The authors will describe in the following pages the reasons why the project of the Multi-functio... more The authors will describe in the following pages the reasons why the project of the Multi-functional Sports Hall from Cluj Napoca is attractive. The main lines of the building are: a hall with a capacity of 7000 seats, a structure dominated by precast concrete elements, a long span roof and an advanced analysis of the connections, all well-kept into the limited funds. The roof solution consists in using steel space trusses made out of square hollow sections (SHS). The truss has a clear span of 63,90 m, a total length of 76,10 m, a maximum height of 4,00 m that is reduced on the length of the structural element, and a triangular cross section being 3,60 m wide. Global stability checks and specific local stability problems were performed and are exposed in the following paper. For the fabrication of the space truss, welded joints between the SHS profiles were designed. As a result of the fact that for characteristic failure checks of the welded TT and KK joints analytical methods are based only on a semi-empirical formulae, developed for Φ=90 degrees (the angle between the diagonal planes), for the design of joints finite element modelling was used. Good agreement between the results of the developed finite element joint model and the analytical method for TT and KK joints has been found, even though the semi-empirical formulae are applied for the analysed truss which had Φ=50 degrees.

In mouse embryo NIH 3T3 fibroblasts, ethanol (60 -80 mM) was found to enhance the stimulatory eff... more In mouse embryo NIH 3T3 fibroblasts, ethanol (60 -80 mM) was found to enhance the stimulatory effects of sphingosine 1-phosphate (S1P) on both DNA synthesis and cell proliferation. Well-detectable potentiating effects of ethanol on S1P-induced mitogenesis required the presence of calcium (>1 mM) and zinc (20 -40 M) in the incubation medium. The amphibian tetrapeptide bombesin, which is known to mobilize intracellular calcium in fibroblasts, had no effect alone, but it approximately doubled the combined stimulatory effects of ethanol and S1P on DNA synthesis. The synergistic mitogenic effects of ethanol and S1P were also slightly enhanced, rather than inhibited, by the alcohol dehydrogenase inhibitor 4-methylpyrazole (5 mM). Of the various growth regulatory enzymes examined, ethanol detectably enhanced the stimulatory effects of S1P on the phosphosphorylation (activation) of p42/p44 mitogen-activated protein (MAP) kinases, but not of p38 MAP kinase. Cotreatment of fibroblasts with ethanol for 10 min also enhanced the stimulatory effects of S1P on the activities of c-Raf-1 kinase and p70 S6 kinase, but neither S1P nor ethanol had effects on phosphatidylinositol 3-kinase and Akt/PKB kinase activities. Ethanol-plus-S1Pinduced DNA synthesis was partially inhibited by both PD 98059 (50 M) and rapamycin (10 nM), inhibitors of p42/p44 MAP kinase kinase and mTOR/p70 S6 kinases, respectively. The results indicate that in NIH 3T3 fibroblasts, ethanol can enhance the mitogenic effects of S1P by a zinc-and calcium-dependent mechanism involving both the rapamycin-sensitive p70 S6 kinasedependent and the c-Raf-1/MAP kinase-dependent growth regulatory pathways.

Alcohol abuse is a risk factor for cancers of the gastrointestinal tract, and it also can precipi... more Alcohol abuse is a risk factor for cancers of the gastrointestinal tract, and it also can precipitate psoriasis characterized by hyperproliferation of epidermal cells. Because these effects of alcohol may involve stimulation of ccll growth, and ethanol (EtOH) was shown to enhancc DNA synthesis in mouse fibroblasts and cpidermal cclls. wc conducted a study to determine whcthcr EtOH can also stimulate mitogcnesis in human fibroblasts and keratinocytes. In kcratinocytes, EtOH had no cffccts on mitogenesis aftcr shorter (17-hr) treatments. but it partially preventcd inhibition of DNA synthesis elicited by longer treatments (3-4 days) with 2 mM calcium (Ca'+), a differentiation-inducing agent. In contrast, trcatmcnt of serum-starved zinc-treatcd (40 wM) human skin fibroblasts with 50-60 mM EtOH for 17 hr resulted in increased DNA synthcsis. EtOH-induced DNA synthesis was blocked by 1 mM EGTA, a specific Ca" chelator. Despite the presence of 1.8 mM Ca" in the ccll culture medium, thc addition of 1 mM extra Ca" (final concentration, 2.8 mM) for 17 hr induced DNA synthesis, prcsumably mediated by Ca'+ rcccptors. In cight independent human skin fibroblast lines examincd, treatment with EtOH for 46 hr, but not for 17 hr, invariably enhanced the effects of Ca'+ on DNA synthesis, consistent with synergistic stimulation of cell prolifcration by EtOH and Ca". Ncomycin, a Ca" rcccptor agonist, and EtOH also exerted synergistic effects on DNAsynthesis aftor longer (46-hr) treatments. In mouse NIH 3T3 fibroblasts, both EtOH-and Ca' ' -enhanced DNA synthesis aftcr 17-hr treatment, but they stimulated cell proliferation only in combination. The results indicate that in human fibroblasts, EtOH can potentiate the longer-term effects of high concentrations of Ca' ' on DNA synthesis whereas, in kcratinocytes, EtOH may inhibit Ca''.-induced differentiation.

