Melioidosis is caused by Burkholderia pseudomallei, a gram negative bacterium which is an oxidase... more Melioidosis is caused by Burkholderia pseudomallei, a gram negative bacterium which is an oxidase positive, motile bacillus present in soil and water of tropical and subtropical areas. It can survive in extreme conditions for years and can infect both humans as well as animals. It is rarely transmitted from human to humans [1], however few cases of nosocomial infections have been reported. It is an emerging problem in India. The persons who are immune compromised or having diabetes mellitus, chronic liver disease, chronic alcoholism, chronic lung disease or occupational exposure are prone to develop it, though melioidosis can develop in persons without risk factors also. The spectrum of disease, severity and outcome varies depending upon the presence and absence of risk factors. The incubation period varies from 1 to 21 days with an average of 9 days. The acute infection can present as fatal septicaemia with septic shock, pneumonia, and acute respiratory distress syndrome, thrombosis of vessels, abscesses in the spleen, liver, lungs, kidneys, prostate, subcutaneous abscess, osteomyelitis and septic arthritis. The chronic infection may present with nonspecific symptoms to pyrexia of unknown origin, which makes the diagnosis difficult. Transmission of infection can occur via inhalation, aspiration and occasionally by percutaneous inoculation or ingestion. Cases of pneumonia following presumptive inoculating skin injuries are well documented, suggesting that the organism can reach the lungs via the haematogenous route [2]. Laboratory-acquired infections and iatrogenic infections from contaminated hospital or surgical equipment occasionally occur. Melioidosis during postpartum period is very rare and to best of our knowledge only one case of melioidosis following lower segment caesarean section (LSCS) was found while reviewing the available literature [3]. Here we report a case of a young lady who developed acute disseminated melioidosis after LSCS surgery.

Background: Cirrhosis progression varies greatly from patient to patient due to a variety of fact... more Background: Cirrhosis progression varies greatly from patient to patient due to a variety of factors, including hepatic reserve, cirrhosis etiology, and the presence of hepatocellular cancer. As a result, determining a prognosis in a patient with cirrhosis remains a difficult task. For nearly three decades, the Child-Pugh score (CPS) has been the gold standard for determining the prognosis of cirrhosis. In the last two decades, many prognostic models and scores like a model for end-stage liver disease (MELD), chronic liver failuresequential organ failure assessment (CLIF-SOFA) score, peripheral blood lymphocyte to monocyte ratio (LMR) have been presented to predict prognosis in patients with cirrhosis and to choose the best therapy option. The aim of our study is to determine which score is more effective in predicting three-month mortality and whether these scores are equally effective in predicting short-term outcomes. Materials & methods: In this hospital-based longitudinal study, we analyzed 140 patients with cirrhosis of liver visiting All India Institute of Medical Sciences Bhopal between July 2019 and July 2020. All the 140 patients were followed up for three months to establish short-term outcomes. The blood investigations were done at the time of presentation from all the patients and after three months in the survivors. Various scores were calculated. Results: The majority of patients (47%) were in Child-Pugh class C. Mean MELD score was 13.54, LMR score was 1.96 and CLIF-SOFA score was 5. The total bilirubin, serum creatinine, international normalized ratio (INR), total leukocyte count, absolute monocyte count, CPS, MELD, CLIF-SOFA were significantly higher in a non-surviving group as compared to the surviving group, whereas the albumin and LMR significantly decreased in the non-surviving group. On performing multivariate regression, LMR and CLIF-SOFA were significant independent risk factors of mortality after adjusting for confounding factors. All the parameters had significant discriminatory power to predict mortality. Discriminatory power of CLIF-SOFA (AUC 0.808; 95% CI: 0.733 to 0.870) was excellent and discriminatory power of CPS (AUC 0.792; 95% CI: 0.716 to 0.856), MELD score (AUC 0.765; 95% CI: 0.685 to 0.832) and LMR (AUC 0.75; 95% CI: 0.669 to 0.819) was acceptable. Among all the parameters, CLIF-SOFA was the best predictor of mortality at a cutoff point of >5 with 80.80% chances of correctly predicting mortality. Conclusion: The significant morbidity and mortality indicators are high total bilirubin, high creatinine, high INR, high TLC, low platelet count, and low albumin. Among the various scores, CLIF-SOFA is a better predictor of mortality and morbidity. Low LMR and high CLIF-SOFA are significant independent risk factors of mortality at three months.

