Methods: A total of 1612 patients having abnormal DRE and/or elevated PSA whom underwent TRUS guided prostate biopsies were included in the study. Any pal-pable induration or nodularity was accepted as abnormal DRE findings. Pathologic features of biopsy speci-mens were compared within groups according to DRE findings and serum PSA level groups of 2.5- 4, 4- 10 and>10 ng/ml. Results: Abnormal DRE was detected in 339 patients; of whom 48.7 % were determined to have cancer. Cancer detection rates of patients having abnormal DRE were found to be 20%, 31.5 % and 68% at PSA ranges 2.5- 4, 4- 10 and>10 ng/ml, respectively. Significantly higher grade cancers were detected by abnormal DRE at each PSA group. "Ödematöz" ya da "hafif" seyirli akut pankreatitte mortalite ve morbiditenin düşük olmasına koşut olarak cerrahi dışı sağaltımda çok sorun yok gibi gözükmektedir. O halde akut pankreatitte "şiddetli" tanımlaması, cerrahi öncesi dönemde önemli bir belirleyiciliğe sahiptir. Önceleri semptomların şiddetine göre; yani ağrı, bulantı ve kusmanın şiddetine göre karar verilirken(2-4), sonraları pseudokist, abse veya ölüm gibi komplikasyonların varlığı akut pankreatitte "şiddet"in tanımlayıcısı olmuştur (5, 6). Objective: To evaluate the correlation of stereologically estimated mean nuclear volume of tumor ... more Objective: To evaluate the correlation of stereologically estimated mean nuclear volume of tumor cells with other clinicopathologic prognostic features and survival in pancreatic ductal adenocarcinoma. Study design: The study included 27 patients with primary pancreatic ductal adenocarcinoma. A stereologic method proposed by Gundersen et al was used for the estimation of mean nuclear volume in hematoxylin and eosin-stained histologic sections of each case. Mean nuclear volume values were compared statistically with histopathologic prognostic feature groups and survival. Results: The mean nuclear volume values of tumor cells ranged from 296.83 to 982.79 microns 3 (mean, 633.906 +/- 212.310). Higher values of mean nuclear volume were significantly related to advanced tumor stage and the presence of distant metastasis (Kruskal-Wallis, P = .036; Mann-Whitney U, P = .020). In contrast, nodal stage, tumor grade, perineural invasion, lymphatic and blood vessel invasion, and size of tumor showed no statistical relation to mean nuclear volume of tumor cells. Mean nuclear volume was found to be a significant predictor of survival in univariate analysis (P = .016). Conclusion: Estimation of mean nuclear volume may help in predicting the extent of disease and clinical behavior in pancreatic adenocarcinoma.

The suture-mediated Prostar XL® VCD is stated to be eligible for up to 26Fr sheaths' puncture sites. The main criticized point of Prostar XL® is the complexity of the deployment steps with resultant experience of the interventionist, which is pointed to be the major cause of adverse events. The aim of this report is to warn the interventionists to take into account of this rare complication.

The diagnosis of alveolar echinococcosis, caused by the larval stage of Echinococcus multilocular... more The diagnosis of alveolar echinococcosis, caused by the larval stage of Echinococcus multilocularis, is often difficult and almost always possible only in the later stages of the disease. In this study, we aimed to evaluate the findings of Doppler ultrasonography images for the diagnosis of intraabdominal non-visceral alveolar echinococcosis in Meriones unguiculatus. Six infected animals were studied. Abdominal Doppler ultrasonography examinations were performed 20-25 days after the implantation. Then, animals were sacrificed and infected specimens were histopathologically examined. The sonographic examinations of the infected animals revealed lobulated, heterogeneous cystic intraabdominal masses. There were echogenic solid areas with hypoechoic and anechoic cystic areas within the lesions. Doppler ultrasound examination revealed vascular islands within those heterogeneous cystic lesions. Color mode showed multiple vascular coding areas within the solid part of the lesions, most of them were venous structures but there were also arterial vessels showing dominant flow with low resistive indices. Vascular structures were supposed to be the invaded native mesenteric vessels within the infiltrating inflammatory mass lesions. We consider that the Doppler ultrasound finding of irregular mesenteric vascular structures within intraabdominal heterogenous mass lesions may be an important sign for the diagnosis of experimental non-visceral alveolar echinococcosis in Meriones unguiculatus.

