Background: Limited research has been conducted on the post-intervention inflammatory status in s... more Background: Limited research has been conducted on the post-intervention inflammatory status in sarcopenic patients, despite previous studies revealing elevated pro-inflammatory markers. This study aimed to investigate the potential elevation of specific pro-inflammatory cytokines in sarcopenic patients and evaluate the effects of exercise and nutritional support interventions on these cytokine levels. Methods: In this post-hoc analysis of a randomized controlled trial (RCT), 57 individuals with sarcopenia from the RCT and 57 non-sarcopenic participants from the same geriatric community cohort that did not participate in the RCT were enrolled. Grip strength and body composition measurements were recorded. Tumor necrotizing factor (TNF)-α, interleukin (IL)-1β, IL-6, and IL-15 levels were assessed at baseline for both groups and after a 12-week intervention consisting of resistive exercise and supplementation with branched-chain amino acids, calcium, and vitamin D3 in the patients with sarcopenia. Results: The sarcopenic group demonstrated significantly lower body weight, body mass index, grip strength, and skeletal muscle mass index. Moreover, sarcopenic patients exhibited higher levels of TNF-α (p=0.007), IL-1β (p<0.001), and IL-6 (p<0.001), while no significant difference was observed in IL-15 (p=0.345) between participants with and those without sarcopenia. Following the intervention, the sarcopenic group experienced significant improvements in grip strength and skeletal muscle mass index with a notable reduction in TNF-α (p=0.003), IL-1β (p=0.012) and IL-6 (p=0.001) levels. Conclusions: Sarcopenic patients exhibit elevated levels of TNF-α, IL-1β, and IL-6, which declined after nutrition support and exercise interventions. However, further research is necessary to evaluate the long-term impact of these interventions on cytokine levels.

Bletilla formosana HAYATA (Orchidaceae) is a perennial herb with rhizomes, corms, or root-stem tu... more Bletilla formosana HAYATA (Orchidaceae) is a perennial herb with rhizomes, corms, or root-stem tuberoids. It is the only variable species of Bletilla in Taiwan. The tubers of Bletilla striata REICHB. FIL. has attracted much attention during the Severe Acute Respiratory Syndrome (SARS) prevalent period due to its use in treating pneumonophthisis and pneumonorrhagia in traditional Chinese medicine. 2) To the best of our knowledge, there has been no phytochemical investigation on B. formosana. As part of our effort to search for bioactive components from local indigenous herbs, we describe herein the isolation of three new dihydrophenanthrenes, along with seven known phenanthrene derivatives, six known flavonoids, a bibenzyl and three phenolic compounds from the whole herb of B. formosana. The ethanolic extract of the whole herbs was successively partitioned with ethyl acetate and n-butanol. The ethyl acetate and n-butanol-soluble fractions were separatedly subjected to silica gel column chromatography. Fractions rich in polyphenolic compounds were combined and purified by Sephadex LH-20 column to give three new dihydrophenanthrenes, 4-methoxy-9,10-dihydrophenanthrene-1,2,7-triol (1), 1-(4-hydroxybenzyl)-4,7-dimethoxy-9,10-dihydrophenanthrene-2-ol (2), and 1,3,6-tri(4-hydroxylbenzyl)-4-methoxydihydrophenanthrene-2,7-diol (3), along with seven known phenanthrene derivatives, 1-(4-hydroxybenzyl)-4-methoxy-9,10-dihydrophenanthrene-2,7-diol (4), 3) 1,6-di(4-hydroxylbenzyl)-4-methoxy-9,10-dihydrophenanthrene-2,7-diol (5), 4) 1,3-di(4-hydroxybenzyl)-4-methoxy-9,10-dihydrophenanthrene-2,7-diol (6), 1-(4-hydroxybenzyl)-4-methoxyphenanthrene-2,7-diol (7), 4) 1-(4-hydroxybenzyl)-4,8-di-methoxyphenanthrene-2,7-diol (8), 5) 1,8-bi(4-hydroxybenzyl)-4-methoxyphenanthrene-2,7-diol (9), 6) and blestriarene B (10), 7) a bibenzyl, 2Ј,6Ј-bis(p-hydroxybenzyl)-3,3Ј-dihydroxy-5methoxybibenzyl (11), 8) six known flavonoids, 8-C-p-hydoxybenzylkaempferol (12), 9) apigenin (13), 10) 6-methoxykaempferol (14), 11) kaempferol (15), 12) isorhamnetin (16), 13) kaempferol 7-O-glucoside (17), 12) 3,4-dihydroxybenzaldehyde (18), protocatechuic acid (19), p-(hydroxymethyl)phenyl-b-D-glucoside (20). Compound 1 was isolated as a pale yellow amorphous powder with a molecular formular C 15 H 14 O 4 by high-resolution (HR)-EI-MS at m/z 258.0951. UV absorption maxima at 218, 268, 279, 297, and 317 sh were characteristic of a dihydrophenanthrene skeleton. The IR spectrum showed ab-sorptions at 3397 (OH), 1598 and 1462 (benzenoid) cm Ϫ1 . Studies on the physicians' health have paid less attention on the Chinese medicine than on the we... more Studies on the physicians' health have paid less attention on the Chinese medicine than on the western medicine professionals. Like western medicine professionals, Chinese medicine physicians' health condition is critical to the quality of care and patients' safety. They also cared a large number of patients in many Asia societies. However, no data have been reported regarding the health problems associated with Chinese medicine physicians. The aim of this study was to examine the mortality patterns of a cohort consisting of 6109 male physicians of Chinese medicine who were followed from 1991 through 2003 in Taiwan. The analyses of female subjects were not included due to small sample size. We calculated the overall and cause-specific standardized mortality ratios (SMRs) and life expectancy of the study cohort using the death rates of some 11 millions of Taiwanese male population as reference. We found out that physicians of Chinese medicine experienced significantly lower death rate of mortality from all causes (SMR = 0.63, 95% > CI: 0.57, 0.69), from malignant neoplasms (SMR = 0.69, 95% CI: 0.57, 0.82) and from cerebrovascular disease (SMR = 0.53, 95% CI: 0.38, 0.72). Additionally, they have longer life expectancy (ranged from 3 years to approximately 15 years). Our study is crucial to evaluate potential health risks associated with Chinese medicine physicians. Although this study did not reveal elevated death rate among Chinese medicine physicians, researchers and policy makers should not overlook other heath problems that Chinese medicine physicians might have experienced. ──── Chinese medicine Physicians; standardized mortality ratio; life expectancy; mortality rate; occupational epidemiology.