OBJECTIVE: To report the appearances of ultrasound (US) and magnetic resonance imaging (MRI) befo... more OBJECTIVE: To report the appearances of ultrasound (US) and magnetic resonance imaging (MRI) before and after surgery for chronic patellar tendinopathy and to correlate postoperative appearances with clinical outcome.DESIGN: A 12-month prospective longitudinal study and a retrospective study, each part using different patients. Prospective study included clinical assessment, ultrasound, and MRI all performed before and 12 months after surgery. Retrospective study included ultrasound and clinical assessment only (i.e., no MRI) 24 to 67 months after surgery.SETTING: Institutional athlete study group in Australia (Victorian Institute of Sport Tendon Study Group).PATIENTS: In the prospective study, 13 patients (all male; 15 tendons) who underwent patellar tenotomy; in the retrospective study, 17 different patients (18 tendons) who had undergone identical surgery.MAIN OUTCOME MEASURES: Ultrasound and MRI appearances and clinical assessment at baseline and 12 months after surgery (prospective study). Ultrasound appearance and clinical assessment 24 to 67 months after surgery (retrospective study). Dimensions of abnormal regions on imaging were measured. Clinical assessment included categorical rating and numerical Victorian Institute of Sport Assessment (VISA) score.RESULTS: In the prospective study, preoperative ultrasound and MRI appearances confirmed the clinical diagnosis of patellar tendinopathy. Postoperative ultrasound and MRI also revealed abnormalities consistent with patellar tendinopathy. Despite this, 11 of 15 (73%) tendons were rated clinically as either good or excellent. Imaging modalities were unable to distinguish tendons rated as good or excellent from those rated poor at 12 months. In the retrospective study, ultrasound images revealed abnormalities despite full clinical recovery. There was no correlation between dimension of ultrasound abnormality and either VISA score or time since surgery.CONCLUSION: After open patellar tenotomy, MRI and ultrasound findings remain abnormal despite clinical recovery. Thus, clinicians ought to base postoperative management of patients undergoing patellar tenotomy on clinical grounds rather than imaging findings. At present, there appears to be no role for routine postoperative imaging of patients recovering slowly after patellar tenotomy. However, this is not to suggest that imaging cannot play a role in special circumstances.

According to our knowledge, there has never been a comprehensive research in Hungary dealing with... more According to our knowledge, there has never been a comprehensive research in Hungary dealing with healthcare university or college students' social background. The main objectives of our cross-section research were to analyze differences in the social background of female students who decided to become doctors, professional nurses and health visitors. This research was conducted among female medical and nursing college students at the Semmelweis University. There were 295 students invited to participate, the response rate was 68,08% (N = 201). Data analysis was performed by SPSS software by using descriptive methods of statistics. We found a decisive difference among medical, nursing and health visitor students while analyzing a number of socio-demographic characteristics. Most medical students came from families where the parents had college or university degrees, but we can state that there is a minimal likelihood in the case of college students that they are descendants of parents with higher educational degrees. We did not find statistical differences in the three student sample groups regarding their marital status, but fewer nursing college students lived in marriage or household partnership. We found a significant difference in the social background of nursing college and medical students. The social disadvantages of nursing and health visitor students are more considerable than those of medical students.

