L'etude retrospective presentee dans cet article a ete realisee sur une serie de 22 cas de tu... more L'etude retrospective presentee dans cet article a ete realisee sur une serie de 22 cas de tumeurs muco-secretantes de l'appendice colligees parmi 8373 patients appendicectomises. Le tableau clinique etait domine par un syndrome appendiculaire (73%). Le diagnostic a ete etabli dans 10 cas en pre ou en peroperatoire et c'etait seulement a l'examen anatomo-pathologique systematique que les 12 autres cas ont ete decouverts. L'etude histologique de la piece d'appendicectomie a montre une mucocele retentionnelle (12 cas), une hyperplasie muqueuse (8 cas) et un cystadenome mucineux (2 cas). Pour 5 malades, le diagnostic de pseudomyxome peritoneal (4 cas) ou extra peritoneal (1 cas) a ete retenu. Tous les patients ont subi une appendicectomie, associee a une evacuation des epanchements peritoneaux gelatineux pour 5 patients dont deux ont subi en plus une omentectomie. Deux cas de recidive ont ete enregistres 5 mois plus tard.

Les auteurs rapportent 12 cas de dilatation kystique congenitale du choledoque (DKC), 8 femmes et... more Les auteurs rapportent 12 cas de dilatation kystique congenitale du choledoque (DKC), 8 femmes et 4 hommes. L'âge moyen est de 25 ans. La symptomatologle est dominee par la douleur abdominale. Le diagnostic est evoque par l'echographie dans 42% des cas et confirme par le scanner lorsqu'il est realise dans 100 % des cas. La chotanglographie per-operatoire realisee de facon systematique a permis de confirmer le diagnostic, en precisant la classification et l'existence ou non d'une anomalie de la jonction bilio-pancreatique. Le traitement a consiste en une exerese du kyste dans 7 cas et une kysto-duodenostomie dans un cas. 4 patients operes en urgence, n'ont pas eu de resection ou de derivation biliaire. Les suites immediates ont ete marquees par une fistule anastomotique. La mortalite a ete nulle. Les suites lointaines ont ete emaillees par une stenose au niveau de l'anastomose hepatico-jejunale. A travers une serie de 14 cas colliges sur une periode de 12 ans, nous avons realise une etude re... more A travers une serie de 14 cas colliges sur une periode de 12 ans, nous avons realise une etude retrospective, analytique et descriptive de ces traumatismes. Nous avons eu pour but de degager des recommandations pour ameliorer leur prise en charge. Il s'agissait de 2 femmes et 12 hommes d'âge moyen de 24,4 ans (extremes : 5 et 60 ans) repartis en 10 lesions pancreatiques, 2 lesions duodenales et 2 lesions duodeno-pancreatiques. L'etiologie etait dominee par les AVP dans 7 cas (50%). Le traumatisme etait ferme dans 13 cas (93%).Les lesions associees ont ete presentes chez 13 de nos patients (92%). Le traumatisme duodeno-pancreatique s'est manifeste par un abdomen chirurgical urgent chez 9 patients (64%), secondairement dans 3 cas, tardivement dans 2 cas : un cas de suppuration profonde a 3 mois apres un traumatisme traite en ambulatoire et un cas d'ascite persistante apres un traumatisme passe inapercu. Les tumeurs carcinoides digestives sont rares. L'appendice constitue la localisation la plus ... more Les tumeurs carcinoides digestives sont rares. L'appendice constitue la localisation la plus frequente de ces tumeurs. Nous avons etudie d'une facon retrospective tous les dossiers des patients qui ont eu une appendicectomie dans le service de chirurgie generale de Sfax sur une periode de 7 ans allant de 1995 a 2001. Au cours de cette periode, 9584 patients ont eu une appendicectomie dans notre service. Parmi lesquels, 46 cas (0,48 %) de tumeur carcinoide de l'appendice ont ete diagnostiques. Dans plus de 95 % des cas, le tableau clinique etait celui d'une appendicite aigue. Aucun de nos patients ne presentait un syndrome carcinoide. Le traitement etait une simple appendicectomie pour 44 patients alors que pour les 2 autres une hemicolectomie droite etait necessaire, en raison d'un envahissement de la base dans un cas et d'une taille tumorale superieure a 2 cm dans l'autre. Prerequis :La cholecystectomie par voie laparoscopique dans le traitement de la lithiase vesicula... more Prerequis :La cholecystectomie par voie laparoscopique dans le traitement de la lithiase vesiculaire est actuellement le « Gold standard ». But: Evaler les resultats de la cholecystectomie sous coelioscopie Methodes: A propos d'une etude retrospective d'une serie de 500 cholecystectomies faites par voie coelioscopique pour lithiase vesiculaire durant une periode allant de Janvier 1996 a Mars 2000, nous avons voulu evaluer notre propre experience et comparer nos resultats a ceux de la litterature. Resultats: Notre serie comporte 420 femmes et 80 hommes avec un sex-ratio de 0,19. L'âge moyen a ete de 50 ans. 16,2% de nos patients sont obeses. 13 patients avaient des antecedents respiratoires et 122 des antecedents cardio-vasculaires particulierement l'hypertension arterielle. Tous nos patients ont eu au moins une echographie hepato-biliaire avant l'intervention. L'antibioprophylaxie a ete administree dans 93,8 % des cas. Dans 23 cas (4,6 %), une conversion s'est imposee pour des causes diverses. On a note 11 complications chirurgicales (2,2 %) dont 2 ont necessite une reprise chirurgicale et 11 complications medicales dominees par les infections broncho-pulmonaires. Nous n'avons pas note de deces.

