Understanding the psychological characteristics and motivations of each patient is fundamental and a part of empathy. But these factors must be controlled because they can affect patient’s satisfaction and cooperation. Therefore, there is a need for clinicians to improve the patient experience of treatment delivery and increase the potential for a successful treatment outcome. The aim of the present study is to present a classification of motivation for orthodontic treatment. In fact, the classification proposal is based on more than 10 years of own research on orthodontic treatment motivation and awareness.

Introduction: The training of dental students in communication skills has been faced with a new c... more Introduction: The training of dental students in communication skills has been faced with a new challenge related to the limitations of COVID 19. Obtaining informed consent and deciding on the need for treatment are part of the twoway dental-patient communication process. While training in Public Dental Health, the students learn the basic rules of patient-oriented communication, discussing models for successful conversation with the patient and the types of verbal and nonverbal communication. The main educational goal is to build dental professionals who can adequately accept the needs of patients, help to realize them and generate trust. The specific educational goal is to organize and discuss specific, evidence-based clinical skills for communicating with patients. Materials and methods: A literature search has been performed and alternatives have been discussed with colleagues. The information search has been conducted using the Medline database (PubMed), Science Direct, and Web of Science. Discussion: The development of the social element has been observed in the relationship dentist-patient. Against the background of growing commercialism in the provision of dental services, patients are feeling increasingly uncertain whether the treatment offered is the needed one. Therefore, the patient must be at the centre of modern dentistry where the primary role has the Medical University in educating ethical principles and building doctors dedicated to humanity. Conclusion: The challenges in teaching students to communicate effectively with the patient are significant, especially in a global pandemic. The roots of philanthropy and empathy are laid back in time. They were described by the first teachers, physicians and philosophers in Medicine such as Hippocrates, Socrates, Plato, Aristotle and others. It is our responsibility just to repeat them.

That's why the Oral Healthcare Providers (OHP) should be convinced the consent given by the patient is valid. It means that at the beginning of the treatment the orthodontist will ask a lot of questions and have expectations to receive appropriate answers. There is a specific lack of awareness about the first orthodontic consultation at 7y of age, occurrence and prevention of most of the common tooth jaw discrepancies which affect the oral health, self-confidence and overall development of the child. A variety of socio-demographic, educational, personal and other factors mostly divided into objective and subjective factors influences the perception of facial attractiveness. The orthodontic treatment lays down on the personal desire and attitudes, depends from the motivation but is not without a risk for the patient. The aim of the current research is to present the most objective and subjective factors identifying the patient's refusal. Material and methods: It's a case report based on preliminary discussion and orthodontic consultation over the cephalometric analysis and cast models. Orthodontic treatment protocol was followed and given informed consent by the individual was received. Results and discussions: An electronic search was conducted using the Medline database (PubMed), Science Direct, and Scopus. In this case report were described the treatment options for Class III malocclusion with an emphasis on maxillary protraction and existing impacted canine 13. The decision making capacity was evaluated and also what are the objective and subjective factors and how to proceed with patient refusal. Conclusions: Despite the orthodontist's efforts to improve the management of the dental practice and to attract new patients, these challenges should never been from the first importance. Contemporary dentistry requires that the patient's right to refuse should be respected and this refusal must be accepted. Because orthodontic treatment is expensive, the process of returning money or sharing responsibility for the treatment depend on the socio-cultural characteristics of both the patient and the doctor. The whole situation requires a very delicate approach, as it affects the image of the dental community in society at whole.

The chief complaint of a patient and his parents are crowded teeth upper and lower and anterior crossbite. "@I'm looking like a crocodile" were the first words of the boy sitting on the dental chair. The main task was functional but also esthetics because it was from the first importance to him. The treated malocclusion is characterized with a narrow maxillary width and prominence of the mandible. The maxilla was deficient in anteroposterior and transverse dimensions, causing a slightly concave and unpleasant profile, a crossbite relationship of lateral incisors and upper and lower anterior crowding. The use of mechanics was preferred and accepted as a better solution for the patient according to the idea to generate well directed growth. The patient was 12 year-old boy with Class I malocclusion and anterior crossbite. A molar and canine class 1 and correction of the malocclusion were obtained. The improvement of facial aesthetics and smile enlarging were also ensured.

The material measurability of the timelessness in which we live does not exist, and the virtues have preserved them above all as human beings. They have the ability to enjoy the simple things in life, which in most cases are not financially commensurate. The connection between the beauty of the smile, positive health and longevity has been consciously sought. All aspects of the life of the Bulgarian population are outlined, which are positive and could lead them out of the black rankings for morbidity and mortality in Europe and the world. Questions are raised related to the health and activity of long-lived and centenarians and what in the modern way of life turn this trend into a negative one.

