Haemoglobin SC (HbSC) disease accounts for 30% of cases of sickle cell disease in the United King... more Haemoglobin SC (HbSC) disease accounts for 30% of cases of sickle cell disease in the United Kingdom and the United States. Unlike other sickle cell carriers, who are relatively asymptomatic, people with HbSC disease have a combination of genotypes with the potential to cause considerable morbidity due to intracellular water loss. Patients can present with acute pain, acute chest syndrome, proliferative retinopathy, splenic and renal complications, or stroke. We present a young man with HbSC disease who developed acute compartment syndrome. This is only the second report of this syndrome in a patient with HbSC disease. This is a very rare complication in HbSC disease, but it can have serious implications.

The non-invasive method of blood pressure measurement is regarded as a safe procedure and the rep... more The non-invasive method of blood pressure measurement is regarded as a safe procedure and the reports of any serious complications are rare. We report a unique case of extensive skin necrosis due to an intermittently inflating blood pressure cuff in a 65-year-old critically ill lady following a third time redo mitral valve surgery. A brief review of the literature on complications associated with noninvasive method of measurement of blood pressure is presented along with possible mechanisms of skin injury and ways to avoid it.

Aim: Cryotherapy is a common treatment for a variety of skin conditions; however, it has to be pe... more Aim: Cryotherapy is a common treatment for a variety of skin conditions; however, it has to be performed with care due to the risks of inadvertent tissue damage including burns, damage to healthy tissue, and scarring. We report on the first case of septic arthritis developing post cryotherapy to a digit. Method: A 57yr-old female retired podiatrist who five days prior received cryotherapy, by a Dermatologist, for a long standing presumed viral wart on the dorsum of her hand. Results: septic arthritis to the distal interphalangeal joint. Conclusions:-The delicate skin and tissues over the dorsum of the fingers and hand can result in significant trauma. Care must be exercised in these areas to prevent full-thickness wounds that involve the joint and result in septic arthritis and functional disability.

Aim: To identify the rate of K-wire infection in hand injuries and ascertain compliance with NICE... more Aim: To identify the rate of K-wire infection in hand injuries and ascertain compliance with NICE guidelines on antibiotic prophylaxis and surgical skin preparation. Method: Retrospective audit/service evaluation-EPR Operations first cycle (August-December 2019), and the second cycle (August-November 2020). Included all cases (adult and paediatric) of hand injuries managed with Kwire fixation-Plastic Surgery Trauma theatre lists. Results: After the first cycle recommendations, the infection rates fell from 15 % to 11 %. Conclusions: Infection rates were lower in the group of patients who had antibiotics on induction and on discharge.

Is... more Radial head fractures are common injuries, occurring in about 20% of all acute elbow injuries. Isolated radial head fractures are not common and include about 2% of all fractures around the elbow. Bilateral radial head fractures are rare and usually associated with severe trauma and associated fractures and dislocations. We report a case of bilateral undisplaced radial head fracture in a woman, following a simple fall. Early recognition, proper management, and physical therapy led to complete recovery and full functional movement of the elbow.

We present a rare case of primary tuberculous tenosynovitis in a young patient with an acute hist... more We present a rare case of primary tuberculous tenosynovitis in a young patient with an acute history of non-penetrating traumatic injury. The patient had recently visited Pakistan and presented with sudden onset pain at the base of their right fifth digit after trying to catch a cricket ball. A provisional diagnosis of haematoma was made; however, ultrasonography revealed a mass attached to the A2 pulley. Surgical excision followed by histological examination and culture identified Mycobacterium Tuberculosis infection. This case serves to raise clinical awareness of this rare condition and highlight the importance of obtaining a travel history.

We present a case of a solitary extramedullary plasmacytoma of the frontal bone presenting as an asymptomatic forehead lump with clinically benign characteristics. This case highlights the need for a high index of suspicion when dealing with enlarging subcutaneous lumps of the forehead and scalp. The significance of this lies in the appropriate sequencing of investigations and the implementation of the necessary treatment regimen.

Minimal important changes and differences describe the smallest changes and differences between i... more Minimal important changes and differences describe the smallest changes and differences between individuals that are relevant to patients following treatment. Minimal important differences may vary between conditions, treatments and lengths of follow-up, and can be calculated in different ways. Minimal important differences for elective hand surgery were reviewed. A total of 99 minimal important differences were identified in 29 articles. The conditions, treatments, outcome measures used and follow-up periods are discussed. The Disabilities of the Arm, Shoulder and Hand had the most estimates of minimal important differences, but these varied. The methods used in the included studies were reviewed and appraised. Most minimal important differences were calculated using retrospective anchors. Future research directions in this area are suggested.