Animal studies have shown that Lactobacillus plantarum can improve microbiota and immune regulation, as well as inhibit uric acid production. However, it is not clear whether L. plantarum can improve HC and intestinal microbiota. We used potassium oxonate (PO) to induce HC in male SD rats and then treated them with L. plantarum TCI227 in a dose-dependent manner (HC + LD, HC + MD, HC + HD) for 4 weeks. We examined organ weight, conducted biochemical examinations of blood and urine, and analyzed the intestinal microbiota in feces through a 16s rDNA sequence analysis. In this study, TCI227 improved body weight, decreased creatinine and serum uric acid, and increased urine uric acid compared to the HC group. Furthermore, TCI227 increased short-chain fatty acids (SCFAs). In the fecal microbiota (family), TCI227 increased the level of Lactobacillaceae and then decreased the levels of Deferribacteres and Prevotellaceae compared to the HC group. Finally, in the fecal microbiota (genus), TCI227 decreased the level of Prevotella and then increased the levels of Lactobacillus and Ruminococcus compared to the HC group. This study suggested that TCI227 can improve HC and can change the composition of intestinal microbiota in PO-induced male HC SD rats.

However, there is currently no routinely used biomarker for differential diagnosis of malignant and premalignant endometrial lesions. Ten-eleven translocation (TET) proteins, especially TET1, were found to play a significant role in DNA demethylation, via conversion of 5-methylcytosine (5-mC) to 5-hydroxymethylcytosine (5-hmC). TET1, 5-mC, and 5-hmC expression profiles in endometrial carcinogenesis are currently unclear. We conducted a hospital-based retrospective review of the immunohistochemical expression of TET1, 5-mC, and 5-hmC in 181 endometrial samples. Early oligomerization of tau involves conformational reorganization into parallel b-sheet structures and supramolecular assembly into toxic fibrils. Despite the need for selective inhibitors of tau propagation, b-rich protein assemblies are inherently difficult to target with small molecules. Here, we describe a minimalist approach to mimic the aggregation-prone modules within tau. We carried out a backbone residue scan and show that amide N-amination completely abolishes the tendency of these peptides to self-aggregate, rendering them soluble mimics of ordered b-strands from the tau R2 and R3 domains. Animal studies have shown that Lactobacillus plantarum can improve microbiota and immune regulation, as well as inhibit uric acid production. However, it is not clear whether L. plantarum can improve HC and intestinal microbiota. We used potassium oxonate (PO) to induce HC in male SD rats and then treated them with L. plantarum TCI227 in a dose-dependent manner (HC + LD, HC + MD, HC + HD) for 4 weeks. We examined organ weight, conducted biochemical examinations of blood and urine, and analyzed the intestinal microbiota in feces through a 16s rDNA sequence analysis. In this study, TCI227 improved body weight, decreased creatinine and serum uric acid, and increased urine uric acid compared to the HC group. Furthermore, TCI227 increased short-chain fatty acids (SCFAs). In the fecal microbiota (family), TCI227 increased the level of Lactobacillaceae and then decreased the levels of Deferribacteres and Prevotellaceae compared to the HC group. Finally, in the fecal microbiota (genus), TCI227 decreased the level of Prevotella and then increased the levels of Lactobacillus and Ruminococcus compared to the HC group. This study suggested that TCI227 can improve HC and can change the composition of intestinal microbiota in PO-induced male HC SD rats.

However, there is currently no routinely used biomarker for differential diagnosis of malignant and premalignant endometrial lesions. Ten-eleven translocation (TET) proteins, especially TET1, were found to play a significant role in DNA demethylation, via conversion of 5-methylcytosine (5-mC) to 5-hydroxymethylcytosine (5-hmC). TET1, 5-mC, and 5-hmC expression profiles in endometrial carcinogenesis are currently unclear. We conducted a hospital-based retrospective review of the immunohistochemical expression of TET1, 5-mC, and 5-hmC in 181 endometrial samples. Early oligomerization of tau involves conformational reorganization into parallel b-sheet structures and supramolecular assembly into toxic fibrils. Despite the need for selective inhibitors of tau propagation, b-rich protein assemblies are inherently difficult to target with small molecules. Here, we describe a minimalist approach to mimic the aggregation-prone modules within tau. We carried out a backbone residue scan and show that amide N-amination completely abolishes the tendency of these peptides to self-aggregate, rendering them soluble mimics of ordered b-strands from the tau R2 and R3 domains.
