Artan yaşla birlikte görülme sıklığı artmaktadır. Kadınlarda daha çok görülen bu eklem hastalığı en sık yük taşıyan büyük kalça ve diz eklemlerini ve zaman zaman el gibi küçük eklemleri etkilemektedir. Hastalığın erken dönemlerinde konservatif tedavi ile hastalığın semptomlarında azalma sağlanırken, hastalığın ilerlediği durumlarda cerrahi yöntemlere ihtiyaç duyulmaktadır. Diz osteoartritinde doğru konservatif tedavi yaklaşımlarının tercih edilmesi, hem semptomlarda azalma sağlayarak hastanın yaşam kalitesini artırmak için hem de cerrahi süreci mümkün ölçüde ertelemek için önemlidir. Fizyoterapi ve rehabilitasyon, en çok tercih edilen konservatif tedavi yaklaşımlarından birisidir. Aerobik egzersizler ve kuvvetlendirme egzersizleri başta olmak üzere egzersizler, diz osteoartritinde konservatif tedavinin temel taşıdır. Egzersiz dışı fizyoterapi yöntemlerinin de uygun hastalarda programa eklenmesi elde edilen kazanımları destekleyecektir. Kilo kontrolü, hasta eğitimi ve baş etme mekanizmalarını geliştirmek konservatif tedavinin başarısını daha da arttıracaktır. Fizyoterapi ve Rehabilitasyon yönünden tedavinin başarısı; doğru tedavi yaklaşımlarının uygulanması ve düzenli egzersiz alışkanlığı geliştirmesi ile yakından ilişkilidir.

To produce a new tool for investigating primary culture and adenoviral infection of chondrocytes ... more To produce a new tool for investigating primary culture and adenoviral infection of chondrocytes from mature mouse articular cartilage (C57BL/6J) Methods: We used four-month old C57 BL/6J mice as provider and dissected articular cartilage from tibial plateaus and femoral condyles for primary culture, then examined several morphological and biochemical features of different passages to determine which passage is suit for deep investigation. Furthermore, we used adenovirus as powerful gene transfer vector system to achieve gene SREBP-2 knock-down. Results: The cells from P0, 1 had the typical chondrocytes morphology, with a rounded or polygonal shape, but till P2,3 the cells became dedifferentiation with the shape changed. Alcian-Blue and Immunofluorescence staining showed that chondrocytes from P1 express proteoglycans and collagen II respectively. The results from RealTime-PCR showed collagen II and aggrecan expression decreased with passages. In contrast, collagen I expression was low in primary cultures and became much higher after passages, indicating that this kind of chondrocytes became fibroblasts and not suit for deep investigation. Gene delivery via adenoviral infection was highly efficient and reproducible at 50 MOI for 48 h, and the RT-PCR of gene SREBP-2 proved the effects. Conclusions: We have developed practical and reliable methods to isolate and culture chondrocytes from articular of mature mouse and to implement acute gene knock-down in these chondrocytes via adenoviral infection. The culture and adenoviral infection of mature mice articular chondrocyte described here should prove a powerful tool for deeper research for virus theory and drug delivery at the cellular or subcellular level.