In these activities, physical education and sport can play an important role in the growth and harmonious development of children and, not the least in their socialization and social integration. In this way, the achieved research is of an exploratory type, and it proposed itself to collect information on how the physical education and sport facilities are employed during specialized activities carried out in foster care centres in Caras-Severin county and how the physical education teachers are involved in the activities of specialized intervention.

We have applied both neutron and X-ray radiography to study the distribution of the residual material in the damaged K5. X-ray radiography visualized the main distribution of the residual materials, like different kinds of B 4 C and water, and the frozen gas bubbles, while neutron radiography displayed the fine structure of the congealed B 4 C. The 3D reconstruction of the most interesting parts of K5 has been done by discrete tomography.

Given that, to our knowledge, it has not been reported in the literature, the objective of this study was to investigate the intertester reliability of sonography for the detection and measurement of hypoechoic areas associated with patellar tendinopathy. The study cohort comprised 8 patients with clinically diagnosed patellar tendinopathy and 4 patients with bilateral asymptomatic patellar tendons. Two equally experienced musculoskeletal radiologists imaged both patellar tendons from each patient (n = 24). All 24 tendons were assessed on the same day with the use of identical sonography machines. Discrete tomography (DT) is a new technique to reconstruct discrete images from their projections... more Discrete tomography (DT) is a new technique to reconstruct discrete images from their projections (like neutron images). The reconstruction methods in DT are different from the conventional ones, because the created images may contain only a few numbers of given discrete values. One of the main reasons to apply DT is that hopefully we need only a few numbers of projections. In many applications we have a situation where we know the material components of the object to be studied, that is, we know the discrete values of the image to be reconstructed. Using discreteness and some a priori information we can apply several DT methods in neutron imaging. Most of the DT reconstruction methods are reducing the problem to an optimization task. To evaluate the ability of ultrasonography to predict eventual symptoms in an at-risk population,... more To evaluate the ability of ultrasonography to predict eventual symptoms in an at-risk population, 52 elite junior basketball players' patellar tendons were studied at baseline and again 16 months later. The group consisted of 10 study tendons (ultrasonographically hypoechoic at baseline) and 42 control tendons (ultrasonographically normal at baseline). By design, all tendons were asymptomatic at baseline. No differences were noted between subjects and controls at baseline for age, height, weight, training hours, and vertical jump. Functional (P < 0.01) and symptomatic outcome (P < 0.05) were poorer for subjects' tendons than for controls. Relative risk for developing symptoms of jumper's knee was 4.2 times greater in case tendons than in control tendons. Men were more likely to develop ultrasonographic changes than women (P < 0.025), and they also had significantly increased training hours per week (P < 0.01) in the study period. We compared the outcomes in 25 subjects (29 tendons) who had had open patellar tenotomy and 23 subjects (25 tendons) who had had arthroscopic patellar tenotomy at a mean follow-up of 3.8 and 4.3 years, respectively. At follow-up, outcomes in the open and arthroscopic groups were as follows: 1) symptomatic benefit was seen in 81% of open and 96% of arthroscopic tenotomy patients, 2) sporting success was seen in 54% of open and 46% of arthroscopic tenotomy patients, 3) median time to return to preinjury level of activity was 10 months for open and 6 months for arthroscopic tenotomy patients, and 4) median Victorian Institute of Sport Assessment score at follow-up was 88 for open and 77 for arthroscopic tenotomy patients. Extra Ca2+ stimulated phosphorylation/activation of p42/p44 mitogen-activated protein kinases by an initially (10 min) zinc-independent mechanism; however, insulin, and particularly zinc, significantly prolonged Ca2+-induced mitogen-activated protein kinase phosphorylation. Midline sagittal magnetic resonance images were taken of 12 knees from 10 patients and of 17 knees from 15 age- and activity-matched subjects who underwent imaging for reasons other than patellar tendinitis. Of the 12 magnetic resonance imaging scans of knees with clinical patellar tendinitis, 3 (25%) exhibited no defect and only 7 (58%) had unequivocal intratendinous lesions. Among the 17 scans of subjects without clinical patellar tendinitis, 5 (34%) showed no defect and 4 (24%) had unequivocal intratendinous lesions. Proximal tendon width was significantly larger for the tendinitis patient group (5.0 +/- 1.7 mm versus 3.9 +/- 1.0 mm), although considerable overlap was present. Here, attached and suspended NIH 3T3 fibroblasts as well as variants of the MCF-7 human breast carcinoma cell line expressing PKC-alpha and a PtdCho-specific PLD activity at widely different levels were used to determine the possible role of PKC-alpha, PtdCho hydrolysis, and choline uptake in the mediation of PMA effect on PtdCho synthesis. In wild-type MCF-7 cells, which express both PKC-alpha and PLD activities at very low levels, PMA had little effects on the uptake or incorporation [14C]choline into PtdCho. In multidrug resistant MCF-7/MDR1 cells, which highly express PKC-alpha but lack the PtdCho-specific PLD activity, 100-nM PMA had relatively small stimulatory effects on the uptake of [14C]choline (approximately 1.5-fold) and [14C]PtdCho synthesis (1.5- to 2-fold). In cight independent human skin fibroblast lines examincd, treatment with EtOH for 46 hr, but not for 17 hr, invariably enhanced the effects of Ca'+ on DNA synthesis, consistent with synergistic stimulation of cell prolifcration by EtOH and Ca". Ncomycin, a Ca" rcccptor agonist, and EtOH also exerted synergistic effects on DNAsynthesis aftor longer (46-hr) treatments. In mouse NIH 3T3 fibroblasts, both EtOH-and Ca' ' -enhanced DNA synthesis aftcr 17-hr treatment, but they stimulated cell proliferation only in combination. The results indicate that in human fibroblasts, EtOH can potentiate the longer-term effects of high concentrations of Ca' ' on DNA synthesis whereas, in kcratinocytes, EtOH may inhibit Ca''.-induced differentiation.

