Con esta cita de Spinoza comienza Erwin Schrodinger el Prefacio de su libro ?Que es la vida? [1]. En el ultimo capitulo y a la pregunta ?Esta basada la vida en las leyes de la fisica? responde Schrodinger: Por lo tanto, no debe desanimarnos que tengamos dificultad en interpretar la vida por medio de las leyes or dinarias de la Fisica. Eso es lo que cabria esperar de lo que hemos aprendido de la estructura de la materia viva. Debemos estar preparados para encontrar un nuevo tipo de ley fisica que la gobierne. ?O tendremos acaso que denominarla ley no-fisica, o incluso ley super-fisica? No, no creo que tengamos que llamarla ley no-fisica. Porque el nuevo principio subyacente es genuinamente fisico. En mi opinion, no es otra cosa que el mismo principio de la teoria cuantica.

There are several techniques for the treatment of lung cancer, among which radiotherapy is one of the most effective and least invasive for the patient. However, it has associated difficulties due to the moving target tumor. It is possible to reduce the side effects of radiotherapy by effectively tracking a tumor and reducing target irradiation margins. This paper presents a custom electromechanical system that follows the movement of a lung tumor. For this purpose, a hysteresis loop of human lung movement during breathing was studied to obtain its characteristic movement equation. The system is controlled by an Arduino, steppers motors and a customized 3D printed mechanism to follow the characteristic human breathing, obtaining an accurate trajectory. Purpose: The common use of nomograms in Low Dose Rate (LDR) permanent prostate brachytherapy (BT)... more Purpose: The common use of nomograms in Low Dose Rate (LDR) permanent prostate brachytherapy (BT) allows to estimate the number of seeds required for an implant. Independent dosimetry verification is recommended for each clinical dosimetry in BT. Also, nomograms can be useful for dose calculation quality assurance and they could be adapted to High Dose Rate (HDR). This work sets nomograms for LDR and HDR prostate-BT implants, which are applied to three different institutions that use different implant techniques. Material and methods: Patients treated throughout 2010 till April 2011 were considered for this study. This example was chosen to be the representative of the latest implant techniques and to ensure consistency in the planning. A sufficient number of cases for both BT modalities, prescription dose and different work methodology (depending on the institution) were taken into account. The specific nomograms were built using the correlation between the prostate vo lume and some characteristic parameters of each BT modality, such as the source Air Kerma Strength, number of implanted seeds in LDR or total radiation time in HDR. Results: For each institution and BT modality, nomograms normalized to the prescribed dose were obtained and fitted to a linear function. The parameters of the adjustment show a good agreement between data and the fitting. It should be noted that for each institution these linear function parameters are different, indicating that each centre should construct its own nomograms. Conclusions: Nomograms for LDR and HDR prostate brachytherapy are simple quality assurance tools, specific for each institution. Nevertheless, their use should be complementary to the necessary independent verification.

The H-type Leipzig applicators are accessories of the microSelectron-HDR system (Nucletron, Veene... more The H-type Leipzig applicators are accessories of the microSelectron-HDR system (Nucletron, Veenendaal, The Netherlands) for treatment of superficial malignancies. Recently, the dose rate distributions in liquid water for the whole set of applicators using both source models available for the microSelectron-HDR afterloaders have been obtained by means of the experimentally validated Monte Carlo (MC) code GEANT4. Also an output table (cGy/hU) at 3 mm depth on the applicator central axis was provided. The output verification of these applicators by the user, prior to their clinical use, present practical problems: small detectors such as thermoluminescent dosimeters or parallel-plate ionization chambers are not easily used for verification in a clinical environment as they require a rigid setup with the Leipzig applicator and a phantom. In contrast, well-type ionization chambers are readily available in radiotherapy departments. This study presents a technique based on the HDR1000Plus well chamber (Standar Imaging) measurements with a special insert, which allows the output verification of the H-type Leipzig applicators on a routine basis. This technique defines correspondence factors (CF) between the in water dose rate output of the Leipzig applicators (cGy/hU) obtained with MC and the reading on the well chamber with the special insert, normalized to the HDR calibration factor with the HDR insert and to the source strength. To commission the applicators (with the well chamber and the special insert used), the physicist should check if the CF value agrees with its tabulated values presented in this work. If the differences are within 5% the tabulated output values can be used in clinical dosimetry. This technique allows the output validation of the Leipzig applicators with a well chamber widely used for HDR Ir-192 source strength measurements. It can easily be adapted to other types of well chambers for HDR source output verification.