Results: Differences between high (0.64 ± 0.16) and low (0.78 ± 0.20) injury risk movement profiles in the early season A:C ratio trended toward significance (t¼-1.772, p ¼ 0.091). A large effect size for pre-season LESS score was observed (d¼0.83); however, our power was limited by number of athletes. There was no effect of pre-season LESS score on in-season average training load (t¼-1.546, p¼0.137; low risk movers ¼ 0.93 ± 0.31, high risk movers ¼ 1.09 ± 0.20). Conclusions: Our results suggest that movement quality associated with musculoskeletal injury risk may influence early season TLs in collegiate male soccer athletes. Bu çalışma Türkiye'de istihdam edilen fizyoterapistlerin özel kuruluşlar ya da kamu kurum ve kuru... more Bu çalışma Türkiye'de istihdam edilen fizyoterapistlerin özel kuruluşlar ya da kamu kurum ve kuruluşlarındaki çalışma ortam ve koşulları ile çalışma sürelerinin; fiziksel şikâyet ve çalışma ortamına yönelik memnuniyetlerine etkisini ortaya koymayı hedeflemektedir. Gereç ve Yöntemler: Çalışmaya İstanbul'da bulunan özel kuruluşlarda ve kamu kurum ve kuruluşlarında çalışan 204 fizyoterapist dahil edildi. Katılımcıların 111'i özel kuruluşlarda (Grup 1) ve 93'ü kamu kurum ve kuruluşlarında (Grup 2) çalışmakta idi. Katılımcıların demografik bilgileri alındıktan sonra tarafımızca hazırlanan 6 bölüm 31 sorudan oluşan "Fizyoterapistlerin Çalışma Ortamı Memnuniyeti Anketi" yüz yüze yöntemiyle uygulandı. Bulgular: Grup 1'in yaş ortalaması 29,88±6,80 yıl, Grup 2'nin 33,85±8,87 yıl idi (p=0,00). Grupların çalışma süreleri karşılaştırıldığında Grup 1'in toplam çalışma süresinin 6,41±6,89 ve Grup 2'nin toplam çalışma süresinin 10,10±8,82 (yıl) olduğu, oturarak geçen sürenin Grup 1'de 3,35±1,35 ve Grup 2'de 2,95±1,21 (saat) olduğu görüldü ve gruplar arasında toplam çalışma süresi (p=0,01) ve oturarak geçen süre (p=0,03) açısından istatistiksel olarak anlamlı fark vardı. Grup 2'nin toplam çalışma süresinin daha fazla ve iş yerinde oturarak geçirdikleri sürenin daha az olduğu görüldü. Grupların çalışma ortamı memnuniyet düzeylerine bakıldığında uygulanan anketin toplam skoru Grup 1'de 60,32±14,80 ve Grup 2'de 43,35±13,87 olarak bulundu. Grupların çalışma ortamı memnuniyet düzeyleri karşılaştırıldığında uygulanan anketin alt bölümleri ve toplam skorda memnuniyet düzeyi açısından Grup 1'in üstün olduğu tespit edildi (p=0,00). Sonuç: Bu çalışma bize özel kuruluşlarda çalışan fizyoterapistlerin, kamu kurum ve kuruluşlarında çalışan fizyoterapistlere göre çalışma ortamı ve koşullarından daha memnun olduklarını gösterdi.

Background: Video-based rehabilitation programs, which are also used in the treatment of neurolog... more Background: Video-based rehabilitation programs, which are also used in the treatment of neurological disorders, could be a beneficial treatment option for patients who cannot receive treatment. Purpose: To determine the efficacy of video-based rehabilitation program in the conservative treatment of partial rotator cuff tears in comparison to a physiotherapist-supervised rehabilitation program. Study Design: A randomized controlled clinical trial. Methods: Thirty-three voluntary patients with partial rotator cuff tear were randomized into two groups: video-based rehabilitation (VBR) group and physiotherapist-supervised rehabilitation (PSR) group, for a common 6 weeks rehabilitation program. After drop outs, thirty participants (mean AE SD age, 50.60 AE 8.54 years; 10 female) were finally completed the trial. Active shoulder range of motion, pain, functional status, and health-related quality of life of the patients were assessed before and after treatment. Also, treatment satisfaction level was assessed at the end of the treatment. Results: There was statistically significant improvement in terms of active shoulder range of motion values (mean differences at 95% confidence interval for abduction: 30.75-51.37 in the VBR group, 34.20-54.45 in the PSR group, P ¼ .001 in both groups) and secondary outcome measures within both groups (P < .05). However, there were no statistically significant differences in terms of all outcome measures between groups (P > .05). The level of satisfaction from treatment in the PSR group was higher than that in the VBR group. Conclusion: A video-based rehabilitation program is an effective option in terms of shoulder range of motion, pain, functionality, and quality of life and has a comparable success level with a PSR program in the conservative treatment of partial rotator cuff tears.