Les liposarcomes sont des tumeurs mesenchymateuses malignes developpees a partir des elements con... more Les liposarcomes sont des tumeurs mesenchymateuses malignes developpees a partir des elements constituant le tissu graisseux a diverses etapes de leur differenciation. C'est l'un des plus frequents sarcomes mesenchymateux. Le retroperitoine est un site privilegie de developpement dans 12 a 15 % des cas. Durant 9 ans, de 1994 a 2002, nous avons collige et opere 5 cas de liposarcomes retroperitoneaux. L'âge median a ete de 50 ans (extremes 34 a 79 ans) avec un sexratio de 1,5. Le diagnostic a ete tardif au dela de six mois. La douleur abdominale et l'apparition d'une enorme masse (20 cm) ont ete les signes revelateurs. Aucune methode d'imagerie n'a permis d'evoquer le diagnostic. La cyto-ponction realisee une fois n'a pas pu determiner avec precision le type histologique. L'immunohistochimie occupe maintenant une place importante pour classer un sarcome. L'exerese chirurgicale large realisee dans les 5 cas n'a pas permis l'ablation complete de la tumeur dans 3 cas vu l'extension locoregionale avancee, ce qui a ete a l'origine de trois recidives. Le type histologique bien differencie a ete note dans 3 cas, le type myxoide dans un cas et le type indifferencie dans un cas.

The aim of this study is to review clinical data, histological and immunohistochemical findings a... more The aim of this study is to review clinical data, histological and immunohistochemical findings and prognosis of stromal gastrointestinal tumors. A retrospective Study of 32 cases of stromal gastrointestinal tumors diagnosed in the Department of Pathology of Mongi Slim Hospital of Tunis from 1991 to 2004. The average age of the patients was of 54.4 years, equal for sex. Tumors were essentially gastric (50%) and of the small intestine (37.5%). All the patients had surgical treatment. Gastro-intestinal Stromal Tumors or GIST represent the most frequent stromal tumor with 56.2% of cases. After immunohistochimestry study, using CD117 antigen, this revision allows better management of GIST. Glivec is the standard treatment of advanced GIST.

