In this report, we discuss the demographic features of patients with post-operative chylothorax, and the treatment approaches used to emphasize the importance of this complication for thoracic surgery. Materials and Methods: Medical records of 13 patients operated between January 2001 and July 2017, and who were diagnosed with chylothorax in the post-operative period, were retrospectively evaluated. Patients' demographic features, type of surgeries, results of histopathological examination, day of chylothorax diagnosis, and treatment results were recorded. Results: Eight male patients (61.5%) and 5 female patients (38.5%) were included in the study. Chylothorax developed after lobectomy in 9 patients (62.23%), total pleurectomy in 2 patients (15.38%), bilobectomy in 1 patient (7.69%), and pneumonectomy in 1 patient (7.69%). Oral nutrition was discontinued immediately after chylothorax diagnosis, and patients were followed-up with parenteral nutrition. A conservative approach was used in 11 patients (84.61%). Two patients (15.39%) did not respond to medical treatment, and underwent surgery. All patients were successfully treated chylothorax. Conclusions: Chylothorax is a rare but serious post-operative complication after thoracic surgeries. While appropriate conservative treatment leads to recovery in majority of the cases, surgery remains an important option for patients with prolonged drainage and persistent disease.

Torasik splenozis, dalak dokusunun sol hemitoraksa ototransplantasyonunu içeren, dalak ve sol hem... more Torasik splenozis, dalak dokusunun sol hemitoraksa ototransplantasyonunu içeren, dalak ve sol hemidiyafragmanın eşzamalı rüptürü sonucu gelişen nadir bir durumdur. Bu yazıda, sırt ağrısı şikayeti nedeniyle muayene edilen ve akciğer grafisi ve göğüs bilgisayarlı tomografisinde malign olduğu değerlendirilen sol plevral nodülleri olan 61 yaşında bir kadın hasta sunuldu. Hastanın travmatik dalak ve sol diyafragma rüptürü öyküsü vardı. Nodüller için video yardımlı torakoskopik biyopsi uygulandı ve histolojik inceleme nodüllerin dalak dokusu olduğunu doğruladı. Torasik splenozis, sol tek plevral nodülü ya da multipl plevral nodülleri ve geçmişinde dalak ve sol diyafragma rüptürü olan hastalarda akılda tutulması gereken bir patolojidir.

Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor ... more Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor antibodies (AchR-Ab) against nicotinic acetylcholine receptors at the postsynaptic neuromuscular junction. In this study, we investigated the effect of AchR-Ab levels to symptoms and treatment in the MG patients underwent surgery for thymic pathology. Materilas and Methods: The records including level of preoperative AchR-Ab, type of thymic pathology, the changes of symptoms and treatment after surgery for thymic pathology of 37 MG patients between January 2007 and December 2015 have been viewed retrospectively. Results: The mean age of 37 patients (21 females, 16 males) was 40.2 ± 14.9 years (range, 18 to 75 years). The mean of the level of AchR-Ab was 144.7 ± 427.6 nmol/L (range, 0.1 to 1806 nmol/L). In the patient group including the reduced use of anticholinesterase after surgery the mean of level of AchR-Ab was 241.3 nmol/L while it was 10.8 nmol/L in the patient group including the non-reduced use of anticholinesterase (p = 0.082) after a mean follow-up period of 23 months postoperatively. The mean levels were 246.7 nmol/L and 8.5 nmol/L for the reduced and non-reduced use of corticosteroid patient groups, respectively (p = 0.001). In the 25 patients with fewer symptom after surgery the mean of the AchR-Ab level was 205.3 nmol/L while it was 18.3 nmol/L in the patients without any changes (p = 0.071). Conclusion: We concluded that the preoperative level of AchR-Ab was associated with postoperative dosage of anticholinesterase and corticosteroids and severity of symptoms postoperatively. We think that the levels of AchR-Ab titers can be a marker for the efficacy of thymic surgery.

