Methods: Skin biopsy imprints or biopsy fragments were assayed via kDNA-PCR/RFLP and FRET-real-time PCR. Results: Of individuals with suspected CL, 18 were positive for Leishmania kDNA. Leishmania (Viannia) braziliensis (61.1%) and Leishmania (Viannia) guyanensis (5.5%) were identified in the positive samples. Conclusions: These results are congruent with the previous reports in Acre and Bolivia, revealing L. braziliensis as the most prevalent species. L. guyanensis identification also corroborates with the epidemiology of the disease in the Amazon Basin.

Cyanoacrylate derivatives have been used as surgical adhesives for many years. Shorter-chain deri... more Cyanoacrylate derivatives have been used as surgical adhesives for many years. Shorter-chain derivatives (methyl- and ethyl-cyanoacrylate) have proved to be histotoxic. Longer-chain derivatives (butyl- and isobutyl-cyanoacrylate) are much less histotoxic. Many surgeons continue to use ethyl-2-cyanoacrylate (Krazy Glue) despite the availability of a less toxic derivative, butyl-2-cyanoacrylate (Histoacryl). In this study, the histotoxicity and bone graft-cartilage binding ability of Krazy Glue and Histoacryl were compared. Bone grafts harvested from the anterior wall of the maxillary sinus were placed in a subcutaneous pocket and glued to auricular cartilage in the rabbit. Krazy Glue and Histoacryl were used in opposite ears, harvesting specimens at 1, 2, 4, 12, 24, and 48 weeks. The Krazy Glue-treated ears developed seromas with histologic evidence of acute inflammation, tissue necrosis, and chronic foreign body giant cell reaction. The Histoacryl-treated ears showed mild acute inflammation and mild foreign body giant cell reaction. The Krazy Glue was completely degraded within 12 months, while some Histoacryl was still present at 1 year. Histoacryl had minimal histotoxic effect and good bone graft-cartilage binding ability, whereas Krazy Glue demonstrated severe histotoxicity.

INTRODUCTION We describe the clinicopathologic and flow cytometric features of 10 cases of basalo... more INTRODUCTION We describe the clinicopathologic and flow cytometric features of 10 cases of basaloid squamous cell carcinoma (BSCC) of the head and neck to determine if DNA ploidy is a useful prognostic indicator. We also provide a review of 80 cases previously reported in the English language literature. MATERIALS AND METHODS The 10 cases were obtained from the surgical pathology files of Presbyterian University Hospital and The Eye and Ear Institute, Pittsburgh, PA (1987-1991). In all 10 cases, the microscopic slides and clinical data were reviewed. Flow cytometry was performed using the Hedley technique and formalin-fixed, paraffin-embedded tissue. RESULTS The mean age of patients with BSCC was 64 years (range, 49 to 75 years). Tumor involved the base of tongue (n = 5), hypopharynx-epiglottis (n = 3), and tonsil (n = 1). One case presented with cervical lymph node metastasis from an unknown primary site. Histologically, BSCC showed a biphasic pattern with basaloid-squamous elements, comedonecrosis, stromal hyalinization, surface dysplasia, and an in situ and/or invasive squamous cell carcinoma component. Flow cytometry revealed six diploid and four aneuploid tumors. Five of six patients with diploid and all four patients with aneuploid tumors developed early regional and/or distant metastases. Of the two patients who died of disease, one had a diploid and the other an aneuploid tumor. CONCLUSION Our study reaffirms the predilection of BSCC for the base of tongue, pyriform sinus, and supraglottic larynx, and its aggressive biologic behavior with a high incidence of cervical lymph node metastasis (64%), distant spread (44%), and death from disease (38% mortality at 17 months median follow-up). However, in contrast to previous reports, tumor ploidy by flow cytometry provided no additional prognostic information beyond that supplied by routine histologic evaluation.