Epicurean philosophy provides an explanation of what makes people happy and how to experience joy and pleasure in life. Gossipiboma is a rare complication that can occur due to human negligence. Gossipiboma’s form as a complication of inflammatory reaction to a surgical sponge or a laparotomy pad left mistakenly in the body of the patient after a surgical procedure. The term Gossipiboma is a combination of two words from two different languages; Gossypium a Latin word which mean cotton and boma a Kiswahili word which means plan of concealment. Their incidence is reported to be 1 in 1000 to 1500 surgery. We present here a case of a 67-year-old male who had a history of right-sided Percutaneous Nephrolithotomy (PCNL) five years back. No immediate post procedure complications occurred however the patient presented to the Outpatient Department (OPD) with left flank pain and his Computed Tomography (CT) scan was done which showed an organized Gossipiboma.Most people find happiness by making money, eating good food, or spending time with friends. The success of orthodontic treatment depends on both the correct clinical assessment and a number... more The success of orthodontic treatment depends on both the correct clinical assessment and a number of social factors. The most significant among them are the health motivation of the patient, the family health culture and the social attitudes towards a positive result of the treatment. The aim of the study was to identify what is the motivation of patients to recommend orthodontist among their friends. It was hypothesized, that there would be significant correlations among treatment-outcome satisfaction, motivation, and expectations. Materials and methods: A sample of 259 patients who received orthodontic treatment completed a questionnaire. The average age of children is 12.56 ± 3.14 years, respectively 12.38 ± 3.22 for boys and 12.69 ± 3.07 for girls, P> 0.05. The questionnaire includes four basic tools in conducting an orthodontic study: (A) Functional Limitations (FL); (B) Oral Symptoms (OS); (C) Emotional Well-being (EW); (D) Social Wellbeing (SW). Results: The age of 9-12-year-old is the most sympathetic and closest to the dentist's effort group, P = 0.01 (χ 2 = 8.38). There is a statistically significant difference in the attitudes of children from different age groups-P = 0.015, χ 2 = 8.38 and understanding to shared responsibility for oral health protection P = 0.12 χ 2 = 5.77, df = 3. Children show high awareness and desire to protect their personal health. Conclusions: There are many different factors that would determine the dentistpatient relationships as excellent. The emotional investment in a long-term and quality service requires a lot of patience and empathy from the dentist, educational approach but not only to the children, also to the parents, treatment without push, etc. Additional factors are relaxed and friendly atmosphere; respect the personality of the patient, an attempt to escape from the clinical setting through game moments and conversations. More important than talking, however, is the ability of the dentist to hear what the patient says and avoid the paternalistic pattern of behavior.

