significantly correlated with aspartate aminotransferase, alanine aminotransferase, and creatinine levels during the observation period. Serum HA and IL-6 levels increased in non-surviving patients throughout the period of observation. Conclusion: Increased serum HA and IL-6 levels are associated with hepatic damage in acute MP. Hyaluronic acid may be a useful marker in the assessment of MP-induced acute liver failure in clinical practice.

Biochemical markers can be helpful for emergency physicians in these critical situations. Heart-type fatty acid binding protein (H-FABP) is one of the promising plasma markers for the detection of tissue injury. H-FABP is known to be released from injured myocardium. It is also expressed in skeletal muscle, the kidney, brain, lactating mammary gland, and placenta. It can be useful in the management of acute coronary syndromes, heart failure, pulmonary embolism, renal and hepatic injury, and some cases of poisonings. In this review, an updated overview of the role of H-FABP in the management of diseases seen frequently in the Emergency Department is presented.

Wild mushroom poisoning (MP) is an important medical emergency that may have serious clinical out... more Wild mushroom poisoning (MP) is an important medical emergency that may have serious clinical outcome. The aim of this study was to evaluate the demographic and clinical features of patients with wild MP. This study was designed retrospectively by examining files of the patients with wild MP who were admitted to Ondokuz Mayis University Emergency Department, between January 2002 and December 2007. Patients ≥16 years of age were included in the study. A total of 317 patients poisoned by wild mushrooms (mean age, 42.0 ± 16.3 years; 67.5% female) were studied. All poisonings were accidental, i.e. consumption of wild mushrooms collected from open fields and woodlands or purchased from local bazaars. The common symptoms and complaints on admission were nausea (86.8%) and vomiting (79.8%). The poisoning latent phase in most cases was &lt;6 hours (86.8%). Most of the poisonings occurred in autumn (59.6%). Three patients died in the hospital due to acute liver failure and complications. The duration of hospitalization was a median 3 days (range: 1—12 days). The public should be informed about the probable hazards of wild mushroom ingestion.

Accidental ingestion of the plant could result in central nervous system depression, respiratory failure and even death. Airway and breathing management followed by fluid resuscitation is the mainstay of treatment. Poison hemlock poisoning is an important differential diagnosis for patients with impaired consciousness and respiratory failure after plant ingestion in areas of natural habitat for the plant. Early and proper diagnosis is vital for patients with poison hemlock poisoning. In this report, we present a case of life-threatening poisoning due to taking the poison hemlock mistaken for Helichrysum arenarium.

The study was carried out with 565 randomly selected patients meeting the inclusion criteria. Of these patients, 187 (33.10%) were treated in the emergency observation unit and 378 (66, 90%) were treated in the emergency outpatient clinic. The rate of the patients who stated that they were satisfied with the service they received was 80.4% (304) in those receiving treatment in the emergency outpatient clinic while 88.2% (1165) in those receiving treatment in the emergency observation unit. Conclusion: Behaviors of nurses and physicians as well as their care and interventions were determined to be the most influential factors on patient satisfaction. In addition, it was revealed that there was a negative correlation between satisfaction and patient education level, where as a positive correlation between satisfaction and patient age and inpatient follow-up.

Ancak akut pankreatit mantar zehirlenmesinin nadir bir komplikasyonudur. Altmış altı yaşında kadın hasta yabani mantarı yemesinden 24 saat sonra bulantı, kusma ve karın ağrısı yakınmaları ile acil servisimize başvurdu. Hastanın fizik muayenesinde epigastrik bölgede hassasiyet vardı. Başvuru anında serum hepatik transaminazlar, amilaz ve lipaz düzeylerinde yükseklik saptandı. Çekilen bilgisayarlı batın tomografisinde peripankreatik mayi görüldü. Hasta mantar zehirlenmesine ve akut pankreatit tanılarıyla acil yoğun bakım ünitesine yatırıldı. Takip süresince klinik durumu iyi olan hastanın hepatik transaminazları ve pankreatik enzim düzeyleri progresif olarak azaldı. Komplikasyon gelişmeyen hasta şifa ile taburcu edildi. Mantar zehirlenmesinin ve akut pankreatitin erken semptomları benzerdir. Mantar zehirlenmeli hastalarda diğer organ yetersizliklerinin yanı sıra akut pankreatit olasılığı düşünülmelidir. Akut pankreatit ve mantar zehirlenmesinin erken tanısı ve uygun tedavisi iyi prognozun sağlanması ve komplikasyonların önlenmesine katkı sağlayabilir.