The patient underwent a magnetic resonance imaging scan of the head for seizure workup, which incidentally revealed a T 1and T2-enhancing cystic mass in the left neck (Fig. 1). There was no pain, dysphagia, or respiratory difficulty associated with the mass. Physical examination revealed a healthy boy with a palpable mass 3 × 4 cm in size, deep to the left sternocleidomastoid muscle in the high jugulodigastric region. The mass was mobile, soft, and spongy. There were no associated overlying skin changes. The mass did not increase in size or change position with the Valsalva maneuver. At excision, the mass was found to be compressing the internal jugular vein but was removed without difficulty. Grossly, the mass measured 3.5 x 2.5 x 0.7 cm and consisted of multiloculated pink-tan tissue with whitish plaques across its surface. Histologically, the mass was a cervical thymic cyst with cuboidal epithelial lining (Fig. 2). Thymic tissue was seen in the cyst wall, and granulation tissue with lipid-laden macrophages and giant cells was seen in the lumen (Fig. 2).

Cribriform adenocarcinoma of the tongue: a hitherto unrecognized type of adenocarcinoma character... more Cribriform adenocarcinoma of the tongue: a hitherto unrecognized type of adenocarcinoma characteristically occurring in the tongue Aims: We report a review of our institutional and consultation files in order to select cases of hitherto unrecognized type of adenocarcinoma occurring in the tongue. Materials and results: Eight cases of a characteristic adenocarcinoma of the tongue resembled solid and follicular variants of the papillary carcinoma of the thyroid. All the tumours were unencapsulated and were divided by fibrous septa into lobules. Major parts of the lesions were composed of areas with solid and microcystic growth patterns. The most striking cytological feature was that the tumour nuclei were pale-staining with a 'ground glass' quality, and they often appeared to overlap. Immunohistochemically, the tumours expressed cytokeratin and S100 protein and, focally, actin; thyroglobulin was negative. Ultrastructurally the cells had clefted nuclei, and the cytoplasm contained a few mitochondria, lysosomes and Golgi apparatus. Many tumour cells had combined features of both myoepithelial and secretory differentiation-well formed microvilli on their apical borders and bundles of microfilaments. At first presentation, all eight patients had metastases in the regional neck lymph nodes, but all are alive 2-6 years after the initial excision and irradiation. Conclusion: We describe a distinctive type of adenocarcinoma of the tongue, for which we propose the name cribriform adenocarcinoma of the tongue (CAT). CAT usually presents with metastases in the neck lymph nodes at the time of presentation. We hypothesize that the tumour might arise from the thyroglossal duct anlage.

THE POTENTIAL usefulness to surgeons of an innocuous tissue adhesive has provided the impetus for... more THE POTENTIAL usefulness to surgeons of an innocuous tissue adhesive has provided the impetus for extended experimental trials of methyl-2-cyanoacrylate. The unique properties of this monomer as an effective adhesive for living wet tissue surfaces,1, 10 are of considerable interest and clinical potential, even though it is becoming increasingly apparent that this substance is histotoxic.7 We have confirmed the necrotizing properties of methyl-2-cyanoacrylate.3' 12 The structural formula of the cyanoacrylate adhesives and their proposed mechanism of polymerization 4 are shown in Fig. 1. The present study is part of an attempt to develop a less necrotizing cyanoacrylate possessing significant adhesive properties. The hypothesis that lengthening the alkoxyl group of the 2-cyanoacrylic acid ester would reduce its histotoxicity has been tested. The adhesive bond strengths, the heats evolved during in vivo polymerization and local responses to methyl-, hexyland decyl-2-cyanoacrylate are reported. Methods Cyanoacrylate Synthesis Methyl-2-cyanoacrylate (Methyl), hexyl-2-cyanoacrylate (Hexyl), and decyl-2-cyanoacrylate (Decyl) were synthesized according to the method of McKeever.9 The alkyl cyanoacetate was allowed to react with paraformaldehyde to form the cyanoacrylate polymer. The cyanoacrylate polymer was then cracked by heating and the monomer fractionally distilled. To the final product were added trace amounts of SO2 and hydroquinone to increase storage stability. Small batches of each monomer stored at-10°C. were warmed to room temperature immediately before use. Monomers were 95 to 98 per cent pure; mass spectroscopy suggested the principal impurity to be acetic anhydride. Implantation Preparations Individually caged Walter Reed strain male white rats weighing 200 to 250 Gm. were given standard laboratory diet and water ad lib. Operative procedures were performed under ether anesthesia observing aseptic technics. The animals were sacrificed by CO2 asphyxiation. Principles of laboratory animal care as promulgated by the National Society for Medical Research were observed. A. Implantation of Monomers. Subcutaneous, hepatic and femoral marrow cavity sites were employed. Through a midline abdominal incision the tips of the right and left anterior liver lobes were resected. Mucinous carcinoma is a rare primary eyelid malignancy. It is, however, more common in other site... more Mucinous carcinoma is a rare primary eyelid malignancy. It is, however, more common in other sites and may metastasize to the eye. Thus, it is important to consider a distant primary when diagnosing mucinous carcinoma of the eyelid. We studied various immunohistochemical markers that may be useful. Two cases of mucinous carcinoma from the eyelid were reacted with antibodies to cytokeratins (35-beta-H11), carcinoembryonic antigen, S-100 protein, gross cystic disease fluid protein-15, alpha-lactalbumin, estrogen receptor, and progesterone receptor. All antigens were positive in both cases. This study shows that immunohistochemistry may help exclude metastatic mucinous carcinoma to the eyelid from many sites, except the breast, which the eyelid primary closely resembles. Thus, a breast primary should be specifically sought and excluded clinically.