These six applicators were used in high-doserate (HDR) afterloaders with the "classic" and v2 192 Ir sources. The applicators have an inner diameter of 1, 2, and 3 cm, with the source traveling parallel or perpendicular to the contact surface. Methods and Materials: The MC GEANT4 code was used to obtain the dose-rate distribution in liquid water for the six applicators and the two HDR source models. To normalize the applicator output factors, a MC simulation for the "classic" and v2 sources in air was performed to estimate the air-kerma strength. To validate this specific application and to guarantee that realistic source-applicator geometry was considered, an experimental verification procedure was implemented in this study, in accordance with the TG43U1 recommendations. Thermolumniscent dosimeter chips and a parallel plate ionization chamber in a polymethyl methacrylate (PMMA) phantom were used to verify the MC results for the six applicators in a microSelectronHDR afterloader with the "classic" source. Dose-rate distributions dependence on phantom size has been evaluated using two different phantom sizes. Results: Percentage depth dose and off-axis profiles were obtained normalized at a depth of 3 mm along the central axis for both phantom sizes. A table of output factors, normalized to 1 U of source kerma strength at this depth, is presented. The dose measured in the PMMA phantom agrees within experimental uncertainties with the dose obtained by the MC GEANT4 code calculations. The phantom size influence on dose-rate distributions becomes significant at depths greater than 5 cm. Conclusions: MC-detailed simulation was performed for the Nucletron Leipzig HDR applicators. The matrix data obtained, with a grid separation of 0.5 mm, can be used to build a dataset in a convenient format to model these distributions for routine use with a brachytherapy treatment planning system.

In recent years, the use of high dose rate (HDR) after-loader machines has greatly increased due ... more In recent years, the use of high dose rate (HDR) after-loader machines has greatly increased due to the shift from traditional Cs-137/Ir-192 low dose rate (LDR) to HDR brachytherapy. The method used to calculate the required concrete and, where appropriate, lead shielding in the door is based on analytical methods provided by documents published by the ICRP, the IAEA and the NCRP. The purpose of this study is to perform a more realistic kerma evaluation at the entrance maze door of an HDR bunker using the Monte Carlo code GEANT4. The Monte Carlo results were validated experimentally. The spectrum at the maze entrance door, obtained with Monte Carlo, has an average energy of about 110 keV, maintaining a similar value along the length of the maze. The comparison of results from the aforementioned values with the Monte Carlo ones shows that results obtained using the albedo coefficient from the ICRP document more closely match those given by the Monte Carlo method, although the maximum value given by MC calculations is 30% greater.

The risk of local recurrence after breast conserving surgery for breast cancer increases with tum... more The risk of local recurrence after breast conserving surgery for breast cancer increases with tumor stage and revolved surgical margins. The risk can be reduced by high doses of postoperative radiotherapy to the tumor bed. Considering the high skin doses with electrons, brachytherapy may be preferable over electrons when high boost doses are used for such patients. Twenty-seven cases of breast cancer treated with breast conserving surgery were postoperatively irradiated with 50 Gy external beam radiotherapy followed by HDR boost of 15-20 Gy. The indications were posinve or close (<1 cm) surgical margins for TI-T2 tumors and all T3 tumors treated with breast conserving surgery. The distribution of the patients to the tumor stages were as follows; TI: 4 pts., T2:6 pts., and T3:17 pts. Thirteen of the cases with T3 tumors and 2 cases with T2 tumors were given 3-4 cycles of neoadjuvant chemotherapy prior to surgery. No surgery was performed to the two cases with T3 tumors following chemotherapy. Surgical margins were involved or close in I 1 cases. (TI: 6 pts., T2:4 pts. and T3: I pt.). Patients were irradiated with 50 Gy of external beam radiotherapy to whole breast and regional lymphatics m 5 weeks with 2 Gy fractions followed by HDR brachytherapy one week after external irradiation Brachytherapy implanlation to tile tumor bed was done with 7-13 needles m two planes and 15-2t) t;y m 4-6 fractions were prescNhed to the reference isodose (3 Gy x 6 in 19 pts.). There were no comphcattons related to the implantations and no severe early or late side effects. Median follow-up was 35 months (14-54 months) from the first day of external irradiation and local recurrence in 3 patients, distant metastasis in 6 patients were developed and 4 patients were died of distant metastasis. Survival analysis was done by Kaplan-Meier method; 3 year local recurrence free survival was 85.1% and 3 year overall survival was 91.5%.

