Microsponge plays an important role in enhancing drug stability, reducing side effects, and modifying drug release profiles. It is mostly used for transdermal delivery. Recent studies also explored their use for oral administration. This study aimed to explore the potential use of the microsponge technique in improving the aqueous solubility and dissolution profile of pentoxifylline (PTX). In this study, microsponges were prepared by a quasi-emulsion solvent diffusion method by varying concentrations of carriers. Nine different ratios of the PTX:Eudragit E-100 with varying amounts of dichloromethane were used. Background: Integrity of ovarian function is a prerequisite for successful implantation of ovum. ... more Background: Integrity of ovarian function is a prerequisite for successful implantation of ovum. The subtle changes in ovarian function can be detected by studying endometrial carbohydrates with special stains. Material and Methods: 87 infertile and 30 normal endometria were studied with the help of Hematoxylin& Eosin (H& E) stain and special stains like Best Carmine, Alcian blue PAS (with and without diastase digestion) and reticulin stain. Results: Special stains were useful in emphasizing the cyclical activity of carbohydrates and stroma in normal endometrium. In infertile endometrium, they helped in precise classification and detection of corpus luteal deficiency, at times (8.7% cases) missed on routine H& E stain. Intrinsic abnormalities like polyps and tuberculous granulomas could be detected with ease on reticulin stain. On application of special stains, incidence of corpus luteal deficiency was 27.5%, and that of anovulatory infertility and normal secretory endometrium was 34.5% each. Conclusion: The special stains helped in detecting precise endometrial response and added to the information obtained by routine H&E stain.

Malassezia spp. are commensal yeasts that can cause cutaneous ailments such as dandruff and sebor... more Malassezia spp. are commensal yeasts that can cause cutaneous ailments such as dandruff and seborrheic dermatitis. We sought to develop a cost-effective, herbal formulation for the treatment of cutaneous ailments related to Malassezia spp. Aqueous and ethanolic extracts of fenugreek (Trigonellafoenum-graecum L.) leaves exhibited activity against a clinical isolate and commercial strain of Malassezia furfur. The extracts were also found to be active against other pathogenic fungi such as Aspergillus niger and Candida albicans. Qualitative and quantitative phytochemical evaluation of aqueous extract showed a predominant presence of flavonoids apart from alkaloids, saponins, carbohydrates, phenols, and proteins. Gel formulation of 30% aqueous fenugreek leaf extract was developed and optimized using sodium alginate as a gelling agent. The formulation showed good physicochemical characteristics and retained activity against M. furfur during 3-month accelerated stability studies. Furthermore, the developed herbal gel formulation did not show any irritation or sensitization in New Zealand rabbits after topical application, proving its cutaneous safety. Thus, topical gel formulation containing fenugreek leaf aqueous extract could be a safe and effective herbal treatment for various cutaneous fungal infections, including dandruff.

To study duodenal histomorphological features and their relationship with Helicobacter pylori (H Pylori) infection in patients of FD. This case control study included 50 cases of FD patients selected according to Rome III criteria and 30 age and sex matched controls. These were subjected to oesophago-gastro-duodenoscopy, rapid urease test for detection of H. pylori on gastric antral biopsy and duodenal biopsy from second part of duodenum for histopathological evaluation by light microscopy. Ten antral urease positive cases of FD with highest Intraepithelial Lymphocyte Count (IEL) were subjected to Immunohistochemistry (IHC). Duodenal inflammation was an invariable feature noted in FD. This report describes a case report of a postmortem performed on a 5-year old patient of Tay-Sach... more This report describes a case report of a postmortem performed on a 5-year old patient of Tay-Sachs disease, presenting with failure to thrive, muscular flaccidity, and cherry-red spots on macula on fundoscopy. There was no history of similarly affected sibling or any other family member. The diagnosis was confirmed by enzyme studies. At postmortem, there was no organomegaly. The brain, on microscopy, showed vacuolated swollen neurons.
A case of disseminated histoplasmosis in a 45-year-old male patient with acquired immunodeficienc... more A case of disseminated histoplasmosis in a 45-year-old male patient with acquired immunodeficiency syndrome (AIDS) from Pune is reported. The patient presented with high-grade fever and pain in hypochondrium. Clinical signs were pallor and hepatosplenomegaly. Bone marrow and splenic aspirate revealed numerous intracellular oval shaped yeast forms. Histoplasma capsulatum was isolated from the bone marrow and splenic aspirate. H. capsulatum infection is an opportunistic infection usually reported from patient with AIDS in areas endemic for H. capsulatum . The present case highlights the fact that histoplasmosis could be an emerging opportunistic infection in India.

This 2x1 micro strip patch antenna array resonates at 2.44 GHz. The dimension of each patch in the array antenna is 0.3λ × 0.23λ. Micro strip inset line feeding technique is used to feed the antenna structure. The width of the feed is designed accordingly as required for the impedance matching at that point of design. The gain and directivity of the antenna goes on increasing as the number of patch array elements go on increasing in the antenna structure.