Vitamin A and synthetic analogues are essential for the normal differentiation of epithelial tissues. The purpose of this study was to determine whether the administration of isotretinoin to chinchillas would prevent propylene glycol exposure from inducing middle ear cholesteatomas. Sixteen chinchillas received 90% propylene glycol to the left middle ear and normal saline to the right. Half the animals were placed in the experimental group and received a daily dose of isotretinoin of 2 mg/kg for 7 days prior to propylene glycol administration and then for 6 weeks until killed. At 6 weeks, cholesteatoma was found in six of eight ears treated with propylene glycol in animals receiving isotretinoin. Two animals in the control group died. Three of the remaining eight had cholesteatoma. No ears treated with saline had cholesteatoma. We conclude that isotretinoin, in our chinchilla model, does not prevent propylene glycol-induced cholesteatoma formation.

Initially, the patient was diagnosed as adrenocortical tumor that was treated with robotic adrenalectomy. Histopathological and immuno-histochemical examination revealed schwannoma. We will report the case and review the literature regarding this rare tumor.

Hodgkin's lymphoma (HL) is a hematological malignancy characterized by a minority of neoplastic c... more Hodgkin's lymphoma (HL) is a hematological malignancy characterized by a minority of neoplastic cells outnumbered by tumor-associated macrophages (TAMs). The overexpression of the CD163 antigen by TAMs is considered to be a significant predictive biomarker for risk stratification. This is likely caused by a genetic single-nucleotide polymorphism (SNP) at the gene promoter. The aim of the present retrospective case-control study was to establish a gene expression profile of a specific biomarker for classical HL (CHL) in order to predict the outcome and survival of CHL patients in Saudi Arabia. The protein expression of CD163 on TAMs was studied using immunohistochemistry (IHC). A prognosis index was calculated for the CD163 protein to assess the risk stratification of CHL. Genotyping of selected SNPs of this antigen was performed for 100 CHL cases and controls. The analysis revealed that the CD163 protein expression level was significantly correlated with disease relapse (DR) and overall survival (OS). In addition, the CD163 index threshold (15.0) was found to be significantly correlated with the relapse rate. Among the studied CD163 SNPs, rs75608120 exhibited a significant correlation with the DR rate of CHL patients, but not with OS. The findings of the present study confirmed that CD163 is a specific marker for TAMs, and its overexpression by TAMs is significantly associated with relapse and reduced survival post-therapy. In addition, a new methodology of indexing CD163 protein expression for HL risk stratification was proposed. Thus, the present study identified a specific predictive molecular and antigenic biomarker for CHL prognosis.

