Objective: To compare the diagnostic efficacy and agreement of the traditional tuberculin skin te... more Objective: To compare the diagnostic efficacy and agreement of the traditional tuberculin skin test with QuantiFERON-Tuberculosis Gold In-Tube test for latent tuberculosis infection in healthcare workers. Methods: The cross-sectional analytical study was conducted between March 1 and 31, 2008, at a specialist tuberculosis hospital in Istanbul, Turkey, and comprised healthcare workers who had been employed for at least one year at the hospital and volunteered to take part. Tuberculin skin test and QuantiFERON-Tuberculosis Gold In-Tube test were both performed simultaneously and their results were compared Using SPSS 12. Results: Out of 34 subjects, 20(58.8%) had a positive tuberculin skin test, and 7(20.6%) had a positive QuantiFERON-Tuberculosis Gold In-Tube test. The two tests agreed in only 15(44.1%) cases and disagreed in 19(55.9%). In 16(47.1%) subjects, the QuantiFERON-Tuberculosis Gold In-Tube test was negative and tuberculin skin testwas positive, while in 3(8.8%) participants QuantiFERON-Tuberculosis Gold In-Tube test was positive and tuberculin skin test was negative. Kappa test revealed discordance between the two tests (k=-0.13; p=0.92). Conclusions: Latent tuberculosis infection prevalence was higher based on tuberculin skin test than QuantiFERON-Tuberculosis Gold In-Tube test. The results of the two tests were discordant.

Erken evre ve endobronşiyal kritik darlığı olmayan ileri evre akciğer kanserlerinin tedavisinde kullanılmaktadır. FDT uygulaması, etkinliği, komplikasyonları ve endikasyonları kısaca anlatılmıştır.

Objective: To analyze data of cases with lung cancer (LC) diagnosed in our clinic over a one year... more Objective: To analyze data of cases with lung cancer (LC) diagnosed in our clinic over a one year period and compare them with data of cases with LC diagnosed ten years previously. Methods: The demographic data, radiological and bronchoscopic findings, diagnostic methods, and histological type and stages of the patients diagnosed with lung cancer in 2005 were evaluated. Results: Over a one year period, 138 patients were diagnosed with LC. 104 (75.4%) were men and 34 (24.6%) were women. Mean age was 63.78±9.53 (38-83). 118 (86.5%) of the patients had 39±16.34 (5-90) pack-years smoking history. Squamous cell carcinoma (SCC) was diagnosed in 40.6% of patients, adenocarcinoma in 29.0%, small cell carcinoma in 21.0%, combined type in 8.0% and carcinoma with unidentified cell type in 1.4%. Squamous cell carcinoma was more common among smoking patients. It was determined that the distribution of histological types in this study was similar to the previous studies, however the incidence of LC was found as increased in females in our study when compared to previous studies. Conclusion: We conclude that SCC is the most common histological type in patients with LC in our clinic and the male to female ratio for LC is decreasing.

We present here the case of a 29-year-old male with primary pleural synovial sarcoma. The patient... more We present here the case of a 29-year-old male with primary pleural synovial sarcoma. The patient had no complaints, no significant medical history and did not smoke. A well-circumscribed giant mass, adjacent to the chest wall was observed on a PA chest X-ray, with a solid mass lesion in the lateral right hemithorax with diffuse amorphous calcifications noted on thorax computed tomography. An F18-FDG uptake with a SUVmax of 5.0 was detected in the mass defined by PET/CT. A Tru-cut biopsy revealed monophasic as histological subtypes composed of a dense cellular and interlaced fascicular proliferation of spindle cells, and SYT-SSX1 fusion (X;18)(p11.23;q11) consistent with synovial sarcoma. The patient was followed-up in the oncology center after surgery and chemoradiotherapy, and has been stable for 1 year. This paper draws attention to a diagnostic approach to primary pleural synovial sarcoma, which is a rare and aggressive tumor, and the need for the prompt initiation of treatment for an improved outcome.

Objective: Early diagnosis is one of the most important steps in the management of tuberculosis, ... more Objective: Early diagnosis is one of the most important steps in the management of tuberculosis, so the con-ventional diagnostic techniques need to be improved. In our study, the diagnostic value of polymerase chain reaction (PCR) in active pulmonary tuberculosis was investigated. Methods: We have examined 79 cases (61 patients and 18 healthy subjects) between 20-22 ages. In these cases Mycobacterium tuberculosis complex DNA specific IS6110 field has been investigated by PCR and the re-sults were compared with the microbiological culture. Results: The sensitivity of PCR results were found to be 96.72% and the specificity was 100%. In 59 out of 61 cases with pulmonary tuberculosis, culture and PCR positivity were shown, PCR was negative for 2 of the culture (+) cases. All control cases were negative in terms of bacteriological culture, PCR, and sputum smear for acid fast bacillus (AFB). Conclusion: PCR is a rapid technique with high sensi-tivity and specificity. However, there is always a risk of contamination with other specimens and the amplifi-cation products. PCR is believed to be useful and time saving if we consider the time needed to wait for the results of microbiological culture, which in turn might cause delay in starting the treatment in time, especially in sputum smear negative cases. Background/Aims: Diagnostic tests for carcinoembryonic antigen (CEA) in ascites have been perform... more Background/Aims: Diagnostic tests for carcinoembryonic antigen (CEA) in ascites have been performed in various malignant cases, but there is only few data on the applicability of CEA for colorectal cancer (CRC) patients with peritoneal carcinomatosis. We aimed to determine the usefulness of CEA in ascites (aCEA) as a diagnostic parameter for CRC with peritoneal carcinomatosis. Methods: Between January 2000 and May 2013, the medical records of 259 patients who underwent paracentesis for the evaluation of ascites were retrospectively reviewed. CRC patients with ascites (n=82) and patients with non-malignant ascites (n=177) were evaluated. Patients who had other malignancies, including gastric or ovarian cancer, with ascites were excluded. The optimal diagnostic cutoff value of aCEA for CRC with peritoneal carcinomatosis was determined using receiver operating characteristic curve analysis. The value of aCEA for predicting the occurrence of peritoneal carcinomatosis was evaluated using a logistic regression model. Results: The optimal cutoff value of aCEA to diagnose CRC with peritoneal carcinomatosis was 3.89 ng/mL, and the area under the curve for aCEA was 0.996 (sensitivity 96.3%, specificity 100%, positive predictive value 100%, negative predictive value 98.3%). Multivariate logistic regression analysis showed that aCEA was an independent factor predicting the occurrence of peritoneal carcinomatosis. Conclusions: In this study, we showed that aCEA may be a useful parameter for diagnosing CRC with peritoneal carcinomatosis, and we propose an optimal aCEA cutoff value of 3.89 ng/mL. Further study that includes patients with other malignant ascites may be necessary to validate these findings.
