Wegen des nicht selten beangstigenden anfanglichen Wachstums, der entstellenden Lokalisation und der gelegentlich funktionellen Beeintrachtigung werden Padiater und Dermatologen von den Eltern zu einer Behandlung gedrangt (C. Ehringhaus, H.C. Dominick, 1986). Uber die Halfte der entstellenden Riesenhaemangiome (64,5%) sind nach Heiner und Zenk (1981) an Kopf und Hals lokalisiert. Madchen sind 2,3mal haufiger betroffen als Jungen.

In a retrospective study, initial diagnosis and method of treatment were evaluated in 118 patient... more In a retrospective study, initial diagnosis and method of treatment were evaluated in 118 patients with invasive basal cell carcinoma (formerly basalioma terebrans) of the head and neck. In 97 of the total 118 patients, radical surgery with resection of important organs had to be performed, and the follow-up results of 107 patients were recorded. In 44 (41%) of the latter 107 cases, recurrences were observed despite radical tumor excision, and only 63 (59%) patients were free of tumor. In 20 patients a second recurrence and in 12 patients a third recurrence was found. The recurrence rate amounted to only 1.9% in a comparative study group of 6932 patients with "simple" basal cell carcinoma. The findings reveal that healing without recurrence was achieved in only half of the patients with invasive basal cell carcinoma (basalioma terebrans). Diagnosis, therapy, and prognosis of 40 patients with malignant melanoma of the mucous membranes,... more Diagnosis, therapy, and prognosis of 40 patients with malignant melanoma of the mucous membranes, treated at one hospital from 1971 to 2006, were evaluated in a retrospective study. The survival rate was compared with that established by the German-Austrian-Swiss Study Group on Tumors of the Head and Neck (DÖSAK) for 121 cases. The cumulated 5-year survival rate amounting to 33% (DÖSAK study 35%) emphasizes the unfavorable prognosis of this tumor. Age, sex, or tumor location had no significant impact on a patient's survival. A modified surgical concept has been developed for the treatment of the primary tumor and its lymphatic drainage area to maintain a patient's quality of life. The entire tumor should be resected under histologic control ensuring a 5-mm safety margin of healthy tissue. Bone should only be resected if invaded by the tumor. Large resections of the upper or lower jaw should be avoided. A neck dissection does not improve the prognosis and should be performed only in the case of histologically confirmed invasion of lymph nodes.

The effects of chronic pressure on the mandible during skin expansion procedures have been invest... more The effects of chronic pressure on the mandible during skin expansion procedures have been investigated. In 19 Göttingen miniature pigs with an average age of 3 months, tissue expanders were placed s.c. in close proximity to the growing ascending ramus of the mandible on the surface of the masseter muscle. After complete filling of the expanders (100 cc) after 6 weeks, a significant transverse discrepancy could be observed compared with the contralateral side. Additionally, severe medial dislocation of the molars were noticed. Ten weeks after the removal of the implant, the occlusal disturbance still existed, deviation of the transverse mandibular diameter still reached up to -13.6 +/- 2.1%. Because only four animals were measured based on a CT investigation, further statistical analysis was not possible. Histological examination showed osteoclastic activity of the expander with consequent bone resorption and remodelling. Strict indications should be agreed for tissue expansion in the vicinity of growing bone, as, for example, when tissue expansion is applied in children.

We compared the split-thickness skin graft donor sites of hair bearing scalp (n=86) and thigh (n=... more We compared the split-thickness skin graft donor sites of hair bearing scalp (n=86) and thigh (n=27) in 113 patients, predominantly adults over a 6-month period. Donor site morbidity (pain, epithelialization, scars) was evaluated, including intensity and duration of pain, number of dressing changes, and duration of epithelialization of the wound. Graft thickness and quality was identical in the two groups. Patients with grafts from the scalp had fewer complaints than those with grafts from the thigh; they also had faster reepithelialization. Costs for staff and material related to the frequency and duration of dressing changes were significantly lower in patients with scalp grafts. The limited size of these grafts must be considered. The scalp as a donor site has the advantage of lower donor site morbidity with minimal pain and fast epithelialization. There was no scarring or alopecia in any of the patients.

Defects result predominantly after tumor surgery or due to trauma followed by scars. The functional and aesthetic aspects of the lid region have to be considered. Anatomically, the lid area consists of three layers: skin with eyelashes, cartilage and mucosa. An original eyelid reconstruction is only possible if a transplant consisting of the same tissue quality and architecture is used. This report is a presentation of an old method first described by Hu¨bner [3] with a description of the surgical technique and a retrospective analysis of 72 patients treated from 1997 to 2003. The tarsomarginal transplant is an ideal method for lid margin reconstruction when functional and aesthetic results are considered. Skin texture, tarsal stability, eye protection with mucosa and growth of eye lashes are all restored. By using three grafts, even complete eyelid reconstruction is possible.

Merkel cell carcinoma (MCC) is a rare, highly malignant carcinoma of the neuroendocrinological sy... more Merkel cell carcinoma (MCC) is a rare, highly malignant carcinoma of the neuroendocrinological system (Bayrou et al., J Am Acad Dermatol 24:198-207, 1992). It is a fast-growing, aggressive tumor with a high rate of local recurrence and early metastases. A radical surgical procedure is the therapy of choice. In case of lymph node metastases, regional lymphadenectomy is mandatory. Sentinel lymph node (SLN) mapping is a standard diagnostic technique to detect micrometastases in certain patients with malignant melanoma and breast cancer. The question is, can the SLN technique provide information as a prognostic factor and can the prognosis of the MCC be improved by detecting micrometastases in SLN at an early stage?

Weltkrieg erlittenen Granatsplitterverletzung ein Angiosarkom im Bereich der Fossa temporalis. Die Tumorresektion bestätigte die enge räumliche Lagebeziehung von Tumor und Granatsplitter. Die Analyse des Granatsplitters konnte keine eindeutige Aussage über die kanzerogene Potenz der Inhaltsstoffe liefern; eine entzündungsbedingte Karzinogenese erscheint wahrscheinlich. In der Literatur sind zwei ähnliche Kasuistiken beschrieben.