An increasing number of elderly patients have been considered for major surgical procedures, such... more An increasing number of elderly patients have been considered for major surgical procedures, such as pancreaticoduodenectomy. The decision to recommend this operation for localized pancreatic cancer or other periampullary process in a very elderly patient is complicated by the frailty of the patient and the poor prognosis of the disease. Moreover, increased surgical experience associated with better patient selection may reduce the mortality rate, even in very elderly patients (over 80 years of age), after pancreaticoduodenectomy. An 84-year-old woman underwent pancreaticoduodenectomy for ampullary adenocarcinoma. The tumor was classified pT3N0M0. A good postoperative outcome was obtained. The patient is still alive, 18 months after operation. Radical resection of periampullary tumors is safe in selected patients of advanced age, with morbidity and mortality rates approaching those observed in younger patients. Age alone should not be a contraindication for pancreatic resection.

The retrospective study presented in this article was realized on a series of 22 cases of mucus s... more The retrospective study presented in this article was realized on a series of 22 cases of mucus secreting tumor of the appendix compiled among 8373 appendicectomies. The clinical picture was dominated by an appendiceal syndrome (73%). The diagnosis was made pre or peroperatively in 10 cases and it was only with to the systematic anatomopathologic exam that 12 other cases were discovered. The histological study of the appendectomy piece showed a retention mucocele (12 cases), a mucosal hyperplasia (8 cases) and a cystadenoma (2 cases). For 5 patients, the diagnosis of peritoneal pseudomyxoma (4 cases) or extraperitoneal (1 case) was retained. All the patients underwent an appendicectomy, associated to an evacuation of the peritoneal gelatinous ascites for 5 patients among whom two underwent in more an omentectomie. Two cases of recurrence were recorded 5 months later.

The appendix constitutes the most frequent localization of these tumors. We studied of a retrospective way all cases that had an appendectomy in the service of general surgery of Sfax on a period of 7 years going from 1995 to 2001. During this period, 9584 patients had an appendectomy in our service. Among which, 46 cases (0.48%) of carcinoid tumor of the appendix have been diagnosed. In more than 95% of cases, patients were admitted due to acute appendicitis; but none of them had the signs of the carcinoid syndrome. The treatment was a simple appendectomy in 44 cases (95%). The right hemicolectomy was necessary in two cases, because of an invasion of the appendicular base in a case and a tumor greater than 2 cm in the other case. We retrospectively studied patients who had undergone surgery for a hydatid cyst (HC) of the abdomen between January 1990 and December 2000 and in whom the exploration had confirmed the diagnosis of acute rupture of a hydatid cyst in the peritoneum. Over this period of 11 Years, 970 patients had undergone surgery for hydatid cysts of the liver and the spleen, 17 of whom had presented an acute rupture of the hydatid cyst in the peritoneum, that is to say 1.75% of the cases. The mean age was 30 Years and sex ratio was 0.41. Rupture was secondary to a trauma in 6 cases and to an intense physical effort in one case. The clinical table was acute peritonitis in 14 cases, acute intestinal occlusion in one case, suggested anaphylactic shock in one case, and a non-complicated hydatid cyst in one case. We aimed to release recommendations to improve their management. It was 2 women and 12 men, whom mean age was 24.4 years (extremes: 50 and 60 years), divided into 10 pancreatic injuries, 2 duodenal injuries and 2 duodeno-pancreatic injuries. Etiology was dominated by traffic accidents 7 cases (50%). Trauma was closed in 13 cases (92%). Associated lesions were present among 13 patients (92%). The traumatism appeared by an urgent surgical abdomen among 9 patients, secondarily in 3 cases, tardily in 2 cases: a case of deep suppuration 3 months after a traumatism treated into ambulatory and a case of persistent ascites after a past unperceived traumatism. It is one of the most frequent mesenchymatous sarcomas. Retro-peritoneum is a privileged site of development in 12 to 15% of cases. During 9 years, from 1994 till 2002, we have colligated and operated 5 cases of retro-peritoneal liposarcomas. Average age was 50 years (extremes 34 to 79 years) with a sex-ratio of 1.5. Diagnosis was late beyond 6 months. Abdominal pain and the appearance of an enormous mass (20 cm) were the revealing signs. No imagery method allowed to evoke the diagnosis. Assesment of Laparoscopic cholecystectomy Through a retrospective series of 500 laparoscopic cholecystectomies during a period going from January 1996 to March 2000, we tried to evaluate our experience by comparing our results to the literature data. There were 420 women and 80 men with a sex ratio of 0.19. Average age was 50 years. 16.2% of our patients were obese. 13 patients had a history of respiratory disease and 122 history of a cardiovascular pathology essentially arterial hypertension. All our patients benefited from at least one hepato-biliary ultrasound examination before the intervention. Antibioprophylaxy was administrated in 93.8% of cases. In 23 cases (4.6%), a conversion was necessary for different causes. L'âge moyen a ete de 50 ans. 16,2% de nos patients sont obeses. 13 patients avaient des antecedents respiratoires et 122 des antecedents cardio-vasculaires particulierement l'hypertension arterielle. Tous nos patients ont eu au moins une echographie hepato-biliaire avant l'intervention. L'antibioprophylaxie a ete administree dans 93,8 % des cas. Dans 23 cas (4,6 %), une conversion s'est imposee pour des causes diverses. On a note 11 complications chirurgicales (2,2 %) dont 2 ont necessite une reprise chirurgicale et 11 complications medicales dominees par les infections broncho-pulmonaires. Nous n'avons pas note de deces.