Nonocclusive mesentery ischemia, a subgroup of "acute mesentery ischemia", is known as a period of intestinal ischemic hypoperfusion without a demonstrable vascular occlusion in the mesentery bed. It can be caused by factors leading to splanchnic hypoperfusion, which can be of cardiac, renal or hepatic origin. In addition, it can be induced by certain drugs such as digitalis, ergotamines and vasoactive agents. In clinical practice, digitalis toxicity is commonly seen. In contrast, non-occlusive mesentery ischemia secondary to digitalis is quite rare. However, nonocclusive mesentery ischemia should be included in the differential diagnosis for patients who develop sudden and diffuse abdominal pain while on digitalis therapy.

The aim of this study was to examine the relationships between the renal resistance index (RI) an... more The aim of this study was to examine the relationships between the renal resistance index (RI) and histomorphometric variables measured in children with glomerular or nonglomerular renal disease. Patients and Methods The medical records, sonography reports, and biopsy specimens of 30 children with glomerular or nonglomerular renal disease (mean age, 11.0 +/- 4.7 years; range, 2. 5-17 years) who underwent sonographic evaluation and biopsy of the right kidney were retrospectively studied. Biopsy specimens were evaluated for glomerular surface density, number of glomeruli in the stroma, capillary surface density, number of capillaries within the glomeruli, and the diameters of the afferent arterioles. The RI was negatively correlated with age in patients 2.5-4.0 years old (rho = -1.0, p < 0.001). The RI was positively correlated with serum creatinine levels (rho = 0.63, p < 0.001), and the RI differed significantly between patients with abnormally high and age-appropriate serum creatinine levels (0.8 +/- 0.1 and 0.6 +/- 0.1, respectively; p = 0.002). No differences in age, histomorphometric variables, RI, or arteriolar diameter were found between patients who had interstitial fibrosis and those who did not, and the RI was not correlated with histomorphometric variables in either group. However, the mean RI was significantly higher in patients with severe fibrosis than in those with mild fibrosis. In those with severe fibrosis, the RI was negatively correlated with number of glomeruli in the stroma (rho = -0.975, p < 0.01), glomerular surface density (rho = -0.931, p < 0.05), number of capillaries within the glomeruli (rho = -0.989, p < 0.01), and arteriolar diameter (rho = -1.00, p < 0.01). The RI was correlated with histomorphometric variables in patients with severe interstitial fibrosis and could be useful in the follow-up of these patients.

Although predictive value of abnormal DRE diminishes with concomitantly decreasing PSA levels, significance of DRE in the diagnosis of prostate cancer cannot be ignored.

We report the case of a hematuric adolescent diagnosed with a high index of suspicion by noninvasive Doppler sonography using the diagnostic indices of Doppler sonography established for adults with hematuria. Compression of the patient's left renal vein at the aortomesenteric portion and dilatation at the hilar portion were visualized by magnetic resonance angiography, which verified the diagnosis. © 2003 Wiley Periodicals, Inc. Assessment of the effects of normovolaemic haemodilution on middle cerebral artery blood flow vel... more Assessment of the effects of normovolaemic haemodilution on middle cerebral artery blood flow velocity with transcranial Doppler ultrasonography, intracranial pressure, cerebral perfusion pressure, arterial oxygen content and cerebral oxygen delivery. Normovolaemic haemodilution was induced in rabbits under general anaesthesia, and the haematocrit was allowed to decrease to 30% in Group 1 (n = 6) and to 20% in Group 2 (n = 6). Peak systolic and diastolic velocities, mean blood flow velocity, and pulsatility and resistance indices of the middle cerebral artery were measured by transcranial Doppler ultrasonography. Changes in intracranial pressure, cerebral perfusion pressure, arterial oxygen content and cerebral oxygen delivery were also assessed. In Group 2, middle cerebral artery blood flow velocity increased from 0.4 +/- 0.01 to 0.51 +/- 0.02 m s(-1) after the induction of normovolaemic haemodilution (P = 0.04), while arterial oxygen content decreased from 16.2 +/- 0.1 to 8.5 +/- 0.1 mLdL(-1) (P = 0.002). The decrease in cerebral oxygen delivery from 6.5 +/- 0.2 to 4.3 +/- 0.2 was also significant (P = 0.02). However, no associated changes in intracranial pressure and cerebral perfusion pressure could be demonstrated. Normovolaemic haemodilution resulted in an increase in the mean blood flow velocity of the middle cerebral artery. However, this increase did not compensate for the consequences of the altered oxygen delivery to the brain when the haematocrit was reduced to 20%.