Aims and objectives: The incidence of primary spontaneous pneumothorax (PSP) is 6-7/100000 among ... more Aims and objectives: The incidence of primary spontaneous pneumothorax (PSP) is 6-7/100000 among all population. Although its etiology is unknown, it is thought to be occurring due to rupturation of bullae or bleb formations localized in apex of lung of tall and thin young adults. In this study, we aimed to evaluate the relation between puberty growth and PSP. Methods: The records of 246 patients who were treated for PSP between 2013-2015 were analyzed. A questionnaire designed by pediatric endocrinology specialists about puberty was performed to 80 patients who answered the recall for an outpatient control. Data about pubertal development and rate was collected. Results: The mean of age and length of patients (74 male, 6 female) were 26.05 ± 7.2 years (range 17 to 34 years) and 179.73 ± 7.2 cm (range, 165 to 197 cm), respectively. Although few number of female patients, their mean age of menarch was 14.50 years and it was higher than the mean age of menarch of Turkish females (12.20 years). The mean age of facial hair shaving of males was 16.81 ± 1.7 years (range, 13 to 22 years) and it was higher than the mean age, 14.9 years, that was reported for US population. After exclusion of females and smokers, the means of length and age of facial hair growing of 33 patients, were 181.09 cm and 16.76 years respectively. Twenty patients in this group described a rapid growth in length after age of 16 years compared to the others at the same age. Conclusions: In this study, it had been seen that the mean puberty age of PSP patients was higher than normal population and their pubertal growth attack was occured in a shorter period. We think that these factors effect the maturation of lung and cause to PSP.

From January 2001 through June 2002, 128 consecutive patients undergoing lung resection for vario... more From January 2001 through June 2002, 128 consecutive patients undergoing lung resection for various diseases were evaluated preoperatively by a cardiologist at our institution in order to predict postoperative cardiac complications in pulmonary surgery. Our assessment algorithm consisted of the following: Smoking, hypertension, hyperlipidemia, advanced age, diabetes mellitus, and history of cardiac disease or angina pectoris were considered as risk factors. A stress test was performed when at least 2 of the first 3 risk factors or at least 1 of the last 3 risk factors was present. Coronary angiography was performed in the case of a positive stress test. Patients were classified as low-risk when there was no need for a stress test. Patients were considered as intermediate-risk when a stress test revealed no ischemia or if there was a history of congestive heart failure or valvular disease. A nasogastric tube (NGT) had been inserted due to the complaints of refusal of oral nutrition and... more A nasogastric tube (NGT) had been inserted due to the complaints of refusal of oral nutrition and aglutition to a sixty-one years old female with schizophrenia. Disturbance in general status, hypotension, dispnea and tachicardia were observed after the initiation of enteral feeding. Chest X-ray revealed left hydropneumothorax therewith tube thoracostomy was performed. Enteral feeding fluid drainage was observed. Thorax computed tomography showed NGT to be in the left pleural space. The patient was followed up after the removal of NGT and the tube thoracostomy was ended on the sixteenth day. The patient was discharged from the hospital without any sequela. Although insertion of NGT is a simple procedure it can result in serious complications like epistaxis, pharyngeal perforation, osephagial rupture and pneumothorax. The localization of the tube must be corrected after insertion in order to avoid and detect these serious complications.

Background: Cryotherapy is an actual treatment and diagnostic method in many fields of medicine. ... more Background: Cryotherapy is an actual treatment and diagnostic method in many fields of medicine. We aimed to evaluate the acute histopathological effects of cryotherapy application on lung tissue. Materials and Methods: In this study 10 male rabbits were used. After providing single lung ventilation of the rabbits, clamshell incision was performed for exploration. Four different cryotherapy applications were performed to determine the different histopathologic changes on atelectatic and ventilated lung, also the difference between one versus two-cycle applications. After all applications, firstly, air leak was checked, then specimens were taken to analyze the acute histopathologic effects. Results: After cryotherapy application, no air leak was detected on ventilated rabbit lung with a pressure of 30 cmH2O. There was no bleeding from lung tissue, but macroscopic parenchymal hematoma developed. The depth of necrosis in lung tissue was evaluated with histopathological analysis. In all samples, alveolar edema and congestion were observed but there was no statistical difference between the depth of necrosis regarding with different cryotherapy applications. Conclusions: This study indicated that different applications of cryotherapy on lung parenchyma is a safe method and do not affect the depth of necrosis in a statistically significant degree.

Aim: To evaluate the results of positron emission tomography (PET) in patients who underwent resection after preliminary diagnosis of malignancy based on fluorodeoxyglucose (FDG) uptake value, in whom the lesions were later classified as pathologically benign. Material and methods: The analysis included the records of 106 (12.3%) patients out of 862 patients who underwent surgery between January 2012 and December 2015 after being initially diagnosed with malignant lung lesions based on PET-CT results, in whom the lesions were later classified as pathologically benign. Diagnoses, PET findings, types of surgery, and demographic data of the patients were recorded. Results: The mean age of the patients was 55.5 (26-79) years. The mean diameter and SUVmax of the lesions were 2 ±2.14 (0.5-13) and 3.55 ±4.35 (0-22.2) cm, respectively. The pathology results were analyzed in five different groups. The SUVmax in the hamartoma group was significantly lower than in the other groups (p < 0.001), while the SUVmax in the granulomatous disease group was significantly higher than in the other groups (p < 0.001). Conclusions: The possibility of false positive PET results must be kept in mind when diagnosing and treating lung cancer. In particular, in the case of suspected granulomatous disease, all available pre-and intraoperative diagnostic procedures must be used.