Les liposarcomes sont des tumeurs mesenchymateuses malignes developpees a partir des elements con... more Les liposarcomes sont des tumeurs mesenchymateuses malignes developpees a partir des elements constituant le tissu graisseux a diverses etapes de leur differenciation. C'est l'un des plus frequents sarcomes mesenchymateux. Le retroperitoine est un site privilegie de developpement dans 12 a 15 % des cas. Durant 9 ans, de 1994 a 2002, nous avons collige et opere 5 cas de liposarcomes retroperitoneaux. L'âge median a ete de 50 ans (extremes 34 a 79 ans) avec un sexratio de 1,5. Le diagnostic a ete tardif au dela de six mois. La douleur abdominale et l'apparition d'une enorme masse (20 cm) ont ete les signes revelateurs. Aucune methode d'imagerie n'a permis d'evoquer le diagnostic. La cyto-ponction realisee une fois n'a pas pu determiner avec precision le type histologique. L'immunohistochimie occupe maintenant une place importante pour classer un sarcome. L'exerese chirurgicale large realisee dans les 5 cas n'a pas permis l'ablation complete de la tumeur dans 3 cas vu l'extension locoregionale avancee, ce qui a ete a l'origine de trois recidives. Le type histologique bien differencie a ete note dans 3 cas, le type myxoide dans un cas et le type indifferencie dans un cas.

The aim of this study is to review clinical data, histological and immunohistochemical findings a... more The aim of this study is to review clinical data, histological and immunohistochemical findings and prognosis of stromal gastrointestinal tumors. A retrospective Study of 32 cases of stromal gastrointestinal tumors diagnosed in the Department of Pathology of Mongi Slim Hospital of Tunis from 1991 to 2004. The average age of the patients was of 54.4 years, equal for sex. Tumors were essentially gastric (50%) and of the small intestine (37.5%). All the patients had surgical treatment. Gastro-intestinal Stromal Tumors or GIST represent the most frequent stromal tumor with 56.2% of cases. After immunohistochimestry study, using CD117 antigen, this revision allows better management of GIST. Glivec is the standard treatment of advanced GIST.