ABSTRACT The purpose of this investigation of lumbar disc herniation patients was to compare the ... more ABSTRACT The purpose of this investigation of lumbar disc herniation patients was to compare the effi cacy of the inversion spinal traction and the conventional mechanical spinal traction on several clinical parameters and computed tomography. This investigation consisted of 29 patients with low back pain and sciatica due to lumbar disc herniation. Patients were randomly assigned into two groups: an inversion spinal traction that was applied to 15 patients, and a conventional traction that was applied to 14 patients for ten sessions. The efficacy of the treatments was evaluated based upon clinical parameters before, immediately after, and three months after the treatment. Computed tomographic (CT) investigation was done before and immediately after the treatment. Both methods of traction were found to be clinically effective. Although there was no statistically significant difference between the two groups based upon the clinical parameters, CT fi ndings of the conventional traction group tended to show more improved parameters than the inverted spinal traction group. Reasons for better results of the conventional traction are discussed and the necessity for further investigations on this topic is emphasized.

Objective: Greater flexibility and smaller sizes for introducer sheaths in the newest stent graft... more Objective: Greater flexibility and smaller sizes for introducer sheaths in the newest stent grafts increase the feasibility of endovascular aneurysm repair but raise concerns about long-term limb patency. The aim of the study was to determine the incidence of and predictive factors for limb occlusion after use of the Endurant stent graft (Medtronic Inc, Minneapolis, Minn) for abdominal aortic aneurysm. Methods: The Endurant Stent Graft Natural Selection Global Postmarket Registry (ENGAGE) prospectively included 1143 patients treated with bifurcated devices who were observed for up to 2 years. Limb occlusions were evidenced by computed tomography, angiography, or ultrasound. To predict stent graft limb occlusion, a two-step model-building technique was applied. We first identified predictors from a total of 47 covariates obtained at baseline and in the periprocedural period. Subsequently, we reduced the set of potential predictors to key factors that are clinically meaningful. To handle large numbers of covariates, we used the Classification And Regression Tree (CART) method. Results: Forty-two stent graft limbs occluded in 39 patients (3.4% of the patients). At 2 years, the rate of freedom from stent graft limb occlusion calculated by Kaplan-Meier plot was 97.9% (standard error [SE], 0.33%). Of the 42 occlusions, 13 (31%) were observed within 30 days and 30 (71%) within 6 months. The strongest independent predictors were distal landing zone on the external iliac artery, external iliac artery diameter #10 mm, and kinking. High-risk vs low-risk patients were identified according to a decision tree based on the strongest predictors. Freedom from stent graft limb occlusion was 96.1% (SE, 0.64%) in high-risk patients vs 99.6% (SE, 0.19%) in low-risk patients. Conclusions: After Endurant stent grafting, the incidence of limb occlusion was low. Classifying patients as high risk vs low risk according to the algorithm used in this study may help define specific strategies to prevent limb occlusion and improve the overall results of endovascular aneurysm repair using the latest generation of stent grafts.

The main criticized point of Prostar XL® is the complexity of the deployment steps with resultant experience of the interventionist, which is pointed to be the major cause of adverse events. The aim of this report is to warn the interventionists to take into account of this rare complication.