The aims of our study were, 1. to assess pain, limitation of movement ability, and functionality ... more The aims of our study were, 1. to assess pain, limitation of movement ability, and functionality in osteoarthritis patients scheduled to undergo total knee arthroplasty, 2. to determine if pain (Group 1) or function loss (Group 2) has a greater influence on the decision of patients to have surgery, and 3. to compare results between Group 1 and Group 2. [Subject and Methods] Fifty-five osteoarthritis patients classified as grades 3 and 4 according to the Kellgren-Lawrence system of classification were evaluated for preoperative pain intensity with the Visual Analogue Scale, knee flexion/extension range of movement with a clinical goniometer, and function with the Western Ontario and McMaster Universities Osteoarthritis Index. Patients were examined to reveal their reasons regarding the decision to undergo total knee arthroplasty (pain or function loss). [Results] The Visual Analog Scale scores at rest and during activity were 5.62 and 7.42, the knee flexion range of movement and extension limitation were 93.17° and −7.04°, and the Western Ontario and McMaster Universities Osteoarthritis Index value was 82.09. Regarding the decision to undergo surgery, 47.3% (n=26) of the knees were in Group 1, and 52.7% were in Group 2; the two groups were not significantly different. There were also no significant differences between the groups in Visual Analog Scale score during activity, the Western Ontario and McMaster Universities Osteoarthritis Index value, and knee flexion range of movement and extension limitation. The only statistically significant difference was found in the Visual Analog Scale score at rest in Group 1, which was significantly higher than that in Group 2. [Conclusion] Our results showed that osteoarthritis patients decided to undergo surgery only if all of the parameters were impaired significantly. Both pain and function loss have a similar impact on a patient's decision to undergo surgery. We observed no significant difference in clinical and self-reported outcomes between patients who decided to undergo surgery due to pain or function loss.

BACKGROUND Studies have emphasized the importance of the presence of myofascial trigger points (M... more BACKGROUND Studies have emphasized the importance of the presence of myofascial trigger points (MTrPs) in patients with rotator cuff pathologies and the high frequency of MTrPs in rotator cuff muscles. OBJECTIVE Evaluate the effectiveness of the treatment of active MTrPs in patients with rotator cuff pathologies. METHODS Fifty-three patients with rotator cuff tear were randomized into two groups. All patients received the same standard conservative treatment twice a week for 6 weeks. Patients in Group 1 additionally received ischemic compression (IC) of MTrPs. Pain, range of motion (ROM), function, and anxiety and depression were assessed. MTrPs in rotator cuff muscles were assessed manually, and the number of MTrPs on the shoulder complex was counted. RESULTS There were no significant differences between the groups in terms of changes in resting/activity/night pain, ROM, function, or anxiety and depression (p> 0.05). Pain scores improved only in Group 1. However, the total number of MTrPs was significantly decreased in Group 1 (p= 0.001). CONCLUSION A six-week course of IC helps treat active MTrPs. A standard conservative treatment program reduced pain and increased function; the addition of MTrP treatment did not improve clinical outcomes in patients with rotator cuff pathologies.

The AOFAS-AHFS is widely used to evaluate disability associated with foot and ankle injuries but has not yet been translated or culturally adapted for Turkish-speaking individuals. The AOFAS-AHFS was translated into Turkish using the Beaton guidelines. The measurement properties of the Turkish AOFAS-AHFS (internal consistency, construct validity, and floor and ceiling effects) were tested in 72 patients (94 feet, 50 females; average ± SD age 44.88 ± 16.30 years) with a variety of foot and ankle pathologic features. Patients were randomly divided into two groups and treated with WBV and/or home-based balance coordination exercises (BCEs). Patients in the WBV-BCE group were asked to stand on the platform in standing, squat, and deep squat positions. During each position, 30-sec vibration (30-35 Hz) and 60-sec rest periods were applied twice (20 sessions in total). The BCE program was performed by subjects twice per day. To produce a new tool for investigating primary culture and adenoviral infection of chondrocytes ... more To produce a new tool for investigating primary culture and adenoviral infection of chondrocytes from mature mouse articular cartilage (C57BL/6J) Methods: We used four-month old C57 BL/6J mice as provider and dissected articular cartilage from tibial plateaus and femoral condyles for primary culture, then examined several morphological and biochemical features of different passages to determine which passage is suit for deep investigation. Furthermore, we used adenovirus as powerful gene transfer vector system to achieve gene SREBP-2 knock-down. Results: The cells from P0, 1 had the typical chondrocytes morphology, with a rounded or polygonal shape, but till P2,3 the cells became dedifferentiation with the shape changed. Alcian-Blue and Immunofluorescence staining showed that chondrocytes from P1 express proteoglycans and collagen II respectively. The results from RealTime-PCR showed collagen II and aggrecan expression decreased with passages. In contrast, collagen I expression was low in primary cultures and became much higher after passages, indicating that this kind of chondrocytes became fibroblasts and not suit for deep investigation. Gene delivery via adenoviral infection was highly efficient and reproducible at 50 MOI for 48 h, and the RT-PCR of gene SREBP-2 proved the effects. Conclusions: We have developed practical and reliable methods to isolate and culture chondrocytes from articular of mature mouse and to implement acute gene knock-down in these chondrocytes via adenoviral infection. The culture and adenoviral infection of mature mice articular chondrocyte described here should prove a powerful tool for deeper research for virus theory and drug delivery at the cellular or subcellular level.