Background New ultrasound techniques (high frequency ultrasound, elastography) have been put in t... more Background New ultrasound techniques (high frequency ultrasound, elastography) have been put in the clinical practice in recent years for the detection of skin involvement in Systemic Sclerosis (SSc). The modified Rodnan skin score (mRSS) is a standard measure used to evaluate severity and extent of skin involvement. The limitations of mRSS include the intraobserver and interobserver variability as well as the necessity of standardization. The pulmonary involvement is very common feature in patient with SSc and the gold standard of its detection was high resolution CT (HRCT). There exist a few studies in the past several years indicating that the Lung ultrasound (LUS) could substitute HRCT in screening of pulmonary interstitial complications. Objectives To determine the correlation between the dermal thickness revealed by high frequency ultrasound and the presence of B-lines at LUS examination in early diffuse SSc. Methods 42 patients with diffuse SSc have been identified in the present stusy by the extent of skin involvement. All subjects have met the ACR diagnostic criteria for SSc. The disease duration have been ≤3 years. The dermal thickness have been measured by means of an 18MHz probe at five different anatomical sites: phalanx, hand, forearm, leg, chest. The obtained values for all sites have resulted in an average skin thickness. LUS have been performed with 2.5-3.5 MHz transducer for anterior, posterior and lateral chest consecutively in determinated areas. LUS examination have been aimed at searching for B-lines, the sonographic marker of interstitial lung disease. Results Patients with diffuse SSc have revealed high dermal thickness during the early edematous phase. This finding correlates with the increased number of B-lines, detected at the LUS examinations. The results of the examinations have been characterized by the following statistical quantities: (i) mean value of dermal thickness x=1.77±0.21, (ii) 73.8±2.4% from the group of patients shows values of B-lines, which are greater than 6, (iv) the correlation is significant at P=0.01 for both methods applied: direct and significant relationship at the following correlation coefficients: Kendal 0.82, Spearman's rxy =0.93, Pearson 0.924. Conclusions A strong correlation has been found in this study between the dermal thickness examined at five anatomical sites and the total amount of B-lines at LUS examination of patients with diffuse SSc. Background: Pulmofibrosis and in general pulmonary involvement is very common feature in patients... more Background: Pulmofibrosis and in general pulmonary involvement is very common feature in patients with systemic sclerosis (SSc). It is detectable in more than 80% of SSc population and is one of the main cause of internal organ involvement and death. The golden standard of pulmonary involvement was high resolution CT (HRCT), but it has high cost and radiological risk for the patient. Typical alterations present increased ground glass opacities-increased lung attenuation in the absence of architectural distortion; intralobular interstitial thickening. Lung ultrasound (LUS) evaluates B-lines-the sonographic marker of pulmonary interstitial complication. Objectives: To estimate the diagnostic value of LUS in patients with SSc compared to HRCT. Methods: 49 patients (women, mean age 57±13 years) that fulfilled the ACR diagnostic criteria for SSc were evaluated by LUS, HRCT and pulmonary functional test. LUS was performed with 2.5-3.5 MHz transducer for anterior, posterior and lateral chest consecutively in determinated areas. All patients underwent echocardiography examination to evaluate the pressure in pulmonary artery, left and right ventricular systolic and diastolic function and to detect pulmonary hypertension. Results: Pulmonary fibrosis was detected in 43 patients (87.76±4.68%) of the SSc population at HRCT and in 41 patients (83.67±5.28%) at lung ultrasound. There was no difference between detected number of B-lines in younger and older patients, duration of disease (P>0.05). One patient was false positive at LUS. The analysis of results showed a corelation between LUS and HRCT (r xy =0.91, p<0.05). ROC curve analysis was used to confirm the analytical relationship between the number of B-lines and the presence of ILD at HRCT (AUC=0.89, Std. Err. =0.92, 95% CI 0.7 to 1.063, p<0.0001; Test results to sensitivity is equal to 0.83; to specificity-0.07. It means that the used lung ultrasound is validated and has a high efficiency and reliability as a prospective, wholesome method. Conclusions: Lung ultrasound is a reliable technique in the diagnostic assessement and recognition of pulmonary involvement in SSc. The presence of B-lines at LUS examination correlates with ILD at HRCT. LUS is very sensitive for detecting ILD. The use of LUS as a screening tool for ILD may be feasible to guide further investigation with HRCT.

METHODS: “Questionnaire for researching the study strategies” (N. Entwisle, M. Henly and D. Hensol, 1979) was used for the purpose for our survey; it was adapted for Bulgarian educational syllabus by J. Tzvetkova, 2001. The questionnaire included 30 statements. Data was processed using variational and nonparametric analysis. For better accuracy analysis for reliability with comparing to Cronbach alpha parameters was used. RESULTS: the obtained results repeat, to a certain extent, the results of J. Tzvetkova, 2001. Therefore, the patient must be at the centre of modern dentistry where the primary role has the Medical University in educating ethical principles and building doctors dedicated to humanity. Conclusion: The challenges in teaching students to communicate effectively with the patient are significant, especially in a global pandemic. The roots of philanthropy and empathy are laid back in time. They were described by the first teachers, physicians and philosophers in Medicine such as Hippocrates, Socrates, Plato, Aristotle and others. It is our responsibility just to repeat them.

That's why the Oral Healthcare Providers (OHP) should be convinced the consent given by the patient is valid. It means that at the beginning of the treatment the orthodontist will ask a lot of questions and have expectations to receive appropriate answers. There is a specific lack of awareness about the first orthodontic consultation at 7y of age, occurrence and prevention of most of the common tooth jaw discrepancies which affect the oral health, self-confidence and overall development of the child. A variety of socio-demographic, educational, personal and other factors mostly divided into objective and subjective factors influences the perception of facial attractiveness. The orthodontic treatment lays down on the personal desire and attitudes, depends from the motivation but is not without a risk for the patient. The aim of the current research is to present the most objective and subjective factors identifying the patient's refusal. Material and methods: It's a case report based on preliminary discussion and orthodontic consultation over the cephalometric analysis and cast models. Orthodontic treatment protocol was followed and given informed consent by the individual was received. Results and discussions: An electronic search was conducted using the Medline database (PubMed), Science Direct, and Scopus. In this case report were described the treatment options for Class III malocclusion with an emphasis on maxillary protraction and existing impacted canine 13. The decision making capacity was evaluated and also what are the objective and subjective factors and how to proceed with patient refusal. Conclusions: Despite the orthodontist's efforts to improve the management of the dental practice and to attract new patients, these challenges should never been from the first importance. Contemporary dentistry requires that the patient's right to refuse should be respected and this refusal must be accepted. Because orthodontic treatment is expensive, the process of returning money or sharing responsibility for the treatment depend on the socio-cultural characteristics of both the patient and the doctor. The whole situation requires a very delicate approach, as it affects the image of the dental community in society at whole.