In this study, we aimed to evaluate the findings of Doppler ultrasonography images for the diagnosis of intraabdominal non-visceral alveolar echinococcosis in Meriones unguiculatus. Six infected animals were studied. Abdominal Doppler ultrasonography examinations were performed 20-25 days after the implantation. Then, animals were sacrificed and infected specimens were histopathologically examined. The sonographic examinations of the infected animals revealed lobulated, heterogeneous cystic intraabdominal masses. There were echogenic solid areas with hypoechoic and anechoic cystic areas within the lesions. Doppler ultrasound examination revealed vascular islands within those heterogeneous cystic lesions. Color mode showed multiple vascular coding areas within the solid part of the lesions, most of them were venous structures but there were also arterial vessels showing dominant flow with low resistive indices. Vascular structures were supposed to be the invaded native mesenteric vessels within the infiltrating inflammatory mass lesions. We consider that the Doppler ultrasound finding of irregular mesenteric vascular structures within intraabdominal heterogenous mass lesions may be an important sign for the diagnosis of experimental non-visceral alveolar echinococcosis in Meriones unguiculatus.

Nonocclusive mesentery ischemia, a subgroup of "acute mesentery ischemia", is known as a period of intestinal ischemic hypoperfusion without a demonstrable vascular occlusion in the mesentery bed. It can be caused by factors leading to splanchnic hypoperfusion, which can be of cardiac, renal or hepatic origin. In addition, it can be induced by certain drugs such as digitalis, ergotamines and vasoactive agents. In clinical practice, digitalis toxicity is commonly seen. In contrast, non-occlusive mesentery ischemia secondary to digitalis is quite rare. However, nonocclusive mesentery ischemia should be included in the differential diagnosis for patients who develop sudden and diffuse abdominal pain while on digitalis therapy.

Patients and Methods The medical records, sonography reports, and biopsy specimens of 30 children with glomerular or nonglomerular renal disease (mean age, 11.0 +/- 4.7 years; range, 2. 5-17 years) who underwent sonographic evaluation and biopsy of the right kidney were retrospectively studied. Biopsy specimens were evaluated for glomerular surface density, number of glomeruli in the stroma, capillary surface density, number of capillaries within the glomeruli, and the diameters of the afferent arterioles. The RI was negatively correlated with age in patients 2.5-4.0 years old (rho = -1.0, p < 0.001). The RI was positively correlated with serum creatinine levels (rho = 0.63, p < 0.001), and the RI differed significantly between patients with abnormally high and age-appropriate serum creatinine levels (0.8 +/- 0.1 and 0.6 +/- 0.1, respectively; p = 0.002). No differences in age, histomorphometric variables, RI, or arteriolar diameter were found between patients who had interstitial fibrosis and those who did not, and the RI was not correlated with histomorphometric variables in either group. However, the mean RI was significantly higher in patients with severe fibrosis than in those with mild fibrosis. In those with severe fibrosis, the RI was negatively correlated with number of glomeruli in the stroma (rho = -0.975, p < 0.01), glomerular surface density (rho = -0.931, p < 0.05), number of capillaries within the glomeruli (rho = -0.989, p < 0.01), and arteriolar diameter (rho = -1.00, p < 0.01). The RI was correlated with histomorphometric variables in patients with severe interstitial fibrosis and could be useful in the follow-up of these patients.

Objective: Our aim was to determine the performance of abnormal digital rectal examination (DRE) ... more Objective: Our aim was to determine the performance of abnormal digital rectal examination (DRE) in prostate cancer detection at different PSA levels. Methods: A total of 1612 patients having abnormal DRE and/or elevated PSA whom underwent TRUS guided prostate biopsies were included in the study. Any palpable induration or nodularity was accepted as abnormal DRE findings. Pathologic features of biopsy specimens were compared within groups according to DRE findings and serum PSA level groups of 2.5-4, 4-10 and >10 ng/ml. Results: Abnormal DRE was detected in 339 patients; of whom 48.7% were determined to have cancer. Cancer detection rates of patients having abnormal DRE were found to be 20%, 31.5% and 68% at PSA ranges 2.5-4, 4-10 and >10 ng/ml, respectively. Significantly higher grade cancers were detected by abnormal DRE at each PSA group. The positive and negative predictive values of abnormal DRE according to groups of PSA 2.5-4, 4-10 and >10 ng/ml were 20% and 84.1%, 31.5% and 80.6%, 68% and 66.6%, respectively. Conclusion: At each PSA group DRE resulted in detecting significantly more cancers with Gleason score > 7. Although predictive value of abnormal DRE diminishes with concomitantly decreasing PSA levels, significance of DRE in the diagnosis of prostate cancer cannot be ignored.