Serum levels of S100B protein, NSE, and GFAP were determined on admission. A total of 55 CO-poisoned patients (mean age ± standard deviation, 45 ± 20.3 years; 60% women) were included in the study. The S100B is a calcium-binding peptide and is used as a parameter of glial activation and/or death in many disorders of the central nervous system (CNS). It plays important roles in normal CNS development and recovery after injury. Although S100B is mainly found in astroglial and Schwann cells, it also has extracerebral sources. S100B is a useful neurobiochemical marker of brain damage such as in circulatory arrest, stroke and traumatic brain injury. S100B is also associated with neurodegenerative diseases like Alzheimer's disease or other chronic neurological diseases. Moreover, S100B may have a potential in predicting the efficiency of treatment and prognosis. In this review, an updated overview of the role of S100B in human neurological disorders is presented.

Pediyatrik hastaların anatomik ve fizyolojik farklılıkları ve bu hastalara yaklaşım konusunda standart sağlanamamış olması klinisyenlerin karşılaştıkları zorluklardır. Organize olmayan yaklaşımlar bu hasta grubunda mortalitenin artmasına neden olmaktadır. Pediyatrik travma konusunda özelleşmiş merkezlerin kurulması en iyi yöntemdir. Bu mümkün olamıyorsa acil servis çalışanlarının eğitim ve tecrübe seviyesi arttırılmalı ve diğer bölümlerle iş birliği içinde belli standartların saptanması gerekmektedir. Travmalı hastalar için belli algoritmaların belirlenmesi ve uygulanması hastalar için hayati önem taşıdığı kadar, acil serviste çalışanlar açısından iş yükünün azaltılmasında ve zaman kazanmada fayda sağlayacaktır. Biz bu çalışmamızda, güncel literatür ışığında pediyatrik travmaya yaklaşımın sistematik hale getirilmesine katkı sağlamayı amaçladık.

Objective: To assess the possible use of hyaluronic acid (HA) and interleukin-6 (IL-6) together a... more Objective: To assess the possible use of hyaluronic acid (HA) and interleukin-6 (IL-6) together as a biochemical marker of liver damage in mushroom poisoning (MP). Methods: We prospectively studied patients with MP who were admitted to the emergency service, between April 2005 and April 2007, Samsun, Turkey. Twenty-seven patients with MP were included in the study. Serum HA and IL-6 levels of the patients were determined using enzyme-linked immunosorbent assay daily for a total of 3 days. Ten healthy adults were included in the study to serve as controls. The patients were divided into survivors, and non-survivors. Results: There was no significant difference between the patients and controls with respect to serum HA levels on admission (p>0.05). However, IL-6 levels on admission were significantly higher in the patients than the control group (p<0.01). Serum HA and IL-6 levels on admission, and the following days were significantly higher in non-surviving patients (n=5) than in surviving patients (n=22) (p<0.05). There was a significant correlation between HA and IL-6 (r=0.42, p<0.05) on admission. The HA concentration was also significantly correlated with aspartate aminotransferase, alanine aminotransferase, and creatinine levels during the observation period. Serum HA and IL-6 levels increased in non-surviving patients throughout the period of observation. Conclusion: Increased serum HA and IL-6 levels are associated with hepatic damage in acute MP. Hyaluronic acid may be a useful marker in the assessment of MP-induced acute liver failure in clinical practice.

Biochemical markers can be helpful for emergency physicians in these critical situations. Heart-type fatty acid binding protein (H-FABP) is one of the promising plasma markers for the detection of tissue injury. H-FABP is known to be released from injured myocardium. It is also expressed in skeletal muscle, the kidney, brain, lactating mammary gland, and placenta. It can be useful in the management of acute coronary syndromes, heart failure, pulmonary embolism, renal and hepatic injury, and some cases of poisonings. In this review, an updated overview of the role of H-FABP in the management of diseases seen frequently in the Emergency Department is presented.

The aim of this study was to evaluate the demographic and clinical features of patients with wild MP. This study was designed retrospectively by examining files of the patients with wild MP who were admitted to Ondokuz Mayis University Emergency Department, between January 2002 and December 2007. Patients ≥16 years of age were included in the study. A total of 317 patients poisoned by wild mushrooms (mean age, 42.0 ± 16.3 years; 67.5% female) were studied. All poisonings were accidental, i.e. consumption of wild mushrooms collected from open fields and woodlands or purchased from local bazaars. The common symptoms and complaints on admission were nausea (86.8%) and vomiting (79.8%). The poisoning latent phase in most cases was &lt;6 hours (86.8%). Most of the poisonings occurred in autumn (59.6%). Three patients died in the hospital due to acute liver failure and complications. The duration of hospitalization was a median 3 days (range: 1—12 days). The public should be informed about the probable hazards of wild mushroom ingestion.

Accidental ingestion of the plant could result in central nervous system depression, respiratory failure and even death. Airway and breathing management followed by fluid resuscitation is the mainstay of treatment. Poison hemlock poisoning is an important differential diagnosis for patients with impaired consciousness and respiratory failure after plant ingestion in areas of natural habitat for the plant. Early and proper diagnosis is vital for patients with poison hemlock poisoning. In this report, we present a case of life-threatening poisoning due to taking the poison hemlock mistaken for Helichrysum arenarium.