The unique properties of this monomer as an effective adhesive for living wet tissue surfaces,1, 10 are of considerable interest and clinical potential, even though it is becoming increasingly apparent that this substance is histotoxic.7 We have confirmed the necrotizing properties of methyl-2-cyanoacrylate.3' 12 The structural formula of the cyanoacrylate adhesives and their proposed mechanism of polymerization 4 are shown in Fig. 1. The present study is part of an attempt to develop a less necrotizing cyanoacrylate possessing significant adhesive properties. The hypothesis that lengthening the alkoxyl group of the 2-cyanoacrylic acid ester would reduce its histotoxicity has been tested. The adhesive bond strengths, the heats evolved during in vivo polymerization and local responses to methyl-, hexyland decyl-2-cyanoacrylate are reported. Methods Cyanoacrylate Synthesis Methyl-2-cyanoacrylate (Methyl), hexyl-2-cyanoacrylate (Hexyl), and decyl-2-cyanoacrylate (Decyl) were synthesized according to the method of McKeever.9 The alkyl cyanoacetate was allowed to react with paraformaldehyde to form the cyanoacrylate polymer. The cyanoacrylate polymer was then cracked by heating and the monomer fractionally distilled. To the final product were added trace amounts of SO2 and hydroquinone to increase storage stability. Small batches of each monomer stored at-10°C. were warmed to room temperature immediately before use. Monomers were 95 to 98 per cent pure; mass spectroscopy suggested the principal impurity to be acetic anhydride. Implantation Preparations Individually caged Walter Reed strain male white rats weighing 200 to 250 Gm. were given standard laboratory diet and water ad lib. Operative procedures were performed under ether anesthesia observing aseptic technics. The animals were sacrificed by CO2 asphyxiation. Principles of laboratory animal care as promulgated by the National Society for Medical Research were observed. A. Implantation of Monomers. Subcutaneous, hepatic and femoral marrow cavity sites were employed. Through a midline abdominal incision the tips of the right and left anterior liver lobes were resected. Mucinous carcinoma is a rare primary eyelid malignancy. It is, however, more common in other site... more Mucinous carcinoma is a rare primary eyelid malignancy. It is, however, more common in other sites and may metastasize to the eye. Thus, it is important to consider a distant primary when diagnosing mucinous carcinoma of the eyelid. We studied various immunohistochemical markers that may be useful. Two cases of mucinous carcinoma from the eyelid were reacted with antibodies to cytokeratins (35-beta-H11), carcinoembryonic antigen, S-100 protein, gross cystic disease fluid protein-15, alpha-lactalbumin, estrogen receptor, and progesterone receptor. All antigens were positive in both cases. This study shows that immunohistochemistry may help exclude metastatic mucinous carcinoma to the eyelid from many sites, except the breast, which the eyelid primary closely resembles. Thus, a breast primary should be specifically sought and excluded clinically.

Vitamin A and synthetic analogues are essential for the normal differentiation of epithelial tissues. The purpose of this study was to determine whether the administration of isotretinoin to chinchillas would prevent propylene glycol exposure from inducing middle ear cholesteatomas. Sixteen chinchillas received 90% propylene glycol to the left middle ear and normal saline to the right. Half the animals were placed in the experimental group and received a daily dose of isotretinoin of 2 mg/kg for 7 days prior to propylene glycol administration and then for 6 weeks until killed. At 6 weeks, cholesteatoma was found in six of eight ears treated with propylene glycol in animals receiving isotretinoin. Two animals in the control group died. Three of the remaining eight had cholesteatoma. No ears treated with saline had cholesteatoma. We conclude that isotretinoin, in our chinchilla model, does not prevent propylene glycol-induced cholesteatoma formation.

We will report the case and review the literature regarding this rare tumor.

The overexpression of the CD163 antigen by TAMs is considered to be a significant predictive biomarker for risk stratification. This is likely caused by a genetic single-nucleotide polymorphism (SNP) at the gene promoter. The aim of the present retrospective case-control study was to establish a gene expression profile of a specific biomarker for classical HL (CHL) in order to predict the outcome and survival of CHL patients in Saudi Arabia. The protein expression of CD163 on TAMs was studied using immunohistochemistry (IHC). A prognosis index was calculated for the CD163 protein to assess the risk stratification of CHL. Genotyping of selected SNPs of this antigen was performed for 100 CHL cases and controls. The analysis revealed that the CD163 protein expression level was significantly correlated with disease relapse (DR) and overall survival (OS). In addition, the CD163 index threshold (15.0) was found to be significantly correlated with the relapse rate. Among the studied CD163 SNPs, rs75608120 exhibited a significant correlation with the DR rate of CHL patients, but not with OS. The findings of the present study confirmed that CD163 is a specific marker for TAMs, and its overexpression by TAMs is significantly associated with relapse and reduced survival post-therapy. In addition, a new methodology of indexing CD163 protein expression for HL risk stratification was proposed. Thus, the present study identified a specific predictive molecular and antigenic biomarker for CHL prognosis.