Most human immunodeficiency virus (HIV)-infected patients develop various skin diseases. These sk... more Most human immunodeficiency virus (HIV)-infected patients develop various skin diseases. These skin manifestations not only act as markers but also reflect the patient's underlying immune status. Investigating CD4 counts is costly and not always possible. Thus, the potential value to be gained by using skin manifestations as predictors of low CD4 counts and disease progression should be explored. The present study attempted to correlate the association of various cutaneous disorders found in HIV patients with CD4 and CD8 counts, the CD4 : CD8 ratio and stage of HIV infection. This was a prospective study involving 61 patients who were HIV-positive and demonstrated skin lesions. Punch biopsies of skin were taken for histopathological diagnosis. CD4 and CD8 T cell counts were performed. The study sample included a majority of male patients, most of whom were aged 21-40 years. 16 post transfusion deaths were studied by reviewing clinical and blood bank records and by postm... more 16 post transfusion deaths were studied by reviewing clinical and blood bank records and by postmortem examination whenever possible. 13 of these cases belonged to regional blood bank, 2 were transfused in other hospitals and referred for management of transfusion reactions and one case be longed to other blood bank in the city. 3 deaths could not be attributed to transfusion reaction. Post transfusion endotoxaemia (7 cases) was the most frequent mode of transfusion associated fatality followed by cardiac overload (5 cases) and septicaemia (3 cases). In two cases endotoxaemia coexisted with cardiac overload. The pretransfusion compromised clinical course unfavourably thereby contributing significantly in death. The 3 clinically stable patients succumbed to posttransfusion endotoxaemia. The incidence of transfusion associated fatality in the present institute was 0.028% amongst total transfusions.

Review of records for last 5 years has shown 4 cases of Type I Gaucher's disease in our insti... more Review of records for last 5 years has shown 4 cases of Type I Gaucher's disease in our institute. The cases were diagnosed on bone marrow aspiration, examination of splenectomy specimen, liver biopsy and post mortem in one case. The age range was 2 years to 22 years. Male to female ratio was 3:1. Splenectomy was performed in one case and one case received enzyme replacement therapy with high dose, low frequency regimen for six months without any favorable effect. All were Hindus. Family history of similarly affected and treated twin brother was available in one case. The predominant clinical presentation was pancytopenia and splenohepatomegaly with splenomegaly greater than hepatomegaly. Remarkable constitutional inferiority was noted in one case, which succumbed to death following acute illness and bleeding diathesis. Post mortem performed showed infiltration of spleen and liver with Gaucher cells, fibrosis and myeloid metaplasia in liver and lung.

Medico-legal post-mortems referred to the Department of Pathology, for the histopathological exam... more Medico-legal post-mortems referred to the Department of Pathology, for the histopathological examination, revealed six cases of acute aortic dissection--two in isolation, three in combination with congenital bicuspid aortic valve; and one isolated case of congenital bicuspid aortic valve. One case of isolated aortic dissection was associated with Marfan's syndrome; and one case of aortic dissection with bicuspid aortic valve was associated with polycystic kidneys. History of hypertension could be elicited in two cases. Cystic medial degeneration of aorta was seen in three cases; one of which was associated with Marfan's syndrome. All five cases of aortic dissection belonged to type II of DeBakey classification.

There are very few autopsy studies available on systemic distribution of clofazimine, a drug with... more There are very few autopsy studies available on systemic distribution of clofazimine, a drug with anti-mycobacterial activity, used in multidrug therapy (MDT) regimen of leprosy and in erythema nodosum leprosum (ENL). An autopsy study was done on a 45 year old female of lepromatous leprosy (LL) on MDT and long term high dosage of clofazimine. Patient succumbed to intractable abdominal pain, diarrhoea, hypokalemia following clofazimine treatment. Autopsy study revealed yellowish brown discoloration of skin, viscera and body fluids. Chemical extraction of the drug revealed the highest concentration of the drug in jejunum (1.5mg/gm),followed by spleen (1.2mg/gm), pancreas (0.4mg/gm), adrenal (0.25mg/gm), liver (0.21mg/gm), and less than 0.2mg/gm in lung, fat, large intestine and stomach. It can be inferred from the present study that the drug is absorbed from the jejunum and gets deposited in fat, reticulo-endothelial cells (R-E cells) and hepatocytes. Serum proteins and plasma fibrinogen were estimated in 103 patients in various groups of leprosy ... more Serum proteins and plasma fibrinogen were estimated in 103 patients in various groups of leprosy and 52 patients of reactional leprosy. Total proteins, serum globulin and fibrinogen showed significant rise while serum albumin showed fall over the immunological spectrum from TT to LL. Type II reactional leprosy similarly revealed significant rise in globulin and fibrinogen. The comparison of these parameters between most of the comparable groups of leprosy was statistically significant. ENL patients after complete subsidence of reaction and after steroid treatment showed significant decrease in these protein fractions, thus conferring some prognostic implication on these tests.