The chief complaint of a patient and his parents are crowded teeth upper and lower and anterior crossbite. "@I'm looking like a crocodile" were the first words of the boy sitting on the dental chair. The main task was functional but also esthetics because it was from the first importance to him. The treated malocclusion is characterized with a narrow maxillary width and prominence of the mandible. The maxilla was deficient in anteroposterior and transverse dimensions, causing a slightly concave and unpleasant profile, a crossbite relationship of lateral incisors and upper and lower anterior crowding. The use of mechanics was preferred and accepted as a better solution for the patient according to the idea to generate well directed growth. The patient was 12 year-old boy with Class I malocclusion and anterior crossbite. A molar and canine class 1 and correction of the malocclusion were obtained. The improvement of facial aesthetics and smile enlarging were also ensured.

Gossipiboma’s form as a complication of inflammatory reaction to a surgical sponge or a laparotomy pad left mistakenly in the body of the patient after a surgical procedure. The term Gossipiboma is a combination of two words from two different languages; Gossypium a Latin word which mean cotton and boma a Kiswahili word which means plan of concealment. Their incidence is reported to be 1 in 1000 to 1500 surgery. We present here a case of a 67-year-old male who had a history of right-sided Percutaneous Nephrolithotomy (PCNL) five years back. The most significant among them are the health motivation of the patient, the family health culture and the social attitudes towards a positive result of the treatment. The aim of the study was to identify what is the motivation of patients to recommend orthodontist among their friends. It was hypothesized, that there would be significant correlations among treatment-outcome satisfaction, motivation, and expectations. Materials and methods: A sample of 259 patients who received orthodontic treatment completed a questionnaire. The average age of children is 12.56 ± 3.14 years, respectively 12.38 ± 3.22 for boys and 12.69 ± 3.07 for girls, P> 0.05. The questionnaire includes four basic tools in conducting an orthodontic study: (A) Functional Limitations (FL); (B) Oral Symptoms (OS); (C) Emotional Well-being (EW); (D) Social Wellbeing (SW). Results: The age of 9-12-year-old is the most sympathetic and closest to the dentist's effort group, P = 0.01 (χ 2 = 8.38). There is a statistically significant difference in the attitudes of children from different age groups-P = 0.015, χ 2 = 8.38 and understanding to shared responsibility for oral health protection P = 0.12 χ 2 = 5.77, df = 3. Children show high awareness and desire to protect their personal health. Conclusions: There are many different factors that would determine the dentistpatient relationships as excellent. The emotional investment in a long-term and quality service requires a lot of patience and empathy from the dentist, educational approach but not only to the children, also to the parents, treatment without push, etc. Additional factors are relaxed and friendly atmosphere; respect the personality of the patient, an attempt to escape from the clinical setting through game moments and conversations. More important than talking, however, is the ability of the dentist to hear what the patient says and avoid the paternalistic pattern of behavior.