In a consensus meeting, the Spanish Brachytherapy Group of SEOR and the Spanish Society of Medical Physics (SEFM) agreed on the indications, dose, fractionation schedule, prescription and reporting to be performed in endoesophageal brachytherapy. The results of this consensus are presented here as recommendations for medical practice.

In recent years, the use of high dose rate (HDR) after-loader machines has greatly increased due ... more In recent years, the use of high dose rate (HDR) after-loader machines has greatly increased due to the shift from traditional Cs-137/Ir-192 low dose rate (LDR) to HDR brachytherapy. The method used to calculate the required concrete and, where appropriate, lead shielding in the door is based on analytical methods provided by documents published by the ICRP, the IAEA and the NCRP. The purpose of this study is to perform a more realistic kerma evaluation at the entrance maze door of an HDR bunker using the Monte Carlo code GEANT4. The Monte Carlo results were validated experimentally. The spectrum at the maze entrance door, obtained with Monte Carlo, has an average energy of about 110 keV, maintaining a similar value along the length of the maze. The comparison of results from the aforementioned values with the Monte Carlo ones shows that results obtained using the albedo coefficient from the ICRP document more closely match those given by the Monte Carlo method, although the maximum value given by MC calculations is 30% greater.

Independent dosimetry verification is recommended for each clinical dosimetry in BT. Also, nomograms can be useful for dose calculation quality assurance and they could be adapted to High Dose Rate (HDR). This work sets nomograms for LDR and HDR prostate-BT implants, which are applied to three different institutions that use different implant techniques.

The risk of local recurrence after breast conserving surgery for breast cancer increases with tum... more The risk of local recurrence after breast conserving surgery for breast cancer increases with tumor stage and revolved surgical margins. The risk can be reduced by high doses of postoperative radiotherapy to the tumor bed. Considering the high skin doses with electrons, brachytherapy may be preferable over electrons when high boost doses are used for such patients. Twenty-seven cases of breast cancer treated with breast conserving surgery were postoperatively irradiated with 50 Gy external beam radiotherapy followed by HDR boost of 15-20 Gy. The indications were posinve or close (<1 cm) surgical margins for TI-T2 tumors and all T3 tumors treated with breast conserving surgery. The distribution of the patients to the tumor stages were as follows; TI: 4 pts., T2:6 pts., and T3:17 pts. Thirteen of the cases with T3 tumors and 2 cases with T2 tumors were given 3-4 cycles of neoadjuvant chemotherapy prior to surgery. No surgery was performed to the two cases with T3 tumors following chemotherapy. Surgical margins were involved or close in I 1 cases. (TI: 6 pts., T2:4 pts. and T3: I pt.). Patients were irradiated with 50 Gy of external beam radiotherapy to whole breast and regional lymphatics m 5 weeks with 2 Gy fractions followed by HDR brachytherapy one week after external irradiation Brachytherapy implanlation to tile tumor bed was done with 7-13 needles m two planes and 15-2t) t;y m 4-6 fractions were prescNhed to the reference isodose (3 Gy x 6 in 19 pts.). There were no comphcattons related to the implantations and no severe early or late side effects. Median follow-up was 35 months (14-54 months) from the first day of external irradiation and local recurrence in 3 patients, distant metastasis in 6 patients were developed and 4 patients were died of distant metastasis. Survival analysis was done by Kaplan-Meier method; 3 year local recurrence free survival was 85.1% and 3 year overall survival was 91.5%.

(total dose 40.5-45 Gy). Three-year cause-specific survival and local control are 91 and 88% (95% T1-2, 74% T4, p&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt;0.05). Acute and chronic reactions are like LDR cases. We think that HDR results are equivalent to LDR.