Reduction in cycle time has been gaining significant attention in recent times. The shorter cycle times effect in higher consumer satisfaction, lower manufacturing rate, higher yield, and better potential given tool inventory and facility constraints. This research paper provides a brief review of core approaches related to cycle time and also describes a methodology for cycle time reduction in any manufacturing and automobile production industry. It includes the assessment and potential gains of the projected cycle time reduction methodology.[3]

It is due to a lesion affecting median longitudinal fasciculus, paramedian pontine reticular formation and facial nerve fascicle on the same side at the level of pons. The diagnosis is easily missed as it needs detailed ocular movement examination. It is mainly caused due to infarction or demyelinating conditions. We are reporting an interesting case of a 54-year-old man with right-side eight-and-a-half syndrome due to acute ischaemic stroke and ST-elevation myocardial infarction of the inferior wall.

The diagnosis is usually made by exclusion. A large number of debilitating conditions leading to a loss of fatty tissue such as anorexia nervosa, malabsorption, or hyper catabolic states like burns, major surgery, severe injuries, or malignancies can cause this syndrome. To our knowledge there are no cases reported in the Indian literature, though many such cases are being diagnosed and successfully treated the world over. We recently diagnosed and successfully treated a patient of S.M.A. syndrome in Shri Aurobindo Institute of Medical Sciences, Medical College and PostGraduate Institute. We take the opportunity to review the literature on S.M.A. syndrome.

This modern era's high technological improvements present manufacturers and other organizations w... more This modern era's high technological improvements present manufacturers and other organizations with a plethora of Management Information Systems (MISs) which makes them face challenges when choosing a corporate information system. High initial investment in setting up the information systems make it very difficult for companies to adopt new systems as they come into market before realizing a reasonable return from the previous system. In line with these concerns, a methodology for building a Balanced Scorecard module as a strategic management platform that can be integrated smoothly into already existing information system such as MRP/ERP is presented. The paper uses a case study of a manufacturing company based in Zimbabwe. Various manufacturing based metrics are reviewed with the main intent of showing how these can be tracked in a computerized platform. Sample data extracted from the production system is used to test the built system. The paper shows a methodology for software design, setting up and adopting a BSC system. The proposed approach is used to design a computerized BSC system for the case study company, which incorporates a BSC dashboard for the four main perspectives derived from various operational metrics.

UARS also presents with excessive daytime sleepiness (EDS), which is associated with impaired social functioning, work performance and driving ability. However, UARS is not merely a continuum between chronic snoring and obstructive sleep apnea (OSA). Not only UARS patients have equal gender distribution and thin body habitus but they also do not fulfil Polysomnographic criteria of OSA. UARS diagnosis requires oesophageal manometry for diagnosing increased respiratory efforts against increased upper airway resistance (without complete cessation of airflow or hypoxia) and correlating it with EEG arousals to mark an event. The diagnosis is easily missed as it needs detailed ocular movement examination. It is mainly caused due to infarction or demyelinating conditions. We are reporting an interesting case of a 54-year-old man with right-side eight-and-a-half syndrome due to acute ischaemic stroke and ST-elevation myocardial infarction of the inferior wall.

The diagnosis is usually made by exclusion. A large number of debilitating conditions leading to a loss of fatty tissue such as anorexia nervosa, malabsorption, or hyper catabolic states like burns, major surgery, severe injuries, or malignancies can cause this syndrome. To our knowledge there are no cases reported in the Indian literature, though many such cases are being diagnosed and successfully treated the world over. We recently diagnosed and successfully treated a patient of S.M.A. syndrome in Shri Aurobindo Institute of Medical Sciences, Medical College and PostGraduate Institute. We take the opportunity to review the literature on S.M.A. syndrome.

This modern era's high technological improvements present manufacturers and other organizations w... more This modern era's high technological improvements present manufacturers and other organizations with a plethora of Management Information Systems (MISs) which makes them face challenges when choosing a corporate information system. High initial investment in setting up the information systems make it very difficult for companies to adopt new systems as they come into market before realizing a reasonable return from the previous system. In line with these concerns, a methodology for building a Balanced Scorecard module as a strategic management platform that can be integrated smoothly into already existing information system such as MRP/ERP is presented. The paper uses a case study of a manufacturing company based in Zimbabwe. Various manufacturing based metrics are reviewed with the main intent of showing how these can be tracked in a computerized platform. Sample data extracted from the production system is used to test the built system. The paper shows a methodology for software design, setting up and adopting a BSC system. The proposed approach is used to design a computerized BSC system for the case study company, which incorporates a BSC dashboard for the four main perspectives derived from various operational metrics.

UARS also presents with excessive daytime sleepiness (EDS), which is associated with impaired social functioning, work performance and driving ability. However, UARS is not merely a continuum between chronic snoring and obstructive sleep apnea (OSA). Not only UARS patients have equal gender distribution and thin body habitus but they also do not fulfil Polysomnographic criteria of OSA. UARS diagnosis requires oesophageal manometry for diagnosing increased respiratory efforts against increased upper airway resistance (without complete cessation of airflow or hypoxia) and correlating it with EEG arousals to mark an event.