The modified Rodnan skin score (mRSS) is a standard measure used to evaluate severity and extent of skin involvement. The limitations of mRSS include the intraobserver and interobserver variability as well as the necessity of standardization. The pulmonary involvement is very common feature in patient with SSc and the gold standard of its detection was high resolution CT (HRCT). There exist a few studies in the past several years indicating that the Lung ultrasound (LUS) could substitute HRCT in screening of pulmonary interstitial complications. Objectives To determine the correlation between the dermal thickness revealed by high frequency ultrasound and the presence of B-lines at LUS examination in early diffuse SSc. Methods 42 patients with diffuse SSc have been identified in the present stusy by the extent of skin involvement. All subjects have met the ACR diagnostic criteria for SSc. The disease duration have been ≤3 years. The dermal thickness have been measured by means of an 18MHz probe at five different anatomical sites: phalanx, hand, forearm, leg, chest. The obtained values for all sites have resulted in an average skin thickness. LUS have been performed with 2.5-3.5 MHz transducer for anterior, posterior and lateral chest consecutively in determinated areas. LUS examination have been aimed at searching for B-lines, the sonographic marker of interstitial lung disease. Results Patients with diffuse SSc have revealed high dermal thickness during the early edematous phase. This finding correlates with the increased number of B-lines, detected at the LUS examinations. The results of the examinations have been characterized by the following statistical quantities: (i) mean value of dermal thickness x=1.77±0.21, (ii) 73.8±2.4% from the group of patients shows values of B-lines, which are greater than 6, (iv) the correlation is significant at P=0.01 for both methods applied: direct and significant relationship at the following correlation coefficients: Kendal 0.82, Spearman's rxy =0.93, Pearson 0.924. Conclusions A strong correlation has been found in this study between the dermal thickness examined at five anatomical sites and the total amount of B-lines at LUS examination of patients with diffuse SSc. It is detectable in more than 80% of SSc population and is one of the main cause of internal organ involvement and death. The golden standard of pulmonary involvement was high resolution CT (HRCT), but it has high cost and radiological risk for the patient. Typical alterations present increased ground glass opacities-increased lung attenuation in the absence of architectural distortion; intralobular interstitial thickening. Lung ultrasound (LUS) evaluates B-lines-the sonographic marker of pulmonary interstitial complication. Objectives: To estimate the diagnostic value of LUS in patients with SSc compared to HRCT. Methods: 49 patients (women, mean age 57±13 years) that fulfilled the ACR diagnostic criteria for SSc were evaluated by LUS, HRCT and pulmonary functional test. LUS was performed with 2.5-3.5 MHz transducer for anterior, posterior and lateral chest consecutively in determinated areas. All patients underwent echocardiography examination to evaluate the pressure in pulmonary artery, left and right ventricular systolic and diastolic function and to detect pulmonary hypertension. Results: Pulmonary fibrosis was detected in 43 patients (87.76±4.68%) of the SSc population at HRCT and in 41 patients (83.67±5.28%) at lung ultrasound. There was no difference between detected number of B-lines in younger and older patients, duration of disease (P>0.05). One patient was false positive at LUS. The analysis of results showed a corelation between LUS and HRCT (r xy =0.91, p<0.05). ROC curve analysis was used to confirm the analytical relationship between the number of B-lines and the presence of ILD at HRCT (AUC=0.89, Std. Err. =0.92, 95% CI 0.7 to 1.063, p<0.0001; Test results to sensitivity is equal to 0.83; to specificity-0.07. It means that the used lung ultrasound is validated and has a high efficiency and reliability as a prospective, wholesome method. Conclusions: Lung ultrasound is a reliable technique in the diagnostic assessement and recognition of pulmonary involvement in SSc. The presence of B-lines at LUS examination correlates with ILD at HRCT. LUS is very sensitive for detecting ILD. The use of LUS as a screening tool for ILD may be feasible to guide further investigation with HRCT.

Material and methods: By using a sociological method of straight-group questionnaire the subjects (89 young adults) of experimental and control group have been asked to take into account factors such as age and sex, oral-health and perceptions of treatment. Dental aesthetics is assessed by means of the aesthetics component of the Visual Analogue Scale (VAS). By using Visual Analogue Scale every one of the undergraduates self-assessed one's own dental esthetics with the choice of a number of means. The results support the conclusion that favourable dental esthetics and previous orthodontic treatment appear as important variables that affect patient's attitude towards orthodontic treatment.

The individual's awareness of malocclusion, like the necessity for orthodontic treatment, seems t... more The individual's awareness of malocclusion, like the necessity for orthodontic treatment, seems to be different at different ages, as observed in orthodontic referrals. The information originating by orthodontist and proposed to the patient may be filled in writing and in oral form. The contemporary world requires that it can appear on-line, too. The aim of the study is to define the gap of children and parents awareness and motivation for orthodontic treatment. Materials and Methods: We have randomly selected parents (n = 214) and their children (n = 258) from Plovdiv at 2012. The inquiry contains questions designed to provide socio-demographic characteristics, the role of orthodontist and both importance of social media for the awareness in orthodontics. Results: Gender is not a factor for the parents awareness on the qualifications and skills of orthodontists (P > 0.05; χ 2 = 4.71, df = 3) but statistics shows significant difference in the group of orthodontist regarding the necessary motivation of patients (P < 0.05; χ 2 = 12.85). The benefits are complex and diverse, according to the perceptions of parents (P < 0.05; χ 2 = 29.60, df = 1). Mothers have a greater awareness than fathers (P = 0.027; χ 2 = 14.23, df = 6), but fathers take significant involvement in the payment of orthodontic services. Conclusions: Patient consent may be helpful to anticipate the problems that might arise during treatment. The cooperation of the patient is the only significant factor that orthodontists comply. The benefits of orthodontic treatment are complex and diverse, according to the perceptions of parents.