We report the case of a hematuric adolescent diagnosed with a high index of suspicion by noninvasive Doppler sonography using the diagnostic indices of Doppler sonography established for adults with hematuria. Normovolaemic haemodilution was induced in rabbits under general anaesthesia, and the haematocrit was allowed to decrease to 30% in Group 1 (n = 6) and to 20% in Group 2 (n = 6). Peak systolic and diastolic velocities, mean blood flow velocity, and pulsatility and resistance indices of the middle cerebral artery were measured by transcranial Doppler ultrasonography. Changes in intracranial pressure, cerebral perfusion pressure, arterial oxygen content and cerebral oxygen delivery were also assessed. In Group 2, middle cerebral artery blood flow velocity increased from 0.4 +/- 0.01 to 0.51 +/- 0.02 m s(-1) after the induction of normovolaemic haemodilution (P = 0.04), while arterial oxygen content decreased from 16.2 +/- 0.1 to 8.5 +/- 0.1 mLdL(-1) (P = 0.002). The decrease in cerebral oxygen delivery from 6.5 +/- 0.2 to 4.3 +/- 0.2 was also significant (P = 0.02). However, no associated changes in intracranial pressure and cerebral perfusion pressure could be demonstrated. Normovolaemic haemodilution resulted in an increase in the mean blood flow velocity of the middle cerebral artery. However, this increase did not compensate for the consequences of the altered oxygen delivery to the brain when the haematocrit was reduced to 20%.

This investigation consisted of 29 patients with low back pain and sciatica due to lumbar disc herniation. Patients were randomly assigned into two groups: an inversion spinal traction that was applied to 15 patients, and a conventional traction that was applied to 14 patients for ten sessions. The efficacy of the treatments was evaluated based upon clinical parameters before, immediately after, and three months after the treatment. Computed tomographic (CT) investigation was done before and immediately after the treatment. Both methods of traction were found to be clinically effective. Although there was no statistically significant difference between the two groups based upon the clinical parameters, CT fi ndings of the conventional traction group tended to show more improved parameters than the inverted spinal traction group. Reasons for better results of the conventional traction are discussed and the necessity for further investigations on this topic is emphasized.

The aim of the study was to determine the incidence of and predictive factors for limb occlusion after use of the Endurant stent graft (Medtronic Inc, Minneapolis, Minn) for abdominal aortic aneurysm. Methods: The Endurant Stent Graft Natural Selection Global Postmarket Registry (ENGAGE) prospectively included 1143 patients treated with bifurcated devices who were observed for up to 2 years. Limb occlusions were evidenced by computed tomography, angiography, or ultrasound. To predict stent graft limb occlusion, a two-step model-building technique was applied. We first identified predictors from a total of 47 covariates obtained at baseline and in the periprocedural period. Subsequently, we reduced the set of potential predictors to key factors that are clinically meaningful. To handle large numbers of covariates, we used the Classification And Regression Tree (CART) method. Results: Forty-two stent graft limbs occluded in 39 patients (3.4% of the patients). At 2 years, the rate of freedom from stent graft limb occlusion calculated by Kaplan-Meier plot was 97.9% (standard error [SE], 0.33%). Of the 42 occlusions, 13 (31%) were observed within 30 days and 30 (71%) within 6 months. The strongest independent predictors were distal landing zone on the external iliac artery, external iliac artery diameter #10 mm, and kinking. High-risk vs low-risk patients were identified according to a decision tree based on the strongest predictors. Freedom from stent graft limb occlusion was 96.1% (SE, 0.64%) in high-risk patients vs 99.6% (SE, 0.19%) in low-risk patients. Conclusions: After Endurant stent grafting, the incidence of limb occlusion was low. Classifying patients as high risk vs low risk according to the algorithm used in this study may help define specific strategies to prevent limb occlusion and improve the overall results of endovascular aneurysm repair using the latest generation of stent grafts.