Results: Differences between high (0.64 ± 0.16) and low (0.78 ± 0.20) injury risk movement profiles in the early season A:C ratio trended toward significance (t¼-1.772, p ¼ 0.091). A large effect size for pre-season LESS score was observed (d¼0.83); however, our power was limited by number of athletes. There was no effect of pre-season LESS score on in-season average training load (t¼-1.546, p¼0.137; low risk movers ¼ 0.93 ± 0.31, high risk movers ¼ 1.09 ± 0.20). Conclusions: Our results suggest that movement quality associated with musculoskeletal injury risk may influence early season TLs in collegiate male soccer athletes. Bu çalışma Türkiye'de istihdam edilen fizyoterapistlerin özel kuruluşlar ya da kamu kurum ve kuru... more Bu çalışma Türkiye'de istihdam edilen fizyoterapistlerin özel kuruluşlar ya da kamu kurum ve kuruluşlarındaki çalışma ortam ve koşulları ile çalışma sürelerinin; fiziksel şikâyet ve çalışma ortamına yönelik memnuniyetlerine etkisini ortaya koymayı hedeflemektedir. Gereç ve Yöntemler: Çalışmaya İstanbul'da bulunan özel kuruluşlarda ve kamu kurum ve kuruluşlarında çalışan 204 fizyoterapist dahil edildi. Katılımcıların 111'i özel kuruluşlarda (Grup 1) ve 93'ü kamu kurum ve kuruluşlarında (Grup 2) çalışmakta idi. Katılımcıların demografik bilgileri alındıktan sonra tarafımızca hazırlanan 6 bölüm 31 sorudan oluşan "Fizyoterapistlerin Çalışma Ortamı Memnuniyeti Anketi" yüz yüze yöntemiyle uygulandı. Bulgular: Grup 1'in yaş ortalaması 29,88±6,80 yıl, Grup 2'nin 33,85±8,87 yıl idi (p=0,00). Grupların çalışma süreleri karşılaştırıldığında Grup 1'in toplam çalışma süresinin 6,41±6,89 ve Grup 2'nin toplam çalışma süresinin 10,10±8,82 (yıl) olduğu, oturarak geçen sürenin Grup 1'de 3,35±1,35 ve Grup 2'de 2,95±1,21 (saat) olduğu görüldü ve gruplar arasında toplam çalışma süresi (p=0,01) ve oturarak geçen süre (p=0,03) açısından istatistiksel olarak anlamlı fark vardı. Grup 2'nin toplam çalışma süresinin daha fazla ve iş yerinde oturarak geçirdikleri sürenin daha az olduğu görüldü. Grupların çalışma ortamı memnuniyet düzeylerine bakıldığında uygulanan anketin toplam skoru Grup 1'de 60,32±14,80 ve Grup 2'de 43,35±13,87 olarak bulundu. Grupların çalışma ortamı memnuniyet düzeyleri karşılaştırıldığında uygulanan anketin alt bölümleri ve toplam skorda memnuniyet düzeyi açısından Grup 1'in üstün olduğu tespit edildi (p=0,00). Sonuç: Bu çalışma bize özel kuruluşlarda çalışan fizyoterapistlerin, kamu kurum ve kuruluşlarında çalışan fizyoterapistlere göre çalışma ortamı ve koşullarından daha memnun olduklarını gösterdi.