From January 2001 through June 2002, 128 consecutive patients undergoing lung resection for vario... more From January 2001 through June 2002, 128 consecutive patients undergoing lung resection for various diseases were evaluated preoperatively by a cardiologist at our institution in order to predict postoperative cardiac complications in pulmonary surgery. Our assessment algorithm consisted of the following: Smoking, hypertension, hyperlipidemia, advanced age, diabetes mellitus, and history of cardiac disease or angina pectoris were considered as risk factors. A stress test was performed when at least 2 of the first 3 risk factors or at least 1 of the last 3 risk factors was present. Coronary angiography was performed in the case of a positive stress test. Patients were classified as low-risk when there was no need for a stress test. Patients were considered as intermediate-risk when a stress test revealed no ischemia or if there was a history of congestive heart failure or valvular disease. A nasogastric tube (NGT) had been inserted due to the complaints of refusal of oral nutrition and... more A nasogastric tube (NGT) had been inserted due to the complaints of refusal of oral nutrition and aglutition to a sixty-one years old female with schizophrenia. Disturbance in general status, hypotension, dispnea and tachicardia were observed after the initiation of enteral feeding. Chest X-ray revealed left hydropneumothorax therewith tube thoracostomy was performed. Enteral feeding fluid drainage was observed. Thorax computed tomography showed NGT to be in the left pleural space. The patient was followed up after the removal of NGT and the tube thoracostomy was ended on the sixteenth day. The patient was discharged from the hospital without any sequela. Although insertion of NGT is a simple procedure it can result in serious complications like epistaxis, pharyngeal perforation, osephagial rupture and pneumothorax. The localization of the tube must be corrected after insertion in order to avoid and detect these serious complications.

Background: Cryotherapy is an actual treatment and diagnostic method in many fields of medicine. ... more Background: Cryotherapy is an actual treatment and diagnostic method in many fields of medicine. We aimed to evaluate the acute histopathological effects of cryotherapy application on lung tissue. Materials and Methods: In this study 10 male rabbits were used. After providing single lung ventilation of the rabbits, clamshell incision was performed for exploration. Four different cryotherapy applications were performed to determine the different histopathologic changes on atelectatic and ventilated lung, also the difference between one versus two-cycle applications. After all applications, firstly, air leak was checked, then specimens were taken to analyze the acute histopathologic effects. Results: After cryotherapy application, no air leak was detected on ventilated rabbit lung with a pressure of 30 cmH2O. There was no bleeding from lung tissue, but macroscopic parenchymal hematoma developed. The depth of necrosis in lung tissue was evaluated with histopathological analysis. In all samples, alveolar edema and congestion were observed but there was no statistical difference between the depth of necrosis regarding with different cryotherapy applications. Conclusions: This study indicated that different applications of cryotherapy on lung parenchyma is a safe method and do not affect the depth of necrosis in a statistically significant degree.

Aim: To evaluate the results of positron emission tomography (PET) in patients who underwent resection after preliminary diagnosis of malignancy based on fluorodeoxyglucose (FDG) uptake value, in whom the lesions were later classified as pathologically benign. Material and methods: The analysis included the records of 106 (12.3%) patients out of 862 patients who underwent surgery between January 2012 and December 2015 after being initially diagnosed with malignant lung lesions based on PET-CT results, in whom the lesions were later classified as pathologically benign. Diagnoses, PET findings, types of surgery, and demographic data of the patients were recorded. Results: The mean age of the patients was 55.5 (26-79) years. The mean diameter and SUVmax of the lesions were 2 ±2.14 (0.5-13) and 3.55 ±4.35 (0-22.2) cm, respectively. The pathology results were analyzed in five different groups. The SUVmax in the hamartoma group was significantly lower than in the other groups (p < 0.001), while the SUVmax in the granulomatous disease group was significantly higher than in the other groups (p < 0.001). Conclusions: The possibility of false positive PET results must be kept in mind when diagnosing and treating lung cancer. In particular, in the case of suspected granulomatous disease, all available pre-and intraoperative diagnostic procedures must be used.