OBJECTIVE: To report the appearances of ultrasound (US) and magnetic resonance imaging (MRI) befo... more OBJECTIVE: To report the appearances of ultrasound (US) and magnetic resonance imaging (MRI) before and after surgery for chronic patellar tendinopathy and to correlate postoperative appearances with clinical outcome.DESIGN: A 12-month prospective longitudinal study and a retrospective study, each part using different patients. Prospective study included clinical assessment, ultrasound, and MRI all performed before and 12 months after surgery. Retrospective study included ultrasound and clinical assessment only (i.e., no MRI) 24 to 67 months after surgery.SETTING: Institutional athlete study group in Australia (Victorian Institute of Sport Tendon Study Group).PATIENTS: In the prospective study, 13 patients (all male; 15 tendons) who underwent patellar tenotomy; in the retrospective study, 17 different patients (18 tendons) who had undergone identical surgery.MAIN OUTCOME MEASURES: Ultrasound and MRI appearances and clinical assessment at baseline and 12 months after surgery (prospective study). Ultrasound appearance and clinical assessment 24 to 67 months after surgery (retrospective study). Dimensions of abnormal regions on imaging were measured. Clinical assessment included categorical rating and numerical Victorian Institute of Sport Assessment (VISA) score.RESULTS: In the prospective study, preoperative ultrasound and MRI appearances confirmed the clinical diagnosis of patellar tendinopathy. Postoperative ultrasound and MRI also revealed abnormalities consistent with patellar tendinopathy. Despite this, 11 of 15 (73%) tendons were rated clinically as either good or excellent. Imaging modalities were unable to distinguish tendons rated as good or excellent from those rated poor at 12 months. In the retrospective study, ultrasound images revealed abnormalities despite full clinical recovery. There was no correlation between dimension of ultrasound abnormality and either VISA score or time since surgery.CONCLUSION: After open patellar tenotomy, MRI and ultrasound findings remain abnormal despite clinical recovery. Thus, clinicians ought to base postoperative management of patients undergoing patellar tenotomy on clinical grounds rather than imaging findings. At present, there appears to be no role for routine postoperative imaging of patients recovering slowly after patellar tenotomy. However, this is not to suggest that imaging cannot play a role in special circumstances.