An increasing number of elderly patients have been considered for major surgical procedures, such... more An increasing number of elderly patients have been considered for major surgical procedures, such as pancreaticoduodenectomy. The decision to recommend this operation for localized pancreatic cancer or other periampullary process in a very elderly patient is complicated by the frailty of the patient and the poor prognosis of the disease. Moreover, increased surgical experience associated with better patient selection may reduce the mortality rate, even in very elderly patients (over 80 years of age), after pancreaticoduodenectomy. An 84-year-old woman underwent pancreaticoduodenectomy for ampullary adenocarcinoma. The tumor was classified pT3N0M0. A good postoperative outcome was obtained. The patient is still alive, 18 months after operation. Radical resection of periampullary tumors is safe in selected patients of advanced age, with morbidity and mortality rates approaching those observed in younger patients. Age alone should not be a contraindication for pancreatic resection.

The retrospective study presented in this article was realized on a series of 22 cases of mucus s... more The retrospective study presented in this article was realized on a series of 22 cases of mucus secreting tumor of the appendix compiled among 8373 appendicectomies. The clinical picture was dominated by an appendiceal syndrome (73%). The diagnosis was made pre or peroperatively in 10 cases and it was only with to the systematic anatomopathologic exam that 12 other cases were discovered. The histological study of the appendectomy piece showed a retention mucocele (12 cases), a mucosal hyperplasia (8 cases) and a cystadenoma (2 cases). For 5 patients, the diagnosis of peritoneal pseudomyxoma (4 cases) or extraperitoneal (1 case) was retained. All the patients underwent an appendicectomy, associated to an evacuation of the peritoneal gelatinous ascites for 5 patients among whom two underwent in more an omentectomie. Two cases of recurrence were recorded 5 months later.

The appendix constitutes the most frequent localization of these tumors. We studied of a retrospective way all cases that had an appendectomy in the service of general surgery of Sfax on a period of 7 years going from 1995 to 2001. During this period, 9584 patients had an appendectomy in our service. Among which, 46 cases (0.48%) of carcinoid tumor of the appendix have been diagnosed. In more than 95% of cases, patients were admitted due to acute appendicitis; but none of them had the signs of the carcinoid syndrome. The treatment was a simple appendectomy in 44 cases (95%). The right hemicolectomy was necessary in two cases, because of an invasion of the appendicular base in a case and a tumor greater than 2 cm in the other case. We retrospectively studied patients who had undergone surgery for a hydatid cyst (HC) of the abdomen between January 1990 and December 2000 and in whom the exploration had confirmed the diagnosis of acute rupture of a hydatid cyst in the peritoneum. Over this period of 11 Years, 970 patients had undergone surgery for hydatid cysts of the liver and the spleen, 17 of whom had presented an acute rupture of the hydatid cyst in the peritoneum, that is to say 1.75% of the cases. The mean age was 30 Years and sex ratio was 0.41. Rupture was secondary to a trauma in 6 cases and to an intense physical effort in one case. The clinical table was acute peritonitis in 14 cases, acute intestinal occlusion in one case, suggested anaphylactic shock in one case, and a non-complicated hydatid cyst in one case. We aimed to release recommendations to improve their management. It was 2 women and 12 men, whom mean age was 24.4 years (extremes: 50 and 60 years), divided into 10 pancreatic injuries, 2 duodenal injuries and 2 duodeno-pancreatic injuries. Etiology was dominated by traffic accidents 7 cases (50%). Trauma was closed in 13 cases (92%). Associated lesions were present among 13 patients (92%). Assesment of Laparoscopic cholecystectomy Through a retrospective series of 500 laparoscopic cholecystectomies during a period going from January 1996 to March 2000, we tried to evaluate our experience by comparing our results to the literature data. There were 420 women and 80 men with a sex ratio of 0.19. Average age was 50 years. 16.2% of our patients were obese. 13 patients had a history of respiratory disease and 122 history of a cardiovascular pathology essentially arterial hypertension. All our patients benefited from at least one hepato-biliary ultrasound examination before the intervention. Antibioprophylaxy was administrated in 93.8% of cases. In 23 cases (4.6%), a conversion was necessary for different causes.