Purpose: To determine the efficacy of video-based rehabilitation program in the conservative treatment of partial rotator cuff tears in comparison to a physiotherapist-supervised rehabilitation program. Study Design: A randomized controlled clinical trial. Methods: Thirty-three voluntary patients with partial rotator cuff tear were randomized into two groups: video-based rehabilitation (VBR) group and physiotherapist-supervised rehabilitation (PSR) group, for a common 6 weeks rehabilitation program. After drop outs, thirty participants (mean AE SD age, 50.60 AE 8.54 years; 10 female) were finally completed the trial. Active shoulder range of motion, pain, functional status, and health-related quality of life of the patients were assessed before and after treatment. Also, treatment satisfaction level was assessed at the end of the treatment. Results: There was statistically significant improvement in terms of active shoulder range of motion values (mean differences at 95% confidence interval for abduction: 30.75-51.37 in the VBR group, 34.20-54.45 in the PSR group, P ¼ .001 in both groups) and secondary outcome measures within both groups (P < .05). However, there were no statistically significant differences in terms of all outcome measures between groups (P > .05). The level of satisfaction from treatment in the PSR group was higher than that in the VBR group. Conclusion: A video-based rehabilitation program is an effective option in terms of shoulder range of motion, pain, functionality, and quality of life and has a comparable success level with a PSR program in the conservative treatment of partial rotator cuff tears.

The aims of our study were, 1. to assess pain, limitation of movement ability, and functionality ... more The aims of our study were, 1. to assess pain, limitation of movement ability, and functionality in osteoarthritis patients scheduled to undergo total knee arthroplasty, 2. to determine if pain (Group 1) or function loss (Group 2) has a greater influence on the decision of patients to have surgery, and 3. to compare results between Group 1 and Group 2. [Subject and Methods] Fifty-five osteoarthritis patients classified as grades 3 and 4 according to the Kellgren-Lawrence system of classification were evaluated for preoperative pain intensity with the Visual Analogue Scale, knee flexion/extension range of movement with a clinical goniometer, and function with the Western Ontario and McMaster Universities Osteoarthritis Index. Patients were examined to reveal their reasons regarding the decision to undergo total knee arthroplasty (pain or function loss). [Results] The Visual Analog Scale scores at rest and during activity were 5.62 and 7.42, the knee flexion range of movement and extension limitation were 93.17° and −7.04°, and the Western Ontario and McMaster Universities Osteoarthritis Index value was 82.09. Regarding the decision to undergo surgery, 47.3% (n=26) of the knees were in Group 1, and 52.7% were in Group 2; the two groups were not significantly different. There were also no significant differences between the groups in Visual Analog Scale score during activity, the Western Ontario and McMaster Universities Osteoarthritis Index value, and knee flexion range of movement and extension limitation. The only statistically significant difference was found in the Visual Analog Scale score at rest in Group 1, which was significantly higher than that in Group 2. [Conclusion] Our results showed that osteoarthritis patients decided to undergo surgery only if all of the parameters were impaired significantly. Both pain and function loss have a similar impact on a patient's decision to undergo surgery. We observed no significant difference in clinical and self-reported outcomes between patients who decided to undergo surgery due to pain or function loss.

BACKGROUND Studies have emphasized the importance of the presence of myofascial trigger points (M... more BACKGROUND Studies have emphasized the importance of the presence of myofascial trigger points (MTrPs) in patients with rotator cuff pathologies and the high frequency of MTrPs in rotator cuff muscles. OBJECTIVE Evaluate the effectiveness of the treatment of active MTrPs in patients with rotator cuff pathologies. METHODS Fifty-three patients with rotator cuff tear were randomized into two groups. All patients received the same standard conservative treatment twice a week for 6 weeks. Patients in Group 1 additionally received ischemic compression (IC) of MTrPs. Pain, range of motion (ROM), function, and anxiety and depression were assessed. MTrPs in rotator cuff muscles were assessed manually, and the number of MTrPs on the shoulder complex was counted. RESULTS There were no significant differences between the groups in terms of changes in resting/activity/night pain, ROM, function, or anxiety and depression (p> 0.05). Pain scores improved only in Group 1. However, the total number of MTrPs was significantly decreased in Group 1 (p= 0.001). CONCLUSION A six-week course of IC helps treat active MTrPs. A standard conservative treatment program reduced pain and increased function; the addition of MTrP treatment did not improve clinical outcomes in patients with rotator cuff pathologies.

The AOFAS-AHFS is widely used to evaluate disability associated with foot and ankle injuries but has not yet been translated or culturally adapted for Turkish-speaking individuals. The AOFAS-AHFS was translated into Turkish using the Beaton guidelines.