According to our knowledge, there has never been a comprehensive research in Hungary dealing with... more According to our knowledge, there has never been a comprehensive research in Hungary dealing with healthcare university or college students&amp;amp;amp;#39; social background. The main objectives of our cross-section research were to analyze differences in the social background of female students who decided to become doctors, professional nurses and health visitors. This research was conducted among female medical and nursing college students at the Semmelweis University. There were 295 students invited to participate, the response rate was 68,08% (N = 201). Data analysis was performed by SPSS software by using descriptive methods of statistics. We found a decisive difference among medical, nursing and health visitor students while analyzing a number of socio-demographic characteristics. Most medical students came from families where the parents had college or university degrees, but we can state that there is a minimal likelihood in the case of college students that they are descendants of parents with higher educational degrees. We did not find statistical differences in the three student sample groups regarding their marital status, but fewer nursing college students lived in marriage or household partnership. We found a significant difference in the social background of nursing college and medical students. The social disadvantages of nursing and health visitor students are more considerable than those of medical students.

We have applied both neutron and X-ray radiography to study the distribution of the residual material in the damaged K5. X-ray radiography visualized the main distribution of the residual materials, like different kinds of B 4 C and water, and the frozen gas bubbles, while neutron radiography displayed the fine structure of the congealed B 4 C. The 3D reconstruction of the most interesting parts of K5 has been done by discrete tomography.

Given that, to our knowledge, it has not been reported in the literature, the objective of this study was to investigate the intertester reliability of sonography for the detection and measurement of hypoechoic areas associated with patellar tendinopathy. The study cohort comprised 8 patients with clinically diagnosed patellar tendinopathy and 4 patients with bilateral asymptomatic patellar tendons. Two equally experienced musculoskeletal radiologists imaged both patellar tendons from each patient (n = 24). The reconstruction methods in DT are different from the conventional ones, because the created images may contain only a few numbers of given discrete values. One of the main reasons to apply DT is that hopefully we need only a few numbers of projections. In many applications we have a situation where we know the material components of the object to be studied, that is, we know the discrete values of the image to be reconstructed. The group consisted of 10 study tendons (ultrasonographically hypoechoic at baseline) and 42 control tendons (ultrasonographically normal at baseline). By design, all tendons were asymptomatic at baseline. No differences were noted between subjects and controls at baseline for age, height, weight, training hours, and vertical jump. Functional (P < 0.01) and symptomatic outcome (P < 0.05) were poorer for subjects' tendons than for controls. Relative risk for developing symptoms of jumper's knee was 4.2 times greater in case tendons than in control tendons. Men were more likely to develop ultrasonographic changes than women (P < 0.025), and they also had significantly increased training hours per week (P < 0.01) in the study period. We compared the outcomes in 25 subjects (29 tendons) who had had open patellar tenotomy and 23 subjects (25 tendons) who had had arthroscopic patellar tenotomy at a mean follow-up of 3.8 and 4.3 years, respectively. At follow-up, outcomes in the open and arthroscopic groups were as follows: 1) symptomatic benefit was seen in 81% of open and 96% of arthroscopic tenotomy patients, 2) sporting success was seen in 54% of open and 46% of arthroscopic tenotomy patients, 3) median time to return to preinjury level of activity was 10 months for open and 6 months for arthroscopic tenotomy patients, and 4) median Victorian Institute of Sport Assessment score at follow-up was 88 for open and 77 for arthroscopic tenotomy patients. Midline sagittal magnetic resonance images were taken of 12 knees from 10 patients and of 17 knees from 15 age- and activity-matched subjects who underwent imaging for reasons other than patellar tendinitis. Of the 12 magnetic resonance imaging scans of knees with clinical patellar tendinitis, 3 (25%) exhibited no defect and only 7 (58%) had unequivocal intratendinous lesions. Among the 17 scans of subjects without clinical patellar tendinitis, 5 (34%) showed no defect and 4 (24%) had unequivocal intratendinous lesions. Proximal tendon width was significantly larger for the tendinitis patient group (5.0 +/- 1.7 mm versus 3.9 +/- 1.0 mm), although considerable overlap was present. Here, attached and suspended NIH 3T3 fibroblasts as well as variants of the MCF-7 human breast carcinoma cell line expressing PKC-alpha and a PtdCho-specific PLD activity at widely different levels were used to determine the possible role of PKC-alpha, PtdCho hydrolysis, and choline uptake in the mediation of PMA effect on PtdCho synthesis. In wild-type MCF-7 cells, which express both PKC-alpha and PLD activities at very low levels, PMA had little effects on the uptake or incorporation [14C]choline into PtdCho.
