A 52-year-old man with a history of adenocarcinoma of the sigmoid colon and hepatic metastases un... more A 52-year-old man with a history of adenocarcinoma of the sigmoid colon and hepatic metastases underwent F-18 FDG PET scan. There was no evidence of active or recurrent metastatic colon cancer. However, focal hypermetabolic activity in the scrotum was noted. A subsequent scrotal ultrasound revealed a hypoechoic heterogeneous lesion with microcalcification in the posterior aspect of the right testis. At surgery, a right testicular mass was confirmed and right inguinal/radical orchiectomy was performed. Histology showed seminoma (classic type) within the testicular parenchyma without infiltration to the tunica albuginea.

A follow-up 1-131 whole-body scintigram performed on a 56-year-old man 2 years after total thyroi... more A follow-up 1-131 whole-body scintigram performed on a 56-year-old man 2 years after total thyroidectomy and ablation therapy with radioactive 1-131 for papillary thyroid carcinoma showed focal midline uptake in the anterior neck compatible with thyroid remnant tissue or metastatic papillary carcinoma. The activity largely dissipated on additional "postwash" imaging after fluid ingestion, indicating accumulation of activity most likely in the patient's known Zenker diverticulum rather than thyroidal tissue. The possibility of a false-positive study resulting from related conditions needs to be considered for proper interpretation. Radiographic correlation helps differentiate a functional thyroid remnant from physiological or false-positive findings.

A cold thyroid nodule was detected in a diffusely enlarged gland in a patient undergoing thyroid ... more A cold thyroid nodule was detected in a diffusely enlarged gland in a patient undergoing thyroid uptake and scan for suspected Graves' disease. Fine-needle biopsy of the nodule suggested neoplasm. Subsequent surgery confirmed the diagnosis of papillary carcinoma of the thyroid. The patient was treated successfully for both Graves' disease and papillary carcinoma of the thyroid with total thyroidectomy.

This study correlates the findings of bone marrow biopsy with those of bone scan in patients with... more This study correlates the findings of bone marrow biopsy with those of bone scan in patients with common cancers and highlights the comparable sensitivities of the two methods at the authors' institution. The results show that bone scintigraphy may be slightly more sensitive than bone marrow examination for the detection of carcinomatous metastases. This experience suggests that institutions should devise clinical pathways based on their testing sensitivities and patient characteristics. These considerations will ultimately result in better and more cost-effective treatment of these patients.

Disseminated synovial lymphoma of the knee with hepatic involvement in a patient after renal transplant was suggested on bone/gallium scintigraphy and confirmed by synovial biopsy. Gallium scanning thus was instrumental in the evaluation, staging, and proper treatment of this rare condition.

Bowel transit in two women with protracted constipation was evaluated after oral administration o... more Bowel transit in two women with protracted constipation was evaluated after oral administration of In-111 DTPA in water. Serial abdominal images were obtained for as long as 96 hours to assess transit through the stomach, small bowel, and colon. In both patients, large bowel transit was delayed. A pattern of colonic inertia was observed in one patient, whereas retention in the distal colon was seen in the other patient. Both patients underwent total colectomy with marked symptomatic relief. Colonic transit studies of these patients are presented, and the literature is reviewed.

Hepatobiliary scintigraphy is an established method for the diagnosis of a bile leak from the bil... more Hepatobiliary scintigraphy is an established method for the diagnosis of a bile leak from the biliary system. A bile leak should be considered in any patient after cholecystectomy who has unexplained abdominal pain after operation. Three patients with bile leak diagnosed by scintigraphy are described, one of whom had an unusual pattern of hepatic subcapsular collection of the bile. The second patient had a bile leak through the postsurgical drainage tube, whereas the third patient had a more typical pattern of leakage into the peritoneal cavity.

Imaging correctly identified the source of bleeding at the anastomotic site in the large bowel, effectively contributing in the patient's treatment planning. Radionuclide scintigraphy provides a simple, noninvasive modality to diagnose and manage difficult clinical situations such as postoperative bleeding.

Studies in the collagen and rabbit models of arthritis have indicated that indium-ill chloride ("... more Studies in the collagen and rabbit models of arthritis have indicated that indium-ill chloride ("mInC3) scintigraphy objectively measures synovial inflammation. Indium-ill chloride scans, with imaging three days after 19 MBq intravenous injection, were performed on 21 patients with definite or classical rheumatoid arthritis (RA), all of whom were functional class II. Standard clinical indices of disease activity were recorded at the time of imaging by the same investigator, who was unaware of the results of joint scans. In addition, eight patients with severe osteoarthritis, four of whom were considered to need hip or knee joint replacement, were similarly scanned. In each patient 16 joints were graded as 0 to 5, based on increasing degrees of "InC13 uptake, by a single investigator blinded to the patient's diagnosis and clinical status. In the group with RA significant correlations were observed between individual joint uptake on scan and peripheral joints with swelling, joints reported to be painful, and joints with any abnormality on physical examination. In the group with osteoarthritis joints positive on scan correlated with the presence of pain. A total scan score (sum of individual joint scores) was calculated for each patient. In the patients with RA values ranged from 0 to 42 with a mean (SEM) of 20-7 (2.7) and correlated with the number of swollen joints and decreasing grip strength. In the group with osteoarthritis the mean total scan score (9-2 (1-5), range 3-14) was significantly lower than in the patients with RA. These data show that "'InCl3 scanning can measure joint involvement by RA.
